Low contrast stimuli enhance PERG sensitivity to the visual dysfunction in Parkinson's disease.

The pattern electroretinogram (PERG) was recorded at different contrast levels (96%, 71%, 47%) in 10 Parkinson's disease patients before and during dopaminergic monotherapy. The data were compared to a control group of 8 normal subjects recorded with the same procedure. PERG P50 latency progressively increased as contrast was decreased both in normal subjects and patients; however, this trend was much more pronounced in PD patients without therapy; consequently in this group the difference between P50 latency obtained with 96% and 47% contrast was statistically significant (P = 0.01, analysis of variance corrected by post-hoc Tukey test). By contrast this was not seen in the control group. Statistical analysis (Bonferroni's t test) showed at the 47% contrast level a significant P50 latency increase (P less than 0.01) in PD patients without therapy if compared with the control group. Dopaminergic monotherapy induced a P50 latency recovery in PD patients. We conclude that low contrast stimuli enhance PERG sensitivity to the visual dysfunction of PD patients. Moreover, the effects observed after therapy confirm that abnormal contrast response functions in PD patients are linked to dopaminergic deficiency.

Electroencephalography.

Recent advances in electroencephalography (EEG) research are presented. Particular emphasis is placed on sleep and epilepsy research in humans. In this context, among others, studies on the modifications of EEG epileptic activity during sleep are discussed in more detail. A large number of reports come from EEG monitoring during presurgical evaluation and surgical treatment of drug-resistant epilepsy; information provided by these studies is therefore considered with special attention.

Spontaneous subarachnoid hemorrhage in an HIV patient.

In an HIV-seropositive patient presenting generalized tonic-clonic seizures, magnetic resonance imaging and cerebrospinal fluid examination disclosed the signs of a previous subarachnoid bleed. No vascular malformation was observed with cerebral angiography. Laboratory tests revealed an autoimmune thrombocytopenia. A spontaneous subarachnoid hemorrhage induced by thrombocytopenia should be considered when investigating HIV patients presenting even non-specific neurological symptoms.

Dopaminergic pharmacological manipulations in normal humans confirm the specificity of the visual (PERG-VEP) and cognitive (P300) electrophysiological alterations in Parkinson's disease.

Retinal and occipital visual evoked potentials and event-related potentials (P300) have been recorded in normal human subjects before and after the administration of the dopaminergic receptor antagonist, haloperidol, and/or the dopaminergic precursor L-DOPA. The data show that either retinal or occipital visual potentials and P300 are delayed by haloperidol. These findings are consistent with the hypothesis that haloperidol in healthy subjects mimicks the electrophysiological abnormalities observed in Parkinson's disease. On the other hand, L-DOPA does not generally modify these latencies in normals, while it is known to decrease the same parameters in parkinsonian patients. This is in accord with the involvement of a specific mechanism in the recovery observed in parkinsonian patients after L-DOPA therapy. Our data confirm that the alterations of visual and cognitive potentials observed in Parkinson's disease are closely related to the impairment of dopaminergic transmission.

P300 variations in parkinsonian patients before and during dopaminergic monotherapy: a suggested dopamine component in P300.

P300 was recorded, using an 'odd ball' paradigm, in 18 parkinsonian patients before and during dopaminergic monotherapy. The data were compared with those of a homogeneous standard group of 20 subjects. The main finding was an increase in the P300 latency of parkinsonian patients before therapy, which recovered during dopaminergic monotherapy. In 11 voluntary healthy subjects the same therapy did not produce a reduction of the P300 latency. The data are discussed in relation to a possible dopaminergic component in P300 origin.

Collet-Sicard syndrome caused by a coiling of the internal carotid artery.

A Collet-Sicard syndrome was observed in a 53-year-old patient with a coiling of the left internal carotid artery just below the skull base. Although impairment of cranial nerves by tortuous vessel compression has frequently been reported, a combined palsy of the last four cranial nerves related to such a mechanism has never been described before.

[Computer program recognition of a cDNA sequence specifying signal peptides].

An application of a computational analysis of cDNA sequences is presented in this paper. The goal is the identification of functional domains on sequence data. The results show the capability of this technique to identify a zone of DNA associated with the signal peptide coding region, whose biological function at DNA or RNA level is still unknown.

[Boundary cerebral infarct in a case of essential thrombocythemia].

We describe a case of essential thrombocythemia in a patient with a mild heart failure. The only clinical manifestation consisted of neurologic symptoms. Cerebral CT was able to show boundary zone infarcts. A possible pathogenetic mechanism for the unusual clinical and radiological finding is proposed.
