[Residual gastric carcinoid complicated with multiple endocrine neoplasia and Cushing's syndrome--a case report].

A 51-year-old man had had his stomach resected 10 years earlier due to gastric ulcer. Ten months before the present illness, he complained of a puffy face and edema of the both lower extremities. Hyperparathyroidism with a high level of parathyroid hormone (0.75 ng/ml) was diagnosed. A submucosal tumor was noted at the residual stomach, and histologically, carcinoid was preoperatively confirmed by endoscopic study. Total gastrectomy with radical lymphadenectomy was performed, and concomitantly a small tumor noted at the body of the pancreas was also extirpated. In addition, three and a half of all four parathyroid glands were resected. Histologically, the pancreatic tumor was adenoma, with the parathyroid glands showing only hyperplastic changes. Subsequently, during the postoperative course, Cushing's syndrome was further diagnosed, and the patient is now under follow-up study.

Comparison of reversed-phase and cation-exchange high-performance liquid chromatography for separating closely related peptides: separation of Asp76-human parathyroid hormone (1-84) from Asn76-human parathyroid hormone (1-84).

Cation-exchange high-performance liquid chromatography (CE-HPLC) was compared with ordinary reversed-phase high-performance liquid chromatography (RP-HPLC) for separating closely related peptides. Some synthetic samples of bradykinin and angiotensins, which were homogeneous according to RP-HPLC, were found to be inhomogeneous when analyzed by CE-HPLC. On the other hand, diastereomeric peptides could be separated much more efficiently by RP-HPLC than by CE-HPLC. These results indicated that the purity of synthetic peptides should be checked not only by RP-HPLC but also by ion-exchange HPLC. In the case of human parathyroid hormone (hPTH), baseline separation of Asp76-hPTH from Asn76-hPTH by RP-HPLC was not possible, but was by CE-HPLC. Using this method we confirmed that the Asn residue in hPTH at position 76 could not be converted into the Asp residue under the conditions used to isolate and purify it from human organs.

Discrimination between thorotrast and iodine contrast medium by means of dual-energy CT scanning.

A dual-energy CT scanning method is proposed to discriminate between thorotrast and iodine contrast medium in patients suspected of having thorotrast deposits in the spinal canal as a consequence of myelography injections. The ratio of CT numbers at two X-ray tube voltages was used to distinguish the two materials in the analysis of dual-energy CT scanning. The K-edge of thorium was used to enhance the discrimination sensitivity. Theoretical analysis predicted the feasibility of the method and this was verified by experiment. The sensitivity for the discrimination between thorium and iodine is much higher than that achieved by the gamma-ray counting technique using a whole-body counter. This method, the ratio of CT numbers at two kV values, may be useful in other clinical applications.

[A simple and direct radioimmunoassay for serum and urinary metanephrine].

A simple, direct and specific radioimmunoassay for serum and urinary metanephrine was developed, in which we used 125I-Synephrine and specific antiserum generated in rabbits by injecting with metanephrine conjugated with bovine serum albumin as described by Grota and Brown. The sensitivity of the assay was 2 pg/tube. Intra- and inter-assay coefficients of variation were 2.6 approximately 7.8% and 5.0 approximately 9.6%, respectively. The recoveries of metanephrine added at two levels of 250 pg/ml and 500 pg/ml to ten serum samples and ten urine samples (hydrolyzed and diluted) were 96.7 and 108% in serum, and 94.8 and 103% in urine, respectively. Normal values of serum metanephrine were 40.3 +/- 25.8 pg/ml (mean +/- SD) from 20 normal subjects. Normal values of 24 hour urinary metanephrine excretion were 12.5 +/- 6.7 ug/day from 24 normal subjects. Serum metanephrine values for 7 patients with pheochromocytoma were 210 approximately 628 pg/ml. Urinary metanephrine values for 9 patients with pheochromocytoma 8.7 approximately 302 ug/day.

[Salivary peptide P-C (III). Its relation to the pathogenesis of diabetes mellitus].

Previous immunohistochemical study showed that salivary peptide P-C like immunoreactivity, originally isolated from whole human saliva, was present not only in human salivary glands but also in human pancreatic B-cells. To elucidate the pathophysiological role of this peptide-like immunoreactivity in human pancreatic B-cells, immunohistochemical study using antisera against both insulin and peptide P-C was carried out on the paraffin embedded pancreatic tissues of 27 diabetic patients and 30 control subjects. Positive immunofluorescence due to insulin was detected in 96% of the diabetic pancreases and 100% of the controls. A pancreas of only one IDDM did not have any immunofluorescence due to insulin. Thus, no significant differences were seen in connection with the presence of insulin between pancreases of NIDDM and the controls, though the present study did not examine whether the number of pancreatic B-cells in the pancreas of NIDDM was the same as that in the controls or not. In contrast, positive immunofluorescence due to peptide P-C like immunoreactivity was demonstrated in only 41% of the diabetic pancreases but in 87% of the controls. In view of the fact that the diabetic pancreases and the controls were fixed and embedded into paraffin in the same department of pathology and under the same conditions, the negative finding for salivary peptide P-C like immunoreactive in the diabetic pancreas seems to result neither from the effects of fixatives nor the destructive postmortem effects of enzymes on the antigenicity of peptide P-C like immunoreactivity but from the decreased content of peptide P-C like immunoreactivity in the diabetic pancreas.(ABSTRACT TRUNCATED AT 250 WORDS)

Solution synthesis of [ASN76]-human parathyroid hormone (1-84).

Human parathyroid hormone, hPTH(1-84), was synthesized by the conventional solution procedure applying the maximal-protection approach. All protecting groups were removed simultaneously by the HF method. The product was purified by CM-cellulose column chromatography, gel-filtration on Sephadex G-50 and in the final stage, by reversed phase HPLC. The structure of the final product was confirmed not only by HPLC analysis but also by peptide mapping of tryptic digests on HPLC. The present product showed 350(249-480) IU/mg on in vitro rat renal adenylate cyclase assay.
