Detection of missing vesicoureteral reflux with bethanechol chloride-aided voiding cystourethrography.

Experience in the detection of absent vesicoureteral reflux with bethanechol-aided voiding cystourethrography is presented. The detection rate with conventional contrast-enhanced voiding cystourethrography among 9 study subjects, in whom vesicoureteral reflux otherwise was highly suspected from accompanying clinical, urographic and endoscopic features, and in whom it was absent contralaterally or bilaterally, was improved from 33 per cent (only 3 of 9 patients were positive for reflux) to 100 per cent (all 9 were proved to have vesicoureteral reflux) after bethanechol stimulation. Some representative cases are presented and the mechanisms of inducing vesicoureteral reflux with bethanechol are discussed as well as its clinical relevance. Bethanechol-aided voiding cystourethrography is suggested as a highly sensitive method to detect absent vesicoureteral reflux.

[Experience of the parenchyma saving procedure in the surgical management of renal cell carcinoma].

The result of renal conserving procedures for the surgical management of renal cell carcinoma experienced in 10 patients was reported. All except two were afflicted with tumor involvement in both kidneys (four cases) or in the solitary functioning kidney (four cases). The tumor stage was I in 8 and IV in 2 patients. One of the stage IV patients died of metastatic disease 8 months postoperatively. Of the 9 patients, 8 remained free of cancer in the follow-up period, while one who received an enucleative surgery and whose pseudocapsule of the tumor was missing had a local recurrence. Four of the 5 tumors having no pseudocapsule or having an invasion in the pseudocapsule were grade 2 or 3. The renal conserving surgery is believed to yield an excellent control in low grade and low stage tumors. Partial nephrectomy, rather than simple enucleation, remains to be a preferred surgical treatment in parenchyma-sparing operation, although the enucleative surgery may have a good indication for multiple and central encapsulated tumors.

[Clinical evaluation of cefixime in the treatment of acute uncomplicated cystitis].

Cefixime (CFIX) was administered 200 mg (2 divided doses) per day to 154 female patients with acute uncomplicated cystitis. Ninety four of the 154 patients who satisfied the criteria proposed by the UTI Committee, Japan were examined for the efficacy of the treatment, but 151 of 154 patients were included in the study of the side effects of the drug. The overall clinical efficacy was excellent in 80.8% and moderate in 18.1% of the patients. Ninety five (97.9%) of 97 strains which were isolated from the patients were eradicated in the urinary specimens by the treatment. Subjective side effects were observed in only one (0.7%) of the 151 patients, who complained of a sensation of heaviness in the head. No drug-related aggravation in the laboratory test was observed except for three patients who showed mild elevation glutamic pyruvic transaminase, alkaline phosphatase and blood urea nitrogen. These results showed that the oral administration of CFIX was excellent and satisfactory in the treatment for acute uncomplicated cystitis.

Captopril renal scintigraphy in a patient with bilateral renal artery stenosis.

A 41-yr-old woman with bilateral renal artery stenosis (RAS) and renovascular hypertension is presented. In this patient, the routine [99mTc]diethylenetriaminepentaacetic acid renal scintigraphy without Captopril was normal and the subsequent study with Captopril showed a marked decrease in glomerular filtration rate of the right kidney alone. Percutaneous transluminal angioplasty of the functionally affected right kidney immediately evoked new renin-dependent hypertension caused by the untreated left RAS. This patient illustrates two major points: (a) as a diagnostic tool, the sensitivity of Captopril renal scintigraphy may be compromised by an inability to detect bilateral RAS, but (b) after angioplasty, it can be useful for evaluating the treated kidney and in revealing contralateral disease that may benefit from angioplasty.

Immunologic abnormality in NZB/NZW F1 mice. Thymus-independent occurrence of B cell abnormality and requirement for T cells in the development of autoimmune disease, as evidenced by an analysis of the athymic nude individuals.

Both NZB nu/+ and NZW nu/+ mice were microbially clean by cesarean section. The (NZB x NZW)F1 hybrid (NZB/W) nu/nu mice and nu/+ littermates were then generated by mating of NZB nu/+ with NZW nu/+mice under specific pathogen-free conditions. The female NZB/W F1 nu/nu mice did not develop autoimmune kidney disease, whereas all of nu/+ female littermates mice exhibited proteinuria and died of renal failure with a 50% survival time of 35 wk. Namely, nude mice had no signs of proteinuria up to the time of their death caused by other diseases rather than glomerulonephritis, and their mean survival time was greater than 45 wk. Nude mice had also no anti-ssDNA antibody in their serum. However, splenic B cells of NZB/W nude mice exhibited hyper-responsiveness to both LPS and B151-TRF2, a T cell-derived polyclonal B cell-stimulation factor, and produced large numbers of Ig-secreting cells and anti-TNP plaque-forming cells as well as anti-ssDNA antibody comparable to the nu/+ littermate mice. Interestingly, thymus-engrafted NZB/W nude mice developed autoimmune disease exemplified by the induction of anti-ssDNA antibody and proteinuria at approximately the same time as their nu/+ littermates. These results indicate that the B cell hyper-responsiveness found in NZB/W mice is apparently determined by the T cell-independent process, and T cells are obligatorily required for the development of autoimmune disease in NZB/W mice.

Further experience with one-stage repair of severe hypospadias and scrotal transposition. Modifications in the technique and its result in eight cases.

Further experience with one-stage repair of severe hypospadias and scrotal transposition performed in eight patients is reported. Primary success was obtained in six, while one patient was cured by secondary repair. While describing the operative technique with some modifications it was reassured that our parameatal preputial flap is a well vascularized one which can safely and easily be constructed into a neourethra. Advantages of improved "glanulomeatoplasty" and scrotoplasty are also discussed. The method is recommended as highly successful to attain excellent functional and cosmetic results in one stage for severe hypospadias.

One-stage total repair of severe hypospadias with scrotal transposition: experience in 18 cases.

This is a report detailing further experience with our one-stage repair of severe hypospadias and scrotal transposition performed on 18 patients. Initial success was obtained in 12 cases, while two more were improved by secondary repair. The operative technique, as modified, assures a parameatal preputial flap, which is well vascularized and easily constructed into a neourethra. The advantages of this improved "glanulomeatoplasty" and scrotoplasty are discussed. The method is recommended as an excellent functional and cosmetic procedure for one-stage correction of severe hypospadias.

[The development of newborn C 57 BL/KsJ-dbm mice produced from eggs fertilized in vitro].

The purpose of this study was to examine the development of newly born C57BL/KsJ-dbm mice produced from eggs fertilized in vitro. The embryos derived from fertilization in vitro (which was performed by using db/db eggs and adrenalectomized db/db (Adrex) spermatozoa,) were transferred to the oviduct of MRL/MpJ pseudopregnant recipients 30 hr after insemination. 376 of these embryos yielded 65 young. Weight gain and urine glucose, plasma glucose and insulin levels were measured in these young as well as in Adrex males. The young produced by fertilization in vitro showed hyperglycemia, hyperinsulinemia and obesity. The physiological abnormalities in these young were similar to those in db/db young produced by natural mating between heterozygote (db/+) males and females. Adrex males did not show hyperglycemia but did show hyperinsulinemia. These results indicate that in vitro fertilization and embryo transfer is an effective means of producing fetuses or newborns with an overt genotype in genetically diabetic obese (db) mice.

[Establishment of a new hereditary-cataract mouse (CSM) strain derived from SENCAR mouse].

A mouse representing a new hereditary cataract strain was found in a mouse colony and a new line was established strain CSM. These mice were investigated genetically, histologically and biochemically. The results suggested that this cataract was apparently inherited through two recessive autosomal genes. Histologically the denucleation process of lens fibers was abnormal and small vacuoles appeared in the equatorial region of the lens cortex at 12 days. Biochemically, insoluble protein and sodium increased in the lens with age.

[Clinical experience of enoxacin complicated urinary tract infection].

Enoxacin (ENX) was administered to 69 patients with complicated urinary tract infections (UTI). Clinical efficacy and safety were evaluated by the criteria proposed by the UTI Committee, Japan. The overall clinical efficacy was excellent in 60.9%, moderate in 10.1% and poor in 29% of the patients. Of the 76 strains isolated from the patients 61 strains (80.3%) were eradicated. Subjective side effect was observed in one patient who complained of slight nausea. No drug-related aggravation in the laboratory test was observed. These results showed that ENX was effective and safety for the treatment of complicated urinary tract infection.

[A method for detecting the optimum day for mating during the 4-day estrous cycle in the rat; measuring the value of electrical impedance of the vagina].

A method for detecting the optimum day for mating in the rat was investigated. Cyclic changes of electrical impedance of vagina (EIV) were studied in the rats. EIV indicated high value (over 3,000 omega) only at proestrus, and it was lower (under 3,000 omega) at other stage of estrous cycle. These results apparently indicate that measuring the EIV made to distinguish proestrus from other phases of estrous cycle. The female caged with male showed high copulation rate (88 approximately 96%) when her EIV had been over 3,000 omega.

[Clinical evaluation of ofloxacin in the treatment of acute cystitis].

Ofloxacin (OFLX) was administered to 236 female patients with acute cystitis. Of them 164 patients who satisfied the criteria proposed by the UTI Committee, Japan were examined for the efficacy of the treatment, but all cases were included in the study of the side effects of the drug. In the 65 patients who received 300 mg (3 divided doses) of OFLX per day, the overall clinical efficacy was excellent in 66.2% and moderate in 33.8% of the patients. In the 99 patients who received 600 mg (3 divided doses) of OFLX per day, the overall clinical efficacy was excellent in 79% (P less than 0.076) and moderate in 18.2% of the patients. No failure of the treatment was observed in the two groups. Of 164 bacterial strains isolated from the urine of the patients, 132 strains were identified as E. coli. All bacterial strains were eradicated in the urinary specimens by the treatment. Subjective side effects were observed in 8 (3.4%) of the 236 patients. Gastrointestinal disturbance was complained by 6 patients. No drug-related aggravation in the laboratory test was observed except for one patient who showed mild leukopenia reduced from 3700/mm3 to 2200/mm3. These results showed that the oral administration of OFLX was excellent and satisfactory in the treatment for acute cystitis.
