[Automated direct antimicrobial susceptibility testing of urine specimens].

Two microscopical screening methods have been used to assay the suitability of urine samples for automated direct antimicrobial susceptibility testing by the means of a "MS-2 Microbiology System" apparatus. Gram stained smears showed if specimens were monomicrobic and fresh-mount bacterioscopy gave information about presumptive counts to establish inoculum size. The results obtained by the automated method have been compared with those of the direct disk diffusion method and the classical Stokes method on a single isolated colony. The three systems were reliable and only 7% of specimens gave major discrepancies and 15% minor errors. The influence of each antibiotic drug and of each bacterial species on the discrepancies has been discussed. Direct susceptibility testing by MS-2 seems to be accurate and in the same time so fast (5-6 h vs. 18-24 h) to appear very useful in the rational guide of chemotherapy in the more severe UTI.

[Applications of gas chromatography in the diagnosis of anaerobic infections].

The isolation and identification of anaerobic bacteria by biochemical tests take long time, so it is very useful the gas-chromatography to detect the volatile fatty acids produced from their metabolism. This technique can give rapidly a presumptive diagnosis of anaerobic infection so that it is possible to start immediately a correct antibiotic therapy. In this study we have examined many samples of pus and exudate, and the results have been compared with those obtained from the culture. We have found a good correlation between the detection by gas-chromatography of isobutyric and butyric acids and the isolation in the culture of anaerobic flora, and particularly of Bacteroides fragilis when the gas-chromatography detected isobutyric, butyric and succinic acids.

[Comparison of the use of antibodies in 1975 and 1978 in a regional hospital].

The budget for the total amount of antimicrobial drugs was in 1975 in Parma Hospital 205 millions lire. In 1976 a big increase of 30% was found while in 1978 expenses have been reduced to the 34% of 1975 and th 54% of the 1976 respectively. 2,000 clinical charts of in-patients in the last three months of 1975 and 2,000 of the last three months of 1978 have been examined to show if the reduction in the use of antibiotics was generalized or if some more rational choice had been made. The results showed a decrease of prophylactic use. Tetracyclines and rifampicin were no longer prescribed, and clinicians preferred to employ beta-lactams. Influence of the antibiotic policy to restricting sensitivity reports and of the microbiological consultation, which both started at the end of 1976, is discussed.
