Early diagnosis of Hepatitis B by Dane particle associated DNA polymerase assay.

We have studied prospectively 178 subjects exposed to hepatitis B and 120 haemodialysed patients for the presence of HGs antigen, e antigen and DNA polymerase as well as for anti-HBs and anti-HBc antibodies. The results suggest that the DNA polymerase assay enables us to diagnose hepatitis B earlier than the radioimmunoassay for HGs and that DNA polymerase might be present in the blood in the absence of HBs in cases of confirmed hepatitis B. A posititive correlation between e antigen and DNA polymerase was observed in 83% of the patients on haemodialysis who developed hepatitis B but only in 9% of normal patients developing the same disease.

[Pediatric home care services in Belgium].

In 1970 a home care service for children was started by the committee for public assistance in Brussells with medical supervision from the department of paediatrics of the University. The main features are as follows: - It is exclusively paediatric and its aims are to shorten or avoid hospital admissions, or to provide medico-social support especially for the underpriveledged. - The medical supervision is undertaken by the family doctor (15% of cases) or by the physician in charge of the home care service with the help of specialists in the department of paediatrics in necessary. - Social workers, nurses and physiotherapists look after social problems, give nursing care and appropriate treatment. - The service has its own secretaries.

Partial 17, 20-desmolase and 17 alpha-hydroxylase deficiencies in a 16-year-old boy.

Thirteen plasma steroids as well as ACTH, LH and FSH were measured by specific RIAs under basal and dynamic conditions in a 16-year-old boy (normal external genitalia, 46, XY karyotype) who presented slowness and unachievement of pubertal development. On the delta 4-pathway: basal levels of testosterone and dihydrotestosterone were low- with a normal ratio-, delta 4-androstenedione and 11 beta-hydroxyandrostenedione were in the low normal range. Meanwhile, 17 alpha-hydroxyprogesterone and progesterone levels were markedly elevated. On the delta 5-pathway: dehydroepiandrosterone was extremely low while 17 alpha-hydroxypregnenolone and pregnenolone were almost normal; dehydroepiandrosterone sulfate was subnormal while pregnenolone sulfate was normal. Cortisol, aldosterone were normal while ACTH was moderately increased. Basal and responsive levels of LH and FSH were markedly increased. ACTH stimulation induced a subnormal rise of cortisol and 11 beta-hydroxyandrostenedione, a low or absent rise of dehydroepiandrosterone, 17 alpha-hydroxypregnenolone, androstenedione and 17 alpha-hydroxyprogesterone contrasting with a marked rise of pregnenolone and progesterone. After hCG stimulation, responses were low for testosterone, extremely high for 17 alpha-hydroxyprogesterone with a normalisation of the 17 alpha-hydroxyprogesterone/progesterone ratio. Fluoxymesterone dramatically reduced the pathologically high basal levels of progesterone and 17 alpha hydroxyprogesterone. Dexamethasone induced only a minute decrease in the delta 4-progestagens, a marked decrease in pregnenolone, with a more than 80% reduction of 17 alpha- hydroxypregnenolone, dehydroepiandrosterone, dehydroepiandrosterone sulfate and androstenedione. These data suggest a defect involving the cytochrome P450 common to both 17 alpha-hydroxylase and 17, 20-desmolase activities.(ABSTRACT TRUNCATED AT 250 WORDS)

[Ambulatory pediatrics in the hospital and at home].

In paediatrics, ambulatory care under hospital control is ensured both by the out-patient department and by the emergency unit. If socio-economic level and parental availability are taken into account, the hospital remains the most accessible health-care structure in large towns. The common illnesses, in most cases infectious and respiratory, should not encumber the emergency unit, but should instead be taken in charge by a structure where an extended-timetable dispensary and an out-patient clinic by appointment work in parallel. In the most favourable cases, a home care unit completes this out-patient structure. Its main characteristics are as follows: it is exclusively paediatric, and it aims at shortening or avoiding hospitalization. It improves the quality of treatment, provides the family with medico-social support and follow-up that is particularly useful in less favoured settings, and can also offer preliminary training in heavy or chronic cases. Notwithstanding its obvious advantages, budgetary cuts have led to the suppression of home care at Hôpital Saint-Pierre in 1983.

Interleukin-2 and soluble interleukin-2 receptor levels in children with active pulmonary tuberculosis and atypical mycobacterial adenitis.

Interleukin-2 (IL-2) level was measured in sera and in supernatants of Purified Protein Derivatives of Tuberculin (PPD) stimulated peripheral blood mononuclear cells (PBMC) cultures from children with active primary pulmonary tuberculosis (TB), or adenitis caused by mycobacteria of the group Mycobacterium avium, intracellulare, scrofulaceum (MAIS). The control groups included BCG vaccinated children (BCG) and children with negative skin test to PPD (NST). High mean IL-2 level was exclusively found in sera of mild TB patients (MTB), and not in sera of MAIS infected or BCG vaccinated children. The IL-2 level increased even more in MTB during treatment. In severe TB (STB) the IL-2 level was not elevated before treatment, but increased also during treatment. IL-2 production in supernatants of PPD stimulated PBMC cultures was increased in MTB as well as in MAIS and BCG subjects. Further, soluble IL-2 receptor (sIL-2R) levels were measured in the different groups of children. With the exception of the STB group, there was otherwise no significant increase of the receptor in the sera levels between groups. During treatment the sIL-2R levels decreased in MTB as well as in STB. A slight but non significant augmentation was found in the supernatants of PBMC cultures stimulated with PPD. This work suggests, along with other referable studies, that IL-2 and sIL-2R levels are inversely modulated by the disease. Indeed, the IL-2 seems to increase in MTB comparatively to NST children, and in treated TB comparatively to non treated TB children. On the other hand, the sIL-2R level was found to decrease in TB under treatment.(ABSTRACT TRUNCATED AT 250 WORDS)
