Effects of (DesGly9-Arg8) vasopressin on cognitive processes in alcoholic patients.

The effect of intranasally administered (DesGly9-Arg8) vasopressin (DGAVP) on certain aspects of cognitive functions of alcoholic patients was studied. The investigation was carried out in 103 chronic alcoholic patients. None of the patients suffered from Korsakoff's syndrome, portal encephalopathy or brain damage of other etiology. The trial was double-blind, placebo controlled. Psychological testing, which included certain aspects of attention, short-term and long-term memory and spatial orientation, was carried out on the day before (baseline measurements), on the last day (II), and seven days after the last administration of DGAVP (III). No treatment effect of DGAVP was observed. The results show that investigations with humans are infinitely more complex than those with animals, since a number of physiological and socio-psychological factors must be controlled. In this study of alcoholics two such factors: 1) duration of abstinence and 2) adaptation to test situation were identified.

[Narcissistic depression in schizophrenia].

In individual analytically oriented psychotherapy as a research method, observing a random sample of 34 schizophrenics with the symptoms of depressive syndrome, the author has found that depression in schizophrenia is determined by narcissistic injuries, by a collapse of narcissistic satisfactions through the loss of "ego" functions, completeness, competitiveness, and competence. The depressive model is most frequently encountered in florid schizophrenic psychosis, at the beginning of hospitalization, but depending on the intensity and depth of narcissistic traumas and losses, it can develop also in any phase of the therapeutic process and the course of illness. In the author's view, the depressive model in schizophrenia is not conditioned by the neuroleptic treatment. Its recognition is very important for the therapy of suicidal tendencies, and for their prevention in particular.

[Resistance in the psychotherapy of depression].

Analysing sources, forms, and dynamic significance of resistance in individual analytically orientated psychotherapy applied to a randomly composed sample of 22 patients with a major depressive episode (12 females and 10 males), the author has found that resistance comes from all dynamic systems and that it should be looked upon through the prism of a deep narcissistic regression as reaction to narcissistic traumas and the concept of depressive disturbances as the interaction between genetic-biochemical, psychological, and sociological factors. Forms of resistance and their functions are described in more detail. The dynamic significance of resistance lies in the defence of the narcissistic injuries of the vulnerable self and a further disorganization of mental functions. The nature and dynamic importance of resistance in depressive patients determine the kind of therapeutic interventions which essentially differ from procedures in the psychotherapy of neuroses.

[Aggression in depression].

In the random sample of 18 patients (10 female and 8 male) with the symptoms of the major depressive episode, V/296, DSM-III the author examines the questions of the aetiology, aim, form and mechanisms or aggression by the psychoanalytically oriented individual psychotherapy. The author finds that the aetiology of aggression in patients with depression arises not only from the biological basis but also from the dynamic psychopathological constellations of the personality structure and the configuration of the unconscious. The aggression aim in these patients is the protection of the fulfillment of the primary narcissism pathological needs. Among other things the author finds that their narcissism looks for cosmic spaces and that it breaks under the laws of the natural constants and relativity, whereas the loss of space and the flow of time have the meaning of the sequestrations of their self and the cognition of the crushing defeat. Their basic mechanism is masochistically sadistic. The psychotherapy of these patients requires big efforts, the control of the countertransfers and the adaptation of techniques.

Aggression

[Analysis of conflict in the psychotherapy of schizophrenia].

Considering schizophrenia as the result of the deficiency of ego functions and existing conflicts, the author proposes a psychotherapeutic method based on a constructed integral psychotherapeutic model which departs from the classical or orthodox psychoanalytical approach but unites the principle of the structuring and the principle of the analysis of the conflicts at hand. By the use of this psychotherapeutic method, in a sample of 20 schizophrenics, he achieved better and quicker therapeutic results regarding the increased ego capacity, fuller remissions, and significantly more successful functioning in reality than in the control group of the same size treated with neuroleptics alone. In this integral psychotherapeutic method the author sees possibilities for a scientific investigation of mechanisms and regularities in schizophrenic processes and phenomena and emphasizes the need for its development.
