The same HLA-DQ alleles determine either susceptibility or resistance to different coxsackievirus-mediated autoimmune diseases.

An important characteristic of autoimmune diseases is their association with major histocompatibility complex class I and class II alleles. In this study, we compared insulin-dependent diabetes mellitus (IDDM) with idiopathic dilated cardiomyopathy (IDC) from a strictly immunologic perspective. Although the target organs are different, being in one case the insulin-producing beta cells of the pancreas and in the other case the myocytes of the heart, many aspects of the tissue-specific immune destruction are common. Similar yet different Coxsackievirus B strains with either pancreotropic or cardiotropic specificity are able to perpetrate the first injury of the respective target tissue. Their shared capacity of inducing a superantigenic reaction further enhances the damage. Once previously secluded autoantigens are then exposed to the immune system, the tissue injury is completed via a more conventional type of immune response. On the basis of the compounded results we obtained, it is possible to propose that the same HLA-DQ molecules which are able to protect the individuals from IDDM (e.g., HLA-DQA1*0102, DQB1*0602) seem to favour the enteroviral attack to the myocardium, while alleles which confer the strongest susceptibility to IDDM (e.g., DQA1*0301, DQB1*0302), seem unable to sustain the immune attack against the heart.

The level and the persistence of islet cell antibodies in healthy schoolchildren are associated with polymorphic residues of the HLA-DQ beta chain.

Cytoplasmic islet cell antibodies (ICA) were determined in a group of non-diabetic Caucasian schoolchildren (n = 4208). The prevalence rate for ICA positivity was 1.05 per cent (95 per cent confidence interval: 0.8-1.4 per cent). Analysis of HLA risk factors revealed that HLA-DRB1*03 (p less than 0.01), HLA-DRB1*04 (p less than 0.01) and HLA haplotypes with non-charged amino acids (non-Asp) at codon 57 of the HLA-DQ beta (p less than 0.01) chain were significantly increased when compared to controls. High levels of islet cell antibodies, i.e. Juvenile Diabetes Federation units (JDF units) equal to or greater than 30 JDF units were found to be associated with amino acids other than aspartic acid at codon 57 of the DQ beta chain molecule. Also the persistence of circulating ICA was found to be associated with non-Asp homozygosity of the proband (p less than 0.03).

Piroxicam-beta-cyclodextrin in induced migraine attacks: a SPECT study with Tc-99m HM-PAO split-dose method.

To verify the effect of piroxicam-beta-cyclodextrin (P-B-CD), a novel non-steroidal anti-inflammatory compound, on regional cerebral blood flow (rCBF) during migraine attacks, 7 female patients (mean age: 37.4 years), suffering from migraine without aura, were studied. The patients, after induction of a migraine-like attack with sublingual nitroglycerin (NTG), underwent a rCBF evaluation using Single Photon Emission Computerized Tomography (SPECT) and Tc-99m examethyl-propylene-amino-oxyma split-dose method before and after oral administration of P-B-CD (20 mg). In addition, a spontaneous migraine attack and NTG test in 7 control subjects were investigated. All patients observed relief of pain, evaluated by direct and indirect parameters (Pain Intensity; Pain Intensity Difference; Sum of Pain Intensity Difference). SPECT revealed a uniform and symmetrical distribution of the radiocompound in 5 out of 8 patients, while in the remaining 3 symmetrical parietal or parieto-occipital areas of decreased tracer uptake were detected. In these cases, the intake of P-B-CD cancelled out the cerebral perfusion abnormality.

A case of chronic cluster-like headache in a patient with cerebrovascular disease.

The case of a cerebrovascular patient is presented, who at the age of 70 developed unilateral headache, localized in the left orbital region with radiation to the temporo-parietal homolateral region; the attacks lasted 15 to 60 min, presented a chronic temporal pattern from their onset and were accompanied by homolateral lacrimation, conjunctival injection, nasal stuffiness and ptosis of the eyelid. The headache also presented a circadian timing with more frequent occurrence in the evening or early morning h. Verapamil therapy was effective. The possible relevance of cerebrovascular disease with respect to the late onset of the headache is discussed.
