Complete familial Currarino triad in association with Hirschsprung's disease: magnetic resonance imaging features and the spectrum of anorectal malformations.

Currarino syndrome, a rare hereditary condition, is defined as a partial sacral agenesis associated with a presacral mass and anorectal malformation. The authors present two siblings with complete Currarino triad and their mother with incomplete triad. The complete Currarino triad in the older sibling was associated with Hirschsprung's disease as the second reported case in the literature. Anorectal malformations, whether suspected of Currarino syndrome or not, should be examined with lumbosacral magnetic resonance imaging.

Rapid susceptibility test for Mycobacterium tuberculosis to isoniazid and rifampin with resazurin method in screw-cap tubes.

Multi-drug resistant (MDR) Mycobacterium tuberculosis is still a serious public health problem all over the world. MDR tuberculosis (MDR-TB) caused by these strains has emerged within the last decade and rapid detection is critical for the effective treatment of patients. Recently, a resazurin microtiter assay plate for detecting MDR strains was developed. In this study, it was adapted to screw-cap tubes and the activity of isoniazid (INH) and rifampin (RIF) to 50 M. tuberculosis clinical isolates was tested by this method for the first time. Results were compared with the radiometric reference method for the susceptibility testing of M. tuberculosis complex. The results of both methods were in 100% and 96% agreement for RIF and INH, respectively. Specificity, sensitivity, positive predictive value and negative predictive value were 91.7%, 100%, 92.8% and 100% for INH, respectively. All of these values were 100% for RIF. Susceptibility testing results were obtained on the 8th day of incubation for 42 isolates and on the 9th day for the other eight strains. Our results indicate that this method is suitable for the early determination of INH and RIF resistance in developing countries because it is inexpensive, rapid and easy to perform.

Blood agar for susceptibility testing of Mycobacterium tuberculosis against first-line drugs.

OBJECTIVE: To evaluate the performance of blood agar for the susceptibility testing of 50 Mycobacterium tuberculosis clinical isolates against isoniazid (INH), rifampicin (RMP), streptomycin (SM) and ethambutol (EMB). DESIGN: The activity of the drugs was determined by the proportion method on blood agar instead of Middlebrook 7H10 agar according to Clinical Laboratory Standard Institute recommendations. The final concentrations of INH, RMP, SM and EMB were 0.2 microg/ml, 1 microg/ ml, 2 microg/ml and 5 microg/ml, respectively. RESULTS: The results were compared with the radiometric proportion method as the reference, and the agreements were determined as 100% for INH and RMP, 92% for SM and 96% for EMB. The specificity, sensitivity, positive predictive value and negative predictive value were 90.4% and 97.5%, 100% and 90%, 66.6% and 90% and 100% and 97.5% for SM and EMB, respectively, while these values were 100% for INH and RMP. The results of susceptibility testing were obtained on the 14th day of incubation. CONCLUSION: According to this preliminary study, our results suggest that blood agar can be used as an alternative medium for the susceptibility testing of M. tuberculosis strains against INH, RMP, SM and EMB in resource-limited countries. However, further studies are needed before implementating the method in diagnostic laboratories.

Imaging meningiomas: is there a need for post-contrast FLAIR?

AIM: The aim of this study was to compare post-contrast fluid-attenuated inversion recovery (FLAIR) imaging with post-contrast T1-weighted images (T1WI) in depicting meningiomas. MATERIALS AND METHODS: Twenty-nine patients with 46 meningiomas were included in this study. FLAIR and T1WI were obtained before and after intravenous administration of gadopentetate dimeglumine. The contrast enhancement degree, contrast enhancement pattern, lesion conspicuity, and the detection of the dural sign were compared between post-contrast FLAIR images and post-contrast T1WI. RESULTS: The enhencement degree on FLAIR was equal or less than T1WI for all meningiomas. Among 46 meningiomas 38 (83%) enhanced homogeneously and eight (17%) inhomogeneously on T1WI. On contrast-enhanced FLAIR images, of the total 46 meningiomas 22 (48%) enhanced homogeneously, eight (17%) inhomogeneously, whereas 14 (30%) meningiomas showed a peripheral rim enhancement not observed on T1WI. Two (5%) meningiomas showed no contrast enhancement on post-contrast FLAIR images. Among the 14 meningiomas showing rim enhancement using FLAIR imaging, 12 (85%) were measured to be 2 cm or more in diameter. A dural tail sign was found in 16 (35%) and 23 (50%) meningiomas on post-contrast T1WI and FLAIR images, respectively. CONCLUSION: In contrast to other extra-axial diseases, post-contrast FLAIR sequence was not found to be a valuable adjunct to contrast-enhanced T1WI in the depiction of meningiomas.

Effects of ram introduction after the second prostaglandin F2 alpha injection on day 11 on the LH surge characteristics in fat-tailed ewes.

The aim of this study was to evaluate the effects of ram introduction after the second prostaglandin F2alpha (PG F2alpha) injection on day 11 on the secretion characteristics of pre-ovulatory LH surge of fat-tailed ewes. Multiparous Morkaraman ewes (n=12) were divided into three groups by balancing the groups for liveweight (BW) and body condition score (BCS). On the day of second PGF2 alpha injection (0 h), performance tested rams (n=2) were either introduced to the ewes at 0 h (ram 0 group, n=4) or at 18 h (ram 18 group, n=4) or were not introduced (control group, n=4). Blood samples were collected at 6, 18, 42, 48, 56, 62, 66, 70, 74, 78 and 90 h for the determination of pre-ovulatory LH surge. BCS and BW during the experimental period were 2.2 +/- 0.2 units and 50.9 +/- 2.3 kg, 2.4 +/- 0.4 units and 49.2 +/- 6.2 kg, 2.1 +/- 0.3 units and 45.9 +/- 4.4 kg, respectively for the ram 0, ram 18 and control groups (p > 0.05). No significant difference was observed in LH surge characteristics for the experimental groups. Peak LH concentrations were also not different between groups (p > 0.05) and they were 12.2 +/- 8.3, 29.1 +/- 9.9 and 15.8 +/- 9.5 microg/l for the ram 0, ram 18 and control groups, respectively. There was, however, a significant correlation between peak LH concentrations and BCS (p < 0.05, R2=0.373). In conclusion, it appears that, compared with ram introduction, variability in body condition of the ewe has much pronounced effect on the amount of LH secreted after the usage of two PGF2 alpha injections (11 days apart) as a tool for oestrus synchronization.

Progesterone profiles of fat-tailed tuj ewes following tactile and visual separation of rams at the beginning of the breeding season.

The aim of this study was to determine the level and duration of progesterone secretion during the sequential oestrous cycles in fat-tailed Tuj ewes following tactile and visual separation of rams during the breeding season. For these purposes, rams were separated from the main flock for 50 days starting from the beginning of the breeding season and 21 ewes were randomly selected for the current experiment. In order to assess luteal activity and the length of oestrous cycles, the ewes were blood-sampled thrice or twice weekly for the measurement of progesterone in plasma. The data showed that 3 +/- 0.2 oestrous cycles were observed in this time window, and the first oestrous cycle observed was shorter (P < 0.05) than the following cycles (mean +/- SEM, 14.6 +/- 0.82, 16.5 +/- 0.48 and 17.0 +/- 0.54 days, respectively, for cycles 1, 2 and 3). Progesterone production was significantly lower in the first oestrous cycle compared with the second and third cycles on days 9, 12 and 14 of the cycles. The follicular periods of ewes showed four accumulations (maximum values using a 3-day moving average technique) throughout the study and the percentages of ewes at each accumulation (peak day +/- 1 day) were 50, 35, 65 and 80% for the first, second, third and fourth accumulations, respectively (P < 0.05). It was concluded that progesterone production was lower and the oestrous cycles were shorter during the first oestrous cycle and that tactile and visual separation of rams at the beginning of the breeding season might enhance the synchronizing effect towards the end of the breeding season in the fat-tailed Tuj ewes.

Effect of medetomidine-butorphanol-ketamine anaesthesia and atipamezole on heart and respiratory rate and cloacal temperature of domestic pigeons.

The purpose of this study was to evaluate the sedative-anaesthetic effects of a combination of medetomidine (M, 50 microg per pigeon), butorphanol (B, 50 microg per pigeon) and ketamine (K, 25 mg per pigeon) in domestic pigeons. Eight domestic pigeons (four male and four female, 8-15 months old) were used. The combination of Medetomidine and butorphanol injectable solutions were used to produce sedation. Ten minutes after M + B administration, K was injected. The anaesthetic effects of the drugs were reversed by administration of Atipamazole (AT) at 60 min after K administration. All drugs were injected into the pectoral muscles. The sedative-anaesthetic effects of the M + B-K combination and, alterations in respiratory rate (RR), heart rate (HR), electrocardiographic (ECG) findings and cloacal temperature (CT) were investigated before and 10 min after pre-medication with M + B, at 5, 15, 30, 45 and 60 min during the onset of K anaesthesia and at 1, 5, 10, 20, 30 and 60 min following the administration of AT. The HR and RR of pigeons decreased within 10 min following M + B administration and remained lower until 1st and 5 min of AT injection, respectively. In ECG, no significant alterations in P, Q, R and S-values were observed, however, arhythmia was recorded for three pigeons, which returned to normal values following AT administration throughout the measurement. Cloacal temperature decreased gradually during the anaesthesia from 41.0 to 32.7 degrees C. The drug combination used in this study produced a satisfactory general anaesthesia for seven of the eight pigeons. All pigeons were unconscious within 5 min after K administration as indicated by disappearance of the palpebral and corneal reflexes and lack of reaction to the pain stimuli during the study. The effect of AT administration was observed within 10 min as all pigeons responded partly against stimuli and all reflexes. It is concluded that M + B-K anaesthesia in pigeons is a safe and reliable anaesthetic protocol for surgery.

Investigation of cross-resistance between rifampin and rifabutin in Mycobacterium tuberculosis complex strains.

In order to indicate the cross-resistance between rifampin (RMP) and rifabutin (RBU), minimal inhibitory concentrations (MICs) of RBU were investigated in 50 Mycobacterium tuberculosis strains. The MIC values of 25 RMP-susceptible (to 2 microg/ml) and 25 RMP-resistant (to 2 microg/ml) M. tuberculosis strains against RBU were determined by the Bactec TB 460 system. All of the RMP-susceptible strains were also susceptible to RBU (MIC < or = 1 microg/ml). Three out of 25 (12%) RMP-resistant strains were determined as susceptible to RBU. The high level cross-resistance (88%) obtained in this study highlights the importance of testing susceptibility to RBU prior to its inclusion in the tuberculosis treatment regimens at the Istanbul Faculty of Medicine.

Analysis of heart rate variability in children with primary nocturnal enuresis.

Although nocturnal enuresis is probably the most common developmental disorder in children, the pathogenesis and management remain unclear. Autonomic dysfunction is one of the proposed mechanisms for nocturnal enuresis in children. The objective of current study was to evaluate autonomic nervous system functions in enuretic children. Twenty-four-hour ambulatory electrocardiographic recordings were obtained, and the time domain variables of HRV were calculated. The results of the present study suggest that sympathetic nervous system hyperactivity is present in enuretic children. This may explain why some enuretic children do not respond to anticholinergic medications. If these conflicting results are confirmed by large-scale clinical studies, Holter ECG examinations may be used for rational approaches in treatment of nocturnal enuresis.

[Eight polymorphic blood protein systems in Arab horses from Turkey].

Analysis of the blood protein system was used to study the genetic composition of Arabian horses. Biochemical markers of eight polymorphic loci (Tf, Al, Es, AlB, Gc, Hb, PGD, and PGM) were electrophoretically identified in blood samples. A total of 43 phenotypes were identified for these polymorphic systems. The Tf, Hb, and Es loci appeared to be more polymorphic than the other loci studied. Statistically significant differences between the observed and expected genotypic frequencies were found for the PGD and PGM loci (P < 0.05 and P < 0.01, respectively). Individual allele frequencies, observed and expected phenotype frequencies, and the average heterozygosity were estimated for each polymorphic locus.

Effects of octreotide in patients with hypertrophic obstructive cardiomyopathy.

Hypertrophic obstructive cardiomyopathy (HOCM), the cause of which is unknown, is a heart disease characterized by obstruction of the left ventricular outflow tract and an increase in interventricular septum thickness. Octreotide, a synthetic analogue of somatostatin, was administered subcutaneously to 15 patients for 6 months in order to determine its efficacy in HOCM. Echocardiographic examination was performed in each patient before we had initiated treatment and after treatment. Interventricular septum thickness, interventricular septum thickness/left ventricular posterior wall thickness, and subaortic gradient decreased significantly at the end of treatment. The ratio of the mitral valve E to A waves increased significantly. We observed that octreotide treatment caused a significant decrease in interventricular septum thickness and subaortic pressure gradient. Before and after therapy left ventricular enddiastolic diameter, left ventricular endsystolic diameter, ejection fraction and fractional shortening were not changed. No adverse effect was observed during the therapy. According to our results, octreotide has some beneficial effects on HOCM and it seems to be a new therapeutic approach for HOCM.
