[Specific features of performing anesthesia and artificial circulation in cardiosurgical interventions during pregnancy].

The authors summed up the main principles of performing anesthesia and artificial blood circulation in cardiosurgical interventions in 5 women with 20-27 week pregnancy. In pregnant women with progressing cardiac failure due to a heart disease, thrombosis of the cardiac valve prosthesis, acute infectious endocarditis with the altered valve, large size tumor of the heart with disturbed valve function an emergency operation must be performed under conditions of artificial circulation. Operative treatment is the only effective method for saving life of the mother and fetus. The proposed complex preoperative preparation, intra- and postoperative management of the patients are thought to be an effective method of supporting operations on the heart under conditions of artificial blood circulation in pregnant women. In order to prevent possible complications antioxidants should be included in the complex of anesthesiologic means. During operations with artificial circulation in pregnant women it is necessary to strive for performing normothermal perfusions with high volume velocities.

[Artificial pneumothorax in the combined therapy of destructive pulmonary tuberculosis].

44 patients with destructive pulmonary tuberculosis received chemotherapy (intermittent antibacterial drugs, i.v. isoniazid) and artificial pneumothopax (AP). 42 patients had infiltrative disease (destructive in 15, disseminated in 27 cases). Discharge of the bacilli occurred in 95.4%. AP application was followed by thoracocautery in 3-4 weeks to form appropriate collapse of the affected lung portions. Short adhesions were burnt, plane ones were extrapleurally detached according to the technique developed in the Novosibirsk Research Institute of Tuberculosis. A complete correction of pneumothorax was achieved in 43 patients. Neither intra- nor postoperative complications were reported. The discharge stopped in 100%, the caverns closed in 93.1% of the cases.

[Analysis of lethality in tuberculosis].

The analysis of tuberculosis lethal outcomes occurring in a large antituberculous hospital in 1990-1992 showed that: mean age of the dead made up 49.7 +/- 0.8 years, 70.9% of them did not work (46.3% were invalids due to tuberculosis). 32% of the deceased were observed by the specialist no longer than a year, 1/6 were dead within treatment month 1.68% of the above patients were admitted to the hospital in grave condition. Intensive chemotherapy for advanced destructive tuberculosis with bacterial discharge was given to the majority of the patients. 57.4% of the patients did not survive a month of hospital stay, 24.3% were dead within the first week in hospital. 84.6% of the lethal outcomes resulted from progression of pulmonary tuberculosis.

[Pregnancy and labor in women in the region of the accident at the Chernobyl Atomic Electric Power Station].

The authors analyze the tentative results of a multiprofile study, including a review of labor histories, examinations of pregnant women and parturients, measurements of fetoplacental hormones, study of the immunity status and microbiocenosis of the mothers and newborns, living in the first (up to 15 Ci/km2) and second (15 to 40 Ci/km2) zones of radioactive contamination. The detected shifts of a compensatory nature were found mostly in women living in the first zone. The disorders found in the women living in the second zone evidenced a decrease of the defense potential of the body, this necessitating nonspecific and correcting therapy during pregnancy, in labor and the postpartum period.

[Characteristics of central and peripheral hemodynamics in infants with hemolytic disease of newborn].

Overall 82 full-term neonates (52 children with hemolytic disease and 30 healthy neonates) were examined. The general circulation was investigated over time (on days 1, 3.5-7 of life). Use was made of echocardiography and monitor studies of arterial pressure and heart rate; lability of arterial pressure and general peripheral vascular resistance were computed. As a result of the studies and correlations, it has been revealed that neonates with hemolytic disease manifest appreciable disorders of the cardiovascular system, correlating with the disease gravity. Earlier times of the closure of fetal communications were established as were the hyperkinetic type of +cardiac hemodynamics, arterial hypotension accompanied by arterial pressure instability and a decrease of the general peripheral vascular resistance which should be taken into consideration in choosing therapeutic measures.

[The immune status of nonpregnant and pregnant women living constantly under ionizing radiation exposure conditions].

A total of 455 women, both pregnant and nonpregnant ones, permanently living in zones I and II of ionizing irradiation, were examined in 13 months after the Chernobyl Power Plant accident. Monocyte phagocytic activity and humoral immunity parameters were under study. An imbalance of the immunity parameters were characteristic of all the examinees living in zone I: they presented with disordered phagocytic activity of the monocytes, elevated levels of circulating R proteins, reduced production of natural and specific antibodies, changed concentrations of the principal immunoglobulin classes. The immune status of women living in the zone II was found lowered at the expense of reduced phagocytic activity of the monocytes, production of natural antibodies and immunoglobulins, shifted to either sides R protein levels, this necessitating immunocorrective therapy before pregnancy onset.

[Means of lowering the perinatal mortality in pregnant women with diabetes mellitus].

The paper gives some current approaches to the management of pregnant women with diabetes mellitus, in terms of perinatal medicine. These are based on a comprehensive detailed clinical examination of 104 patients, which involves the hormonal fetoplacental complex function, hypophyseo-thyroidal system, hemocoagulation system by using ultrasonic tools (fetal biometry and echocardiography, Doppler echometric studies of the fetoplacental blood flow in the umbilical vein, umbilical artery, and fetal aorta). A differential approach was proposed to the management of pregnancy and delivery of patients with diabetes mellitus, which allows perinatal mortality to be reduced.

[Possibility of predicting intrauterine fetal growth retardation by a single ultrasonic examination (using varying standards)].

The paper deals with early prediction of fetal growth retardation and its severity in a newborn from single ultrasound fetal biometric findings (biparietal head size, chest and belly diameters) at week 20 of pregnancy. The prediction was made by employing the developed varying standards for these parameters as percentile curves and tables. A stepwise prediction of fetal growth retardation was proposed for obstetric in- and outpatient settings, which was presented as an IBM personal computer dialogue program. The positive diagnostic value of fetal growth retardation prediction was found to be 69.7%, its negative value was 86.9%. The paper discusses whether therapeutic measures and pregnancy length affect the efficiency of its prediction and stresses that the prediction is valuable for individual well-grounded tactics for pregnancy management.

[Chronic placental insufficiency: incidence and causes].

This population study has examined the incidence and determinants of placental insufficiency. Predictors of placental dysfunction were ascertained. They included somatic diseases, gestational complications, a positive obstetric and gynecologic history and a series of constitutional factors. Populations of primiparae+- and multigravidae at risk of placental insufficiency were identified.
