[Regenerative medicine with adult stem cells].

The present state of clinical regenerative medicine with adult stem cells in the cardiology, digestive, corneal and neurological fields are reviewed. From the cardiology point of view, there is clinical experience with bone marrow stem cells and peripheral blood cells and with skeletal myoblasts. At present, the adult stem cells (bone marrow hematopoietic or mesenchymal) constitute the best option for the regeneration of heart tissue, the clinical studies showing favorable results without ethical or safety problems. Most of the studies with skeletal myoblasts have also been demonstrated to significantly contribute to improve heart function, above all, the systolic one. However they have the disadvantage that has not been totally clarified that they induce malignant ventricular arrhythmias. In either case, the clinical studies are in the initial phase and new studies, above all randomized, are necessary. In the digestive field, there is the pioneer experience of the Hospital La Paz on the use of stem cells from abdominal fat in the treatment of fistulous condition of patients with Crohn's disease. In ophthalmology, the limbal corneal transplant is a recognized practice, using cells from the contralateral eye when the damage is in a single eye and cells from a donor when the damage is bilateral. Finally, in the neurological field, different zones of the adult mammal brain where there are stem cells have been identified: the hippocampus, subventricular zone, olfactory bulb and periependymal zone of the spinal cord. On the other hand, neurons may be obtained from adult stem cells from other tissues, such as the bone marrow or adipose tissue, which means a practically unendable source of neural precursors, either by direct implant after their selection or after their in vitro culture. However, most of the experimentation is animal up to now, clinical trails on safety in amyotrophic lateral sclerosis are now being initiated.

[P. aeruginosa bacteremias: analysis of prognostic factors. A prospective study, 1992-1998].

INTRODUCTION: P. aeruginosa causes serious infections with high mortality. OBJECTIVES: Identify the prognostic factors associated with higher mortality in P. aeruginosa bacteremias (PAB). METHODS: 211 consecutive cases of PAB were analyzed prospectively between 1992-1998. Blood cultures, isolation, and antimicrobial sensitivity were carried out according to microbiology standard methodology. The variables analyzed as prognostic factors were: sex, age, source of infection, background, main disease, initial clinical severity, foci, presence of complications, leukocyte count, type of antibiotic treatment and adaptation. Bivariate and multivariate statistical analyses were carried out by the method of logistic regression. RESULTS: Global mortality was 27.96%; factors associated with higher mortality in the bivariate study were main disease rapidly and eventually fatal, diabetes, a situation of critical initial clinical severity, lung focus, complications, neutropenia and inadequate antibiotic treatment. The logistic regression study, the critical initial clinical severity, and the presence of complications were the variables associated with worse prognosis. We did not find significant differences in the evolution among the patients who received monotherapy and those on combinations of antimicrobial drugs against pseudomonas. CONCLUSION: Higher mortality of PAB is statistically associated to the situation of critical initial clinical severity and to the presence of complications; therefore, an early diagnosis and adequate treatment to improve the morbidity and mortality are recommended.

Congenital diverticulum of the right ventricle associated with coarctation of aorta, atrial and ventricular septal defect and ductus.

Congenital right ventricular muscular diverticula are extremely rare and are usually associated with other congenital cardiac anomalies, (in half of the cases tetralogy of Fallot). They functionally behave like an accessory ventricular chamber which contracts synchronously with the normal ventricles. Less than 30 patients with a right ventricular diverticulum have been reported in literature. An apical right ventricular diverticulum occurs in patients with thoraco-abdominal midline defects or abnormalities of the cardiac position([1]). However, an antero-superior diverticulum is usually associated with other congenital cardiac defects, such as a ventricular septal defect, tetralogy of Fallot, double outlet right ventricle and pulmonary stenosis([2--9]). We report an 11-year-old boy with an antero-superior diverticulum of the right ventricle associated with a coarctation of aorta, ductus arteriosus, and atrial and ventricular septum defects. To the best of our knowledge, such an association has not been reported before.

[Usefulness of predictors of angiographic restenosis to predict clinical restenosis after coronary stent placement].

INTRODUCTION: After coronary stenting, several predictors of angiographic in-stent restenosis have been identified in different studies, however, little is known about predictors of clinical restenosis, a more functional aspect of coronary restenosis. AIM: To assess whether risk factors for angiographic restenosis previously described, are able to predict clinical restenosis and at what rate in current practice. PATIENTS AND METHODS: 216 consecutive patients (271 stents in 256 lesions) with procedural success were followed-up for 17.6 +/-10 months during periodic visits. Clinical restenosis was defined as the presence of symptoms or signs of myocardial ischemia, associated with >= 50% diameter stenosis on the angiogram. RESULTS: Clinical restenosis occurred in 33 lesions (13%), which were revascularized with 34 stents associated with unstable angina in 29, acute myocardial infarction in three and death in one case. Multivariate analysis identified as independent predictors of clinical restenosis, a vessel diameter less than 3 mm (p < 0.001, OR 4.5), a restenotic lesion (p = 0.01, OR 2.9) and the presence of residual stenosis by visual estimate (> 0%) after implantation (p = 0. 02, OR 2.5). These three risk factors explained most clinical restenosis (73%), with rates of 22% when at least one was present and 4% in absence of all these. The presence of diabetes mellitus, the location in the anterior descending coronary artery or at coronary ostium, and the number or total length of stents per lesion did not achieve an independent, significant association as predictors of clinical restenosis. CONCLUSIONS: Most clinical restenosis after coronary stenting can be predicted by the restenotic character of the revascularized lesion, the diameter of the vessel being less than 3 mm and the presence of residual stenosis by visual estimate at the end of procedure.

[Continuous practice and advanced training in interventional cardiology. Recommendations for the assessment and maintenance of proficiency in interventional cardiology. A statement for physicians and advanced training units from the Section of Hemodynamics and Interventional Cardiology of the Spanish Society of Cardiology].

This report reflects the interest of the Section of Hemodynamics and Interventional Cardiology of the Spanish Society of Cardiology in increasing quality, safety and applicability of percutaneous procedures, by giving scientific keys aimed at improving related functions of teaching or planning in this field and enhancing competence and prestige of Spanish interventional cardiologists. The purpose of the document is to describe the importance of current interventional cardiology, to identify quality references and to establish minimum acceptable requirements for assessing and maintaining the competence of practicing or providing advanced training in this discipline. To achieve this goal, a search for a gold standard of the different techniques of general interventional practice was carried out, and predictors of postprocedural outcome were analyzed, as well as their relation with different kinds of circumstances. This analysis identified coronary angioplasty as the standard on which recommendations regarding competence in overall interventional cardiology standards of quality and assessment and maintenance of proficiency must be based. On the other hand, the strong influence of experience and knowledge of results has been documented, especially in high-risk or high-complexity settings. On this basis, the report establishes specific recommendations about proficiency for practice and advanced training. It also suggests that interventional cardiology should be considered as a subspecialty, of cardiology requiring specific credentials.

[Effect of obesity on the morphology of the left ventricle].

BACKGROUND: To analyze the relationship between obesity in its different degrees and the left ventricle morphology. PATIENTS AND METHODS: M-mode echocardiography was used to estimate the mass, wall thickness and internal dimension of left ventricle in 48 obese women with different degrees of obesity, defined according to the body mass index. 25 women with normal weight were used as controls. RESULTS: The body mass index was correlated with left ventricular mass, as well as with both the wall thickness of the left ventricle and its diastolic internal dimension. The abnormalities in the heart morphology increased according to the obesity degree, ranging from a 59% in the lesser obesity group up to a 100% in the more obese women. The incidence of the left ventricular hypertrophy determined by echocardiography also increased along with the body mass index, ranging from a 29% in the lesser degree of obesity women up to an 82% in the patients with a body mass index > 35 kg/m2. CONCLUSIONS: Obesity, even in its lowest degrees, shows frequent alterations in the heart morphology. This is related with a left ventricular mass increase and a higher incidence of the left ventricular hypertrophy. The left ventricular mass increase is due to an increase in the left ventricular walls thickness and also to a dilatation of its cavity.

[Effect of the location of myocardial infarction on the variability of heart rate: a study during the acute phase].

OBJECTIVES: The influence of the location of acute myocardial infarction on the autonomic tone and its evolution during the first hours post-infarct has not been fully evaluated. The aim of this study was to analyze this effect using a spectral analysis of the heart rate variability. PATIENTS AND METHODS: Forty-nine consecutive patients with acute myocardial infarction (22 anterior and 27 inferior) in sinus rhythm and free of diseases and drugs which could affect heart rate variability were studied. Five-minute Holter recordings within each hour between 10 and 33 hours after the onset of symptoms were analyzed, calculating the standard deviation of NN intervals and the spectral power of the high and low frequency bands using normalized units. RESULTS: The standard deviation was higher in inferior infarcts (51.4 +/- 23.4 ms vs. 38.6 +/- 14.8 ms in anterior location; p < 0.05) and gradually decreased over time in both locations. The relative distribution of high- and low-frequency bands did not show significant differences related to the infarct location. An inverse significant correlation between the high-frequency component and time was observed for anterior infarcts (r = -0.98; p < 0.001) as well as in the inferior group (r = -0.75; p = 0.04). Conversely, the low-frequency power gradually increased in anterior infarcts (r = 0.98, p < 0.001) while remaining stable in inferior locations (r = -0.08; NS). CONCLUSIONS: A gradual reduction of heart rate variability was observed in patients with acute myocardial infarction during the time of monitorization. The spectral analysis suggests that anterior infarcts present a progressive increase of sympathetic activity and a reduction of vagal tone, whereas inferior infarcts show a parallel reduction in both components of the autonomous nervous system.

[Retrospective study of auricular fibrillation of recent onset in a hospital emergency service: frequency, clinical presentation, and predictive factors of early conversion to sinus rhythm].

AIM: To evaluate the proportion of emergencies due to recent-onset atrial fibrillation (AF), its clinical characteristics and in-hospital follow-up. The clinical predictors of conversion to sinus rhythm within the first 24 hours were analyzed. PATIENTS AND METHODS: 34,445 consecutive reports from patients presenting themselves at the emergency room of a community hospital during 15 months were retrospectively studied. The clinical reports of all patients (n = 186) with symptoms of recent onset AF (< 15 days) were reviewed. RESULTS: Hypertension (n = 77.41%) and lone AF (52 patients, 28%) were the most common etiologies. Forty seven patients (25%) presented with heart failure and the onset time was > 24 hours in 77 cases (41%). Conversion to sinus rhythm was observed in 71 out of 166 patients with at least 24 hours of follow-up (42.8%). Age < 60 years, the absence of cardiac disease, a NYHA functional class I, the absence of heart failure at the emergency room and the time from onset < 24 hours were significantly associated with conversion to sinus rhythm. The last two variables were selected as independent predictors by logistic regression analysis (sensitivity: 80%, specificity: 68%). CONCLUSIONS: We conclude that recent-onset AF represents 0.54% of all the hospital emergencies. The time from onset and the presence of heart failure predict the probability of conversion to sinus rhythm within the first 24 hours.

[Recurrent syncope without angina: an uncommon presentation of coronary spasm].

We report the case of a 64-year-old patient admitted for repetitive syncope as an isolated clinical manifestation probably due to coronary artery spasm. The patient had no history of previous cardiac disease, and was studied because of two nonspecific syncopes. Long-term electrocardiographic monitoring showed many episodes of transient ST segment elevation, associated with premature ventricular beats and runs of ventricular tachycardia. Coronary angiography during ergonovine infusion was performed to confirm the diagnosis. We discuss the incidence of coronary spasm provoking syncope and the need to establish a correct diagnosis in order to administer an effective therapy to the patient.

[The spectral analysis of the P wave in patients with paroxysmal atrial fibrillation].

INTRODUCTION AND OBJECTIVE: The aim of this study is to establish if patients with paroxysmal atrial fibrillation have an abnormal spectrum of the P wave. PATIENTS AND METHODS: Thirty-five patients with previous documented atrial fibrillation were compared with a control group of 29 patients. A signal-averaged ECG was performed using an ART-1200-EPX system, and a segment covering the last 75 ms of the P wave and the next 25 mg was analyzed in each of three orthogonal leads (X, Y, Z) and in a combined one (C). The area under the spectral curve between 20 and 50 Hz was divided by the area between 0 and 20 Hz (x 100 = AR50). RESULTS: Patients with paroxysmal atrial fibrillation had greater AR50 on leads Y (78.3 +/- 42.9 vs 54.4 +/- 14.8; p < 0.01) and C (82.2 +/- 52.1 vs 58.4 +/- 14.6, p < 0.05) when compared with the control group. In the subset of patients without structural heart disease AR50 in lead X was greater. The criterion "AR50 > 55 in lead Y" identified paroxysmal atrial fibrillation with a sensitivity of 77.1% and a specificity of 85%. CONCLUSIONS: This method may be useful to detect patients at risk for paroxysmal atrial fibrillation.

[Disorders secondary to iodine deficiency in the autonomous community of the Murcia area].

Hypothesis. To know goiter prevalence, iodine excretion and disorders associated to iodine deficiency in the region of Murcia, with the aim to define the need for establishing iodine prophylaxis programs. Method. Transversal study on 1956 pupils of the region of Murcia, with an age between four and seventeen years old, stratified by sex, age and area of origin, evaluating thyroid size, grade, weight, height, school performance and problems, menarche age, fish and iodine salt intake and iodine excretion. Results. Global prevalence of goiter was 29%, we found a mean ioduria of 93.92 +/- 56.83 micrograms/l. Anthropometric mean values for each age group did not differ between pupils affected by the lack of iodine and the others. A lower school performance was found in pupils with iodine deficit. Fish intake did not play a role in goiter prevalence. Conclusions. Murcia is an area affected by disorders derived from mild iodine deficiency, which implies the need to establish a continuous plan of effective prophylaxis with iodine salt.
