Effects of collateral blood vessel ligation and electrical conditioning on blood flow in dog latissimus dorsi muscle.

Utilization of skeletal muscle as a myocardial substitute requires it to undergo two major modifications: mobilization to the site of action and adaptation to continuous activity. We have examined the effects of collateral blood vessel ligation, which would accompany mobilization, on blood flow in control and electrically conditioned canine latissimus dorsi (LD) muscle. Blood flows were measured at rest and during a vigorous isometric fatigue test. In 22 control muscles, electrical stimulation during the fatigue test resulted in a sevenfold increase in muscle blood flow (0.26 +/- 0.18 ml/g/min at rest, 1.69 +/- 0.84 ml/g/min during stimulation). No difference was detected in flow to distal and proximal portions of the muscle. In three muscles where collateral vessels were ligated immediately before measurement of blood flow, flow in the proximal portion of the muscle was not significantly different from control, but in the distal portion, stimulation failed to elicit an increase in flow (0.12 +/- 0.13 ml/g/min at rest, 0.16 +/- 0.07 ml/g/min during stimulation). In animals allowed a 3-week recovery period following collateral vessel ligation, stimulation-induced increases in blood flow were detected but remained lower than control. Muscles which had been conditioned by continuous electrical stimulation for 6-7 weeks at 2 or 10 Hz generated less peak isometric tension than controls (peak tension = 4.5 +/- 1.7 kg control, 2.4 +/- 0.7 kg following 2 Hz conditioning, 1.6 +/- 0.4 kg following 10 Hz conditioning). Indium-111 leukocyte scintigraphic detection of subclinical osteomyelitis complicating delayed and nonunion long bone fractures: a prospective study.

Twenty patients were studied prospectively with indium-labeled leukocyte imaging to evaluate its effectiveness in differentiating noninfected delayed or nonunion from osteomyelitis complicating these entities. All patients underwent an open surgical procedure within 24 h of the scan. Bone specimens from the nonunion site were obtained for microbiological and histological analysis to confirm the presence or absence of osteomyelitis. In these twenty patients, the sensitivity of the indium scintigraphy was 100%, the specificity 100%, and the overall accuracy 100%. Indium-labeled leukocyte scintigraphy is significantly more accurate than 99mtechnetium and 67gallium imaging had been, when studied earlier, in detecting subclinical osteomyelitis complicating nonunion. Indium-labeled leukocyte scintigraphy should supplant sequential technetium and gallium studies in this patient population when the surgeon must determine whether subclinical osteomyelitis is complicating fracture management of delayed and nonunions.

Thyroid imaging with high-field-strength surface-coil MR.

Magnetic resonance (MR) imaging of the thyroid was performed with a 1.5-T system and local receiver coil in 19 "healthy" subjects and 34 patients with various focal and diffuse thyroid disorders. The normal gland was typically homogeneous with increased intensity relative to that of muscle on images obtained with long repetition times (TRs) and long echo times (TEs). Adjacent structures in the neck and upper mediastinum were well displayed. Thyroid nodules as small as 4-5 mm were identified. Follicular adenomas appeared as well-circumscribed nodules of heterogeneous intensity, increasing significantly in signal with long TRs/TEs. Colloid cysts and hemorrhagic cysts had homogeneous high signal with both short and long TRs/TEs. Two of three carcinomas were seen as poorly marginated lesions with associated cervical lymphadenopathy clearly depicted as increased intensity with long TRs/TEs. A follicular adenoma containing microscopic papillary carcinoma appeared similar to other benign adenomatoid nodules. A functioning nodule was isointense with normal gland at all pulse sequences. Characteristic patterns of diffuse abnormality were observed in cases of multinodular goiter, Hashimoto thyroiditis, and Graves disease, although additional cases are required to determine specificity. High-field-strength surface-coil MR imaging appears to be a sensitive method for identifying gross morphology of focal, multinodular, and diffuse disorders of the thyroid and involvement of surrounding structures in the neck.

Oxygen consumption of chronically stimulated skeletal muscle.

The latissimus dorsi muscles of six dogs were made fatigue resistant by chronic electrical conditioning. Once the muscles were conditioned, oxygen consumption was measured during periods of exercise. The ratio of the tension developed to oxygen consumed during moderate stimulation (300 msec on) for the control and the electrically conditioned muscles was 16.3 +/- 3.5 and 36.5 +/- 6.7 kg-sec/ml oxygen, respectively. During intense stimulation (800 msec on) the ratio was 12.6 +/- 2.1 and 54.2 +/- 8.9 kg-sec/ml oxygen, respectively. Thus the conditioned muscle was able to develop and maintain tension with a considerably reduced oxygen expenditure. The increased efficiency of the conditioned muscle helps to explain its increased resistance to fatigue and the ability of pumping chambers constructed from electrically preconditioned skeletal muscle to perform sustained cardiac type work.

Multiple-hospital survey of ejection-fraction variability using a cardiac phantom.

A dynamic cardiac phantom was used to provide identical input data at 11 different nuclear medicine laboratories throughout the Philadelphia area, and the variability in the resulting calculations of ejection fraction (EF) was assessed. The variability observed between different operators using the same computer system averaged 3 EF units, which was similar to that between different observers using different types of computers. In the range of low ejection fractions, however, there was a suggestion that EFs calculated with an MDS computer were slightly lower than those from a DEC computer.

Wilms' tumor: diagnostic therapeutic implications.

Twenty-four cases of Wilms' tumor were analyzed. The gray-scale echographic characteristics of the tumors were categorized. Correlation with pathologic characteristics, clinical presentation, prognosis, and therapy response was assessed. Two echographic patterns emerged: (a) hypoechoic solid--3; and (b) hyperechoic solid--21. Necrotic degeneration as well as decrease in tumor size were echographic features correlated with positive response to therapy. The results of this analysis showed no initial correlation between the echographic patterns and the clinical presentation or prognosis. On the other hand, the echographic features of Wilms' tumor seem distinctive enough to help in etiologic differentiation and therapy management.
