A study of the psychological status and sexuality in middle-aged Bulgarian women: significance of the hormone replacement therapy (HRT).

OBJECTIVE: The psychoemotional and sexual status of Bulgarian urban postmenopausal women 10 years after the beginning of the period of extensive political, social, economic and cultural transitions was studied. SUBJECTS AND METHODS: Three hundred and thirty-two postmenopausal women (83.5% in natural menopause and 16.5% in surgical menopause) and 295 women with normal menstrual cycle in several Bulgarian cities and the capital were studied. A campaign for free-of-charge bone density measurement was announced and the examined subjects filled in an individual anonymous questionnaire. SPSS for Windows version 8.0 was used for the statistical processing of the results. The comparison was made between normally menstruating women (n=295), postmenopausal women on hormone replacement therapy (HRT) (n=36) and postmenopausal subjects without HRT (n=296). RESULTS: A significant correlation was observed between depression and sexual aversion, depression and pain with intercourse, depression and the change in the sexual life after menopause. The correlation between self-esteem and sexual thoughts, self-esteem and sexual desire, self-esteem and the arousal from the partner, self-esteem and orgasm also proved important. About 20% of the studied subjects reported depressive thoughts, 50% reported anxiety and the self-esteem was very low in 13%. These results can be explained in part with the economic stagnation in the country, the unemployment and uncertainty. However, 94% of Bulgarian women believe that they do well in life and rely on their professionalism and social positions. All parameters of sexual life are significantly lower in the postmenopausal group without HRT as compared both with menstruating women and postmenopausal women on HRT. Marital status influences them too. CONCLUSION: The change in the sexuality is related to the decline in the estrogen activity, the influence of the psychosocial factors, the lack of a permanent intimate partner, and the derogatory attitude of the society towards the sexuality of postmenopausal women.

Various statistical methods in use for evaluating human malignant gastric specimens.

This paper presents the use of certain statistical methods (comparison of means - independent samples t-test, multiple linear regression analysis, multiple logistic regression analysis, analysis of clusters, etc.) included in the SPSS Statistical Package used to classify the patients quantitatively evaluated after a subtotal resection of their stomachs. The group consisted of 40 patients subdivided into two groups: primary neoplasia of the stomach (20 patients), and corresponding lymphogenic deposits in the abdominal perigastric lymph nodes (20 patients). Paraffin-embedded tissue sections (thickness 4-5 microm) prepared as consecutive hematoxylin-eosin-stained slides were morphometrically measured by a rotation of a graduated eyepiece-micrometer; thus, we obtained the minor and major axes' lengths of the elliptic nuclear profiles and the minor and major caliper diameters of the corresponding cellular profiles. These four variables were used to determine the dynamic changes in quantitative features of human gastric lesions when passing from normal histological structures, through hyperplastic processes (chronic gastritis), gastric precancer (ulcers and polyps with or without malignancy) till the development of primary carcinomas and their corresponding lymphogeneous metastases. Besides the increased cytomorphometrical measures, we also noted an opportunity to classify the patients according to these data as well as to add to the knowledge of our consultation system for clinical aid and use, recently published in the literature.

[Attempts at suicide among girls].

Suicides represent one of the main medico-social problems in Bulgaria. Every day three lives are lost because of suicide in our country. The authors present information on the attempted suicide among young girls from an epidemiological study, provided in 1995. It was found out that the leading motives for suicidal behaviour were the conflicts in the family between the girls and their parents. These conflicts induce fear from punishment, getting sometimes to mishandling. Another part of the suicide attempts were directed to draw their parent's attention, even to bring together separated parents. The frequent cases of premature sexual connections and of sexual misure are discussed. Attention is turned to the group suicide attempts among girl-friends and sister. The authors draw inferences on the necessity of directing the primary prevention among the young girls toward the achievement of mental stability, self-recognition and building up social competence.

Personality dimensions of opiate addicts.

A survey of 80 opiate addicts included in a detoxification program was conducted at the Institute on Addictions in Belgrade. In addition to a dependence diagnosis and mental disorders based on DSM-III-R, we applied a Tridimensional Personality Questionnaire (TPQ) that measures the 3 major personality dimensions: novelty-seeking (NS), harm avoidance (HA) and reward dependence (RD). When compared with a control group (a sample of Yugoslav undergraduate students), the opiate addicts demonstrate significantly high NS dimension as well as significant divergences of HA and RD subscales. The surveyed opiate addicts demonstrate a high percentage of personality disorders specifically in cluster B. The personality dimensions of opiate addicts showed certain temperament traits, such as: impulsiveness, shyness with strangers, fear of uncertainty and dependence. NS, HA and RD determined by temperament specifics may be an etiological factor in forming of a personality disorder, an affective disorder as well as of a drug choice.

[Experience in using cluster analysis and the prognosis of the stages of epizootic risk for Newcastle disease in poultry].

An attempt was made to use clastic analysis and projection of the steps of the epizootic hazard with regard to Newcastle disease. There were no records of the disease after 1982 but still it is the object of consideration on the part of specialists. A clastic analysis was made on the base of a factorial model of Newcastle disease in this country for the 1970-1979. Used was a programme already worked out and practised at the Pilot Computor Center of the Medical Academy, Sofia. The districts in the country were grouped in a dendogramme, 16 groups being distinguished out of a total of 27 districts. This showed that regardless of the small territory of the country the districts are sufficiently differing between each other (due to the various degrees of integration) so that they could not be grouped together by similar values of intensity of poultry breeding and epizootic conjuncture with regard to Newcastle disease. Districts of epizootic hazard ranging from 1st to 5th degree substantiate the use of a differential approach in building up the tactics of prophylaxis and control of the disease, while measures concerning the poultry dressing combines should be at a high level regardless of the category of the respective district. The grouping of neighboring districts on a territorial principle disclosed the existence of reasons of climatic-and-geographic nature that could predispose equal degrees of intensity of poultry breeding and epizootic hazard.

[Attempt to design a factorial model of Newcastle disease in poultry in Bulgaria for the period 1970-1979].

Literature data on the statistical methods relating to many objects (informational, factorial, clusteral, regressional, etc.) in the study of epizootic processes have shown that these methods could be employed to raise the quality of the epizootiologic analysis. An attempt has been made to build a factorial model for Newcastle disease in this country. Use has been made of a programme for factorial analysis, substantiated by a computer. The information is presented in the form of a matrix, containing 27 rows (the number of districts) and 15 columns (the number of indices). The latter include parameters of the epizootic processes as well as indices, characterizing the economic status of the individual districts. The factorial model has made it possible to state the correlations of a great number of measurable indices and through these to disclose 4 basic factors that cannot be measured directly which may largely contribute to the elucidation of the epizootic status. The concept has been confirmed that the character of the epizootic process with the Newcastle disease within the size of concrete territorial-and-administrative units (the districts) is determined to a great extent by the economic features of the districts. The programme used in the factorial analysis of Newcastle disease can be applied also to investigate the epizootic process with other infectious diseases of farm animals and birds along with the use of a computer and available information.

[Transsexualism and sex change].

The article deals with the psychosexually disturbed identity known as transsexualism. The medical practice is occupied with this problem and phenomenon since the sixth decennium of this century. However, in this country it was not present at that time. In Serbia the treatment of transsexualism began at the beginning of 1989, and there have not yet been published articles on this problem. This paper is the first article dealing with transsexualism in Serbia. It is based on personal authors' experience. The essential characteristic of transsexualism is the existence of such psychical grasp and conduct which do not correspond to bodily sexual characteristics. The difference is so great that the patient must force himself (herself) to accept the existing psychical properties or inborn bodily particularities which do not harmonize. Therefore the patient decides to take psychial things as they are but to alter his (her) body. Psychiatry cannot alter his (her) soul but surgery and endocrinology can alter his (her) body. In that case the patients body is first shaped with hormonal therapy according to his (her) wishes (female or male), and then genital organs are appropriately formed by surgery. The popular nema of this procedure is sex reassignment. The whole problem and the therapeutic procedure are considered as hermaphroditism. In true hermaphroditism ovarian or testicular tissue is present in the same patient; in false hermaphroditism female or male organs predominate; and in transsexualism only one way of alteration is possible i.e. bodily sexual alteration. The latter is the most complex form and physicians finally realized that spiritual feelings can be the property of one sex and bodily characteristics of the other.(ABSTRACT TRUNCATED AT 250 WORDS)
