Symptomatic wandering accessory spleen in the pelvis: sonographic findings.

We describe the case of a mobile left lower quadrant mass associated with recurrent abdominal pain; at surgical exploration, the mass was found to be an accessory pelvic spleen. Although accessory spleens are present in 10-30% of individuals, a wandering accessory spleen located in the pelvis is not frequently seen. On sonography, the mass in our patient appeared well-defined and homogeneous. Spectral analysis and color Doppler imaging demonstrated a normal vascular branching pattern and high diastolic flow due to low resistance in the vascular bed. The parenchymal resistance index of the mass was similar to that of the native spleen.

Primary stenting for complex atherosclerotic plaques in aortic and iliac stenoses.

PURPOSE: To evaluate the efficacy of primary stenting for complex atherosclerotic plaques in aortic and iliac stenoses that are not amenable to balloon angioplasty alone. METHODS: Nineteen patients with complex atherosclerotic plaques were treated with a Palmaz stent (n = 19), Wallstent (n = 1), Strecker stent (n = 1), or Memotherm stent (n = 1). A total of 22 stenoses presenting with complex plaque morphology including ulcerated plaques, ulcerated plaques with focal aneurysms, plaques with heavy calcification, severely eccentric plaques, plaques with overhanging edge, and plaques with spontaneous dissection were stented. The lesions were in the aorta (n = 1), common iliac artery (n = 19), or external iliac artery (n = 2). RESULTS: Immediate angiography after stent placement revealed restoration of patency of the stented segment. Focal aneurysms and ulcerated areas were occluded in the follow-up angiographies obtained 4-12 weeks after the procedure. In one case with poor distal runoff and multiple complex lesions of the iliac artery, subacute occlusion occurred. Clinical and angiographic follow-up (3-46 months) revealed patency of all other stented segments. CONCLUSION: Primary stenting is an effective and reliable approach for complex plaques in stenoses. Patency of the arterial segment with a smooth lumen can be created without the risk of acute complications such as distal embolization, dissection, or occlusion.

Neonatal thermoregulation.

The human being is a homeotherm. Homeothermy is a result of thermoregulation which includes many physiological processes. Thermoregulation maintains an equilibrium between heat production (thermogenesis) and heat loss (thermolysis). There are three principal modes of heat production: 1. Voluntary muscle activity. 2. Involuntary tonic or rhythmic muscle activity known as "shivering". 3. Non-shivering thermogenesis (NST) essential for newborns. Heat loss occurs in two stages: 1. The flow of heat from the center of the body to its surface. 2. The flow of heat from the body surface to the environment by conduction, convection, radiation or water evaporation. Even in the very small premature baby, we find that metabolic and vasomotor control responses are developed. To protect the newborn from stress resulting from hypo or hyperthermia, one should take into consideration the concept of the neutral temperature range which is also called the "Thermoneutral Zone" in (TNZ) or "Thermal Neutrality". Curves, proposed in 1971 by Hey are essential for keeping newborns in the TNZ.

[Progressive multifocal leukoencephalopathy in patients with AIDS. Findings with computerized tomography and magnetic resonance].

Progressive Multifocal Leukoencephalopathy (PML) is a subacute demyelinating opportunistic infection of the central nervous system which frequently occurs in AIDS. CT and MR, together with clinical and virological findings, can suggest a correct diagnosis in most cases, thus avoiding stereotactic biopsy which is too invasive considering the lack of therapy and the poor prognosis of this disease. In this study we reviewed the CT and MR findings of 16 proved AIDS-related PML cases. PML lesions appeared as hypodense on CT, hypointense on T1w, hyperintense on PDw and T2w MR images. CT was less sensitive than T2w MR images and underestimated the number of lesions and/or disease extent. On the basis of our findings during the progression of the lesions we observed two different patterns of PML presentation and evolution i.e., "single" and "multifocal". Single lesions generally involve subcortical white matter (arcuate fibers) of parietal lobe and spread to the contralateral hemisphere across the corpus callosum; multiple "patchy" lesions can be localized variably in the cerebral hemispheres and also in the brainstem and cerebellar hemispheres.

Should an echocardiographic scan be done routinely for infants of diabetic mothers?

Several cardiologic pathologies are seen in infants of diabetic mothers (IDMs). Though asymmetrical septal hypertrophy (ASH) is a frequent pathology in IDMs, it is not routinely searched for with an echocardiographic scan. We have performed an echocardiographic examination for all IDMs (56 neonates) hospitalized between January 1987 and December 1992 in our neonatology and neonatal reanimation units. Of 56 patients, the diagnosis of 17 cases of ASH 930%) was made. The group with ASH (17 neonates) had a greater corporeal index than the group without ASH (39 neonates) (p < 0.05). Four of the 17 IDMs (24%) with ASH and one of the 39 IDMs (3%) without ASH presented with a cardiac insufficiency (p < 0.05). ASH is a pathology which should be searched for routinely IDMs.

Intraventricular hemorrhage in preterm newborns.

Intraventricular hemorrhage (IVH) originating from germinal matrix is the major brain injury of the premature. We studied IVH incidence and risk factors which are implied in this pathology in newborns, hospitalized in the neonatal intensive care unit (NICU) of istanbul University Cerrahpaşa Faculty of Medicine. Ninety-seven premature newborns with a birth weight of less than 2500 g were examined systematically with cranial ultrasonography (CU). Mean birthweight was 1540 +/- 430 g (720-2450) and gestational age 31.6 +/- 2.4 weeks (26-37). IVH was diagnosed in 40 cases (41%). The significant risk factors are prematurity (gestational age < 33 weeks), artificial ventilation and infusion of fresh frozen plasma.
