Metal sensitivity in patients with a hinge arthroplasty of the knee.

Fifty patients who had received a hinge arthroplasty of the knee were investigated for possible metal sensitivity. Patients were patch tested against all the metal constituents of the prosthesis. Positive patch tests were found in 32% of patients. Seventeen patients had either lossening or a persistent sterile discharge from the knee. No correlation was found between these complications and metal sensitivity. It was concluded that metal sensitivity is probably not a primary factor in the pathogenesis of complications, particularly loosening.

alpha 1-antitrypsin phenotypes in fibrosing alveolitis and rheumatoid arthritis.

alpha 1-antitrypsin (alpha 1-A.T.) phenotypes were determined in 55 patients with rheumatoid arthritis (R.A.), 33 patients with R.A. and either obstructive airways disease or recurrent chest infections, 49 patients with fibrosing alveolitis (F.A.), 22 patients with R.A. and F.A., and 200 healthy controls. A highly significant increase in the frequency of MZ phenotype was found among patients with F.A., both with and without R.A. Patients with R.A. alone had a normal distribution of phenotypes. Inherited modification of immune function may predispose to F.A. Alternatively, lowered levels of alpha 1-A.T. associated with non-M phenotypes may predispose to tissue damage with subsequent fibrosis.

Salmonella typhimurium arthritis in rheumatoid disease.

Septic arthritis is a well recognized complication of rheumatoid arthritis (British Medical Journal, 1976; Mitchell et al., 1976), particularly after joint replacement (Freeman, 1976). We report here infection with an unusual organism--Salmonella typhimurium.

Studies in man of phenytoin absorption and its implications.

The absorption of phenytoin was studied in man. It is concluded that phenytoin absorbed from the intestine is recirculated via the bile, so that blood levels do not accurately reflect absorption. Phenytoin is loosely bound to serum proteins and is found in red cells in concentrations similar to those in plasma. It is rapidly lost from the blood stream after intravenous administration, which is an important factor to be considered in the treatment of status epilepticus.

An assessment of penicillamine therapy in rheumatoid arthritis and the influence of previous gold therapy.

One hundred and fourteen patients with rheumatoid arthritis who had received treatment with penicillamine are reviewed. Side effects occurred in 50 patients. Sixty patients were considered to be improved with the drug. Low and high maintenance regimes were compared. There was no significant difference in the incidence of side effects or clinical improvement. Previous gold therapy did not influence the therapeutic response or total incidence of side effects observed with penicillamine. Rashes were, however, more common if patients had previously had treatment with gold.
