Sleep patterns, attention span, and infant temperament.

Infant temperament ratings and parental reports of sleep patterns and sleep durations were obtained on 105 normal infants. Boys with more difficult temperaments and active sleep patterns had brief attention spans compared to the other children (p = 0.005). Among all infants, active sleep pattern was associated with maleness and increased activity ratings when awake (both p less than 0.05). Infants with easy temperaments slept longer than those with difficult temperaments (14.9 +/- 0.2 versus 12.8 +/- 0.5 hours, p less than 0.001). Gender, maternal prenatal smoking, method of feeding, and infant sleep position did not affect attention span ratings or sleep durations.

Signs of airway obstruction during sleep and behavioral, developmental, and academic problems.

This case control study of healthy children utilized parents' reports to determine whether behavioral, developmental, or academic problems were associated with signs of partial airway obstruction (snoring, difficult or labored breathing, and mouth breathing) during sleep. Results suggest an association between the signs of airway obstruction during sleep and problems when awake. Children with behavioral, developmental, or academic problems had a significantly later bedtime hour, briefer duration of total sleep, longer night awakenings, and an increased latency to sleep, as compared to children without those problems.

Chinese-American infant temperament and sleep duration: an ethnic comparison.

Parents' responses to the Carey Infant Temperament Questionnaire and results of a standard interview regarding sleep duration were obtained for 23 Chinese-American and 60 non-Chinese infants. Chinese-American infants were rated as significantly more withdrawing, less adaptable, and more negative in mood than the other infants. Total sleep duration for the Chinese-American infants was 13.2 hours, compared to 14.2 hours for the other infants (p less than .05). Night sleep duration had a significant negative correlation with mood ratings in the Chinese-American babies. These findings suggest that there are ethnic differences in infant temperament and that the relationship between negative mood and brief sleep is present among different ethnic groups.

Infantile colic and near-miss sudden infant death syndrome.

Frequent awakenings occur in infantile colic acid sleep-related respiratory control disorders. To evaluate the possibility that colic may be associated with impaired respiratory control, prevalence and clinical features of colic among infants who died from Sudden Infant Death Syndrome (SIDS), infants with a clinical diagnosis of Near-Miss Sudden Infant Death Syndrome (N-M SIDS), and a control group were retrospectively determined. Colic did not occur in any of the 17 infants in the control group who had normal ventilatory responses. Colic did occur in 14 (33%) of 42 infants with N-M SIDS who had impaired ventilatory responses. Ventilatory responses among the N-M SIDS infants were similar in those with and without colic. Four (27%) of the 15 SIDS victims had colic, but the spells had disappeared before their deaths. The proportion of infants with colic among the N-M SIDS infants is significantly greater than among control infants (p less than 0.05). In summary, colic related night wakings may represent an alternative and protective behavioral arousal mechanism in N-M SIDS infants, a group in which a variety of sleep-related respiratory control deficits have previously been documented.

Report of an infant with GM1 gangliosidosis type I and extensive and unusual mongolian spots.

An infant with GM1 gangliosidosis was found to have an eruption at birth consisting of extensive and unusual slate blue macules resembling mongolian spots. All areas of skin were involved except face, scalp, palms, and soles. A biopsy of a macule obtained at 5 months of age demonstrated melanocytic cells in the dermis consistent with monogolian spot but also a perivascular histiocytic infiltrate. At 8 months of age, absence of beta-galactosidase activity was documented in both peripheral leukocytes and skin fibroblasts confirming the diagnosis of GM1 gangliosidosis. The dermal histiocytic cells noted on skin biopsy were interpreted as a manifestation of this storage disease. The coexistence of the hyperpigmented lesions and the heritable enzyme defect was believed to be coincidental.

Gastroesophageal reflux. A review.

Gastroesophageal reflux (GER) may produce vomiting, failure to gain weight, esophagitis, heartburn, or pulmonary symptoms. Medical or surgical management depends on the severity of the symptoms. Newer diagnostic procedures such as esophageal manometry, esophageal pH probes, and nuclear scans may confirm the presence of GER. The mechanism by which GER produces different symptoms is not well understood.
