[Intraoperative electrocardiographic control of the position of the cardiac part of the ventriculoatrial valve catheter].

The author reports 86 cases treated surgically at the Department of Neurosurgery, Medical Academy in Warsaw in the years 1974--1977 for hydrocephalus of different aetiology, who had Pudenz and Hakim valves implanted and in whom the position of the intracardiac part of the device was checked by ECG during the operation. The method included filling of the cardiac portion of the device with 10% saline and connecting it to the ECG on which the ECG curve was recorded continaually during insertion of the catheter into the venous system. Obtaining of P wave in the record corresponding in height to the QRS complex was a reliable indicator of the end part of the catheter reaching the middle part of the right atrium. The described method is technically simple and no complications have been observed as yet connected with it.

[Case of giant aneurysm of the basilar artery].

The authors report a case of a giant aneurysm of the basilar artery of the brain diagnosed accidentally on autopsy. Slowly progressive syndrome of bulbar involvement began after craniocerebral trauma. Of considerable interest was the misleading result of pneumoencephalography which showed a normal pattern of basal cisterns. In the light of results of positive-contrast ventriculography an inoperable expanding lesion of the anterior part of the brain stem with associated hydrocephalus was diagnosed and symptomatic treatment was started inserting a Pudenz-Heyer valve. Despite normalization of intracranial pressure the patient died. Vertebral angiography would not have permitted the aneurysm to be diagnosed intravitally since its lumen was filled with clots.

[Value of axial computerized tomography for the evaluation of the effectiveness of the ventriculo-atrial shunt].

The authors describe 32 cases of hydrocephalus of different aetiology in which the effectiveness of the ventriculoatrial shunt was evaluated by means of repeated computerized axial tomographic (CT) investigations. This investigation made it possible to demonstrate in a way not exposing the patients to any complications that the shunt was effective in 87.4% of cases with reduction of the size of the ventricular system of significant and moderate degree. Simultaneously with reduction of the size of the ventricular system an improvement of the clinical state was observed in these patients with disappearance of the signs of increased intracranial pressure. It was observed that in cases of normotensive hydrocephalus presence of cortical atrophy in CT investigation made the prognosis unfavourable despite treatment with the shunt. The analysis of the presented material demonstrated a high usefulness of CT in the observation of hydrocephalus dynamics in cases of different aetiology treated by means of the shunt.

[Intravascular embolization by the so-called balloonization method in the treatment of cerebral arteriovenous malformations].

The authors describe the method of baloonization used in the treatment of intracranial vascular anomalies. The method was used in the treatment of 7 cases with these anomalies in a period from 1979 up to now. For the procedure patients were qualified after angiography. In 4 cases this investigation showed presence of carotid-cavernous fistula which developed in 3 cases after trauma and in one cases spontaneously. In another 3 patients arteriovenous angioma was diagnosed, in 2 of them the malformation was situation in the cerebral hemisphere and in one in the coverings of the skull. In only one cases the general condition of the patient was severe before the procedure. In 5 cases the procedure was performed under local anaesthesia puncturing directly the common carotid artery, while in two other cases the carotid artery was exposed surgically. Catheters with baloons used for the procedure were made in the centre where the authors were working. Early therapeutic results were estimated on the basis of angiography performed after the end of the procedure. In 4 patients with carotid-cavernous fistulas the abnormal communication was closed with baloons filled with autopolymerising substances, in 2 of these cases the blood flow through the internal carotid artery to the brain was preserved after the procedure. In the group of patients with arteriovenous angiomas cure was obtained in one case (reported), improvement in another, and the third patient remained unimproved. Late results were estimated after 2 - 8 months of follow up. The condition of the patient was evaluated by control outpatient examination which showed that in 6 cases it was satisfactory and in one unchanged. The method is simple and effective, especially in cases of carotid-cavernous fistula, and safe since no complications were observed. The method should be regarded as treatment of choice when the usual surgical intervention is dangerous.

Mechanical performance and coronary flow adjustments to changes in workload are not affected by inhibiting nitric oxide production in isolated working rat heart.

The importance of NO-induced vasodilator tone in maintaining adequate coronary flow to sustain hemodynamic function in aerobic heart and the role of endothelial release of NO in reactive hyperemia was studied in isolated working and Langendorff perfused rat heart, respectively. Raising the preload from 0 to 20 cm H2O caused a linear increase in coronary flow, myocardial oxygen consumption (MVO2) and cardiac output in the untreated working hearts and in the hearts in which basal release of NO has been inhibited by NG-monomethyl L-arginine (L-NMMA, 200 microM). L-NMMA, but not L-arginine, reduced coronary flow by 48% and 21.5% at preload levels of 0 and 20 cm H2O, respectively. A steeper coronary flow/preload relation and an increased O2 extraction in L-NMMA treated hearts allowed for the fact that their MVO2, lactate release and cardiac output were not significantly different from those in the untreated hearts at any of the preload level tested. SIN-1, 10 microM, augmented coronary flow to 136% and 107% at preload levels of 0 and 20 cm H2O, respectively, indicating that coronary vascular reserve was preserved at all preloads tested. L-NMMA-treatment greatly reduced reactive hyperemia following 30 s occlusion period and it significantly increased hyperemia following 10 min occlusion period. These findings provide evidence that while the inhibition of NO synthesis reduces coronary flow it does not induce a state of ischemia in isolated rat heart.

[Case of false meningeal hematoma in a patient with meningioma of the cerebral convexity].

The authors presented a case of meningioma of cerebral convexity which produced in cerebral angiography findings resembling meningeal haematoma (false meningeal haematoma). On the basis of a survey of literature the authors discuss other intracranial pathological processes which may give rise to similar angiographic changes. They believe that the possibility of false meningeal haematoma should be taken into account in the choice of surgical treatment.
