Agenesis of the celiac trunk: an angiographic case.

In the angiography performed for a clinical diagnosis, a unique case of "agenesis of the celiac trunk" was encountered in a 42-year-old Turkish male. There was instead an artery which arose from the aorta at the level of the first lumbar vertebra, and supplied blood to the territory of both the celiac and superior mesenteric arteries by giving rise to the splenic, the jejunal, the ileal, the pancreaticoduodenal, the proper hepatic and the left gastric arteries, consecutively.

Unusual branching and course of the testicular artery.

In a Turkish male, the right testicular artery was found to originate from the aorta at nearly the same level as and anterior to the renal artery. This testicular artery was seen to give off the inferior phrenic and superior suprarenal arteries. During its course the artery was posterior to the inferior vena cava. In addition to the above mentioned characteristics, the artery arose 5 mm below the renal vein and, after ascending to the superior border of the renal vein, arched over and descended in front of it to continue along its downwards course. This case implies that the lateral mesonephric arteries form anastomoses and that different arteries may interfere with each other's development. Another significant fact in this case is that the testicular artery crossed over the renal vein and descended anterior to it; a condition which may cause pathological disturbances.

Early detection of left ventricular aneurysm rupture by two-dimensional echocardiography and surgical salvage.

A 52-year-old man was admitted to hospital emergency room with cardiogenic shock. The electrocardiogram showed normal tracing. Left ventricular aneurysm rupture at the posterolateral wall and haemopericardium was diagnosed with two-dimensional and colour Doppler echocardiography. The patient was operated on immediately and the defect was repaired. The patient made a good recovery. Four months later coronary angiography and left ventriculography were performed. The coronary vessels were found patent and there was minimal stenosis (lesser than 50%) at the proximal 1/3 portion of left anterior descending (LAD) artery. Left ventriculography showed anterolateral and posterolateral hypokinesis. The patient was symptom free and leading an active life 4 months later.

An accessory flexor digitorum longus muscle.

An accessory flexor digitorum longus muscle (M. flexor digitorum longus accessorius) in a 60-year-old male cadaver was encountered during routine dissections in our laboratory. The muscle originated from the deep transverse fascia of the leg, covering flexor digitorum longus, flexor hallucis longus, and tibialis posterior muscles, and additionally from some fibres of these muscles. After the tendon of the muscle passed behind the medial malleolus, it inserted into the tendon of the flexor digitorum longus muscle where the latter separated into four digital tendons.

The acrocallosal syndrome in a Turkish boy.

A 6 month old Turkish boy with the acrocallosal syndrome is reported. The patient, born to consanguineous, healthy parents, presented with macrocephaly, a prominent forehead, hypertelorism, polydactyly of the fingers and toes, severe motor and mental retardation, hypotonia, and absence of the corpus callosum. The mode of inheritance is discussed and our case is compared with previously reported cases of the syndrome.

Heterogeneity of X-linked recessive (spino)cerebellar ataxia with or without spastic diplegia.

Ataxia with spastic diplegia was seen in seven males of a Turkish family, obviously transmitted as an X-linked recessive trait. The first clinical sign in infancy was nystagmus; ataxia and pyramidal signs were noted at age 2-3 years. Patients were never able to walk. Dysarthria, orthopedic impairment, and mild mental retardation appeared later as the disorder progressed. Death occurred in the 3rd or 4th decade from infectious diseases. The syndrome resembles X-linked spinocerebellar ataxia and X-linked spastic paraplegia in some aspects but is different if compared with previously published reports. Laboratory and neurophysiological studies showed no abnormalities. Various aspects of X-linked ataxia are discussed: genetic heterogeneity is apparent from observations reported.

Surgical treatment of hydatid cysts of the lung: report on 1055 patients.

Of 1055 patients treated surgically for pulmonary hydatid disease, most (950) had isolated lung cysts, the other 105 having both liver and lung cysts. The chest radiograph was most valuable in diagnosis; the Casoni and Weinberg tests and blood eosinophil counts were found to be diagnostically unreliable. One thousand and seventy seven primary operations were performed. Cystotomy and capitonnage were carried out in 906 patients, 40 of whom also had decortication of the pleura. Other procedures included cystotomy with wedge resection of locally damaged lung (29 patients) and cyst removal with capitonnage by Ugon's method (33) or the Perez-Fontana procedure (8) and with costal resection for osteomyelitis in two cases. More radical surgery was carried out in 99 patients for longstanding infection or severe lung destruction. Postoperative complications occurred in 37 patients (3.5%) and the 30 day mortality rate was 1.7%. It is concluded that a lung conserving surgical operation is the treatment of choice for most patients with pulmonary hydatid disease. In patients with coexisting liver cysts the thoracic transpleural approach allowed the lung and liver cysts to be removed at the same session.

Surgical treatment of bilateral hydatid disease of the lung via median sternotomy: experience in 60 consecutive patients.

Sixty patients who underwent operation between 1979-1987 for bilateral pulmonary hydatid disease using median sternotomy approach are reported on. Although some authors prefer two-stage thoracotomy and operate on the side with the larger cyst first, or perform simultaneous bilateral thoracotomies, we have preferred one-stage operation via median sternotomy. There were 29 female and 31 male patients, ranging in age from 5 to 55 years (mean 26.4 +/- 6.3). A total of 173 cysts were operated in 120 lungs. The usual operative technique was cystotomy and capitonnage. Operative and postoperative courses were uneventful in all but three cases (5%). The causes of mortality in these patients were excessive hemorrhage, mediastinitis, and septic shock respectively. It is concluded that median sternotomy is a better alternative method for the treatment of bilateral hydatid disease of the lung, as the duration of hospitalisation is shorter, the approach is better tolerated by the patients than thoracotomy, and it possibly prevents a second general anesthesia.

Evaluation of parietal pleural invasion of lung cancers with breathhold inspiration and expiration MRI.

The goal of this study was to investigate the feasibility of detecting invasion of the parietal pleura by lung cancer with inspiratory-expiratory magnetic resonance imaging (MRI). Twenty-three patients with lung cancer in contact with pleura were studied with multisectional MRI during deep inspiration and expiration on the axial and coronal planes. Changes in the tumor's relation relative to the chest wall markers, vertebral bodies, or mediastinal structures were noted. Surgical correlation was available for each case. Twelve tumors were located at the right middle and lower lobes. Five patients who had a significant change in the position of the tumor were found not to have pleural invasion at surgery. The remaining seven tumors without change in their position with respiration were surgically found to invade the parietal pleura. Only 1 of the 11 upper lobe and apical segments of the lower lobe-located tumors showed a change in its position, which had no parietal pleural invasion at surgery. The remaining 10 tumors did not show any movement. Four of them had parietal pleural invasion that were surgically proven, and six tumors had no parietal pleural invasion at surgery. In conclusion, MRI performed during deep inspiration and expiration is a complementary method in detecting parietal pleural invasion for the tumors located in the lower and middle lobes.

Nitric oxide and endothelin relationship in intestinal ischemia/reperfusion injury (II).

Endothelins ( ETs ) are potent vasoconstrictors derived from vascular endothelium. They have primary roles in many pathophysiologic states including ischemia/reperfusion (I/R) injury. The relationships between nitric oxide (NO) and ETs are still under investigation. In this study on rats we want to focus on the interaction of NO and ET especially in I/R injury. For this purpose ET-1 and PD-156252, a nonselective ET receptor blocker, were given in a mesenteric I/R model and reactive oxygen species were detected directly using chemiluminescence of the ileal tissue. ET administrations to sham and I/R groups caused significant increases in NO concentrations whereas, in terms of peroxynitrite, which is a highly reactive group of free radicals, its increasing effects were seen only in I/R groups. This suggests that in I/R where superoxide levels increase together with NO, the conversion to peroxynitrite is likely and this effect is augmented with ET administration. On the other hand PD administration decreases superoxide and thereby peroxynitrite levels and this study shows that the effect of PD-156252 is established through this mode of action. These data suggest therapeutic approaches that may be beneficial in the treatment of I/R injury.

Complications and management of long-term central venous access catheters and ports.

PURPOSE: Although prolonged venous access devices (PVADs) are used in case prolonged intravenous therapy is required, implantation and use of these devices is associated with complications. The purpose of this study was to evaluate perioperative and long-term complications associated with PVADs and the management of these complications. METHODS: A retrospective review was undertaken of 225 PVADs implanted in 217 patients from February 1993 to June 2004. This included 144 single-lumen port infusion systems, 49 single-lumen Hickman catheters and 32 double-lumen Groshong catheters. The PVADs were inserted using either the percutaneous Seldinger method (n=183) or cutdown access to the subclavian vein (n=42). Indications for placement were as follows: chemotherapy in 66.2% of patients, drug-infusion treatment in 31.6% of patients and total parenteral nutrition in 2.2% of patients. RESULTS: Perioperative complications occurred in 13 patients (5.7%): catheter malposition in seven patients (3.1%), pneumothorax in three patients (1.3%), hemorrhage in two patients (0.9%) and catheter embolization in one patient (0.4%). Long-term complications appeared in 15 patients (6.6%): infection in five patients (2.2%), thrombosis in three patients (1.3%), extravasation in three patients (1.3%), and catheter fracture in four patients (1.8%). The fractured fragments were removed by the Amplatz snare device. In 10 patients (4.4%) only were PVADs removed prior to completion of the intended therapy. Indications for removal were catheter infection in five patients (2.2%) and catheter fracture in five patients (2.2%). CONCLUSIONS: PVAD implantation is associated with some risk of serious perioperative and long-term complications. Care of the catheter and the patient should be maintained with the proper and immediate evaluation of the perioperative and long-term complications.

The comparison of electron microscopy and scintigraphy in determining the protective effect of dimethylsulphoxide (DMSO) on ischemia/reperfusion injury through Pringle maneuver.

BACKGROUND/AIMS: We investigated the role of the electron microscopy and hepatobiliary scintigraphy in determining the effect of DMSO (dimethysulphoxide) and ischemia/reperfusion injury in the liver after the Pringle maneuver. METHODOLOGY: Twenty-four rabbits were divided into the following groups; A: Control group, B: Pringle, C: 10 mg/kg DMSO, D: 1 g/kg DMSO + Pringle. Group A was considered as a control group and only laparotomy was applied. Group B was exposed to Pringle maneuver only. Group C was given 10 mg/kg of DMSO via the vena cava inferior. Group D was given 1 g/kg of DMSO. A clamp was fastened for the groups of B, C and D in the 30th minute of the Pringle maneuver and a biopsy was applied five minutes later. Fifteen minutes later a dynamic hepatobiliary scintigraphy was applied. From dynamic images, liver peak time and activity half time of the liver were obtained. RESULTS: It was found that liver peak time and liver activity half time values of the group B, C and D were significantly longer than group A. Liver peak time and liver activity half time values of group B was not different from group C. However, some values of group D were found to be significantly shorter than groups B and C. In the electron microscopy examination, only in group B were some specific degenerative changes observed in the sinusoids. We observed less irreversible changes in group C than in group B. On the other hand, the least irreversible changes were in group D. CONCLUSIONS: As a conclusion, while electron microscopy is regarded as the "gold standard", hepatobiliary scintigraphy may be thought of as an easily applicable method in determining the ischemic reperfusion injury in the clinical comparison of the protective agents.
