Qualitative and quantitative differences in hLH species in the first and second LH release induced by continuous stimulation with synthetic LHRH in normal menstrual cycle as assessed by isoelectrofocusing.

It is known that continuous stimulation with LHRH induces a biphasic release of LH. The two pools theory is generally accepted in interpreting this phenomenon. The present study was conducted in order to elicit qualitative differences in LH included in the two pools. Five hundred micrograms synthetic LHRH was infused for 2h in 10 women in various phases of the menstrual cycle and 5 h blood sampling was performed during and after the infusion. Biphasic release of LH was demonstrated in all the cases investigated. The responsiveness to LHRH was most predominant in the midcycle, followed by the luteal phase. Pools of plasma samples at 30-105 min and 150-225 min of the LHRH infusion (plasma pools I and II; corresponding to the initial and the second LH release, respectively) were subjected to isoelectrofocusing (IEF) fractionation, pH range of 3.5-10. LH in plasma was classified in terms of isoelectric point (pI); i.e. A(pI = 9.13), B(8.60), C(8.16), D(7.67), E(7.24), and F(LH species migrating in the acidic pH area). Percentage of high alkaline LH species was significantly elevated in plasma pool I of the midcycle. No phasic difference was observed in the IEF profiles in plasma pool II. Consistent and significant increase and decrease in the percentage of acidic and high alkaline LH species, respectively, were observed throughout the menstrual cycle when the IEF profiles of plasma pool II were compared with those of pool I. The ratio of biological to immunological LH activities (B/I ratio) was markedly depressed in acidic LH species. The highest B/I ratios were obtained in the LH species migrating in the mid-alkaline region. The acidic LH species might represent the young generation of LH molecules prior to acquisition of biological potency. LH species migrating in the mid-alkaline region might be mature and bio-potent forms of the hormone. High alkaline LH species might represent LH molecules over-processed and having lost a degree of biological potency during a prolonged period of storage.

[Anterior pituitary hormone in severe head injury--with special reference to the GH, TSH, LH and FSH response to TRH and LH-RH].

Measurement of the serum level of anterior pituitary hormones (GH, TSH, LH and FSH) and the response of these hormones to their releasing hormones were made in 45 uncomplicated head injury patients within 24 hours after injury. We classified these 45 patients into three groups according to the Glasgow Outcome Scale of six months after head injury, such as good outcome group (GR or MD), poor outcome group (SD or PVS) and dead group. The serum level of the anterior pituitary hormones in dead group were higher than those in the good or poor outcome group, especially the level of GH. The serum level of GH was 14.7 +/- 3.0 ng/ml in the dead group, whereas 5.8 +/- 0.9 ng/ml in the good outcome group and 4.0 +/- 2.1 ng/ml in the poor outcome group. The response of TSH, LH or FSH to the administration of TRH or LH-RH were depressed in the dead group. On the other hand, there were normal response of TSH, LH or FSH following administration of TRH or LH-RH in the other two survival groups. These findings suggested that the measurement of the anterior pituitary hormones and the response of these hormones to their releasing hormones were useful to evaluate the severity of head injury.

[Prevalence of antibody to adult T-cell leukemia virus in Hokkaido District].

In order to investigate the spread of adult T-cell leukemia (ATL) virus in the inhabitants of ATL-non-endemic Hokkaido, we surveyed the antibodies to ATL-associated antigen (ATLA) in 29516 healthy individuals by indirect immunofluorescence test using MT-1 for target cell. Of the healthy inhabitants, only 219 individuals (0.74%) were anti-ATLA positive as carriers. Prevalence of anti-ATLA was 0.63% (111 of 17590) in males and 0.91% (108 of 11926) in females. This difference by sex is significant. In several regions, the inhabitants were found to show relatively high incidence (greater than 2%) of the antibodies, and these regions have a tendency to cluster around the western side of Hidaka mountains or Akan area. The highest rate (5.2%) of anti-ATLA positive carriers was shown in a certain town of Hidaka area.

[The effects of aorto-coronary bypass grafting to totally occluded coronary arteries on left ventricular contractility].

The effects of aorto-coronary bypass grafting (ACBG) on totally occluded coronary arteries (TOCA), postocclusion vessels of which were demonstrated angiographically through collaterals and left ventricular contractility were studied in eleven patients consisted of nine with transmural myocardial infarction (TMMI) on ECG in the area perfused by TOCA and without TMMI. Of eleven ACBG consisting of eight left anterior descending coronary arteries (LAD) and three right coronary arteries (RCA), all eight grafts to LAD were patent, whereas two of three to RCA were obstructed on the postoperative angiogram. The following evaluations were undertaken in the cases with patent grafts. In seven cases with TMMI (six LAD, one RCA), left ventricular ejection fraction (EF) increased from 0.56 +/- 0.08 (Mean +/- SD) to 0.65 +/- 0.07 (p less than 0.01). PLVSP/LVESV slightly increased, but not significantly. Mean Vcf increased from 1.07 +/- 0.33 to 1.83 +/- 0.88 circ/sec (p less than 0.02). Left ventricular segmental wall motion also improved from 23.1 +/- 6.8 to 29.5 +/- 7.2% (p less than 0.01), markedly in the anterior and apical segment. In two cases without TMMI, all of EF, PLVSP/LVESV, Mean Vcf and segmental wall motion improved much more than the cases with TMMI. In conclusion, it was suggested that ACBG to TOCA, especially LAD visualized angiographically through collaterals resulted in the excellent graft patency rate and the significant improvement of left ventricular contractility, even in the cases with TMMI.

[The immunological studies on the splenectomy and the splenic autotransplantation in BALB/C mice].

Recently, it has been demonstrated that the severe infectious diseases are often caused after splenectomy. The significance of spleen on humoral immunity has been pointed out, however the alterations of cellular immunity by splenectomy has not yet been investigated sufficiently. In this study, the effects of splenectomy on the immunological aspects were examined. And the reconstruction of immunological responses by the splenic autotransplantation was also examined. The results obtained were as follows. The antibody titers against SRBC of the splenectomized groups continued to be apparently lower than those of sham-operated groups. The IgM of the splenectomized groups showed lower values than that of the sham-operated groups, but concerning IgG, no distinct differences were noticed among these two groups. On the proliferation of peripheral lymphoid cells, the splenectomized groups from 4th week later showed a higher response than the sham operated groups. The splenic autotransplanted groups showed a similar response to the sham operated ones in the antibody production against SRBC, and the proliferations of lymphocytes. The splenic autotransplantation might be suggested to be a worthy application.

[Biological and immunological characterization of human luteinizing hormone in plasma discharged by LH-RH infusion in normal adult women].

Five hundred micrograms synthetic LH-RH was infused for 2 hours in women having a normal menstrual cycle. Measurement of the plasma hLH level revealed two phases of hLH discharge; i.e. an acute and a delayed discharge (plasma pools I and II, respectively). These plasma pools were subjected to a column isoelectrofocusing (IEF) fractionation. The IEF fractionation separated plasma LH in both plasma pools into 6 distinct species; A (pI: 9.03 +/- 0.11), B (8.60 +/- 0.10), C (8.12 +/- 0.13), D (7.67 +/- 0.09), E (7.24 +/- 0.10), and F (acidic LH). The ratio of biological to immunological LH activities in each species was determined after concentration by ultrafiltration and found to be 1.12, 2.09, 2.21, 1.87, 1.29, and 0.49, respectively. The acidic LH was more enriched in plasma pool II throughout the menstrual cycle than in its counterpart. The majority of LH activity was found in B, C and D. The relative amount of LH migrating in the high alkaline region (A) was greater in plasma pool I of the late follicular phase where the secretion of the hormone was depressed. In conclusion, LH subpopulations separated by IEF may represent the stage of molecular maturation of the hormone. The acidic LH is probably the LH newly synthesized and not well processed. The LH species possessing alkaline pIs (B, C and D) are mature types of the hormone. The LH migrating in the high alkaline region (A) may be the LH of over-maturation.

[A case of multilocular renal cyst showing neoplastic features].

A 45-year-old woman was hospitalized with complaint of upper right abdominal pain. By serial examination, it was difficult to distinguish renal cyst from renal cancer. But, because of the frequency of the attacks of upper abdominal pain, nephrectomy was undertaken. The tumor measured 5 X 5 X 3 cm, was distinct from normal renal tissue, and consisted of a small cyst. Histologically, we diagnosed a multilocular renal cyst, and its lining cells showed small atypia and adenomatous proliferation. We concluded that this multilocular renal cyst showed neoplastic features.

[Management of hemorrhage secondary to renal angiomyolipoma].

A case of bilateral angiomyolipoma which was complicated with retroperitoneal massive hemorrhage and in which right-nephrectomy was performed is reported. Eight years ago, this patient had tuberous sclerosis and underwent open renal biopsy with which the diagnosis was confirmed. This patient was admitted for shock caused by the retroperitoneal massive hemorrhage from tumor. For about 30 days, the patient was treated conservatively, such as blood transfusion and hyperalimentation, but retroperitoneal massive hemorrhage reoccurred and the patient underwent right-nephrectomy. The treatment for bilateral angiomyolipoma complicated with retroperitoneal massive hemorrhage is discussed.

Brain abscess secondary to intracranial extradural epidermoid cyst.

A case of brain abscess in the right temporal lobe secondary to an intracranial extradural epidermoid cyst in the right parasellar region is reported. The etiology of the brain abscess in this particular case was deduced using the findings of computed tomography, carried out several times over a 3-year period, after an initial operation to remove the epidermoid cyst. One of the scans showed a very-low-density spot in the right parasellar region compatible with air, suggesting a communication between the intracranial space and the paranasal sinuses.
