CD44H plays an important role in peritoneal dissemination of scirrhous gastric cancer cells.

The role of the adhesion molecule CD44H in the peritoneal adhesion and invasion of cancer cells was assessed using cell lines with low and high peritoneal seeding ability, OCUM-2M (2M) and OCUM-2MD3 (2MD3), respectively. The in vitro binding ability to peritoneal components (mesothelial cells, fibronectin and type I collagen) and invasive ability of 2MD3 cells were higher than those of 2M cells. The expression level of CD44H on 2MD3 cells was higher than that on 2M cells as determined by western blot analysis and flow cytometry. The adhesiveness of 2MD3 cells to hyaluronic acid, which is expressed on the surfaces of mesothelial cells, was greater than that of 2M cells. The binding ability of 2MD3 cells to mesothelial cells was inhibited in the presence of anti-CD44H monoclonal antibody, but that of 2M cells was not. These results suggested that the 2MD3 cell binding to mesothelial cells is regulated by the CD44-hyaluronic acid dependent system. The in vitro binding to submesothelial components and the invasiveness of 2MD3 cells were also inhibited in the presence of anti-CD44H antiody. The in vivo inoculation of 2MD3 cells treated with an anti-CD44H antibody resulted in a significiant prolongation of survival time as compared with control mice that were inoculated with 2MD3 cells alone. In conclusion, CD44H was associated with attachment not only to hyaluronic acid on mesothelial cells, but also to peritoneal stromal components. Thus, CD44H may play an important role in cancer cell binding and invasion in the peritoneal dissemination of scirrhous gastric cancer cells.

Hospital facilities available to patients with Kawasaki disease: results of a national survey of Kawasaki disease in Japan.

Since 1970, thirteen nationwide epidemiologic surveys of Kawasaki disease have been conducted, and questions on the current situation of hospitals and their facilities have been included in the survey form to assess the care available to patients with Kawasaki disease. To clarify the characteristics of medical care for Kawasaki disease, this paper summarizes the present condition of hospital facilities available to patients with Kawasaki disease. A questionnaire form including questions about the situation of hospital facilities was sent to all pediatric departments of hospitals with 100 or more beds throughout Japan. The proportion of hospitals in which successive observations with echocardiography (UCG) is available (UCG available hospital) was significantly increased compared with the ninth survey (1985-86), whereas that of the hospitals in which coronary angiography (CAG) is available (CAG available hospital) was significantly decreased. Almost all (96.2%) of the patients were reported from the UCG available hospitals. The proportion of patients reported from hospitals with a set policy for intravenous gamma globulin (IVGG) therapy was 74.5%. The proportion of UCG available and CAG available hospitals and that of hospitals with a set policy for IVGG therapy correlated with the increase in the number of patients. In conclusion, hospital facilities available to patients with Kawasaki disease in Japan have satisfactory capability for diagnosis and management of Kawasaki disease.

Epidemiology of infant Kawasaki disease with a report of the youngest neonatal case ever reported in Japan.

Our objectives were to describe epidemiological pictures of infant Kawasaki disease (KD) and to report the youngest patient ever reported in Japan. A Japanese database of 105755 KD patients reported in the 25 year period since 1970 was analyzed. Of all the cases registered, there were only six cases aged 30 days or younger and 1768 cases (1.67%) aged 90 days or younger. We reported a typical case of KD (female, the onset at 20 days old) who was the youngest patient ever reported in Japan. The epidemiology of KD in early infants does not contradict the infection theory of the etiology; the rare incidence in the neonatal period can be explained by the protective effects of the passive immunity transferred from the mother and by exposure to the unknown infectious agents.

Intraglomerular deposition of intact cross-linked fibrin in IgA nephropathy and Henoch-Schönlein purpura nephritis.

To investigate the significance of intraglomerular coagulation and fibrinolysis in IgA nephropathy (IgA-N) and Henoch-Schönlein purpura nephritis (HSPN), the distribution of intact cross-linked fibrin (XFb) modulated by plasmin activity was examined in 25 patients with IgA-N and in 12 with HSPN. In addition to the conventional method detecting fibrin-related antigen (FRA) with an antibody against fibrinogen, the enhanced intensity of immunoreactivity of cross-linked FRA (KL-FRA) using the monoclonal antibody DD3B6/22 after plasmin exposure was evaluated to assess intraglomerular deposition of intact XFb. Also, intraglomerular invasion of macrophages was detected using the monoclonal antibody KP1 against CD68. Sixteen of a total of 37 specimens (43%) showed increased intensity of XL-FRA staining after plasmin treatment which is considered to reflect the distribution of intact XFb. Increases in the intensity of XL-FRA staining were observed mainly in mesangium and partially along glomerular capillary loops and also in a few cases in the crescents. The incidence (67%) of increases in XL-FRA staining after plasmin exposure in HSPN specimens was significantly higher than that in IgA-N specimens (32%; p < 0.05). In the group positive for XL-FRA after plasmin exposure, the numbers of macrophages per glomerulus were significantly higher (n = 15; mean +/- SD = 1.6 +/- 0.9) than in the negative group (n = 6; 0.5 +/- 0.6; p < 0.01). In HSPN, the number of macrophages per glomerulus (n = 8; 1.9 +/- 1.0) was higher than that in IgA-N (n = 13; 0.9 +/- 0.9; p < 0.05). Based on these results, we conclude that XFb is often produced and distributed in intact form in the glomeruli both in IgA-N and HSPN, associated with a relatively low intraglomerular plasmin activity, and that intraglomerular coagulation may progress in accordance with macrophage infiltration, especially in HSPN.

Evans' syndrome associated with Graves' disease.

A 36-year-old woman who had had Graves' disease for 6 years was admitted with severe thrombocytopenia. Evans' syndrome was diagnosed. The patient's family history showed multiple cases of Graves' disease but no cases of Evans' syndrome. Both conditions in this patient improved with corticosteroid and thiamazole therapy. Several autoimmune antibodies were found, but a common autoimmune mechanism was not clearly shown. Although the combination of Graves' disease and Evans' syndrome had not occurred previously in her family, genetic factors may play an important role in the pathogenesis of both conditions.

Update of the epidemiology of Kawasaki disease in Japan--from the results of 1993-94 nationwide survey.

OBJECTIVE: The objective of this article is to describe the epidemiologic pictures of Kawasaki disease(KD) in Japan during the 2-year period of 1993 and 1994 based on the data obtained by the thirteenth nationwide epidemiologic survey of KD which was conducted by the Japan Kawasaki Disease Research Committee. METHODS: A survey form and diagnostic criteria of KD were sent to all hospital pediatric departments (2,640) throughout Japan with 100 or more beds. The patients to be reported in this surveys were all the incident KD cases who satisfied the diagnostic criteria. RESULTS: (1) A total of 1,730 (65.5%) hospitals responded, reporting 11,458(5,389 in 1993 and 6,069 in 1994; 6,729 males and 4,729 females; male/female ratio = 1.42) with an incidence rate of 95.1 per 100,000 children of age < 5 years. (2) The monthly number of patients was higher in winter and summer, although the monthly difference was not marked. (3) Age specific incidence rates showed a uni-modal peak at one year of age. (4) The proportion of patients with family history of KD in sibling was 1%. (5) The proportion of recurrent patients was 3%. (6) The proportion of patients with cardiac sequelae in one month after disease onset was 13%. (7) Number of fatal patients reported was 9, which conforms to 0.08% of total patients.

[Mechanism of peritoneal metastasis and the possibility of therapeutic agents].

Peritoneal metastasis has been clinically considered to be a terminal stage, so aggressive therapy has not been performed. Recently, the molecular biologic study of cancer metastasis has progressed, so it should be important in the future to examine new therapies based on the mechanism of metastasis. In this paper, the mechanism of peritoneal metastasis, especially adhesion to peritoneum, was investigated using a scirrhous gastric cancer cell line, OCUM-2MD3. The possibility of therapeutic agents for the metastasis based on the mechanism was discussed.

[Mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) with chronic renal failure: report of mother-child cases].

A 25-year-old man developed nausea, vomiting, severe headache, and confusion. He had a past history of hyperuricemia and mild renal dysfunction. On admission he had somatic growth retardation, hypertrichosis, and bilateral auditory impairment. A cranial CT scan showed a small area of low density in the left temporal lobe and cerebellar atrophy. Five days later, he developed right homonymous hemianopia, sensory aphasia, and sensory inattention, and a new, large area of low density in the left occipital lobe on a cranial CT scan. On laboratory examination, lactate, pyruvate, and the lactate-to-pyruvate ratio were elevated in both the serum and cerebrospinal fluid. The biopsied muscle showed ragged red fibers and strongly SDH-reactive blood vessels. Gene analysis revealed the presence of the A 3243 G point mutation of the mitochondrial tRNA(Leu) gene in his blood leucocytes and muscle. Serum concentrations of BUN and creatinine were elevated to 46 mg/dl and 2.2 mg/dl, respectively. Creatinine clearance was 14.1 ml/min. An abdominal CT scan disclosed atrophy of his left kidney with subcapsular calcification and the findings of his abdominal ultrasonography were compatible with chronic renal failure. His mother, who suffered from renal failure and became dialysis dependent in her late forties also bore the A 3243 G mutation of the mitochondrial tRNA(Leu) gene in her circulating leucocytes. Though the association between MELAS and renal dysfunction still remains obscure, we speculate that renal failure can be a manifestation of MELAS.

Results of 12 nationwide epidemiological incidence surveys of Kawasaki disease in Japan.

OBJECTIVE: To describe the long-term trend of incidence and other epidemiological pictures of Kawasaki disease in Japan, by using the results of 12 nationwide epidemiological surveys on Kawasaki disease that have been conducted throughout Japan (ie, surveys that have encompassed the pediatric departments of hospitals with more than 100 beds) every 2 years since 1970. RESULTS: At the end of December 1992, the total number of patients with this disease was 116,848 (males, 67,815; females, 49,033; male-female ratio, 1.38). The number of cases increased year by year, with three outbreaks in 1979, 1982, and 1986. There have been no cyclical changes since 1986. The incidence was higher in males and in those children who were 1 year of age or younger. The fatality rate decreased from 1% in 1974 to 0.04% in 1992. CONCLUSIONS: The patterns of descriptive epidemiology, such as seasonality and cyclical changes in incidence, supported the theory of an infectious agent as the causal agent. However, the incidence data since 1986 provide less support for an infectious theory.

Establishment of two new scirrhous gastric cancer cell lines: analysis of factors associated with disseminated metastasis.

Determination of the differences between cell lines which are derived from a primary tumour and a disseminated metastatic lesion from the same patient may aid in elucidating the factors associated with disseminated metastases. We report on the establishment and characterisation of two new scirrhous gastric cancer cell lines, designated OCUM-2M and OCUM-2D, derived from a 49-year-old female. OCUM-2M was derived from a primary gastric tumour, and OCUM-2D was derived from a sample of disseminated metastasis. The two cell lines were derived from the same patient. We investigated biological differences between the two cell lines to study mechanisms involved in disseminated metastasis. The growth activity of OCUM-2D cells as determined by doubling time and tumorigenicity was greater than that of OCUM-2M cells. The level of epidermal growth factor receptor (EGFR) expression in OCUM-2D cells was about twice that of OCUM-2M cells and the growth of OCUM-2D cells was stimulated more by epidermal growth factor (EGF) than that of OCUM-2M cells. The invasive activity of OCUM-2D cells was higher than that of OCUM-2M cells and was increased after addition of transforming growth factor-beta 1 (TGF-beta 1). An increase in the number of attached and spreading cells was found following the addition of 10 ng ml-1 TGF-beta 1. These findings suggest that high growth and invasive activity may play an important role in disseminated metastasis and that EGF and TGF-beta 1, which affect the growth and invasive activity of OCUM-2D cells, might be factors associated with metastasis in scirrhous gastric carcinoma. The two cell lines OCUM-2M and OCUM-2D should be beneficial for analysing mechanisms of tumour progression.

Iv gamma globulin treatment of Kawasaki disease in Japan: results of a nationwide survey.

The administration of i.v. gamma globulin (IVGG) for Kawasaki disease was investigated throughout Japan in 1993 by obtaining information from the pediatric departments of 2652 hospitals with more than 100 beds. A total of 1826 hospitals (68.9%) responded, reporting on 11,221 patients who were diagnosed during the survey period from January 1991 to December 1992. There were 8958 patients (79.8%) who received IVGG treatment. The most common treatment modality was 200 mg/kg (29.6%), followed by 400 mg/kg (18.7%) and 300 mg/kg (12.9%), all for 5 days. The distributions of total dose were: 1000 mg/kg or less, 45.7%; 1001-1500 mg/kg, 27.3%; and over 1500 mg/kg, 23.8%. For all patients to whom IVGG was administered, treatment was started in 53.8% by day 5 of illness and in 86.1% by day 7. The proportion of those with cardiac sequelae was higher in patients who were treated with IVGG, possibly due to the fact that those who were more severely affected were more likely to be treated with IVGG.

[Role of carbohydrate antigens in cell-cell and cell to extracellular matrix adhesion on pancreatic cancer].

We investigated the variable adhesiveness of pancreatic cancer cells by glycosylation inhibitor to elucidate the role of carbohydrate antigens in cell-cell and cell to extracellular matrix adhesion. Various carbohydrate antigens were strongly expressed on pancreatic cancer cell (SW1990) by ELISA, and these expression correlated to its in vitro invasiveness and in vivo metastatic potential. Cellular adhesion to Matrigel, laminin and collagen type IV and homotypic cellular aggregation in SW1990 were enhanced by reducing O-linked glycosylation with its inhibitor (Bzl-GalNAc), and these effect were reversed by additional purified mucins. These results suggested that O-linked glycoproteins on SW1990 cell surface may inhibit its adhesiveness to the substratum and aggregation properties by hindering surface receptors, and this variability of adhesiveness induced by altered glycosylation may have an another important role in highly metastatic potential of well glycosylated cancer cells.

[The changes in nuclear DNA content in acquisition of a high capacity for metastasis of human cell lines].

Quantification of nuclear DNA content in cancer is considered a prognostic factor, and several investigators have demonstrated that aneuploid tumors are more aggressive than diploid tumors. In this report, we investigated the relationship between metastatic capacity and nuclear DNA content using gastric and colon cancer cell lines. We established a highly peritoneal metastatic cell line (2M-D3) from a gastric cancer cell line (2M) and a highly liver metastatic cell line (LM-H3) from colon cancer cell line (LM) in our laboratory. Both the highly metastatic cell lines have more capacity for growth and metastasis, but lower nuclear DNA content and chromosome numbers than these parental cell lines. The results indicated that metastatic capacity was inversely proportional to nuclear DNA content. We cannot provide a satisfactory explanation for this inverse correlation between metastatic capacity and nuclear DNA content, but it is supposed that each cancer cell may have suitable and stable nuclear DNA content in metastasis.

[Correlation between malignancy of gastric cancer and c-myc DNA amplification or overexpression of c-myc protein].

The amplification of c-myc DNA for fresh frozen tissue of 31 gastric adenocarcinomas was examined with dot blot hybridization method. Using the avidin-biotin peroxidase complex method, the expression of c-myc protein was examined immunohistochemically for formalin-fixed paraffin embedded tissue of 51 gastric adenocarcinomas including the same cases for c-myc DNA analysis. c-myc DNA amplification was detected in only 4 cases (12.9%). However, c-myc protein overexpression was recognized in 21 cases (41.2%). Only one of 4 cases showing c-myc DNA amplification indicated overexpression of c-myc protein. There was no statistically significant relation of c-myc protein overexpression with clinicopathological factors (histology, depth of invasion, invasion of the vessels, metastasis, intraabdominal dissemination and histological staging). These results suggest overexpression of c-myc protein could be recognized even in early gastric cancer indicating the relation of this gene product with proliferation of gastric cancer cells, although its role in the progression and metastasis of gastric adenocarcinoma might be minimal.

Epidemiologic pictures of Kawasaki disease in Japan: from the nationwide incidence survey in 1991 and 1992.

OBJECTIVE: Since 1970, twelve nationwide epidemiologic surveys of Kawasaki disease (KD) have been conducted throughout Japan every two years to describe KD in Japan. By the end of 1992, a total of 116,848 cases were reported. This paper summarizes the statistical analysis of the latest survey for the 2-year period from January 1991 through December 1992. METHOD: A questionnaire form and diagnostic guidelines for KD were sent to all pediatric departments of hospitals with 100 or more beds throughout Japan and information was obtained on patients with KD diagnosed during the 2-year period from January 1991 through December 1992. RESULTS: The summary of the results is: 1) the number of patients reported was 11,221 (6604 males and 4617 females; male/female ratio = 1.43) with a yearly incidence rate of 90 per 100,000 children < 5 years old; 2) the monthly number of patients was higher in winter and summer, although the monthly difference was not marked; 3) age-specific incidence rates showed a unimodal peak at 1 year of age; 4) the proportion of patients with a family history of KD in a sibling was 1%; 5) the proportion of recurrent patients was 3%; 6) the proportion of patients with cardiac sequelae 1 month after disease onset was 13%; and 7) the number of patients who died was 9, which conforms to 0.08% of total patients. CONCLUSION: The incidence rates of KD in Japan are ten times higher than those reported in western countries and almost constant over 6 years. The descriptive epidemiology of the disease, which supports the infection theory, does not change for years.

[Establishment of a new scirrhous gastric cancer cell line OCUM-2M from a primary gastric tumor].

We report on the establishment and characterization of a scirrhous gastric cancer cell line, designated OCUM-2M, derived from a 49-year-old Japanese female. OCUM-2M was derived from a primary tumor of stomach taken by total gastrectomy. The cell line grew singly or in clusters in the cultured medium. The cell line continued to multiply for more than one year. Doubling time was 37.3 hours, chromosomal mode was 70. The DNA ploidy pattern was aneuploid and DNA index was 1.59. It produced several tumor-associated antigens such as CEA, CA19-9, SLX and SPan-1. The cell line was transplantable in athymic BALB/c nude mice, and histological findings of the xenografted tumor showed poorly differentiated adenocarcinoma. The growth of OCUM-2M was stimulated following the addition of EGF, b-FGF and KGF, and decreased following the addition of TGF-beta 1. This cell line is useful in vitro and in vivo systems for studies of the biology of scirrhous gastric carcinoma.
