A nationwide incidence survey of Kawasaki disease in 1985-1986 in Japan.

Nationwide epidemiological surveys of Kawasaki disease have been conducted nine times in Japan since 1970. By the end of 1986, 83,857 (male:female ratio, 1.4) cases were reported. We summarize the results of these surveys, especially the latest survey of cases from January 1985 to December 1986. There were three epidemic years - 1979, 1982, and 1986. The ratios of the number of patients diagnosed in each of those years to the number in the preceding year were 2.0, 2.4, and 1.7, respectively. The last epidemic started in a metropolitan area of Tokyo in December 1985 and propagated northwards and southwards to involve almost all of the country in six months. The age-specific incidence curve showed a unimodal peak at nine to 11 months of age. The proportion of sibling cases was approximately 2%. The epidemiological pictures suggested that the disease was caused by an unknown biologic agent that is common in the community and that spreads easily among very young children.

Dietary effects on liver and muscle glycogen repletion in exhaustively exercised rats: energy composition and type of complex carbohydrates.

Previous reports have indicated that administration of a glucose-citrate (G-C) drink after a bout of exhaustive exercise results in more effective glycogen repletion in liver and skeletal muscle in rats as compared with administration of glucose alone. The present studies report the effects of the energy pattern and the type of carbohydrates, dextrin or starch from rice, in diet given following the G-C drink after exercise, on further glycogen repletion in the tissues of rats. Rats were adapted to meal-feeding 3 times a day and trained with light swimming for 7 to 10 days. On the final day of experiments, rats received the G-C drink after 2 h of exhaustive swimming and were then fed on diets with different energy patterns or carbohydrate types. Results showed that a high-carbohydrate diet is more effective than a high-fat diet for further glycogen repletion in liver and skeletal muscle. In addition, dextrin was revealed to be superior to starch as a carbohydrate source in tissue glycogen repletion. As compared with the high-fat diet, the high-carbohydrate diet, however, resulted in a lower serum free fatty acid concentration 4 h after ingestion of food possibly by decreasing adipose tissue lipolysis.

Effect of voluntary exercise and dietary protein levels on serum lipoprotein distributions and lecithin: cholesterol acyltransferase (LCAT) activity of mice.

The effect of voluntary exercise on serum lipoprotein distributions, lecitin:cholesterol acryltransferase (LCAT) activity and serum electrophoretic patterns of mice fed different levels of dietary protein were investigated. Serum cholesterol of all exercise groups (E) showed a lower value than that of the non-exercise groups (NE). Ratios of cholesteryl ester to serum total cholesterol tended to be higher in the exercise groups than in non-exercise groups. High density lipoprotein(HDL)-cholesterol/serum total cholesterol ratios and HDL-cholesterol/low density lipoprotein(LDL)-cholesterol ratios were increased by voluntary exercise. With regard to low density lipoprotein cholesterol levels, there were significant differences between 20% E and 20% NE groups, 4% NE groups, respectively. It was found that HDL fractions in serum lipoprotein patterns of exercise groups differed from those of non-exercise groups. This seemed to be prominent in low protein diet groups. LCAT acitivity showed decreasing values as dietary protein levels decreased and its activity was raised by voluntary exercise in all groups.

Effect of voluntary exercise and dietary protein levels on the activity of serum glutamic pyruvic transaminase and the morphological picture of liver in mice.

In the present investigation, effects of voluntary exercise on serum glutamic pyruvic transaminase (SGPT) activity and histological alterations of liver tissue of mice fed 4%, 6% and 20% casein diet were examined. AGPT activity showed decreasing values accompanied by dietary protein level increases. In 4% casein diet groups, SGPT activity of the exercise group was markedly lower than that of the non-exercise group. Furthermore, there was a negative correlation between SGPT and LCAT activity. In histological evaluation, the non-exercise group on 4% casein diet revealed diffuse fatty degeneration of the liver, but the exercise group did not show such changes.

Effect of the combination of voluntary exercise and dietary protein levels on the deposition of glycogen, liver and serum lipids in mice.

The effects of voluntary exercise on the growth, glycogen of muscle and lipid contents of the liver and serum of mice fed different levels of dietary protein were investigated. In both the exercise and non-exercise groups, body weight gains were significantly greater in the 20% and 30% protein diet groups than in the 6% and 4% protein diet groups. After 6 weeks of age, it was shown that the amount of voluntary exercise by the 6% and 4% protein diet groups was greater than that by the 20% and 30% protein diet groups. As for hematological status, the raising of hemoglobin levels due to increasing dietary protein levels was further exaggerated by voluntary exercise. Hematocrit values rose with the increase in dietary protein levels. However the effect of exercise on hematocrit values was not clear. Liver glycogen levels, which were elevated with the increase in dietary protein levels, rose further due to exercise, though no changes were observed in muscle glycogen due to dietary protein levels and exercise. The lipid contents of the liver in all groups tended to be lower in exercise groups compared with non-exercise groups and it was observed that the high levels of dietary protein depressed the increase in liver lipids. Liver triglyceride levels of all groups fed the dietary protein levels except for the 20% casein diet group decreased due to voluntary exercise, and liver triglyceride levels were also lowered as dietary protein levels increased. The levels of serum triglyceride of all groups decreased due to voluntary exercise. This phenomenon was most remarkable in rats fed a 6% casein diet. The tendency for serum cholesterol levels to decrease due to exercise. However it was not further influenced by voluntary exercise in the 4% casein diet group.

Effect of voluntary exercise on physiological function and feeding behavior of mice on a 20% casein diet or a 10% casein diet.

The effects of voluntary exercise on growth and food intake, body composition, organ weight, and fatty acid composition of adipose tissue of mice fed on a 20% casein diet or a 10% casein diet were examined. The weight gain was greater for the 20% casein nonexercise group (20% NE) than that for the 20% casein exercise group (20% E), 10% casein exercise group (10% E) and 10% casein nonexercise group (10% NE). There were no significant differences between the groups except the 20% NE. In 20% E and 10% E, body fats decreased markedly. On the other hand, a very high ratio of protein was present in the body composition of both groups. In the 20% and 10% casein diet groups, food intake was increased by voluntary exercise, but there was no significant difference between 10%E and 10% NE except occasional periods during these experiments. After 6 weeks of age, 10% E had a tendency to undertake more voluntary exercise than 20% E, though the difference was not statistically significant. Development of the heart and gastrocnemius muscles was accelerated by voluntary exercise and epididymal fat tissue was markedly decreased.

Use of intravenous gamma-globulin for Kawasaki disease: effects on cardiac sequelae.

The administration of intravenous gamma-globulin (IVGG) for Kawasaki disease was investigated throughout Japan in 1993 by obtaining information from pediatric departments in 2652 hospitals that had more than 100 beds. Of 11,221 reported patients, 8958 patients (79.8%) received IVGG treatment. Of all the patients to whom IVGG was administered, the most common total dose was 1000 mg/kg (36.3%) followed by 2000 mg/kg (16.9%) and 1200 mg/kg (16.8%). The treatment was started in 53.8% by day 5 of the illness and in 83.7% by day 7. The proportion of those with cardiac sequelae was higher among patients administered > 2000 mg/kg or in those started on IVGG on day 9 of their illness or later. The possible reasons are (1) those who were more severely affected were treated with high-dose IVGG earlier; or (2) IVGG does not effectively prevent cardiac sequelae. We concluded that there is a risk of unfavorable effects with IVGG regarding cardiac sequelae when the IVGG dose is > 2000 mg/kg or if IVGG is started on day 9 or later. We believe that only a randomized controlled trial, undertaken prospectively, can adequately address the question of the optimal use of IVGG.

Effects of gamma-globulin on the cardiac sequelae of Kawasaki disease.

Our aim was to delineate the effect of various factors, such as sex, age, serum albumin levels, and the timing of gamma-globulin (GG) therapy, on cardiac sequelae of Kawasaki disease. The patients with Kawasaki disease who were reported at the 1995-1996 nationwide survey and received 2000 mg/kg at specified hospitals were selected as the subjects of the study. A total of 2221 patients actually received the basic dose. The relationships of the GG therapy with the cardiac sequelae, sex, age, timing of GG administration (the date of initiation and duration of the regimen following disease onset), and serum albumin levels were examined by using logistic regression analysis. The odds ratios for the cardiac sequelae in patients with Kawasaki disease were high in males (1.63), in those under the age of 1 year (1.54), and in those with a serum albumin level <3.2 g/dl (2.64). The odds ratio was low in those who received GG before day 8 of the illness (0.69) or in those for whom the administration period was for 2 days or less (0.67). To prevent cardiac sequelae of Kawasaki disease it is desirable that GG therapy be started as soon as possible and completed within 2 days.

Expression of thrombospondin-1 inversely correlated with tumor vascularity and hematogenous metastasis in colon cancer.

Solid tumors require neovascularization for growth and metastasis. Angiogenesis depends on the local balance between various molecules that induce and inhibit neovascularization. Thrombospondin-1 (TSP-1) is thought to be an antiangiogenic factor. In this study, we examined the correlation between expression of TSP-1 and tumor vascularity, and determined its prognostic significance in colon cancer. Microvessel density, determined by immunostaining for factor VIII-related antigen, was significantly higher in tumors that were TSP-1-negative. TSP-1 expression was inversely correlated with prognosis. Patients with TSP-1-negative tumors had a significantly worse prognosis than did those with TSP-1-positive tumors. Frequency of hepatic recurrence was significantly higher in patients with tumors that were TSP-1-negative. In conclusion, TSP-1 is an important negative-regulator of tumor angiogenesis, and TSP-1 may be useful for predicting recurrence in patients with colon cancer.

Inhibition of invasion and lymph node metastasis of gastrointestinal cancer cells by R-94138, a matrix metalloproteinase inhibitor.

BACKGROUND: MMPs have been implicated in the process of metastasis. Matlystatin analogue, called R-94138, isolated from Actinomadura atramentaria, has been reported to inhibit the activity of MMP-2 and -9 specifically. In this study, we investigated the effect of R-94138 on the invasive and lymphnodal metastatic ability of gastric cancer cells. MATERIALS AND METHODS: Three gastric cancer cell lines, MKN-28, MKN-45, MKN-74, and two colorectal cancer cell lines, HT-29, and LS174T were used. The invasion ability of these cell lines were assayed with R-94138 at the required concentration in vitro. The In vivo effect of R-94138 on lymphnode metastasis was examined passaged by orthotopic implantation. RESULTS: All cell lines revealed activity of both 72 and 92 kDa forms of MMPs by gelatin zymography. R-94138 significantly inhibited the invasiveness of all of these cells to the extracellular matrix. The number of lymph node metastases and the amount of body weight loss were significantly decreased by intraperitoneal administration of R-94138. CONCLUSION: MMP inhibitor, R-94138, has a possible application in adjunctive therapy for lymph node metastasis in gastric carcinoma.

Tranilast (N-(3,4-dimethoxycinnamoyl) anthranilic acid) down-regulates the growth of scirrhous gastric cancer.

BACKGROUND: Fibroblasts have been reported to play an important role in the proliferation of scirrhous gastric cancer cells. It would be an effective cancer therapy to reduce the cancer-stimulating activity of fibroblasts. The aim of the present investigation was to define the efficacy of Tranilast (N-(3,4-dimethoxycinnamoyl) anthranilic acid), a drug used clinically for the treatment of excessive proliferation of fibroblasts, on the growth of scirrhous gastric carcinoma both in vitro and in vivo. MATERIALS AND METHOD: The human scirrhous gastric cancer cell line, OCUM-2M, and the human gastric fibroblasts, NF-8, were used. OCUM-2M cells on the upper well and NF-8 cells in the lower well were co-incubated with Tranilast at the required concentrations in vitro. The in vivo effect of Tranilast was examined by measuring the size and the apoptotic index of coinoculated tumor by OCUM-2M cells and NF-8 cells. RESULTS: The proliferation of OCUM-2M cells was significantly stimulated by co-culture with NF-8 cells. Tranilast significantly suppressed the proliferation of NF-8 cells and subsequently decreased the growth of OCUM-2M cells in vitro. Furthermore, Tranilast depressed gastric carcinoma growth and induced cancer cell apoptosis through its effect in blocking the growth-interactions between fibroblasts and scimbous gastric cancer cells in vivo. CONCLUSION: Tranilast is a useful drug to reduce the proliferation of scirrhous gastric carcinoma.
