A rare accessory coracobrachialis muscle: a review of the literature.

During previous dissections to investigate the incidence of the third head of the biceps brachii in neonate cadavers an accessory coracobrachialis muscle was unilaterally found in a neonate male cadaver. This muscle originated from the coracoid process and the capsule of the shoulder joint. It was inserted into the antebrachial fascia and the medial epicondyle of the humerus. The coracobrachialis muscle is used as a transposition flap in deformities of infraclavicular and axillary areas and in postmastectomy reconstruction. It is also a guide to the axillary artery during surgery and anesthesia. This additional muscle may cause musculocutaneous or high median nerve paralysis. We were unable to find any similar case in the published literature. The anatomic variations of the coracobrachialis muscle may cause confusion during surgery or evaluation of CT and MRI scans.

The role of oxidants and antioxidants in psoriasis.

BACKGROUND: Psoriasis vulgaris is a chronic inflammatory skin disease characterized by well-demarcated erythema and scaly plaques. The pathogenesis of psoriasis still remains unclear. An increased reactive oxygen species (ROS) and insufficient antioxidant activity have been determined in psoriatic lesions. AIM OF THE STUDY: To evaluate and compare superoxide dismutase (SOD) and glutathione peroxidase (GP) activity in erythrocytes, catalase (CAT) activityand malondialdehyde (MDA) levels in serum of subjects with psoriasis and controls as well as MDA levels in skin biopsies from both groups. STUDY POPULATION: Twenty-two psoriatic patients (12 women and ten men) and 22 (12 women and ten men) healthy controls were involved in this study. FINDINGS: Statistically significant decreased levels of erythrocyte SOD and GP activities were noted in psoriatic subjects. Furthermore, a statistically significant increased serum CAT activity was found in the psoriasis group. No statistically significant difference was found in the serum MDA levels in the two groups, however, statistically significant increased tissue levels of MDA were noted in the psoriasis group. CONCLUSIONS: Our results support the hypothesis of an imbalance in the oxidant-antioxidant system in psoriasis.

Is the combination of calcipotriol and PUVA effective in vitiligo?

OBJECTIVE: The objective was to compare the effectiveness of psoralen plus ultraviolet A (PUVA) and the combination of PUVA and topical calcipotriol in the treatment of vitiligo. BACKGROUND: There are several reports on the response rate of patients with vitiligo treated with the combination of PUVA and calcipotriol or calcipotriol alone. SUBJECTS AND METHODS: Twenty-two patients with generalized vitiligo were taken into the study. PUVA treatment was applied on a twice-weekly schedule. Calcipotriol cream was applied to one of the two symmetrical lesions of each patient twice daily. RESULTS: Our results showed that the addition of topical calcipotriol to PUVA treatment did not lead to a significant increase in response rate of patients with vitiligo compared with PUVA treatment alone.

Survey of lead, cadmium, iron, copper and zinc in Kaşar cheese.

Lead, cadmium, iron, copper and zinc contents of Kaşar cheese sold in the markets of Ankara, Turkey, were determined over 12 months. A total of 240 samples comprising 10 different brands were analysed. Graphite-furnace atomic absorption was employed for the determination of lead and cadmium, and flame atomic absorption for iron, copper and zinc. The mean (range) of the lead, cadmium, iron, copper and zinc content of the samples were 86 (10-421) microg kg(-1), 1.8 (0.3-8.3) microg kg(-1), 4.2 (1.0-14.1) mg kg(-1), 0.7 (0.3-1.6) mg kg(-1) and 37.7 (26.5-63.0) mg kg(-1), respectively. The samples in November, December and January contained higher amounts of lead than those in other months (p < 0.01). Moreover, important differences existed in lead content of the samples between different cheese producers (p < 0.01). The differences in cadmium and iron content of the samples for different months were important (p < 0.01). The iron content of the samples among manufacturers also varied significantly (p < 0.01). However, there were no significant differences in copper and zinc contents of the samples over 12 months (p > 0.05). These findings suggested that some contamination occurred during milk production and/or manufacturing of cheese depending on the equipment used. For a consumption of 100 g Kaşar cheese, one would ingest approximately 8.6 microg (4% of the provisional tolerable daily intake, PTDI) of lead, 0.2 microg (0.3%) of cadmium, 0.4 mg (0.9%) of iron, 0.07 mg (2%) of copper and 3.8 mg (6%) of zinc. Therefore, it was concluded that Kaşar cheese is not a significant contributor to the intake of investigated heavy metals.

The changes in ovarian hormone levels and ovarian artery blood flow rate after laparoscopic tubal sterilization.

OBJECTIVE: To investigate the changes in serum ovarian hormone levels and ovarian artery blood flow rate by Doppler ultrasonography following laparoscopic tubal sterilization. METHODS: Laparoscopic tubal sterilization have been performed on 13 voluntary subjects between the sixth and eighth days of the menstrual cycle. Serum ovarian hormone levels and ovarian artery blood flow rate, by Doppler ultrasonography, were determined 3 days before the operation, on the post-operative third day and on the post-operative third month. The results of 10 participants who finished the follow-up period were analyzed. RESULTS: There were no statistically significant changes in serum levels of ovarian hormones after laparoscopic tubal ligation. The end-diastolic blood flow in ovarian artery was found to be decreased following tubal sterilization (8.7+/-2.8 and 7.4+/-1.8m/sec, respectively, P>0.05), while resistivity index (RI) increased after the operation (0.7+/-0.1 and 0.8+/-0.03, respectively, P>0.05). CONCLUSION: There was no change in ovarian hormone levels after laparoscopic tubal sterilization. There is slight but statistically non-significant decrease in ovarian artery blood flow rate following tubal sterilization, signifying a local increase in vascular resistance.

Hepatomesenteric trunk.

An hepatomesenteric trunk, formed by the common hepatic and superior mesenteric arteries, was found in a 50-year-old male cadaver. The left gastric and splenic arteries arose as a common trunk, the gastrosplenic trunk, from the abdominal aorta; no typical celiac trunk was present. In addition, the hepatomesenteric trunk passed posterior to the portal vein. A knowledge of variations of the common hepatic artery may be important in pancreaticoduodenectomy, as well as during hepatic artery infusion chemotherapy.

Effects of a monthly injectable steroidal contraceptive, Mesigyna, on menstrual pattern, lipoproteins, and coagulation parameters.

The objective of this study was to determine the effects of a once-monthly injectable contraceptive (Mesigyna) on menstrual pattern, lipoproteins, and coagulation parameters. Thirty-six women aged 18-35 years requesting monthly injectable contraception were included. Before injecting estradiol valerate 5 mg and norethisterone enanthate 50 mg, coagulation, lipoprotein, and liver function parameters were determined. After the 3- and 6-month injections, the same coagulation and serum lipid measurements and liver function tests were repeated, and women were questioned about their menstrual patterns and side effects. Thirty women who completed 6 months were evaluated. At the end of 3 months, two-thirds of the 30 women had normal menstrual patterns; at the end of 6 months, 80% of the women had normal menses. Serum LDL, total cholesterol, and triglyceride levels did not change significantly, while HDL and VLDL decreased significantly (p = 0.032 and p = 0.039, respectively) at 6 months. PT and aPTT measures did not change at the end of 6 months, while fibrinogen levels were significantly lower (p = 0.013). Serum total bilirubin levels increased (p = 0.022) and albumin levels decreased (p = 0.022) at the end of 6 months. Mesigyna was well tolerated and side effects and menstrual abnormalities were acceptable. There were no clinically significant changes in lipoprotein, coagulation, or hepatic parameters.

Transvaginal sacrospinous colpopexy for marked uterovaginal and vault prolapse.

OBJECTIVE: The transvaginal sacrospinous ligament fixation technique was used as part of the vaginal repair procedure for marked uterovaginal prolapse, and in the treatment of vault prolapse. METHOD: Out of the 26 women treated with sacrospinous ligament suspension of the vaginal vault, 23 had marked uterovaginal prolapse and three had vault prolapse following hysterectomy. Patients with vault prolapse underwent posterior vaginal repair, obliteration of the enterocele sac and sacrospinous colpopexy. Patients with marked uterovaginal prolapse underwent vaginal hysterectomy with high ligation of the enterocele sac, anterior and posterior vaginal repair, and sacrospinous colpopexy. Bilateral salpingoopherectomy was added to the procedure in five patients. All patients were examined 6 weeks after the operation and, subsequently, on an annual basis. The mean follow-up period was 2.6 years (1-5 years). RESULTS: Out of the three patients with previous vault prolapse, none had recurrences. Out of the 23 patients with previous marked uterovaginal prolapse, only two had small cystocele, and one had small enterocele at 36 months following the operation. These patients were asymptomatic and did not need an operation. Vaginal vault prolapse was not observed in any of these patients. Two women had post-operative urinary tract infection and five had buttock discomfort, which subsided after 2 months. No other intra- or post-operative complications occurred. CONCLUSION: Transvaginal sacrospinous colpopexy can be performed together with vaginal hysterectomy, and anterior and posterior vaginal wall repair in patients with marked uterovaginal prolapse because of its high success in avoiding possible vault prolapse, and low intra- and post-operative complication rates.

Comparison of microlaparoscopy and conventional laparoscopy for tubal sterilization under local anesthesia with mild sedation.

STUDY OBJECTIVE: To compare tubal sterilization performed by microlaparoscopy and conventional laparoscopy. DESIGN: Prospective, randomized trial (Canadian Task Force classification I). SETTING: Gazi University School of Medicine. PATIENTS: Twenty women undergoing surgical sterilization. INTERVENTION: Ten sterilizations by conventional laparoscopy and 10 by microlaparoscopy. MEASUREMENTS AND MAIN RESULTS: The techniques were comparable in quality of visualization, operating time, amount of drugs used for sedation and local anesthesia, and intraoperative pain scores. However, the postoperative analgesic requirement was significantly less in women treated by by microlaparoscopy. CONCLUSION: Tubal sterilization by microlaparoscopy does not differ greatly from conventional laparoscopic sterilization.

Clinical use of an intraoral silicoating technique.

UNLABELLED: A ceramic fracture rate of metal-ceramic fixed prosthodontics of up to almost 9% calls for an efficient and effective intraoral repair system. For the repair of fractured veneering material, an intraoral silicoating technique was introduced. In contrast to other repair techniques, the silicoating can successfully be used on a variety of dental restorative materials and, therefore, offers additional applications in the field of the intraoral adhesive technique. This silicoating technique, consisting of a chairside airabrasion device filled with a specific silica-coated air-abrasion medium, has been used intraorally by the authors since 1993 for clinical purposes. The clinical procedure is described for different applications in the field of the adhesive technique: intraoral repair, preconditioning of core buildups, implant abutments, and surfaces of fixed prosthodontics for the bonding of orthodontic brackets or periodontal splints. CLINICAL SIGNIFICANCE: The intraoral silicoating technique promotes reliable adhesion to the major part of metal alloy, composite, and ceramic surfaces of fixed prosthodontics. Preliminary clinical results of intraoral repairs conducted with this technique have shown that it presents a medium-term alternative to the replacement of the restoration. Also, the preliminary results of the clinical applications in other fields of adhesive dentistry are promising.

Metal-free inlay-retained fixed partial dentures.

OBJECTIVE: The treatment procedures used in conjunction with two different metal-free restorative systems are illustrated on the basis of clinical examples. In addition, a report on the initial clinical findings is provided. METHOD AND MATERIALS: Metal-free restorative materials are opening doors to new preparation methods because of their close link to the adhesive cementation technique. As a result of the developments over the past few years, various metal-free systems that can be used to fabricate short-span fixed partial dentures (FPD) are now available. Certain guidelines, however, must be observed in the process. Because of their minimal invasiveness, inlay-retained FPDs offer an interesting solution in cases where the residual dentition exhibits low caries activity. Since the beginning of 1997, a total of 23 metal-free inlay-retained FPDs made of two different types of framework material (11 of high-strength pressed ceramic and 12 of fiber-reinforced composite) have been examined in a clinical study. RESULTS: One inlay-retained FPD made of pressed ceramic had to be replaced because of a fracture. Because the materials have only been on the market for a short time, long-term results are not yet available. CONCLUSION: This type of restoration provides excellent esthetics and reduced invasiveness compared with complete crown-retained FPDs, although indications are limited by the special mechanical properties of the material.

Adenomatoid tumor of the uterus in a patient with chronic renal failure.

A case of adenomatoid tumor of the uterus in a 34-year-old patient, who had received a renal transplant and was undergoing immunosuppresive therapy is presented. At surgery, there were a total of eight nodular intramural and subserous masses thought to be leiomyoma nodules, and tumor excision was unusually, hardly performed because the nodules were strictly adherent to the myometrium. All the specimens yielded the same pathological diagnosis - adenomatoid tumor. This case is unusual because of the multiple nodular pattern and its association with the immunocompromised status of the patient. Although adenomatoid tumors have not been shown to recur, we are in doubt in our case, because the uterus is still intact and the immunocompromised status of the patient might have a role in the extensive growth and also in the possibility of recurrence.

Maxillary sinus augmentation using xenogenic bone substitute material Bio-Oss in combination with venous blood. A histologic and histomorphometric study in humans.

The aim of the present study was to evaluate bone formation following maxillary sinus augmentation using bovine bone substitute material Bio-Oss in combination with venous blood by means of histologic and histomorphometric examination of human biopsies. This involved a total of 15 sinus floor elevation procedures being carried out on 11 patients (average age of 49.6 years) according to the technique described by Tatum (1986). The subantral sinus cavity was augmented using bovine apatite combined with venous blood. After an average healing phase of 6.8 months, trephine burrs were used to take 22 bone biopsies from the augmented sinus region. Then 38 Brånemark implants were inserted in both the osteotomies resulting from bone sampling and in regular sites in the augmented posterior maxilla. Histomorphometric analysis of ground sections from the bone biopsies prepared according to the standard method of Donath & Breuner (1982) produced an average percentage of newly-formed bone of 14.7% (+/- 5.0%) and a proportion of residual xenogenic bone substitute material of 29.7% (+/- 7.8%). Some 29.1% (+/- 8.1%) of the surface of the Bio-Oss granulate was in direct contact with newly-formed bone. Histologically, newly-developed bone became evident, partly invaginating the particles of apatite and forming bridges in the form of trabeculae between the individual Bio-Oss particles. Despite the absence of osteoclastic activity, the inward growth of bone indicates slow resorption of the xenogenic bone graft material. When the implants were uncovered, after an average healing phase of 6 months, 4 of the 38 implants had become loose. Of these 4 implants, 1 had to be subsequently explanted, while the others remained as "sleeping implants" and were not included in the implants superstructure. Thus, the resulting clinical survival rate, prior to prosthetic loading, was 89.5%.

Effects of alendronate and hormone replacement therapy, alone or in combination, on bone mass in postmenopausal women with osteoporosis: a prospective, randomized study.

The effectiveness of hormone replacement therapy (HRT) and alendronate, alone and in combination, was evaluated in 120 postmenopausal patients with osteoporosis with bone mineral density (BMD) measurements at least 2 SD below the mean value for young premenopausal subjects. They had no contra-indications to HRT or alendronate use and were randomized to three different treatment groups. Group I was treated with micronized 17 beta-oestradiol 2 mg and norethisterone acetate 1 mg/day per os, group II received alendronate 10 mg/day per os and group III received micronized 17 beta-oestradiol 2 mg, norethisterone acetate 1 mg/day per os and alendronate 10 mg/day per os for 1 year. Elementary calcium 1500 mg/day was supplied to patients in all three groups. Spinal and femoral neck BMD and markers of bone mineral metabolism were measured on each patient before treatment and 6 and 12 months after treatment in 95 patients. At the end of the 12th month, significant increases in spinal and femoral neck BMD were found in all groups. Increases in spinal BMD were significantly higher in patients treated with alendronate and alendronate with HRT when compared with patients treated with HRT only. No significant difference was found in femoral neck BMD changes between the groups. Significant decreases in bone resorption and markers of bone formation were observed in all groups. Alendronate was found to be more effective than HRT and could have a very beneficial effect when added to the HRT regimen in patients with postmenopausal osteoporosis. Alendronate might also be used in postmenopausal patients with osteoporosis when HRT is contra-indicated or when there is reluctance to use hormonal treatment.
