Addisonian crisis presenting with a normal short tetracosactrin stimulation test.

We report the case of a 70 year old man who presented with physical and biochemical features suggestive of Addison's disease, but had a normal short tetracosactrin (Synacthen) test. Six months later he re-presented with similar clinical features but with an abnormal response to tetracosactrin confirming the diagnosis of Addison's disease. We recommend that if adrenal insufficiency is strongly suggested further investigation should be performed to exclude this diagnosis.

Normal fibronectin levels after surgical treatment of portal hypertension in liver cirrhosis.

We found a significantly higher plasma fibronectin concentration in a group of nine cirrhotic patients who underwent surgical treatment for portal hypertension (either shunting and non shunting procedures) when compared to twenty non operated patients. Significantly shorter prothrombin time and activated partial thromboplastin time in the operated patients were found as well. These results might be related to an increased breakdown of fibronectin during consumption coagulopathy taking place in the extended collaterals and reversed in part by surgical treatment of portal hypertension complicating liver cirrhosis.

A simple micromethod for 25-hydroxyvitamin D estimation.

A quick and simple method for estimating 25-hydroxyvitamin D is described, It involves dichloromethane extraction followed by competitive protein-binding assay without chromatography. This assay can be performed on 100 microliter of serum and enables 50 samples to be estimated in one working day. The simplicity, speed, and sample size of this method make it very suitable for use in a routine clinical biochemistry laboratory. It is particularly useful when calcium disorders secondary to abnormalities in vitamin D intake and metabolism are suspected.
