Angiotensin I-converting enzyme gene polymorphism in a Serbian population: a gender-specific association with hypertension.

Human essential hypertension has a multifactorial origin and is caused by a delicate interaction between susceptibility genes and environmental factors. Candidate genes are selected from the renin-angiotensin system (RAS) and are physiologically implicated in blood pressure regulation. We investigated the association between insertion/deletion (I/D) polymorphism at the angiotensin-converting enzyme (ACE) locus and hypertension in a case-control study conducted in a population of Caucasians (175 females, 210 males). Case subjects were those with untreated borderline hypertension. A significant, moderate, male, gender-specific independent association between DD genotype and high blood pressure was found. Adjusted odds ratio (OR) was 2.0 (95% CI, 1.1 to 3.9; p=0.03) in the whole group and 2.5 (95% CI, 1.2 to 5.1; p=0.01) in the group truncated on the basis of age (< or = 50 years). Our findings support the hypothesis that ACE is a gender-specific candidate gene for hypertension.

Absorbable suture technique: solution to the growth problem in pediatric pacing with endocardial leads.

Endocardial pacing system implantation has been performed in 15 children of mean age 37 months (ranging from 1 day to 89 months). Endocardial lead fixation was performed by means of slowly resorbable suture (Dexon) to allow spontaneous lead migration as the child grows. During a mean follow-up period of 61 months (range 17-108 months), none of the patients needed reintervention for correcting the lead length to allow growth.

Quantitative and qualitative scores on dream test.

A quantitative Dream Test score is defined as the ratio of the number of the key dream words selected by a subject and given a plus sign to the number of the dream words given minus sign as is done on the Initial-letter Word-association Test. The mean quantitative Dream Test score is 2+/3- with a slight prevalence of minus over plus signs and the mean number of selected key dream words of 5. The qualitative Dream Test score was formulated to represent the dream categories and subcategories in which individual dreams are classified according to manifest dream content. From a sample of 122 dreams reported by 122 women, students in psychology (M age 20 yr.), dreams were categorized as Death (27 or 22.0%), Nightmares (53 or 43.1%), Love (35 or 28.3%), Atypical dreams (7 or 5.7%), and No dreams (1 or .8%). The most frequent qualitative Dream Test score was for Nightmares which is in accord with the slight prevalence of minus over plus signs in the Dream Test mean quantitative score.

Laboratory testing in primary health care in Belgrade.

This study aimed to (1) analyse the utilization of tests in 15 laboratories associated with primary health care centres in Belgrade between 1985 and 1992 and (2) assess differences and similarities concerning technologies and costs between two comparable laboratories. The number of tests performed at primary health care laboratories in Belgrade has decreased since 1987 with a drastic fall in 1992, corresponding with crisis and war in the country. The comparative analysis of costs between the two laboratories showed significant differences in the way laboratory technicians spent their time and a marked difference in direct and indirect costs between the two laboratories.

Instructions for additional qualitative scoring of the initial-letter Word-association Test.

An additional scoring method is based on grouping test-words according to whether the same sign is given by subjects to the test-words. In this way five test-word categories are formed, Eros (test-words with double plus signs), demi-Eros (single plus sign), demi-Thanatos (single minus), Thanatos (double minus), and Deviant (+/- and theta signs). The next step in scoring is to count the number of test-words in a given scoring category whose meanings do not conform. The greater the discrepancy between the test-word category and its meaning, the less well adapted is the subject. Several illustrative protocols are discussed.

Quantitative scores of normal and abnormal subjects on Initial-letter Word-association Test.

The quotient of the mean quantitative score on Initial-letter Word-association Test for 193 normal subjects was significantly higher than the quotient of the mean quantitative score of 44 abnormal (psychotic) subjects. This is interpreted as meaning that, in general, the normal subjects have predominantly a positive attitude towards their surrounding world in contradistinction to the abnormal subjects whose attitude was predominantly negative, ambivalent, and repressive. On the other hand, the mean of the quotients of the quantitative scores of the 44 abnormal subjects was higher than the mean of the quotients of the quantitative scores of the normal subjects which suggests increased oral tendencies of the abnormal subjects expressed much as the unwarranted optimism of small children.

Effect of hyperbaric oxygenation on maximal aerobic performance in a normobaric environment.

Eighteen female physical education students, randomly divided into three groups, inhaled 100% O2 for 60 minutes in a hyperbaric chamber. Pressure in the chamber amounted to 2.8 ATA. Three days before the hyperbaric oxygenation (HBO) all the students were subjected to a treadmill test. The first group was retested after 30 minutes, the second after 3 hours and the third 6 hours after the HBO. The first and the second groups of subjects achieved statistically significant maximal oxygen consumption after the HBO (14.4 and 10% respectively) and were capable of sustaining considerably higher exertion on treadmill (12.8%, namely 18.1%, both values were significant on the level of p less than 0.05). The ventilation efficiency parameters (VEO2 and R) did not differ with any of the three groups of subject during the exertion before and after the HBO, although after the HBO the exertion was considerably higher.

Assessment of the effects of cost-sharing in Yugoslavia.

In this study the existing forms and amounts of cost-sharing in the Republic of Serbia (Yugoslavia) are analyzed. The level of cost-sharing is ranked according to the number and type of services involved and the relative importance with which they are viewed within the overall health-care policy of Yugoslavia. The "self-managing community of interest of health care," administratively coincident with the district, is taken as the unit of observation. Until recently, these bodies independently decided whether to introduce cost-sharing as well as the rate of cost-sharing. Therefore, the implementation of this health-policy measure differed among the districts generating inequality within health-care system. The results of simple and multiple correlation indicate that poorer districts with fewer health resources and lower levels of consumption introduced cost-sharing for more services, attempting to provide additional resources for financing of the health-care system. The multiple linear-regression model shows that population/physician ratio and the use of primary care physicians' services are the significant indicators of health-care expenditures in this territory. Considering the current economic situation in Yugoslavia and the level of development of the health-care system, the results of this cross-sectional study imply that cost-containment in health care can be achieved in a more equitable and effective manner by controlling supply of health services than by limiting demands.

Personality assessment by Initial-Letter Word-Association Test.

The case of a psychotic girl was described to show that personality assessment by the Initial Letter Word Association Test may be done by categorizing the subject's test associations according to the oral, anal, and phallic character traits. The similarity between the subject's test and dream contents was also studied. The kind of similarity found was not seen in the normal subjects.
