Segmental bronchial atresia--a case report and a literature review.

Bronchial atresia is a short and juxtahilar bronchus interruption. This uncommon malformation leads to an obstructive and systematized emphysema, often associated with mucoid impaction. The authors report the case of a 5-year-old boy and review the literature in order to recall the radiologic features of this affection, which are characterized by an obstructive segmental or lobar emphysema often located in the upper lobe. The modern imaging means are not necessary for diagnosis. Bronchography confirms the atresia. Neonatal bronchial atresia appears as an opaque segment or lobe with retention of the alveolar liquid.

Percutaneous gastrostomy for enteral nutrition: long-term follow-up of 176 procedures.

OBJECTIVE: To evaluate the efficacy and safety of percutaneous gastrostomy (PG), using small catheters without gastropexy, to deliver enteral nutrition. METHODS: We reviewed the records of 176 consecutive patients in whom PG was attempted for enteral nutrition. Of these cases, 172 catheters were inserted by the Seldinger technique under fluoroscopic guidance alone, 2 were inserted under computed tomographic guidance, and 2 procedures failed. In primary procedures, 8.5- or 10.2-French catheters were used. RESULTS: The technical success rate was 98.9%. Of the 176 procedures, 2 failed because of the high position of the stomach. Seven-day follow-up was obtained in all patients; 30-day follow-up in 145 patients (83%), and long-term follow-up (30 to 1512 days) in 123 patients (70%). The 30-day mortality rate was 13.2%. One death (0.5%) was directly related to the procedure. Major complications occurred in 4 patients (2.2%), and minor complications in 12 patients (6.9%). No patient required surgery for complications attributable to the gastrostomy procedure. CONCLUSION: PG without gastropexy using small catheters is a simple, safe and effective technique for delivering enteral nutrition.

[Helical computed tomography of the aorta and its branches].

Imaging of the aorta has greatly benefited from the recent development of helical computed tomography. This noninvasive and widespread imaging technique could be considered as a viable alternative to invasive modalities in aortic disease assessment, especially in the acute onset. Radiologists should be familiar with the principles of this technique and its clinical applications. This report develops technical aspects, typical and atypical features of a variety of noncongenital aortic diseases including dissections, aortic trauma, aneurysms, nonaneurysmal atherosclerotic diseases and arteritis. The role of helical CT as noninvasive tool for imaging the aorta and its branches is compared to the other imaging methods.

[Cystic meningioma. Apropos of 6 cases].

We report six cases of meningioma, uncommon tumors which accounted for 2.7 of all meningiomas observed in our series. Computed tomography was performed in all patients and produced the diagnosis in only 40% of the cases. Surgical treatment was given. Histological examinations of the surgical specimens confirmed the diagnosis. CT and magnetic resonance imaging were sometimes misleading, suggestive of metastasis or glioblastoma. Selective carotid arteriography with opacification of the external carotid artery established the diagnosis. Prognosis was good after surgery.

[Hydatid cyst of soft tissues. Radiological aspects].

Hydatid disease of the soft tissues is rare, with a frequency between 2.4 and 5.3% of all sites of hydatid disease. We report a retrospective study of 6 cases of hydatid cyst of soft tissues. The mean age of our patients was 36 years (range: 25 and 63 years) and the sex-ratio was 5 females to 1 male. The clinical symptoms consisted in a mass which had been present for a mean of 12 months. Plain X-rays showed a water density mass with no associated bone lesions. Ultrasonography and computed tomography defined the features of the hydatid cysts, which were situated in muscle in 5 cases and in the subcutaneous tissue in 1 case. Their dimensions varied between 1.5 and 25 cm. All types of Gharbi's classification were observed. Secondary hydatid cysts outside of the soft tissues were observed in 2 patients. Medical imaging, essentially ultrasonography, was suggestive of the diagnosis. Computed tomography provided more precise information concerning the number, site and morphology of the lesions and was able to eliminate neoplastic disease.

[Ultrasound signs of pseudoneoplastic forms of hepatic hydatid cysts. A prospective analysis of 50 cases].

Pseudoneoplastic hydatid cyst of the liver (type IV), still raises diagnostic difficulties on ultrasonography (US), and often leads us to perform computed tomography. We therefore, conducted a prospective study of 50 cases of hepatic hydatid cyst (HHC) type IV in order to define the US and Doppler features allowing easy diagnosis with the best cost-effectiveness ratio. HHC type IV represented 23.7% of all types. The average age was 43.5 years with a female predominance. Many signs were described and their frequency was established. The main signs were daughter cysts (82%), hypo or hyperechoic spirals (66%), and peripheral transonic collarette (54%). At least one of these signs was present in 90% of cases. The Doppler study showed the avascular nature of the lesion in 100% of cases. Hydatid serology using modern immunologic techniques confirmed the diagnosis in 74% of cases. The US-serology combination can establish the positive diagnosis of HHC type IV in 94% of cases. Computed tomography therefore has a small place in this setting.

[Cancer of the breast in men. Apropos of 50 cases].

Breast cancer, rarely encountered in men, accounts for 0.2 to 1.5% of the cancer morbidity in men and 1 to 5.7% of all breast cancers. The aim of this work is to analyse the different epidemiological, therapeutic and outcome features of this disease in men using a retrospective multicentric approach to 50 cases reported in Casablanca from 1980 to 1992. The mean age of the patients was 60.6 years. No significant risk factor was found. The mean delay to consultation was 22.5 months. Diagnosis was made at an advanced stage as, the tumour was located behind the nipple in 98% of the cases and ulcerated in 52%. Laterality was almost identical. Invasive galactophoric adenocarcinoma was found in 86% of the cases. Treatment was usually Patey mastectomy with positive nodes in 84%. Complementary treatment included radiotherapy in 60%, chemotherapy in 36% and hormonotherapy in 1 case. Recurrence was observed in 71% of the patients who has achieved complete remission. Surviceal in evaluable cases was 59 and 25% at 2 and 5 years respectively. The results in this series was less satisfactory that it is generally reported in the literature which suggests the need for better codification and management.
