Kinetic analysis and mechanistic aspects of autoxidation of catechins.

A peroxidase-based bioelectrochemical sensor of hydrogen peroxide (H(2)O(2)) and a Clark-type oxygen electrode were applied to continuous monitoring and kinetic analysis of the autoxidation of catechins. Four major catechins in green tea, (-)-epicatechin, (-)-epicatechin gallate, (-)-epigallocatechin, and (-)-epigallocatechin gallate, were used as model compounds. It was found that dioxygen (O(2)) is quantitatively reduced to H(2)O(2). The initial rate of autoxidation is suppressed by superoxide dismutase and H(+), but is independent of buffer capacity. Based on these results, a mechanism of autoxidation is proposed; the initial step is the one-electron oxidation of the B ring of catechins by O(2) to generate a superoxide anion (O(2)(*-)) and a semiquinone radical, as supported in part by electron spin resonance measurements. O(2)(*-) works as a stronger one-electron oxidant than O(2) against catechins and is reduced to H(2)O(2). The semiquinone radical is more susceptible to oxidation with O(2) than fully reduced catechins. The autoxidation rate increases with pH. This behavior can be interpreted in terms of the increase in the stability of O(2)(*-) and the semiquinone radical with increasing pH, rather than the acid dissociation of phenolic groups. Cupric ion enhances autoxidation; most probably it functions as a catalyst of the initial oxidation step of catechins. The product cuprous ion can trigger a Fenton reaction to generate hydroxyl radical. On the other hand, borate ion suppresses autoxidation drastically, due to the strong complex formation with catechins. The biological significance of autoxidation and its effectors are also discussed.

Visualization of sigma1 receptors in eyes by ex vivo autoradiography and in vivo positron emission tomography.

Sigma receptors are present on the neurons of the central nervous system and in peripheral organs. They have also been demonstrated in ocular tissues by in vitro membrane binding assays. We have investigated whether sigma(1) receptors can be demonstrated in rat eyes by ex vivo autoradiography using [(11)C]SA4503, a selective radioligand. We also tested whether in vivo positron emission tomography (PET) can be used to show sigma(1) receptors in rabbit eyes. In rats, a high accumulation of [(11)C]SA4503 was found in the iris-ciliary body and retina. A carrier-loading experiment showed that the receptor-specific binding of [(11)C]SA4503 was approximately 75% of the total binding in the brain. Sigma(1) receptors were also detected in the projecting terminals of the retina to the superior colliculus. PET showed radioactivity in the anterior segment including the iris-ciliary body and retina, and pretreatment or displacement by a sigma receptor ligand (haloperidol), suggested that the PET signal reflects radioligand-receptor binding. The high density of sigma(1) receptors in the iris-ciliary body and retina was confirmed by ex vivo autoradiography. In conclusion, the iris-ciliary body and retina are rich in sigma(1) receptors, and PET may be used to investigate the in vivo distribution of these neuroreceptors in the eye.

Indocyanine green angiography in patients with human T cell-lymphotropic virus type 1 uveitis.

PURPOSE: To determine the indocyanine green (ICG) angiographic features and to evaluate the choroidal involvement of human T-cell lymphotropic virus type 1 (HTLV-1)-associated uveitis. METHODS: We performed ICG angiography using scanning laser ophthalmoscopy in 54 eyes of 27 patients (8 men and 19 women) diagnosed with HTLV-1 uveitis. The patient's mean age was 51.5 years with a range of 24-65 years. RESULTS: The early phase of ICG angiography revealed ICG leakage from the choroidal vessels in the posterior pole, hyperfluorescent spots that which were not detected with fluorescein angiography, and small hypofluorescent lesions in the macula which most likely corresponded to microcirculatory disturbances in the choriocapillaris. CONCLUSIONS: We suggest that the ICG angiographic findings reflect choroidal lesions such as infiltration with leukocytes and edema. ICG angiography may provide useful information on choroidopathy in HTLV-1 uveitis.

Long-term visual prognosis of choroidal neovascularization in high myopia: a comparison between age groups.

OBJECTIVE: To analyze visual outcome in highly myopic patients of different age groups with choroidal neovascularization (CNV). DESIGN: Retrospective observational case series. PARTICIPANTS: We reviewed the medical records of 63 consecutive patients (73 eyes) with myopic CNV. The patient population was divided into two groups according to age at onset of CNV (< or =40 and >40 years old). INTERVENTION: Demographic and clinical data were obtained from the patients' medical records. MAIN OUTCOME MEASURES: Visual acuity at least 3 years after CNV diagnosis. RESULTS: Group 1 (< or =40 years old) consisted of 22 patients (26 eyes), and group 2 (>40 years old) consisted of 41 patients (47 eyes). Throughout the follow-up period, group 1 retained better visual acuity than group 2. Almost half the patients in group 1 retained a final visual acuity better than 20/40. No significant change occurred in the logarithm of the minimum angle of resolution (logMAR) in group 1 during follow-up. Group 2 had worse visual acuity at the initial evaluation than did group 1, and a statistically significant worsening of logMAR was found during the follow-up period. More than half of the patients in group 2 had a final visual acuity less than 20/200. In addition, group 2 had a larger area of CNV, and chorioretinal atrophy was more frequently seen after the regression of CNV than in group 1. CONCLUSIONS: The visual prognosis of myopic CNV is influenced by age at onset. The results of this study indicate that patient age at the time of onset of myopic CNV should be considered when determining the therapeutic course.

Optical coherence tomography of choroidal neovascularization in high myopia.

AIM: To investigate morphologic changes in the eye with choroidal neovascularization (CNV) in high myopia using optical coherence tomography (OCT). METHODS: Optical coherence tomography was performed in 35 patients (42 eyes) with myopic CNV. Myopic CNV was divided into active, scar, or atrophic stages based on funduscopic and fluorescein angiographic findings. The characteristics of OCT findings in each stage were identified. RESULTS: In the active stage (11 eyes), OCT clearly displayed a neovascular membrane as a highly reflective dome-like elevation above the retinal pigment epithelium (RPE). No apparent subretinal fluid accumulation around the CNV was identified. In the scar stage (12 eyes), only the surface of the CNV showed high reflectivity, which was markedly attenuated below the surface. In the atrophic stage (19 eyes), the CNV had become totally flat and chorioretinal atrophy around the regressed CNV showed high reflectivity. CONCLUSIONS: Optical coherence tomography demonstrated characteristic features at each stage of myopic CNV. Optical coherence tomography appears to be useful in evaluating the stage and activity of myopic CNV.

Effect of natural and synthetic retinoids on the proliferation and differentiation of three canine melanoma cell lines.

The effect of two natural retinoids and synthetic retinoids with or without retinoid synergists on the proliferation and differentiation of 3 melanoma cell lines were investigated in vitro. No retinoids showed significant growth inhibitory effect on these cell lines when used alone, however, cell differentiation and significant growth inhibition were observed when treated with a combination of retinoids and a retinoid synergist. This study may suggest that, though the cells showed low susceptibilities when retinoids were treated alone, the combination of retinoids and a retinoid synergist may be effective to control the growth of canine melanoma cell lines.

The expression of sialyl Lewis X in canine and feline mammary gland tumors.

The expression of sialyl Lewis X (sLe(x)) in 93 canine and 15 feline mammary gland tumors (MGT) obtained by surgical resection at Veterinary Medical Center, the University of Tokyo was examined by immunohistochemistry. Their clinicopathological features and prognosis were also reviewed. Approximately 60% of MGT tissues showed sLe(x) positive expressions, while all normal mammary gland tissues were negative. However, its expression was not correlated with clinicopathological features and prognosis significantly. This study suggests that sLe(x) may be a tumor-associated antigen in canine and feline MGTs.

Effects of medetomidine-midazolam, acepromazine-butorphanol, and midazolam-butorphanol on induction dose of thiopental and propofol and on cardiopulmonary changes in dogs.

OBJECTIVE: To evaluate dose-sparing effects of medetomidine-midazolam (MM), acepromazine-butorphanol (AB), and midazolam-butorphanol (MB) on the induction dose of thiopental and propofol and to examine cardiopulmonary changes in dogs. ANIMALS: 23 healthy Beagles. PROCEDURE: Dogs were administered MM, AB, MB, or physiologic saline (0.9% NaCI) solution (PS) IM, and anesthesia was induced with thiopental or propofol. Cardiopulmonary measurements were obtained before and after administration of medication and 0, 5, 10, and 15 minutes after endotracheal intubation. RESULTS: Induction doses were reduced significantly by preanesthetic administration of MM, AB, and MB (thiopental, 20, 45, and 46% after administration of PS; propofol, 42, 58, and 74% after administration of PS, respectively). Recovery time in dogs administered MM-thiopental or MM-propofol and AB-propofol were significantly prolonged, compared with recovery time in dogs administered PS-thiopental or PS-propofol. Relatively large cardiovascular changes were induced by administration of MM, which were sustained even after the induction of anesthesia. Administration of AB and MB induced cardiovascular changes during and immediately after endotracheal intubation that were significantly decreased by induction with thiopental or propofol. However, mild hypotension developed with AB-propofol. Apnea was observed in dogs administered MM during induction of anesthesia, but most respiratory variables did not change significantly. CONCLUSIONS AND CLINICAL RELEVANCE: Preanesthetic medication with MM greatly reduced the anesthesia induction dose of thiopental and propofol but caused noticeable cardiopulmonary changes. Preanesthetic medication with AB and MB moderately reduced the induction dose of thiopental and propofol and amelio rated cardiovascular changes induced by these anesthetics, although AB caused mild hypotension.

[A clinical evaluation of uveitis-associated secondary glaucoma].

PURPOSE: To investigate the clinical features of secondary glaucoma associated with uveitis. SUBJECTS AND METHODS: The subjects of the study were 1,604 eyes of 1,099 patients with uveitis at Miyata Eye Hospital, Miyakonojo, Miyazaki, between October 1974 and January 2000. The intraocular pressure (IOP) and clinical data were analyzed retrospectively. Secondary glaucoma was defined as being an IOP higher than 21 mmHg and needing treatment with medication to control the high IOP. RESULTS: Secondary glaucoma was found in 293 eyes (18.3%) of 217 patients (19.7%) among the uveitis patients. The clinical entity with the highest frequency of secondary glaucoma was Posner-Schlossman syndrome in 100%, followed by sarcoidosis in 34.1%, herpetic anterior uveitis in 30.4%, Behçet's disease in 20.8%, HLA-B 27 related acute anterior uveitis in 20.0%, Vogt-Koyanagi-Harada' disease in 16.4%, and HTLV-1 uveitis in 16.2%. Among the 293 eyes with secondary glaucoma, the majority (72%) had active anterior uveitis at the time of high IOP. Only 7.5% of secondary glaucoma eyes had peripheral anterior synechia wider than 180 degrees of trabecular meshwork. Steroid glaucoma was found only in 8.9% of the secondary glaucoma eyes. Surgical therapy mainly with trabeculectomy with anti-metabolites was performed in 38 eyes and the postsurgical IOP was controlled to 20 mmHg or lower in 36 eyes. Despite the medical and surgical therapy for secondary glaucoma, defect of the visual field was found in 38% of the secondary glaucoma eyes. CONCLUSIONS: The frequency of secondary glaucoma in 1,604 eyes with uveitis was 18.3%, and the frequency of secondary glaucoma differed depending upon the clinical entity of uveitis. The evaluation and the management of IOP are very important in treatment patients with uveitis, in addition to the management of intraocular inflammation.

The presence of macrophage migration inhibitory factor in human trabecular meshwork and its upregulatory effects on the T helper 1 cytokine.

PURPOSE: To investigate the expression and secretion of macrophage migration inhibitory factor (MIF) in human trabecular meshwork (HTM) and evaluate its role in ocular inflammation. METHODS: Tissue samples of HTM cells were isolated from donor human eyes or corneoscleral buttons, and the HTM cells were cultured. The expression of MIF on HTM cells was evaluated by RT-PCR, Western blot analysis, and ELISA. T-cell clones (TCCs) were established from ocular infiltrating cells of patients with uveitis. ELISA was used to evaluate the pathologic role of MIF, in relation to regulatory effects on cytokine production by T cells. RESULTS: MIF was detected in the HTM by RT-PCR and Western blot analysis. MIF was also shown by ELISA to be secreted by the HTM cells in culture. The HTM supernatant enhanced IFN-gamma production by TCCs, but not IL-10; and these effects were neutralized by anti-MIF antibodies. Similarly, recombinant MIF enhanced the IFN-gamma production by the TCCs. CONCLUSIONS: MIF is expressed and secreted in the HTM, and MIF has the capacity to enhance T helper 1 cytokines and may play a role as an inflammatory cytokine in the eye.

[The quality of life in patients with pathologic myopia].

PURPOSE: To evaluate the functional status of daily life and the quality of life of pathologic myopia patients. METHODS: A cross-sectional study was conducted using data from pathologic myopia patients(n = 211) and control subjects(n = 144). The influence of the disease on the daily life and the quality of life of patients were evaluated using a self-rating questionnaire. The questionnaire covered the full range of the status of patients' daily life and the quality of life of pathologic myopia patients, including daily tasks depending on visual acuity, social and emotional handicaps, and cognition of disease. RESULTS: The functional status of daily life and the quality of life of patients was reduced compared with control subjects. The influence of pathologic myopia on the patients' daily lives was primarily the result of three major factors: handicap, disability, and support. All three correlated with the quality of life, the degree of handicap having the strongest correlation. CONCLUSION: The functional status of daily life and the quality of life of patients was reduced; this reduction in quality of life was attributed to disability and handicap caused by pathologic myopia.

Microdissection and gene rearrangement analysis of paraffin-embedded specimens of orbital malignant lymphoma.

PURPOSE: To determine whether a definite diagnosis of malignant lymphoma can be made from paraffin-embedded archived orbital specimens by gene rearrangement analysis using microdissection and polymerase chain reaction (PCR). METHODS: Specimens from four patients with histopathologically diagnosed orbital malignant lymphoma were examined. The malignant cells were microdissected off the paraffin-embedded specimens. DNA was extracted from the cells, and the immunoglobulin heavy chain ( IgH) gene was amplified by PCR. Gene rearrangements were detected by using primers for the third framework (FR3A), the second framework (FR2A), and the complementary determining region 3 (CDR3). Translocation of the B-cell lymphoma/leukemia-2 ( bcl-2) gene was also examined. RESULTS: Malignant cells were present on the slides of the paraffin-embedded specimens of three of four cases. The specimens from these three cases showed IgH rearrangements for FR3A, FR2A, and CDR3. A bcl-2-associated translocation was also detected in one case. CONCLUSIONS: Gene rearrangement analysis is applicable to paraffin-embedded archived orbital specimens to confirm a diagnosis of malignant lymphoma. The advantage of this method is that only a small specimen is needed because the detection sensitivity is high.

Oscillopsia associated with dysfunction of visual cortex.

PURPOSE: To investigate the cause of oscillopsia without nystagmus or vestibular dysfunction in a 31-year-old man with no past history of serious illness. METHODS: The changes in the regional cerebral blood flow elicited by visual stimuli were studied by positron emission tomography (PET) in the patient and were compared with changes in normal subjects. The primary visual cortex (PVC) and area V5 were defined on Talairach coordinates, as shown in previous PET studies, and were confirmed with their activation patterns in our subjects. RESULTS: Flicker stimulation (8 Hz) and a stationary random dot pattern activated area V5 in the patient, but not in the six normal controls. A moving random dot pattern activated both the PVC and the extra-striate cortex in both the patient and the controls. CONCLUSIONS: Oscillopsia in this case is associated with dysfunction of the visual cortices and may be caused by it.

Ocular discomfort at the initial wearing of rigid gas permeable contact lenses.

PURPOSE: To evaluate the relationship between ocular discomfort in the initial wearing of rigid gas permeable contact lenses (RGPCL) and the time required to adapt to RGPCL as measured by using the visual analogue scale (VAS). METHODS: RGPCL were prescribed for 89 patients (178 eyes) with myopia or myopic astigmatism and no history of wearing contact lenses. The patients who stopped wearing RGPCL after 1 week because of ocular discomfort were defined as dropouts, and those who adapted to regular full-time RGPCL wear were defined as successful wearers. VAS scores were measured at several intervals for 1 month after the initial wearing of the lenses. The number of days required to adapt to wearing RGPCL was defined as the adaptation time. RESULTS: Four patients were dropouts due to ocular discomfort (eight eyes; 4.5%). VAS scores were significantly lower in successful wearers than in dropouts at 1 week, although their VAS scores were not different at 1 day. VAS scores of the successful wearers at 1 week were significantly lower than those at 1 day. The average time required to adapt to RGPCL was 23.0 +/- 22.1 days for the 85 successful wearers. Lower VAS scores at 1 day or 1 week were significantly correlated with a shorter adaptation time in the successful RGPCL wearers. CONCLUSIONS: Our results indicate that the VAS score might be useful for predicting successful RGPCL use and the adaptation time required by successful wearers.

Glucose metabolism in the visual structures of rat monocularly deprived by eyelid suture after postnatal eye opening.

PURPOSE: To determine the changes in the visual centers of rats following monocular visual deprivation after postnatal eyelid opening (PEO). METHODS: Monocular eyelid suture was performed on rats on days 1, 3, 5, 7, 14, and 28 after PEO, and the glucose metabolism was measured 1, 2, 5, and 7 days after the eyelid suture. Ex vivo autoradiography with (14)C- or (18)F-labeled 2-deoxy-D-glucose was carried out. Effects of monocular enucleation or dark rearing were also determined. RESULTS: Monocular eyelid suture did not decrease the glucose metabolism in any contralateral visual structures on day 1 after visual deprivation in the PEO1 or PEO3 lid-sutured rats. However, there was a decrease on day 1 after the eyelid suture in PEO7 and older rats. Similarly, monocular enucleation on PEO1 did not reduce the glucose metabolism in the visual cortex (VC), but enucleation on PEO7 and thereafter did. Eyelid suture on PEO8 following dark rearing until PEO7 did not reduce the glucose metabolism 1 day after suture, but reduced it at 7 days after suture. CONCLUSIONS: Glucose metabolism was altered by visual deprivation on PEO7 and thereafter. Five days of visual experience after PEO were required for initiation of visual function in the rat visual system.

Retinal pigment epithelium atrophy secondary to dilated choroidal artery in the macula of a highly myopic patient.

BACKGROUND: Indocyanine green angiography revealed a markedly bent and tortuous choroidal artery in the macula of the left fundus in a highly myopic patient. CASE: The patient's corrected visual acuity was 1.0 in both eyes, and he had no visual symptoms. OBSERVATIONS: Ten years after the initial examination, atrophy of the retinal pigment epithelium (RPE) developed above the dilated choroidal artery. CONCLUSIONS: Optical coherence tomography demonstrated that the RPE was stretched above the markedly dilated choroidal artery. Thus, mechanical stretching of the RPE by an abnormally dilated choroidal artery might underlie the development of RPE atrophy.

Fundus characteristics of high myopia in children.

PURPOSE: To evaluate the fundus characteristics of highly myopic eyes in children. METHODS: We reviewed the medical records of 46 children (1 to 8 years old; mean age, 6.8 years) (80 eyes) with high myopia (4 D or more for children younger than 5 years, 6 D or more for children aged 6-8 years) seen consecutively during a 10-year period at the high-myopia clinic in our hospital. Children of up to 8 years of age at the initial visit were included in the study. RESULTS: Fundus examination revealed posterior staphyloma in only one eye (1.3%) and mild chorioretinal atrophy around the optic disc in 13 eyes (16.3%). There were no patients with choroidal neovascularization or geographic atrophy in the posterior fundus. Myopic peripapillary crescent was observed in 26 eyes (33.8%), but the area of the crescent was relatively small (mean, 0.5 disc area). CONCLUSIONS: The results of the present study showed that myopic fundus changes are uncommon and mild in children. They suggest that aging, in addition to mechanical stretching of the eyeball, might be important for the development of myopic fundus changes.
