Woakes' syndrome and albinism.

Nasal polyposis is a very common and multifactorial disease. Whereas eosinophil-dominated polyps often are sensitive to anti-inflammatory treatment like corticosteroids, the therapy of polyps without eosinophils is more difficult and disappointing. We report the clinical course of a 29-year-old albino patient suffering from a extreme manifestation of Woakes' syndrome, which is characterized by severe recurrent nasal polyps, often without eosinophils on histological examination and with broadening of the nose. In this case, the recurrent fibrotic polyps without eosinophils were resistant to conventional medical and surgical treatment and required further treatment with radiotherapy with awareness of all possible future sequelae. The pathoetiology and treatment of Woakes' syndrome as well as of albinism were discussed.

Invasive sphenoidal aspergillosis: successful treatment with sphenoidotomy and voriconazole.

Treatment of invasive sphenoidal aspergillosis is surgical, followed by antifungal therapy, mostly amphotericin B. To optimize the adjuvant antifungal treatment, which is often limited by severe side effects, the new triazole antifungal agent voriconazole with broad coverage of fungal pathogens including Aspergillus was investigated in a study of 4 patients with clinical, radiological and histological signs of invasive sphenoidal aspergillosis. They first underwent endoscopic sphenoidotomy with drainage and extraction of the fungal mass. Postoperatively, 2 patients were immediately treated with voriconazole. Two patients initially received amphotericin B; but this treatment had to be stopped because of acute renal toxicity. Finally, all patients were treated orally with 200 mg voriconazole twice a day for 12-14 weeks. After this combined treatment all patients were asymptomatic and there were no endoscopic or radiological signs of residual fungal disease. The only side effects were nausea in one and transient visual disturbances in 2 other patients. In the 4 patients presented and treated, voriconazole was shown to be effective and less toxic than amphotericin B in adjuvant treatment of invasive sphenoidal aspergillosis.

Insertion of double bicanalicular silicone tubes after endonasal dacryocystorhinostomy in lacrimal canalicular stenosis: a 10-year experience.

To assess the long-term results of double bicanalicular silicone tubes in canalicular (presaccal) stenosis of the nasolacrimal duct system, a retrospective follow-up was performed on patients operated between 1992 and 2002. Forty-five patients were included in the study, 44 of whom had eyes with canalicular stenosis (primary dacryocystorhinostomy), 4 with congenital agenesis of the lacrimal duct system, and 3 cases after primary external dacryocystorhinostomy. Double bicanalicular silicone tubes were inserted, fixed in the vestibulum nasi, and left in place as long-term space holders. Of the 51 endonasal primary dacryocystorhinostomies with canalicular stenosis, 32 (63%) became symptom-free after one procedure. Nineteen (37%) patients had a relapse. Double silicone bicanalicular tube insertion as long-term spacers in canalicular stenoses has a number of advantages, but canalicular stenoses still remain a challenging area.

Elective neck dissection for carcinomas of the oral cavity: occult metastases, neck recurrences, and adjuvant treatment of pathologically positive necks.

BACKGROUND: Supraomohyoid neck dissection (SOHND) is currently performed in patients with carcinoma of the oral cavity with clinically negative neck. Most investigators consider SOHND as a staging procedure. METHODS: Records of 100 patients with cancer of the oral cavity and clinically negative neck undergoing SOHND were reviewed. The rate and significance of occult metastases are evaluated, the neck recurrences are analyzed and the indication of adjuvant radiation of pN+ necks is discussed. RESULTS: In 34 of 1814 of analyzed lymph nodes, metastatic disease was detected as follows: 30 macrometastases and 4 micrometastases. In 13 of 34 metastases (38%), extracapsular spread was observed. Twenty of 100 patients (20%) had to be upstaged. In 9 of 87 (10%) patients without local recurrence and with a minimal follow-up of 24 months, 5 ipsilateral (4 within the dissection field) and 5 contralateral neck recurrences were observed. Regional recurrence developed in 4% and 35% of patients with pN0 and pN+ necks, respectively. CONCLUSIONS: In 20% of patients with oral cavity tumors and pN0 neck, occult metastases were disclosed. Neck recurrences developed significantly more often in patients with pN+ than in those with pN0 necks. To evaluate the exact indication for an adjuvant treatment of patients with cN0/pN+ necks, prospective studies should be performed.

Postoperative radiation therapy for T1 and T2 primary parotid carcinoma: is it useful?

OBJECTIVE: The benefit of postoperative radiation for advanced primary parotid carcinoma has been reported previously, whereas studies to evaluate the usefulness of postoperative radiation for T1 and T2 parotid carcinomas have never been performed. STUDY DESIGN AND SETTING: Retrospective analysis on 58 previously untreated patients with T1 and T2 parotid carcinomas. In 34 patients, postoperative radiation was included in the treatment protocol and in 24 patients, no postoperative radiation was applied. RESULTS: A local recurrence was observed in 8 of 24 (33%) patients without and in 1 of 34 (3%) patients with postoperative radiation (P < 0.5). The 5-year actuarial and disease-free survival rate was 83% and 70% for patients without postoperative radiation and 93% and 92% for patients with postoperative radiation. CONCLUSION AND SIGNIFICANCE: Local recurrence was less often observed in patients with postoperative radiation. Nevertheless, prospective randomized studies are needed to confirm the usefulness of postoperative radiation in early carcinomas. EBM RATING: B-3b.

Radionecrosis or tumor recurrence after radiation of laryngeal and hypopharyngeal carcinomas.

OBJECTIVE: To analyze the incidence and diagnostic difficulties of radionecrosis vs tumor recurrence of laryngeal and hypopharyngeal carcinomas. STUDY DESIGN AND SETTING: Retrospective study on 341 patients treated by radiation alone or radiochemotherapy. The clinicopathologic findings, work-up, treatment, and follow-up of 20 patients with symptoms suggestive but negative for tumor recurrence on initial imaging studies and endoscopy were analyzed. RESULTS: The incidence of chondroradionecrosis in 341 irradiated patients was 5%. Ten of 20 patients initially negative for tumor recurrence were treated by total laryngectomy; in all laryngectomy specimens, chondroradionecrosis was present, in six specimens associated with tumor recurrence. Ten patients were treated by tracheotomy and tumor recurrence was detected in one patient during follow-up. CONCLUSION: Chondroradionecrosis is a relatively rare treatment complication. Typical imaging findings suggestive of radionecrosis are often missing. Tumor recurrence may be present beneath an intact mucosa and missed by endoscopy.

A novel computational method for real-time preoperative assessment of primary dental implant stability.

A novel methodology which allows for fast and fully automatic structural analysis during preoperative planning for dental implant surgery is presented. This method integrates a fully automatic fast finite element solver within the framework of new concepts in computer-assisted preoperative planning for implant surgery. The planning system including optimized structural planning was validated by experimental results. Nine implants were placed in pig mandibles and mechanically loaded using a testing rig. The resulting displacements were measured and compared with those predicted by numerical analysis during planning. The results show that there were no statistically significant differences (P = 0.65) between the results of the models and the experiments. The results show that fast structural analysis can be integrated with surgical planning software allowing the initial axial implant stability to be predicted in real time during planning. It is believed that such a system could be used to select patients for immediate implant loading and, when further developed, be useful in other areas of preoperative surgical planning.

Improved targeting device and computer navigation for accurate placement of brachytherapy needles.

Successful treatment of skull base tumors with interstitial brachytherapy requires high targeting accuracy for the brachytherapy needles to avoid harming vital anatomical structures. To enable safe placement of the needles in this area, we developed an image-based planning and navigation system for brachytherapy, which includes a custom-made mechanical positioning arm that allows rough and fine adjustment of the needle position. The fine-adjustment mechanism consists of an XYZ microstage at the base of the arm and a needle holder with two fine-adjustable inclinations. The rotation axes of the inclinations cross at the tip of the needle so that the inclinational adjustments do not interfere with the translational adjustments. A vacuum cushion and a noninvasive fixation frame are used for the head immobilization. To avoid mechanical bending of the needles due to the weight of attached tracking markers, which would be detrimental for targeting accuracy, only a single LED marker on the tail of the needle is used. An experimental phantom-based targeting study with this setup demonstrated that a positioning accuracy of 1.4 mm (rms) can be achieved. The study showed that the proposed setup allows brachytherapy needles to be easily aligned and inserted with high targeting accuracy according to a preliminary plan. The achievable accuracy is higher than if the needles are inserted manually. The proposed system can be linked to a standard afterloader and standard dosimetry planning module. The associated additional effort is reasonable for the clinical practice and therefore the proposed procedure provides a promising tool for the safe treatment of tumors in the skull base area.

Automatic method to assess local CT-MR imaging registration accuracy on images of the head.

BACKGROUND AND PURPOSE: Precise registration of CT and MR images is crucial in many clinical cases for proper diagnosis, decision making or navigation in surgical interventions. Various algorithms can be used to register CT and MR datasets, but prior to clinical use the result must be validated. To evaluate the registration result by visual inspection is tiring and time-consuming. We propose a new automatic registration assessment method, which provides the user a color-coded fused representation of the CT and MR images, and indicates the location and extent of poor registration accuracy. METHODS: The method for local assessment of CT-MR registration is based on segmentation of bone structures in the CT and MR images, followed by a voxel correspondence analysis. The result is represented as a color-coded overlay. The algorithm was tested on simulated and real datasets with different levels of noise and intensity non-uniformity. RESULTS: Based on tests on simulated MR imaging data, it was found that the algorithm was robust for noise levels up to 7% and intensity non-uniformities up to 20% of the full intensity scale. Due to the inability to distinguish clearly between bone and cerebro-spinal fluids in the MR image (T1-weighted), the algorithm was found to be optimistic in the sense that a number of voxels are classified as well-registered although they should not. However, nearly all voxels classified as misregistered are correctly classified. CONCLUSION: The proposed algorithm offers a new way to automatically assess the CT-MR image registration accuracy locally in all the areas of the volume that contain bone and to represent the result with a user-friendly, intuitive color-coded overlay on the fused dataset.

Nasal and paranasal sinus manifestation of Rosai-Dorfman disease.

Rosai-Dorfman disease is a rare, idiopathic, benign, histiocytic proliferation usually seen in younger patients. Most commonly neck lymph nodes are involved with a predominant infiltration of sinusoidal histiocytes. Nearly half of the patients have extranodal manifestation. Two cases of Rosai-Dorfman disease involving the nose and the paranasal sinuses are presented. The clinical presentation, radiographic findings, treatment and histological characteristics of the disease are discussed.

Identification of selective basophil chemoattractants in human nasal polyps as insulin-like growth factor-1 and insulin-like growth factor-2.

In a search for novel leukocyte chemoattractants at sites of allergic inflammation, we found basophil-selective chemoattractant activity in extracts of human nasal polyps. The extracts were fractionated by reverse phase HPLC, and the resulting fractions were tested for leukocyte-stimulating activity using sensitive shape change assays. The basophil-selective activity detected was not depleted by a poxvirus CC-chemokine-binding protein affinity column. This activity was further purified by HPLC, and proteins in the bioactive fractions were analyzed by tandem electrospray mass spectrometry. Insulin-like growth factor-2 (IGF-2) was identified in these HPLC fractions, and the basophil-stimulating activity was inhibited by an anti-IGF-2-neutralizing Ab. Recombinant IGF-2 induced a substantial shape change response in basophils, but not eosinophils, neutrophils, or monocytes. IGF-2 stimulated chemokinesis of basophils, but not eosinophils or neutrophils, and synergized with eotaxin-1/CCL11 in basophil chemotaxis. IGF-2 also caused up-regulation of basophil CD11b expression and inhibited apoptosis, but did not stimulate degranulation or Ca(2+) flux. Recombinant IGF-1 exhibited similar basophil-selective effects as IGF-2, and both growth factors were detected in nasal polyp extracts by ELISA. This is the first demonstration of chemokinetic factors that increase the motility of basophils, but do not act on other granulocytes or monocytes. IGF-1 and IGF-2 could play a role in the selective recruitment of basophils in vivo.

New method to assess the registration of CT-MR images of the head.

Due to their complementary information content, both x-ray computed tomography (CT) and magnetic resonance (MR) imaging are employed in certain clinical cases to improve the understanding of pathology involved. o spatially relate the two datasets, image registration and image fusion are employed. However, registration errors, either global or local, are common and are nonuniform within the image volume. In this paper, we propose a new algorithm that assesses the quality of the registration locally within the CT-MR volume and provides visual, color-coded feedback to the user about the location and extent of good and bad correspondence between the two images. The proposed registration assessment algorithm is based on a correspondence analysis of bone structures in the CT and MR images. For that purpose, a custom segmentation algorithm for bone in MR images has been developed that is based on a stochastic threshold computation method. This segmentation method for MR images and the CT-MR registration assessment algorithm were validated on simulated MR datasets and real CT-MR image pairs of the head. Some partial-volume effects occur at the borders of the bone structures and at the bone interfaces with air, which cannot be separated from bone in the MR image. The presented assessment method of CT-MR image registration offers the user a new tool to evaluate the overall and local quality of the registration. With this information, the user does not have to blindly trust the fused CT-MR datasets but can easily identify areas of inaccurate correspondence. The application of the algorithm is so far limited to T1-weighted MR and CT images of the head area.

Internal carotid artery aneurysm with life-threatening hemorrhages in a pediatric patient: endovascular treatment options.

PURPOSE: To describe the management of a pediatric patient with recurrent, life-threatening nasopharyngeal hemorrhages due to a pseudoaneurysm of the internal carotid artery (ICA), most likely caused by deep neck infection following peritonsillar abscess. CASE REPORT: An 11-year-old boy presented with lymphadenopathy of the neck, torticollis, and difficulty swallowing that had persisted for some weeks. After sneezing, the patient suffered massive recurrent nasopharyngeal hemorrhages leading to anemia. Computed tomography identified a 2 x 2-cm hematoma in the left parapharyngeal space; angiography revealed a saccular aneurysm (14.1 x 8.2 mm) of the extracranial ICA adjacent to the skull base, without ongoing hemorrhage. A bare Wallstent was initially implanted, but total exclusion was achieved only after secondary placement of a Symbiot covered stent within the bare stent. CONCLUSIONS: This case demonstrates the utility of an endovascular approach to an extracranial ICA pseudoaneurysm in a pediatric patient.

A-mode ultrasound-based registration in computer-aided surgery of the skull.

OBJECTIVE: To evaluate the integration and accuracy of A (amplitude)-mode ultrasound-based surface matching for noninvasive registration of the head into a frameless computer-aided surgery system for otorhinology and skull base surgery. DESIGN: Experimental study and case series. SETTING: Academic medical center. PATIENTS: Twelve patients underwent anterior and paranasal skull base surgery with the routine use of a computer-aided surgery system. INTERVENTIONS: A computer-aided surgery system, based on an optoelectronic localizer, was used to track the skull and the surgical tools, including the A-mode ultrasound probe. The A-mode probe was a 10-MHz immersion transducer. An acoustic lens attached to the transducer focused the ultrasonic beam to a depth of 1 to 10 mm. Accuracy tests were performed for the ultrasound setup. Different surface point distributions were evaluated with respect to matching accuracy on a human cadaver skull specimen equipped with fiducial markers. The matching comparison was based on the fiducial registration error. For the clinical evaluation, the laboratory setup was transferred to the operating room. MAIN OUTCOME MEASURES: Noninvasive registration of the skull by using A-mode ultrasound in computer-aided surgery (practical and clinical measurements). RESULTS: The accuracy tests on the human skull specimen revealed that the mean +/- SD fiducial registration error was 1.00 +/- 0.19 mm in the best series for A-mode ultrasound surface matchings and was robust with respect to different sets of surface points. The mean +/- SD root mean square error from the 12 A-mode ultrasound matchings in the patient study was 0.49 +/- 0.20 mm. CONCLUSION: A-mode ultrasound surface matching can be used as a noninvasive and accurate registration procedure in computer-aided surgery of the head.

Neck dissection shoulder syndrome: quantification and three-dimensional evaluation with an optoelectronic tracking system.

Objective 3-dimensional biomechanical changes of the shoulder at rest or during arm elevation were measured by means of a new specific method using an optoelectronic detection system that was developed for computer-aided surgery. Additionally, the shoulder syndrome following neck dissection was evaluated by the recognized orthopedic shoulder Constant score. The statistical evaluation encompassed 12 patients with unilateral radical neck dissection (RND), 12 patients with unilateral modified radical neck dissection (MRND) with preservation of the accessory nerve, and 10 healthy subjects. The healthy shoulders showed normal kinematic behavior, the so-called "scapulohumeral rhythm" (SHR). After MRND, the static scapular position and SHR showed no significant 3-dimensional variations. In contrast, RND produced a highly significant scapular displacement at rest (p < .01) and a near-total abolition of SHR. The Constant scores were significantly lower after RND than after MRND (p < .01). Three-dimensional evaluation of the shoulder syndrome supports the Constant score, quantifying what can be measured objectively.

Medical treatment of nasal squamous papilloma with imiquimod cream.

Warts are common lesions. The prevalence and incidence of squamous papilloma of the nasal vestibulum are not known, and it is not certain how often human papilloma viruses are involved. Numerous medical or surgical treatment modalities have been reported in the literature with variable, sometimes unsatisfactory results. Imiquimod cream is a topically applied immunomodulator. It is used for warts and other skin tumours on different locations of the body. We report results of six patients treated with imiquimod five per cent cream for nasal squamous papilloma.

Valuable use of computer-aided surgery in congenital bony aural atresia.

Congenital aural atresia repair is difficult owing to unpredictable anatomy. Benefits may be gained from computer-aided surgery (CAS), but its exact role has yet to be clearly defined. This is a retrospective study of 18 patients with bony type C (Schuknecht classification) congenital atresia. In the first group (n = 9), repair was performed with CAS while in the second group (n = 9), similar intervention was applied without CAS. Intra- and post-operative clinical and audiological findings were compared. CAS computed tomography (CT) images correlated well with intra-operative findings giving the surgeon more security and reducing operative time by 25 minutes. In our estimation, CAS is valuable for type C congenital aural atresia repair. It serves as an educational tool and as a guide for the experienced surgeon in critical situations where anatomical landmarks are distorted and where access is limited.
