[Frequency of adenoid hypertrophy in children with allergic diseases].

Tonsils and adenoid are part of Waldeyer's ring, the basic function of which is antibody formation, they react later against a great variety of antigens. The role of enlarged adenoids and tonsils in upper airway obstruction has become well-established earlier, thus, adenoidectomy with, or without, tonsillectomy is one of the most commonly performed surgical procedures in the paediatric population. Research done in respect to Waldeyer's ring on people dealing at the same time with allergy, is seldom the subject of dissertation in polish as well as world wide medical literature. To examine the influence of atopy on the adenoid hypertrophy we studied 115 children ages 4 to 9 years, with an average age of 5.9 who had allergic rhinitis asthma or atopic dermatitis. The study suggested that allergic rhinitis is an important risk factor for an adenoid hypertrophy (AH) in children, therefore, early prevention of exposure to them may help reduce occurrence of AH.

[Serum immunoglobulin E levels in relation to Waldeyer's ring surgery].

Tonsils and the adenoid are part of Waldeyer's ring, the basic function of which is antibody formation, they react later against a great variety of antigens. Tonsillectomy and adenoidectomy procedures are among the oldest surgical procedures still performed today. The different immunoglobulin serum levels in patients having undergone extirpation of the tonsils, adenoids or both, have been studied before and after surgery by different authors, with discordant results. Immunoglobulin E (IgE) is important in allergic reactions and host defense against parasites. In this study we measured serum levels of immunoglobulin E before and 3 months after tonsillotomy, adenoidectomy, and adenotonsillectomy. There were statistically significant differences between pre and post-operative levels of serum IgE, which decreased after tonsillotomy, but we have not noticed any statistically significant differences in the IgE values after adenoidectomy and adenoidotonsillotomy with respect to those prior to surgery.

[The present methods of diagnosing adenoidal hypertrophy in children].

The role of enlarged adenoids and tonsils in upper airway obstruction has become well-established earlier, thus, adenoidectomy with, or without, tonsillectomy is one of the most commonly performed surgical procedures in the paediatric population. Since the difficulty to objectively assess the adenoidal tissue and the nasopharyngeal airway in children, it is necessary to find out an accurate method of measuring these structures pre-operatively. In this work, the following methods are described: recording of obstructive symptoms, transoral posterior rhinoscopic examination, radiological evaluation, such as lateral radiography of the adenoid and nasopharyngeal magnetic resonance imaging, transnasal fiberoscopic examination, acoustic rhinometry and other.

[The influence of birch pollen on adenoid hypertrophy in children with seasonal allergic rhinitis].

The adenoid is a part of Waldeyer's ring, the basic function of which is antibody formation, they react later against a great variety of antigens. Pediatric adenoidal obstruction of the nasal airway is associated with significant morbidity and is a frequent indication for surgery, but its relationship to allergy has not been often studied. To examine the influence of atopy on the adenoid hypertrophy we studied 9 children ages 4 to 8 years, who had hypersensitivity to birch pollen and the seasonal allergic rhinitis. We conclude that allergy and sensitivity to birch pollen and probably to other kinds of alrallergens is an important risk factor for a AH in children. Therefore, early prevention of exposure to them may help reduce occurrence of AH.

[The results of tympanometry in children with adenoid hypertrophy and coexisting allergy].

UNLABELLED: Adenoid, nasal and middle ear diseases are frequent health problems for young children. Allergic diseases of the upper respiratory tract are becoming one of the leading factors in the etiology of Eustachian tube dysfunctions and this dysfunction is the most significant etiologic factor of secretory otitis (OMS). To examine the influence of atopy on the OMS we studied 89 children ages 4 to 10 years, with an average age of 6.2 who had adenoid hypertrophy and pathologic audiometry. In 36 children the skin tests showed an allergy, mostly to different kinds of pollen or house dust. There was a statistically significant difference in tympanometry by chi-square analysis, between allergy positive and allergy negative group. Type B tympanogram was found in 72% children whit coexisting atopy and in 21% without atopy however type C only in 19% allergic children and in 70% children without atopy. CONCLUSION: Children with allergic disorders of upper respiratory tract should be investigated by tympanometry for OMS.

[Efficacy of the medical treatment of adenoid hypertrophy in children with house dust mite allergy].

Allergic sensitisation of the airways occurs in the mucosa of the shock organ, or in the lymphatic stations draining these structures. The lymphatic structure closest to the nasal mucosa in humans is the adenoid. Pediatric adenoidal obstruction of the nasal airway is associated with significant morbidity and is a frequent indication for surgery. Because efficacious medical alternatives to adenoidectomy are lacking, we assessed the potency of standard-dose topical nasal fluticasone propionate and Cetirizine in reduction of adenoidal obstruction of the nasal airway. To examine the influence of these medicines on the adenoid hypertrophy (AH) we studied 43 children ages 4 to 9 years, who had AH and house dust mite allergy. Properly administered aqueous nasal fluticasone propionate and Cetirizine in standard doses can significantly reduce adenoidal hypertrophy and nasal airway obstructive symptoms in atopic children.

[Zafirlukast in treatment of nasal polyps in patients with aspirin intolerant bronchial asthma--preliminary report].

Aspirin intolerance is characterized by polypous rhinosinusitis, bronchial asthma and adverse reactions to aspirin. The common intolerance to all cyclo-oxygenase inhibitors allows us to focus study of the pathogenesis of AI on the metabolism of arachidonic acid. Nasal polyposis is a chronic inflammatory condition that is mostly characterized by an infiltration of eosinophils. How this eosinophilic inflammation leads to polyp formation remains largely unclear. Eosinophils are the major source of cysteinyl leukotrienes--potent pro-inflammatory mediators they can by responsible for nasal blockade, rhinorrhoea and hyperresponsiveness. A role of prostaglandins (PGs) and leukotrienes (LTs) in the pathogenesis of nasal polyps has been recently suggested and leukotriene levels have been shown to be elevated in patients with asthma as well as in those with sinonasal polyposis and sinusitis. Because leukotriene receptor antagonists inhibit activity of endogenous leukotrienes and decrease tissue eosinophilia it is speculated that these drugs may be useful also in the management of nasal polyps. Our study was designed to determine the efficacy of zafirlukast, a cysteinyl leukotriene receptor antagonist, in nasal polyposis. We conclude that antileukotrienes might play a role in controlling polyposis and their symptoms, and they might be a alternative to long-term steroid therapy and repeated surgical therapy.

[A case of otogenic cerebellar abscess].

Cerebellar abscess is a serious complication of ear disease that requires early diagnosis and treatment. Computerized axial tomography with contrast enhancement is very useful for the diagnosis of these abscesses. This fact is illustrated by a case report of a cerebellar abscess secondary to chronic otitis media with cholesteatoma detected in one of our patients. On examination otorrhoea, fever, headache, vomiting and had hemilateral conductive hearing loss. The therapy included radical operation, excision of the abscess and antibiotic therapy.

[The occurrence of atopic hypersensitivity in children with adenoid hypertrophy].

UNLABELLED: Allergic sensitization of the airways occurs in the mucosa. The lymphatic structure closest to the nasal mucosa is the adenoid. Adenoid hypertrophy (AH) may cause significant morbidity in children but its relationship to atopy has not been intensively studied. To examine the influence of atopy on the adenoid hypertrophy we studied 134 children ages 4 to 8 years, with an average age of 5.4 who had AH. In the analysed group 102 children suffered from difficulty in nose breathing, 61 of children from recurrent upper respiratory tract infections, 51 from recurrent otitis and 40 from hearing disorders. On the basis of allergic and immunological examinations, in 30.6% atopy was confirmed--41 out of 134 children showed positive skin test for allergy, mostly to different kinds of pollen or house dust. From these positive patients the respective allergens could be determined by RAST tests in serum of 87%. Increased total IgE level in serum was confirmed only in 17.5%. 8.5% of children showed peripheral blood eosinophilia. Some children were qualified for adenotomy and removed adenoid was examined histopathologically. Pharmacological treatment was also recommended. CONCLUSIONS: The study shows that allergy and sensitivity to different kinds of allergens is an important risk factor for a greater degree of AH in children. Therefore, early prevention of exposure to them may help reduce occurrence of AH.

[Specific-pollen immunotherapy in the treatment of oral allergy syndrome in patients with tree pollen hypersensitivity].

UNLABELLED: Over the last three decades several authors have described the existence of an association between sensitivity to different pollens and sensitivity to diverse edible vegetables. Most patients with birch pollen allergy report oral allergy symptoms after eating fresh apples and other vegetable foods. Major birch pollen and apple allergens, Bet v 1 and Mal d 1, and other antigens are highly homologous; as a consequence, pollen-specific immunotherapy (SIT) might be expected to improve apple hypersensitivity. A prospective study was carried out in 27 birch pollen-sensitive patients with apple-induced OAS (oral allergy syndrome) who received injection immunotherapy for 24, or 36 months. Eight patients not submitted to SIT and followed up for 24-36 months were used as a control. Sixteen patients (59%) reported a significant reduction (37%) or a total disappearance (22%) of OAS symptoms after SIT. CONCLUSION: SIT with birch pollen extracts effectively reduces clinical apple sensitivity in some cases; these effects are not paralleled by a similar reduction of specific IgE and skin reactivity.
