Nonsurgical approaches for the treatment of periodontal diseases.

This article reviews nonsurgical approaches for the management of periodontal diseases. A brief review of the pathogenesis of periodontitis allows the reader to identify the potential points of intervention. Assessment of risk factors for periodontitis and the potential for risk reduction are introduced as the first part of a three-pronged approach to therapy. Antimicrobial approaches, including the use of mechanical therapy, antiseptics, and antibiotics, are described next. Host modulatory therapy is addressed as the fi-nal component of the nonsurgical approach to periodontal therapy. Clinical applications of these nonsurgical options are presented as part of the treatment strategy.

Clinical attachment level change as an outcome measure for therapies that slow the progression of periodontal disease.

Attachment levels are excellent indicators of past destruction of the periodontal attachment apparatus and can be used to monitor the progression of periodontitis. They have been used in clinical trials to monitor the efficacy of a variety of therapeutic modalities that may either slow the progression of periodontal disease or allow for regeneration of lost attachment and supporting structures. Inherent difficulties in accurately assessing attachment levels include inflammation, which causes coronal displacement of the gingival margin without a concomitant migration of the dentogingival epithelium to a level apical to the cementoenamel junction, and recessions, in which an obvious loss of attachment has occurred, but there is no increase in probing depth. Attachment level measurements are more frequently used as clinical end-points in clinical trials than by private practitioners to determine the periodontal status of patients and to monitor patient responses to periodontal therapy. Clinical attachment level measurements have been used in clinical trials to evaluate a systemic host modulatory agent, demonstrating their utility as surrogate markers of efficacy.

Host modulation: conceptualization to clinical trials and integration into clinical practice.

A better understanding of the pathogenesis of periodontitis has resulted in pharmacotherapeutic advancements, addressing both the microbes and the host response, leading to improved management of this chronic progressive disease by the dental practitioner. The adjunctive use of host-modulatory agents can enhance therapeutic responses, slow the progression of the disease, and allow for more-predictable management of patients. This article will review the pathogenesis and risk factors associated with periodontitis and address in detail the concept and clinical utility of host modulation as a therapeutic strategy.

Clinical applications for host modulatory therapy.

A better understanding of the pathogenesis of periodontitis has resulted in pharmacotherapeutic advancements that address both the microbes and the host response, leading to improved management of this chronic progressive disease by the dental practitioner. The adjunctive use of host modulatory therapy can enhance therapeutic responses, slow the progression of the disease, and allow for more predictable management of patients, particularly in those patients at increased risk caused by factors beyond the reach of conventional therapeutic approaches. This article reviews the pathogenesis and risk factors associated with periodontitis and addresses, in detail, the concept and clinical use of host modulation as a therapeutic strategy.
