[Effect of rilmenidine on intraocular pressure in rabbits, interaction with efaroxan and rauwolscine].

PURPOSE: Glaucoma is the main cause of blindness in the developed countries. Its progress can be diminished by decreasing intraocular pressure (IOP) using pharmacological or surgical treatment. Antiglaucoma agents, alpha 2-adrenergic's receptor agonists have been known for several years as IOP lowering. Due to the fact that the majority of them turned out to be imidazoline receptor agonists, it is worth checking if selective imidazoline receptor (l1) agonists alter IOP. Preliminary animal experiments show that they lower IOP. In our study we examined the influence of rilmenidine, a potent l1 receptor agonist, on IOP in rabbits. Furthermore, we tried to find out whether l1 and alpha 2 receptor antagonists (efaroxan and rauwolscine) counteract the pharmacological effect of rilmenidine. MATERIAL AND METHODS: The study was conducted on adult male White New Zealand rabbits. All the substances were administered topically, and IOP was measured by applanation tonometry after topical anaesthesia before and 1, 3 and 5 hours after drug instillation. RESULTS: Rilmenidine showed the lowering effect on IOP at the concentration of 0.4%. Efaroxan and rauwolscine partly inhibited rilmenidine effect. CONCLUSIONS: Rilmenidine is a potential antiglaucoma agent, though further studies are necessary.

[Coexistence of Alzheimer's disease with pseudoexfoliation syndrome PEX].

Alzheimer disease is a type of cerebral amyloidosis, which is most frequently recognized after the age of 80. In pathophysiology of Alzheimer's disease, Parkinson's disease, amyotrophic lateral sclerosis and age-related cataract changes in protein aggregation play important role, for that reason they form common group of diseases, called conformational diseases. Patients with Alzheimer disease have more common pseudoexfoliation syndrome (PEX), characterized by the accumulation of an abnormal pathognomonic material in the anterior segment of the eye. Regarding common feature of the structure of fibrillar pseudoexfoliation and amyloid material, it is considered that eye examination could be useful in the early diagnosis of Alzheimer disease.

[Obstructive sleep apnea syndrome as a risk factor of glaucomatous neuropathy].

The consequences of sleep apneas in patients with obstructive sleep apnea (OSA) syndrome may influence the function of visual system. In 21 patients with OSA syndrome (apnea/hypopnea index--AHI 50+/-20) the detailed ophthalmologic studies, including tonometry, biomicroscopic evaluation of the anterior segment of the eye, automatic perimetry and stereoscopic examination of the eye fundus have been performed. In 4 patients (19%) the signs of lesions in the optic tract have been found. These were: defects in visual field caused by glaucoma (primary open-angle glaucoma and normal-tension glaucoma) in 2 patients and defects in visual field (concentric and quadrantic homonymous) caused by diffuse lesions in the cortico-nuclear tract in 2 patients. In patients with the lesions in the optic tract there were severe disorders of breathing during sleep: AHI >60, mean SaO2 at the end of the apneas <86% and minimal SaO2 at the end of the apneas <70%. The authors conclude that in OSA patients there is a high risk of the lesions of the optic tract as a consequence of severe and repetitive hypoxemia during sleep.

[Histopathologic studies in experimental uveitis].

UNLABELLED: Experimental uveitis is one of the main models in the diseases of autoimmunological background. The purpose of this paper was to analyze the quantitative histological changes in the experimental uveitis, induced by different types of homogenous endotoxin salts of Havnia alvei. We studied 74 eyes of Lewis rats (males) divided into 4 groups. Each group received a homogenous salt of Havnia alvei in a single subcutaneous injection. In the 1 group-LPS Ca++, in 2 group-LPS Na++, in 3 group-LPS 981, in 4 group physiological salt (control group). The histologic and immunocitochemical examinations were performed after 24, 48 hours, and after 4 and 7 days following the injection. The histologic changes were analyzed (he intensity of inflammatory reaction) using a Highly Optimazed Microscope Enviroment system. The most intensive inflammation was observed in experimental group after 24 hours (n the LPS 981D group in 5 rats out of 6, in LPS Ca++ group in 3 rats out of 6). After 48 hours the intensity of inflammatory reaction visibly decreased. On the fourth day the inflammation revealed a minimal intensity and after 7 days was practically absent. In the control group minimal inflammation was observed only in a few rats. Cilliary body hypermia was present for 48 hours in most of the experimental rats. Only a few of them had hyperemia on the fourth day. In the posterior segment minimal inflammation was noted at the end of the first day (I and II-nd group after 24 hours); this process continued until the fourth day, on the 7th day disappeared. CONCLUSIONS: The most intensive inflammation of the anterior and posterior choroidal segment is caused by homogenous salts of Havnia alvei 981.

[Ocular symptoms of autoagression in patients with idiopathic inflammatory bowel disease].

PURPOSE: of this research was to present a prospective estimation of the ocular symptoms appearance in a group of patients with ulcerous colitis (CU) or Crohn disease (CD). It was also, to demonstrate dependence between the frequency, intensity, and duration of the main disease and the way of its treatment. The research program established a retrospective analysis of symptoms presented typically in non-specific colitis in a group of patients with recurrence acute iridocyclitis (IA) and HLA B27 positive. MATERIALS AND METHODS: The research group contained 63 patients: 51 with CU and 12 with CD. Clinical symptoms and inflammatory indicators such as CRP, OB, pulse, and body temperature were taken into consideration by designation of intensity of main disease. Second group contained 28 patients with IA and duration of CU or CD symptom was analyzed in it. RESULTS: Symptoms of iridocyclitis acute non granulomatous were observed in 7.9% of cases. In 6.3% episcleritis was diagnosed and in one patient with CU scleritis (1.5%). In the second group of patients only in one case (3.5%) typical symptoms for chronic colitis were presented. CONCLUSIONS: In patients with non-specific chronic colitis careful ophthalmological examination should be performed, especially oriented for uveitis diagnosis.

[Clinical use of the interaction between cyclosporine A and its inhibitors].

This article concerns cyclosporine A and cyclosporine--sparing agents. The drugs are evaluated due to their clinical usefulness in elevating cyclosporine blood level and their safety in long-term administration. We analyzed imidazole antifungal agents, calcium channel blockers and, additionally, pharmacological properties of cyclosporine A.

[The influence of nitric synthase inhibitor (L-NAME) on the intraocular pressure in endotoxin induced uveitis in rats].

We studied the influence of the nitric synthase inhibitor (L-NAME) on the induction of uveitis in Wistar rats. The inflammation was induced by a footpad injection of endotoxin in rat. The clinical signs were evaluated in a slit-lamp, the intraocular pressure was measured using Tono Pen tonometer. We showed, that in endotoxin-induced uveitis the intraocular pressure elevation was observed and that L-NAME application reduced the inflammation and the level of the intraocular pressure.

[The role of color vision disturbances in diagnostics of early diabetic retinopathy].

PURPOSE: The evaluations of color vision sensitivity in children with type I diabetes mellitus without retinopathy. MATERIAL AND METHOD: We examined 96 young patients. They was divided into three groups: I: 35 children from 7 to 16 years old with insulin-dependent diabetes mellitus duration of 1-8 years, II: 30 children with type I diabetes lasting more then 8 years, III--31 non-diabetic subjects as a control-matched for age and sex, without visual or systemic symptoms. The examinations of colour vision sensitivity were done with the IF-2AII-color Anomaloscope. In all cases were tested the dynamic blue-green equation of Moreland and two variables were determined: setting (matching) range (SR), calculated mid point (matching mid point) (CMP). RESULTS: In the blue-green equation setting range (SR) was significantly (p < 0.01) enlarged in the II group (diabetes mellitus duration > 8 years) and calculated mid point (CMP) was shifted but no significant. The results indicate a diminution of the colour discriminating sensitivity in the short wavelength half of the visible spectrum and diminution of the blue cone sensitivity in early diabetic retinopathy. CONCLUSIONS: Blue-green colour vision testing with the anomaloscope may serve as an additional test in the diagnosis of early diabetic retinopathy in children without vascular changes at the eye fundus.

Retinal antigens are recognized by antibodies present in sera of patients with multiple sclerosis.

Multiple sclerosis (MS) is frequently accompanied by visual symptoms including those related to retinal disorders. Since they may be a consequence of an autoimmune reaction, we examined whether sera of patients with diagnosed MS and changes in visual-evoked potentials contain antibodies against retinal antigens (retAgs). Immunoblot analysis revealed that MS sera recognized mainly a 46-kD antigen, a 41-kD antigen, retinal arrestin, to a smaller extent also 70-, 56-, 43-, and 36-kD proteins. Patients whose sera showed the highest reactivity with 41- and 46-kD antigens had deficiencies in visual acuity, visual fields, ophthalmoscopy, and electroretinograms. Our observation suggests that antibodies to these retAgs may play a role in the origin of ophthalmologic impairment in MS.

[Influence of nocturnal hypoxemia on the function of the visual tract in the course of the obstructive sleep apnea syndrome].

BACKGROUND AND PURPOSE: In patients with obstructive sleep apnea (OSA) syndrome the episodes of upper airway obstruction lead to hypoxemia during sleep. The aim of the study was to establish the influence of sleep hypoxemia on the function of the visual tract in OSA patients. MATERIAL AND METHODS: The latency and amplitude of wave P100 of visual evoked potentials have been studied in 35 patients with OSA syndrome (mean apnea index 48+/-19). The diagnosis of OSA was established on the basis of continuous recordings of the respiratory function during sleep with additional full polysomnography in 17 patients. RESULTS: Mean absolute latency of P100 was longer in OSA patients than in healthy controls (117.0+/-8.8 ms vs. 104.3+/-4.6 ms, p<0.001). The differences in the amplitude of P100 were not significant (5.9+/-2.6 mV in OSA patients and 7.62+/-3.04 mV in healthy persons). In 60% of patients the latency of P100 exceeded 118 ms; in this group of patients the mean SaO2 during sleep apneas was lower than in patients with normal P100 latency (46+/-15% vs. 69+/-10%, p<0.05). Full polysomnographic studies revealed that in patients with prolonged latencies as compared with patients with normal P100 latencies there were lower: minimal SaO2 during NREM sleep (63+/-12% vs. 78+/-8%, p<0.05), as well as mean and minimal SaO2 during REM sleep (53+/-15% vs. 80+/-5% and 46+/-15% vs. 69+/-10%, p<0.05), without differences in apnea index or apnea duration. CONCLUSIONS: In patients with OSA syndrome the electrophysiological abnormalities suggesting damage of the optical tract may develop probably as a consequence of profound sleep hypoxemia.
