Pseudotumor cerebri complicating measles: a case report and literature review.

A previously healthy 8-year-old girl patient was referred with the complaints of severe headache associated with nausea and vomiting. Three weeks prior to her admission, she had measles manifested with fever and typical skin eruptions. Fundoscopic examination revealed bilateral swollen optic discs with tortuous blood vessels. Other physical examinations were unremarkable, except for photophobia. Lumbar puncture demonstrated a cerebrospinal fluid opening pressure of 30 cm H(2)O, no cells, normal levels of glucose and protein. Serum measles immunoglobulin M level was elevated (183 AU). Our further investigations revealed that the patient had pseudotumor cerebri (PTC) following measles infection.

An uncommon presentation: chronic meningococcaemia associated with cholestatic hepatitis in a Turkish child.

Chronic meningococcaemia is a very rare clinical manifestation of invasive infection by Neisseria meningitidis. A 9-year-old girl was admitted to our clinic with complaints of fever, headache, arthralgia, and maculopapular rash. The diagnosis was made by the growth of Neisseria meningitidis in the blood cultures. Four days after admission, liver function tests were increased and were compatible with cholestatic hepatitis. Thereafter, the patient was successfully treated and symptoms were completely resolved. To our knowledge, there have been no previous reports of Neisseria meningitidis causing cholestatic hepatitis. Herein, we present an unusual child patient with chronic meningococcaemia associated with cholestatic hepatitis.

Recurrent pneumococcal meningitis in homozygous C3 deficiency.

Congenital deficiencies of complement system proteins are rare. A 4-year-old girl was admitted for meningitis. She had had repeated attacks of pneumococcal meningitis and otitis media at the age of 3 years. Analysis of cerebrospinal fluid showed that this meningitis was due to pneumococcal infection. Complement 3 and CH50 values of the proband and her brother were low, while her parents were normal. The patient was given polyvalent pneumococcal and anti-haemophilus vaccines plus ceftriaxone. Recovery was complete after 15 days of antibiotic therapy. This is the first description of a case of recurrent meningitis with C3 and CH50 deficiency in a Turkish family.

Adrenal hemorrhage due to vitamin-K deficiency.

Adrenal hemorrhage has many etiologies including bleeding diathesis. A 10-day-old female baby was admitted to our clinic with the complaint of abdominal distention. Hemorrhage was determined on the right adrenal gland with abdominal ultrasonography and computerized tomography. Laboratory investigations showed PT 44 sec and aPTT longer than one minute. This article reports here an infant diagnosed as adrenal hemorrhage due to Vitamin-K deficiency presenting as an abdominal mass.

Pulmonary tuberculosis and cutaneous mycobacterial infection in a patient with incontinentia pigmenti.

Lupus vulgaris is reinfection tuberculosis of the skin and may result from direct extension, or hematogenous or lymphatic spread from a tuberculosis focus. Lupus vulgaris following bacille Calmette-Guerin (BCG) vaccination is a rare entity. Incontinentia pigmenti is an X-linked dominant genodermatosis in which vesicular, verrucous, and pigmented lesions are associated with various developmental defects. There is evidence of altered immunologic reactivity in some patients with incontinentia pigmenti. A 12-year-old girl hospitalized for pulmonary tuberculosis presented with bizarre-shaped brown macules following Blaschko lines on the left deltoid area, compatible with incontinentia pigmenti, which had appeared following BCG vaccination at the age of 7 years. Histopathologic examination found noncaseated granulomas in the dermis. Antituberculous treatment for pulmonary and cutaneous tuberculosis was initiated along with genetic counseling. Immunologic abnormalities have been reported in conjunction with incontinentia pigmenti. Simultaneous occurrence of pulmonary and cutaneous tuberculosis in our patient might be either coincidental or indicate derangements in the cellular immune system.

Meningitis due to Salmonella in preterm neonates.

Meningitis due to Salmonella is a rare condition. Here we report seven cases with neonatal Salmonella meningitis diagnosed and treated in the Pediatric Neonatology Unit of Ondokuz Mayis University Faculty of Medicine between January 1985-February 2001. Five cases were cured with appropriate treatment; in two of them no neurological sequelae were seen. Of the remaining three, reversible hydrocephalus occurred in two cases and subdural empyema was found in one. Two patients died on the 30th and 40th days of hospitalization: one of them had hydrocephalus and the other ventriculitis. In this report we point out the importance of neonatal meningitis due to Salmonella serotypes, though it rarely occurs.

Neonatal tetanus in the middle Black Sea region of Turkey.

We retrospectively analyzed 62 cases with neonatal tetanus (NT) seen in the Department of Pediatrics of Ondokuz Mayis University from 1989 to 2001. Epidemiological characteristics and prognostic factors on survival were investigated. We grouped the patients into two groups, the survivors and those who died. All patients were born in non-hygienic conditions, coming from rural regions. None of the mothers had been immunized against tetanus. The mortality rate was 40% (25 of 62 cases) and the only two poor prognostic factors on survival were age younger than five days at onset and the presence of fever. There was no significant difference between the two groups regarding the other known prognostic factors. Although the NT rate is declining, we must continue to protect against it by strictly enforcing preventive policies, especially in rural regions.
