Hypoplastic pancreas and ectopic spleen as an abdominal mass: a case report.

We report a 35-year-old female patient who presented with pseudotumoral abdominal mass with final diagnosis of ectopic spleen and hypoplastic pancreas. Clinical diagnosis is difficult due to lack of symptoms. Laboratory findings are commonly non-specific; diagnosis can be confirmed by imaging studies. This patient complained only of abdominal painless mass. The laboratory findings were all within normal limits. Diagnostic images revealed ectopic spleen and absence of the dorsal pancreas. This interesting and rare combination has not been reported previously in the literature.

ERCP subsequent to retroperitoneal perforation caused by endoscopic sphincterotomy.

BACKGROUND: Perforation occurs after endoscopic sphincterotomy in 0.4% of cases. With recognition of a perforation, the procedure usually is aborted and further attempts at ERCP are thought to be precluded by the complication. The aim of this study was to determine the timing and the outcome of ERCP after retroperitoneal perforation caused by endoscopic sphincterotomy when the initial ERCP was incomplete. METHODS: A total of 1787 patients underwent endoscopic sphincterotomy during a period of 29 months. A type II duodenal perforation was recognized in 15 patients, whereupon the ERCP, including further intervention, was halted. Eight patients agreed to undergo a second therapeutic ERCP to complete the treatment of the primary disease. OBSERVATIONS: Therapeutic ERCP was repeated in all patients from 11 to 15 days after the perforation. Treatment was successfully completed in all patients without complication. CONCLUSIONS: Therapeutic ERCP may be repeated and has a high success rate in patients who sustain a perforation caused by endoscopic sphincterotomy.

Inflammatory myoglandular polyp: a rare cause of hematochezia.

Inflammatory myoglandular polyp is characterized by inflammatory granulation tissue in the lamina propria, proliferation of smooth muscle, and hyperplastic glands with variable cystic change. The etiology is obscure. Mucous diarrhea, tenesmus, and hematochezia are main symptoms. We hereby report an 80-year-old man with diagnosis of inflammatory myoglandular polyp.

Two cases with hepatocellular carcinoma and spleen metastasis.

Metastasis of hepatocellular carcinoma to various organs is frequently seen. The lungs, regional lymph nodes, kidney, bone marrow and adrenals are relatively common sites of metastasis. However, metastasis to the spleen is extremely rare. Herein, two patients with hepatocellular carcinoma who developed splenic metastasis are described and distinctive features of spleen metastasis are summarized.

Experience with liver dialysis in acetaminophen induced fulminant hepatic failure: a preliminary report.

BACKGROUND/AIMS: Our aim was to assess the efficacy of the liver dialysis unit (LDU) in the treatment of patients with acetaminophen-induced fulminant hepatic failure. METHODS: Seventeen patients with acetaminophen-induced fulminant hepatic failure between January 1996 and December 2001 were retrospectively studied. A liver dialysis unit became available in our Unit in July 2000, and as of December 2001, four of these 17 patients had undergone treatment with liver dialysis. RESULTS: Mean age was 29 years (range: 14-47) and 76% were women. Four of 17 patients underwent a total of 12 (range: 1-4) LDU sessions. Ammonia level tended to be lower following the LDU and all four patients recovered without a need for orthotopic liver transplantation (OLT). Prior to July 2000, eight of 13 patients survived with supportive therapy, three expired and two underwent OLT. No major bleeding episodes were observed during the LDU course. A significant difference was found between the three cases who died and the cases that survived with respect to the grade of encephalopathy (p<0.001). There was significant difference in the admission serum creatinine among survivors (p<0.05). CONCLUSION: In conclusion, the Liver Dialysis Unit treatment appeared to benefit patients with acetaminophen-induced fulminant hepatic failure by reverse of the encephalopathy and spontaneous recovery of the damaged liver in selected patients.

Are serum and biliary carcinoembryonic antigen and carbohydrate antigen19-9 determinations reliable for differentiation between benign and malignant biliary disease?

BACKGROUND/AIMS: The value of serum tumor markers carbohydrate antigen 19-9 (CA19-9) and carcinoembryonic antigen (CEA) in the differential diagnosis of obstructive biliary disease is dubious. We aimed to define their usefulness prospectively. METHODS: Thirty-seven consecutive patients (12 female, 25 male, median age: 54, range: 19-83 years) who were referred for endoscopic retrograde cholanugiopancreatography (ERCP) or percutaneous transhepatic cholangiography (PTC) examination for obstructive jaundice were included (all with dilatation of biliary tree in ultrasonography or computerised tomography). Bile was obtained through the nasobiliary or external drainage catheter, placed with ERCP or PTC, respectively. Serum samples were taken from all patients at the time of acquisition of bile. Serum and bile samples were stored at -50 oC until they were tested. CA19-9 and CEA levels were measured with chemiluminescent enzyme immunoassay methods in serum and bile samples by using immulite GI-MA and CEA commercial kits, respectively (DPC(r), Los Angeles). RESULTS: In 22 patients with malignant disease, serum CEA levels were 38.6+/-115.8 ng/ml and CA19-9 were 386.9+/-409.7 U/ml, while in 15 patients with benign disease the serum CEA levels were 1.8+/-1.6 ng/ml and CA19-9 were 128.9+/-302.2 U/ml. The difference for both values was significant (p<0.05). In malignant disease bile CEA and CA19-9 levels were 160.8+/-457.8 ng/ml, 14000.9+/-19798 U/ml respectively, while in benign disease the corresponding levels were 21.08+/-48.6 ng/ml for CEA and 14818.9+/-24665.7 U/ml for CA19-9. The differences were not significant in this case (p>0.05). CONCLUSION: It was concluded that serum CA19-9 levels are increased both in malignant and benign obstructive biliary diseases, albeit more significantly in the former. However, increase in serum CEA is mostly restricted to malignant diseases. Measurement of these markers in bile is of no value.

Is celiac sprue an etiology of amyloidosis?

Gluten-sensitive enteropathy is a chronic disease due to malabsorption, and chronic inflammatory disorders may be a cause of secondary amyloidosis. We hereby report the first case with diagnosis of amyloidosis and gluten-sensitive enteropathy.

A rare entity in the rectum: malignant melanoma.

Malignant melanoma is a rare entity in the rectum. Long-term survival is rare, as most patients die of disseminated systemic disease regardless of treatment. For this reason, some authors do not consider extensive radical surgery as the treatment of choice. We describe here a case of this rare entity.

Unusual presentation of hepatic vascular tumors as fulminant hepatic failure.

Epithelioid hemangioendothelioma and angiosarcoma of the liver are rare neoplasms of vascular origin. They can present with nonspecific symptoms such as malaise and weight loss, as well as with liver-related symptoms such as abdominal pain, tender hepatomegaly and jaundice. Portal hypertension and rarely liver failure can occur. We hereby report two cases of fulminant hepatic failure that were eventually diagnosed with epithelioid hemangioendothelioma and angiosarcoma of the liver.
