Image fusion analysis of 99m Tc-HYNIC-octreotide scintigraphy and CT/MRI in patients with thyroid-associated orbitopathy: the importance of the lacrimal gland.

The aim of this study was to describe the anatomical structures that show uptake of the somatostatin analogue octreotide in patients with thyroid-associated orbitopathy (TAO). The study population comprised a series of 20 TAO patients attending the out-patient thyroid clinic and 12 patients presenting head or neck tumours. Scintigraphy was carried out with our newly developed tracer, technetium-99m labelled EDDA-HYNIC-TOC ((99m)Tc-TOC). Morphological imaging was done with either magnetic resonance imaging or X-ray computed tomography without contrast medium. Both imaging procedures were done within an interval of 3-4 weeks. For the image fusion procedure, specific external reference markers were used for each imaging modality. The markers were screwed onto a reference frame, which was held in place via a vacuum-fixed mouthpiece. The anatomical structure showing tracer uptake that was most frequently recognised was the lacrimal gland, followed by the retronasal area, cervical lymph structures, salivary glands, the anterior insertion points of the extra-ocular muscles and discrete areas of the neck extensor muscles. The lacrimal gland and the retronasal area showed the highest and most frequent uptake of (99m)Tc-TOC in TAO patients, whereas such uptake did not occur in the retrobulbar space. In spite of knowledge of these results of image fusion, no changes in the involved structures could be detected on morphological imaging. It is concluded that binding of (99m)Tc-TOC is more frequently localised to the anterior compartment of the eye and to the neck. The previously used term "orbital" uptake should be abandoned and replaced by a descriptive term relating to the anatomically recognised structure showing tracer accumulation, i.e. the lacrimal gland. The uptake of octreotide by lymphoid and salivary glands opens a new field of investigation related to the physiology of somatostatin.

In utero cocaine, discriminative avoidance learning with low-salient stimuli and learning-related neuronal activity in rabbits (Oryctolagus cuniculus).

Daily injections of cocaine administered to pregnant rabbits (Oryctolagus cuniculus) throughout gestation were associated with neural and behavioral changes during development and in adulthood, including altered neuron structure and function in areas receiving dopaminergic projections and retarded Pavlovian eyeblink conditioning with low-salient conditional stimuli. Studies of discriminative avoidance learning have shown changes in learning-related cingulothalamic neuronal activity, but no behavioral learning impairment in cocaine-exposed offspring. Here, low-salient stimuli were used during discriminative avoidance conditioning. Impairments early in behavioral acquisition were found, as well as alterations of anterior cingulate and medial prefrontal cortical, medial dorsal thalamic, and amygdalar neuronal response profiles and learning-related neuronal activity. These results elucidate the neural processes, impaired by prenatal cocaine, that support conditioning with low-salient stimuli.

Consideration of a unified model of amygdalar associative functions.

This paper considers issues arising from presentations by Barry Everitt and Gorica Petrovich concerning amygdalar involvement in appetitive conditioning. Studies reported by these researchers are beginning to unravel the distribution of associative functions among amygdalar nuclei and their efferent targets. The main issue addressed here concerns how to integrate the information from studies of appetitive conditioning with conclusions from studies of other forms of associative learning, including pavlovian fear conditioning and instrumental conditioning. Most students of this issue will agree that no prima facie integration is readily apparent in current scholarship. Although we do not here provide the definitive integration, we do attempt to take some preliminary, broad-brush steps towards that goal.

An intrapatient comparison of 99mTc-EDDA/HYNIC-TOC with 111In-DTPA-octreotide for diagnosis of somatostatin receptor-expressing tumors.

UNLABELLED: The aim of this study was to compare the imaging abilities of the recently developed somatostatin analog, (99m)Tc-hydrazinonicotinyl-Tyr(3)-octreotide ((99m)Tc-HYNIC-TOC [(99m)Tc-TOC]), with (111)In-diethylenediaminepentaacetic acid-D-Phe(1)-octreotide ((111)In-OCT [Octreoscan]) in patients undergoing routine somatostatin receptor (SSTR) scintigraphy. METHODS: Forty-one patients (20 men, 21 women; age range, 29-75 y; mean age, 56.7 y) with either histologically proven or biologically and clinically suspected endocrine tumors were enrolled in the study. Four groups were distinguished: (a) patients being evaluated for the detection and localization of neuroendocrine tumors (n = 6), (b) tumor staging (n = 19), (c) patients being investigated to determine the SSTR status of tumor lesions (n = 11), and (d) patient follow-up studies (n = 5). Each patient received a mean activity of 150 MBq (111)In-OCT and 350-400 MBq (99m)Tc-TOC. Scintigraphy with (99m)Tc-TOC was performed 4 h after injection and scintigraphy with (111)In-OCT was performed 4 and 24 h after injection. SPECT studies of areas of interest were performed 4 h after injection for both tracers as well as at 24 h after injection for (111)In-OCT. The time interval between the studies using each tracer ranged from 2 to 22 d (mean interval, 9.3 d). RESULTS: (111)In-OCT and (99m)Tc-TOC showed an equivalent scan result in 32 patients (78%), 9 cases showed discrepancies (22%), false-negative results with (111)In-OCT were seen in 6 cases (14.6%), whereas (99m)Tc-TOC was false-positive in 2 cases (4.9%). (111)In-OCT was true-negative in both cases. The false-positive findings of the (99m)Tc-TOC studies were caused by nonspecific uptake in the bowel. In 1 case, (99m)Tc-TOC correctly identified a metastasis in the lumbar spine but both scan results were false-positive because of an inflammatory process. In 21 patients with SSTR-expressing tumors, the semiquantitative region-of-interest analysis showed that (99m)Tc-TOC achieved higher tumor-to-normal tissue ratios than (111)In-OCT. CONCLUSION: This study revealed a higher sensitivity of (99m)Tc-TOC as compared with (111)In-OCT as an imaging agent for the localization of SSTR-expressing tumors. To avoid false-positive findings with (99m)Tc-OCT due to nonspecific tracer accumulation, additional scanning at 1-2 h after injection should be done.

Limbic thalamic lesions, appetitively motivated discrimination learning, and training-induced neuronal activity in rabbits.

A substantial literature implicates the anterior and mediodorsal (limbic) thalamic nuclei and the reciprocally interconnected areas of cingulate cortex in learning, memory, and attentional processes. Previous studies have shown that limbic thalamic lesions severely impair discriminative avoidance learning and that they block development of training-induced neuronal activity in the cingulate cortex. The present study investigated the possibility that the limbic thalamus and cingulate cortex are involved in reward-based discriminative approach learning, wherein head-extension responses yielding oral contact with a drinking spout that was inserted into the conditioning chamber after a positive conditional stimulus (CS+) were reinforced with a water reward but responses to the spout after a negative conditional stimulus (CS-) were not reinforced. In this task, the rabbits learned primarily to omit their prepotent responses to the spout on CS- trials. Acquisition was severely impaired in rabbits given limbic thalamic lesions before training. As during avoidance learning, posterior cingulate cortical neurons of control rabbits developed learning-related neuronal responses to task-relevant stimuli, but this activity was severely attenuated in rabbits with lesions. These results support a general involvement of the cingulothalamic circuitry in instrumental approach and avoidance learning. The fact that learning consisted of response omission indicated that the cingulothalamic role is not limited to acquisition or production of active behavioral responses, such as locomotion. It is proposed that cingulothalamic neurons mediate associative attention, wherein enhanced neuronal responses to stimuli associated with reinforcement facilitate the selection and production of task-relevant responses.

Effects of stretching before and after exercising on muscle soreness and risk of injury: systematic review.

OBJECTIVE: To determine the effects of stretching before and after exercising on muscle soreness after exercise, risk of injury, and athletic performance. METHOD: Systematic review. DATA SOURCES: Randomised or quasi-randomised studies identified by searching Medline, Embase, CINAHL, SPORTDiscus, and PEDro, and by recursive checking of bibliographies. MAIN OUTCOME MEASURES: Muscle soreness, incidence of injury, athletic performance. RESULTS: Five studies, all of moderate quality, reported sufficient data on the effects of stretching on muscle soreness to be included in the analysis. Outcomes seemed homogeneous. Stretching produced small and statistically non-significant reductions in muscle soreness. The pooled estimate of reduction in muscle soreness 24 hours after exercising was only 0.9 mm on a 100 mm scale (95% confidence interval -2.6 mm to 4.4 mm). Data from two studies on army recruits in military training show that muscle stretching before exercising does not produce useful reductions in injury risk (pooled hazard ratio 0.95, 0.78 to 1.16). CONCLUSIONS: Stretching before or after exercising does not confer protection from muscle soreness. Stretching before exercising does not seem to confer a practically useful reduction in the risk of injury, but the generality of this finding needs testing. Insufficient research has been done with which to determine the effects of stretching on sporting performance.

Her2/neu expression in C-cell hyperplasia and medullary thyroid carcinomas.

Medullary thyroid carcinomas (MTC) are rare neoplasms derived from the calcitonin-producing thyroid parafollicular C-cells. Interestingly about 20% of cases are related to inherited tumor syndromes. As precursor lesion, C-cell hyperplasia can be detected in numerous MTCs. In our series of 6 patients with MTC, including 5 patients with adjacent C-cell hyperplasia, Her2/neu levels were immunohistochemically evaluated on formalin-fixed, paraffin-embedded tissues using c-erbB-2/Her-2/neu Oncoprotein Ab-17 monoclonal antibody(mAb). Statistically, a highly significant correlation was found between high Her2/neu levels and extrathyroidal growth. C-cell hyperplasias always presented with higher amounts of stained cells than their corresponding invasive MTCs. Whereas in hyperplastic areas large stained cell clusters were found, the invasive tumors showed mainly single cells reacting in areas of variable size. These results present the impact of Her2/neu oncoprotein concerning development and biological behaviour, especially aggressive growth, of invasive MTCs and suggest a major role of hyperplastic C-cell areas in the development of these malignant tumors.

Her2/neu expression in poorly-differentiated and anaplastic thyroid carcinomas.

The Her2/neu expression in poorly-differentiated thyroid carcinomas (PDTC) and in anaplastic thyroid cancer (ATC) was investigated retrospectively. Tumors of 25 patients suffering from PDTC and 25 from ATC were evaluated, including 15 cases of PDTC with highly-differentiated tumor parts. Her2/neu levels were immunohistochemically detected on formalin-fixed, paraffin-embedded tissues using c-erbB-2/Her2/neu Oncoprotein Ab-17 monoclonal antibody(mAb). In statistical analysis using 1-way ANOVA, Her2/neu protein overexpression was highly significantly correlated with the differentiation status of the investigated tumor parts. Whereas ATCs showed only few positive tumor cells, PDTCs presented with large reacting tumor areas and highly-differentiated parts of PDTCs revealed the highest staining intensity. These findings suggest a decreasing impact of Her2/neu oncoproteins correlating to dedifferentiation, reflecting the aggressive biological behaviour of these tumors.
