Effects of cryogen spray cooling and high radiant exposures on selective vascular injury during laser irradiation of human skin.

BACKGROUND: Increasing radiant exposure offers a means to increase treatment efficacy during laser-mediated treatment of vascular lesions, such as port-wine stains; however, excessive radiant exposure decreases selective vascular injury due to increased heat generation within the epidermis and collateral damage to perivascular collagen. OBJECTIVE: To determine if cryogen spray cooling could be used to maintain selective vascular injury (ie, prevent epidermal and perivascular collagen damage) when using high radiant exposures (16-30 J/cm2). DESIGN: Observational study. SETTING: Academic hospital and research laboratory. PATIENTS: Twenty women with normal abdominal skin (skin phototypes I-VI). INTERVENTIONS: Skin was irradiated with a pulsed dye laser (wavelength = 585 nm; pulse duration = 1.5 milliseconds; 5-mm-diameter spot) using various radiant exposures (8-30 J/cm2) without and with cryogen spray cooling (50- to 300-millisecond cryogen spurts). MAIN OUTCOME MEASURE: Hematoxylin-eosin-stained histologic sections from each irradiated site were examined for the degree of epidermal damage, maximum depth of red blood cell coagulation, and percentage of vessels containing perivascular collagen coagulation. RESULTS: Long cryogen spurt durations (>200 milliseconds) protected the epidermis in light-skinned individuals (skin phototypes I-IV) at the highest radiant exposure (30 J/cm2); however, epidermal protection could not be achieved in dark-skinned individuals (skin phototypes V-VI) even at the lowest radiant exposure (8 J/cm2). The red blood cell coagulation depth increased with increasing radiant exposure (to >2.5 mm for skin phototypes I-IV and to approximately 1.2 mm for skin phototypes V-VI). In addition, long cryogen spurt durations (>200 milliseconds) prevented perivascular collagen coagulation in all skin types. CONCLUSIONS: Cryogen spurt durations much longer than those currently used in therapy (>200 milliseconds) may be clinically useful for protecting the epidermis and perivascular tissues when using high radiant exposures during cutaneous laser therapies. Additional studies are necessary to prove clinical safety of these protocols.

Tissue engineering.

Tissue engineering will potentially change the practice of plastic surgery more than any other clinical specialty. It is an interdisciplinary field that promises new methods of tissue repair. There has been more than $3.5 billion invested in this field since 1990. Relevant areas of progress include advanced computing, biomaterials, cell technology, growth factor fabrication and delivery, and gene manipulation. Beneficial clinical techniques will emerge from continued investigation in each of these areas. Techniques that are developed must be scaled up to industry with products cleared by regulatory agencies and acceptable to clinicians and patients. A goal of tissue engineering is to change clinical practice, yielding improved patient outcomes and lower costs of care.

Role of white matter lesions, cerebral atrophy, and APOE on cognition in older persons with and without dementia: the Cache County, Utah, study of memory and aging.

Neuropsychological, qualitative, and quantitative magnetic resonance imaging findings were examined in subjects with Alzheimer's disease (AD), non-AD dementia or mixed neuropsychiatric disorder, subjects characterized as mild/ambiguous, and controls, all with known apolipoprotein E (APOE) genotype. Neuropsychological tasks included an expanded Consortium to Establish a Registery for Alzheimer's Disease (J. T. Tschanz et al., 2000; K. A. Welsh, J. M. Hoffman, N. L. Earl, & M. W. Hanson 1994) battery and the Mini-Mental Status Examination (M. F. Folstein, S. E. Folstein, & P. R. McHugh, 1975). Periventricular white matter lesions were the most clinically salient, and generalized measures of cerebral atrophy were the most significant quantitative indicators. APOE genotype was unrelated to imaging or neuropsychological performance. Neuropsychological relationships with neuroimaging findings depend on the qualitative or quantitative method used.

The accuracy of stereolithography in planning craniofacial bone replacement.

Stereolithography can be used to produce physical models of the craniofacial skeleton from three-dimensional computed tomography (CT) data. The purpose of this study was to assess its accuracy for modeling osseous defects of the midface. Maxillary resections simulating unilateral maxillectomy (N = 3), bilateral maxillectomy (N = 3), and unilateral orbitomaxillectomy (N = 3) were performed as for sinus tumor resection on nine fresh cadaver skulls. Stereolithographic models (SLMs) were made from the specimen's CT data. The accuracy of SLMs was determined by comparing distances between key landmarks on the skulls and SLMs. Each SLM was grossly accurate with some loss of thin delicate structures. The mean differences in overall dimensions between the SLMs and skull specimens were 1.5 mm (range: 0-5.5 mm) for craniofacial measures, 1.2 mm (range: 0-4.8 mm) for skull base measures, 1.6 (range: 0-5.8 mm) for midface measures, 1.9 mm (range: 0-7.9 mm) for maxilla measures, and 1.5 mm (range: 0-5.7 mm) for orbital measures. The mean differences in defect dimensions were 1.9 mm (range: 0.1-5.7 mm) for unilateral maxillectomy, 0.8 mm (range: 0.2-1.5 mm) for bilateral maxillectomy, and 2.5 mm (range: 0.2-7.0 mm) for orbitomaxillectomy defects. Midface SLMs may be more prone to error than those of other craniofacial regions because of the presence of thin walls and small projections. Thus, one should consider designing midface bone replacements that are larger in critical dimensions than those predicted by preoperative modeling. These findings have important implications for the planning of current surgical methods as well as future applications of tissue-engineered bone replacement.

Continuous oxygen monitoring of mammalian cell growth on space shuttle mission STS-93 with a novel radioluminescent oxygen sensor.

A compact, flow-through oxygen sensor device based on luminescence quenching was used to monitor dissolved oxygen levels during mammalian cell growth on the STS-93 mission of the Columbia space shuttle. Excitation of an oxygen-sensitive ruthenium complex was provided by a radioluminescent light source (0.9 mm in diameter, 2.5 mm long), and the intensity of the resulting luminescence was measured by a simple photodiode detector. The use of radioluminescence for the excitation light source is a unique approach that provides many features important for long-term and remote monitoring applications. For the spaceflight experiment, human lung fibroblast cells (WI-38) were grown in hollow-fiber bioreactors. Oxygen concentration was measured in the flow path both before and after the bioreactor cartridge in order to gain information about the metabolism of the cells. The sensor was found to be nonperturbing to cell growth and withstood the challenging physical conditions of shuttle launch and landing while maintaining a stable calibration function. In addition, the sensor provided physically meaningful oxygen predictions.

Dementia, asymmetry of temporal lobe structures, and apolipoprotein E genotype: relationships to cerebral atrophy and neuropsychological impairment.

We examined asymmetry of hippocampal volume as well as other temporal lobe structures (temporal lobe, temporal horn of the lateral ventricular system, parahippocampal and fusiform gyri) in 194 subjects from the Cache County, Utah study, with varying disorders [85 with Alzheimer's disease (AD), 59 with some cognitive or neuropsychiatric disorder-referenced as a Mixed Neuropsychiatric group, 30 with mild ambiguous/mild cognitive impairment (MA/MCI) and 20 controls] and APOE genotypes. Asymmetry was determined by subtracting left-side volume from the right corrected by total intracranial volume. No significant asymmetry was observed to be associated with presence of the epsilon4 allele. Since the AD-epsilon4 allele risk effect may be expressed early in the course of the disorder, we also examined asymmetry indices in AD, MA/MCI and Mixed Neuropsychiatric subjects early in the course of their disorder (2 years or less) to those with longer duration illness (greater than 2 years). We observed a leftward asymmetry (i.e., left side larger) regardless of APOE genotype in hippocampal volume where both AD and MCI subjects demonstrated a leftward shift in hippocampal size when length of disease (LOD) was less but not more than 2 years. Leftward asymmetry was not associated with LOD in the Mixed Neuropsychiatric group. These findings do not support an association between epsilon4 and hippocampal asymmetry in dementia. We also examined whether asymmetry influenced neuropsychological performance, but minimal effects were observed. Where significance or strong trends were observed, better neuropsychological performance was associated with larger parenchymal volume of temporal lobe structures. These findings were interpreted as representing cognitive reserve effects where larger volume was protective against impairment. The role of asymmetry research in understanding neuropsychological performance in dementia is discussed.

Aesthetic and functional outcomes using osseous or soft-tissue free flaps.

The authors compared functional and aesthetic outcomes following reconstruction of the posterior mandible (i.e., unilateral defects distal to the mental foramen) with free flaps containing vascularized bone (VBFs) or only soft-tissue (STFs). Fifty-eight reconstructions of the posterior mandible were performed with STFs (n=32) or VBFs (n=26) and were available for review. All patients (n=58) completed a functional outcome questionnaire containing questions about diet, oral competence, pain, trismus, speech, cosmesis, social reintegration, and return to work. Computer-assisted image analysis was performed comparing facial proportions of patients with STFs (n=10), VBFs (n=11), or unoperated controls (n=10). VBFs yielded statistically significantly superior functional scores compared to STFs in 5/12 categories. Normal scores were found for diet in 31 percent (10/32) vs. 21 percent (3/26) ( p=0.006); oral competence in 56 percent (18/32) vs. 35 percent (9/26) ( p=0.05); and speech in 47 percent (15/32) vs. 30 percent (7/26) ( p=0.026). VBF patients returned to public dining (72 percent, 23/32) more frequently than STF patients (35 percent, 9/26), ( p=0.003). VBFs yielded superior results for midline symmetry with a mean mandible deviation of 3.3+/-2.3 mm vs. 5.8+/-3.6 mm (STFs), and 1.2+/-0.7 mm (controls, p=0.001). An objective method for the evaluation of aesthetic and functional outcomes in posterior mandible reconstruction was developed. This method demonstrated that posterior mandible reconstruction with VBFs is associated with superior results in speech, diet, and midline symmetry. However, rigorous analyses of oropharyngeal soft-tissue defects and the radiation therapy protocols utilized, were not performed in this study. Without these additional analyses, one cannot definitively conclude that VBFs are superior to STFs.

Limb salvage for soft-tissue malignancies of the foot: an evaluation of free-tissue transfer.

Free flaps may safely allow meaningful ambulation, durable limb preservation, and better quality of life in patients undergoing resections of soft-tissue cancers of the foot. To prove this, the records of a series of patients at The University of Texas M. D. Anderson Cancer Center (n = 67) who underwent limb salvage following tumor-related resection (n = 71 procedures) from 1989 to 1999 were retrospectively reviewed. Eighteen patients who were not candidates for local flaps or skin grafts received a total of 20 free flaps to preserve their limbs. Most defects (mean size, 78 cm2; range, 20 to 150 cm2) were on a weight-bearing surface of the foot (nine on a weight-bearing heel, three on a plantar foot); the remainder were on a non-weight-bearing surface (six on dorsum, two on a non-weight-bearing heel). Melanoma was diagnosed in nine cases (50 percent); soft-tissue sarcoma, in seven (39 percent); and squamous cell carcinoma, in two (11 percent). Fasciocutaneous and skin-grafted muscle flaps were used on both weight-bearing and non-weight-bearing surfaces. Free-tissue transfer was successful in 17 of 20 cases (85 percent); the three flap losses occurred in two patients. Minor complications (i.e., small hematoma, partial skin graft loss, and delayed wound healing) occurred in five patients. In all cases of successful free-tissue transfer, patients began partial weight bearing at a mean of 7.4 weeks (range, 2 to 12 weeks), and all ultimately achieved full weight bearing. Sixty-seven percent still required special footwear. In one patient, an ulceration on the weight-bearing portion of the flap resolved after a footwear adjustment. Only one patient was lost to follow-up (mean, 23 months). In the 17 remaining patients, limb salvage succeeded in 15 (88 percent). Of these, nine (60 percent) were alive without evidence of disease, three (20 percent) were alive with disease, and three (20 percent) had died of disease. Local recurrence developed in two patients but was successfully treated by excision and closure. No late amputations were required for local control. Thus, it seems that free flaps help facilitate limb salvage and that they may preserve meaningful limb function in patients who undergo resection of soft-tissue malignancies of the foot.

Traumatic brain injury and atrophy of the cingulate gyrus.

The medial surface areas of the cingulate gyrus (CG) and other midline structures (corpus callosum, thalamus, lateral ventricle) were examined in 27 traumatically brain injured (TBI) and 12 age- and gender-matched control subjects from an established TBI data base. Significant atrophy, primarily in the posterior CG, was found in TBI patients. Degree of atrophy was related to severity of injury. TBI subjects also had significantly reduced corpus callosum and thalamic cross-sectional surface areas with associated increased lateral ventricular volume, as well as reduced brain volume and increased ventricle-to-brain ratio. Despite significant atrophy of the posterior CG, neuropsychological performance was not related to changes in CG cross-sectional surface area in the TBI subjects. This apparent discrepancy is discussed.

Comparison of contact lens multipurpose solutions by in vitro sodium fluorescein permeability assay.

PURPOSE: This article reports the use of in vitro sodium fluorescein permeability assay to study the effects of various contact lens disinfecting solutions on the integrity of epithelium. METHODS: We compared the sodium fluorescein permeability of monolayer cultures of Madin-Darby canine kidney cells (MDCK) after exposure to ReNu Multi-Purpose Solution, ReNu MultiPlus Multi-Purpose Solution, OPTIFREE Express Multi-Purpose Solution, OPTIFREE Express Solution with Aldox, SOLO-care Solution and Complete Comfort Plus Solution. Additional cell monolayer inserts were prepared for each treatment and were analyzed using a scanning electron microscope. The assay has been used to measure the potential for ocular irritancy of a test solution by evaluating its effect on the cell membranes and tight junctions of a cultured canine kidney epithelial cell monolayer. RESULTS: The sodium fluorescein permeability assay and scanning electron microscopy demonstrated that the two OPTIFREE Express solutions were significantly more damaging to the MDCK epithelium than ReNu and ReNu MultiPlus multipurpose solutions (Bausch & Lomb, Rochester, NY), SOLO-care (CIBA Vision, Duluth, GA) solution, and Complete Comfort Plus (Allergan, Irvine, CA) solution. Cultures treated with the two OPTIFREE Express (Alcon, Ft. Worth, TX) solutions were more permeable to sodium fluorescein than cultures treated with the other multipurpose solutions, because of the loss of tight junctions and cell membrane damage. CONCLUSIONS: This experiment demonstrates that the sodium fluorescein permeability assay can be used to determine the effect that various contact lens disinfecting products have on the integrity of an epithelium in culture.
