A metaanalysis comparing the results of pallidotomy performed using microelectrode recording or macroelectrode stimulation.

OBJECT: There is an active debate regarding whether pallidotomy should be performed using macroelectrode stimulation or the more sophisticated and expensive method of microelectrode recording. No prospective, randomized trial results have answered this question, although personnel at many centers claim one method is superior. In their metaanalysis the authors reviewed published reports of both methods to determine if there is a significant difference in clinical outcomes or complication rates associated with these methods. METHODS: A metaanalysis was performed with data from reports on the use of unilateral pallidotomy in patients with Parkinson disease (PD) that were published between 1992 and 2000. A Medline search was conducted for the key word "pallidotomy" and additional studies were added following a review of the references. Only those studies dealing with unilateral procedures performed in patients with PD were included. Papers were excluded if they described a cohort smaller than 10 patients or a follow-up period shorter than 3 months or included cases that previously had been reported. The primary end points for outcome were the percentages of improvement in dyskinesias and in motor scores determined by the Unified PD Rating Scale (UPDRS). Complications were categorized as mortality, intracranial hemorrhage, visual deficit, speech deficit, cognitive decline, weakness, and other. There were no significant differences between the two methods with respect to improvements in dyskinesias (p = 0.66) or UPDRS motor scores (p = 0.62). Microelectrode recording was associated with a significantly higher (p = 0.012) intracranial hemorrhage rate (1.3 +/- 0.4%), compared with macroelectrode stimulation (0.25 +/- 0.2%). CONCLUSIONS: In reports of patients with PD who underwent unilateral pallidotomy, operations that included microelectrode recording were associated with a small, but significantly higher rate of symptomatic intracranial hemorrhage; however, there was no difference in postoperative reduction of dyskinesia or bradykinesia compared with operations that included macroelectrode stimulation.

Survey of antimalarial use in lupus pregnancy and lactation.

OBJECTIVE: To obtain the experience of selected experts in the use of antimalarial drugs (AM) in pregnancy and lactation in systemic lupus erythematosus. METHODS: Seventy-eight lupus experts identified from North America and UK were mailed a 19 question survey regarding their experience using AM in pregnancy and lactation. RESULTS: The 52 (67%) respondents with usable questionnaires treated a median of 75 lupus patients/year, including 4-5 lupus pregnancies/year. Thirty-five (69%) continued AM sometimes, often, or always during pregnancy. Continuing AM increased with the number of pregnant lupus patients seen (p < 0.01). None reported having seen any fetal toxicity with AM use, and pregnancy was never terminated because of AM, other than at patient insistence (n = 1). Postpartum, 29 (63%) continued AM and advised breast-feeding. Responses were consistent among North American and UK experts. CONCLUSION: The majority of lupus experts continue AM during pregnancy. This was particularly true for those who treated a larger number of pregnant lupus patients per year. The majority advise breast-feeding and continue AM postpartum. These practices are supported by the limited literature available.

Dietary measures and exercise training contribute to improvement of endothelial function and atherosclerosis even in patients given intensive pharmacologic therapy.

PURPOSE: Atherosclerosis contributes to cardiovascular mortality and morbidity even with aggressive lipid management. Our objective is to determine whether a combined pharmacological and lifestyle intervention can improve atherosclerosis. METHODS: We conducted a 2-year observational study at a specialized clinic in a tertiary care hospital. One hundred fifty-six subjects with coronary disease were enrolled in an intensive pharmacological management and lifestyle measures (including counseling and exercise training) program designed to reach specific targets. The main outcome measures were carotid intima media thickness and plaque area; brachial artery flow-mediated dilation; nitroglycerin-mediated dilation; flow-mediated dilation-nitroglycerin-mediated dilation ratio; laboratory parameters including lipids, glucose, creatinine, and homocysteine; and physical fitness. RESULTS: At completion, there were improvements in lipids and physical fitness. There were no overall changes in flow-mediated dilation, nitroglycerin-mediated dilation, or carotid intima media thickness in the entire cohort. However, multivariate logistic regression showed that dietary and exercise variables, such as increasing fiber intake and reducing body weight and body fat percentage, were independent predictors of improvements in endothelial function and carotid plaque burden. CONCLUSIONS: Even in the setting of intensive pharmacological therapy, lifestyle interventions, including exercise training and dietary changes, are important determinants of improved endothelial function and atherosclerosis.

Rising incidence of hospital-reported drug-facilitated sexual assault in a large urban community in Canada. Retrospective population-based study.

BACKGROUND: Drug-facilitated sexual assault (DFSA) occurs when an individual has been sexually assaulted due to the surreptitious administration of drug(s) thereby rendering her/him unable to give consent. Our study aim was to calculate the age- and sex-specific annual incidence of hospital-reported DFSA and to determine whether a one-year increase in DFSA observed in 1999 in a pilot study on the same population was a significant and sustained trend. METHODS: We identified cases of DFSA by reviewing the sexual assault examination records of all the individuals who presented to a hospital-based sexual assault care referral service in Vancouver, British Columbia during the study time period (January 1, 1993 to May 31, 2002). The annual sex- and age-specific incidence and temporal trends of drug-facilitated sexual assault were examined using population data from the British Columbia Ministry of Health. RESULTS: The mean annual incidence of female DFSA increased from 3.4 per 100,000 (years 1993--1998) to 10.7 per 100,000 (years 1999--2002). Age-adjusted relative risks for female DFSAs were significantly higher in 1999 (2.77, 95% CI 1.85-4.15), 2000 (3.01, 95% CI 1.97-4.57), 2001 (3.14, 95% CI 2.07-4.78) and 2002 (4.88, 95% CI 2.84-8.37) compared to 1993-1998. Women aged 15-19 years had the highest DFSA incidence, with a year-adjusted relative risk of 3.89 (95% CI 2.75-5.50) compared to all other age groups. CONCLUSION: This study demonstrates that the incidence of hospital-reported DFSA has shown a marked and sustained increase since 1999. Young women in their teens are particularly vulnerable to this form of sexual assault and further efforts are needed to develop and evaluate prevention programs for this group.
