Extension of corticocortical afferents into the anterior bank of the intraparietal sulcus by tool-use training in adult monkeys.

When humans use a tool, it becomes an extension of the hand physically and perceptually. Common introspection might occur in monkeys trained in tool-use, which should depend on brain operations that constantly update and automatically integrate information about the current intrinsic (somatosensory) and the extrinsic (visual) status of the body parts and the tools. The parietal cortex plays an important role in using tools. Intraparietal neurones of naïve monkeys mostly respond unimodally to somatosensory stimuli; however, after training these neurones become bimodally active and respond to visual stimuli. The response properties of these neurones change to code the body images modified by assimilation of the tool to the hand holding it. In this study, we compared the projection patterns between visually related areas and the intraparietal cortex in trained and naïve monkeys using tracer techniques. Light microscopy analyses revealed the emergence of novel projections from the higher visual centres in the vicinity of the temporo-parietal junction and the ventrolateral prefrontal areas to the intraparietal area in monkeys trained in tool-use, but not in naïve monkeys. Functionally active synapses of intracortical afferents arising from higher visual centres to the intraparietal cortex of the trained monkeys were confirmed by electron microscopy. These results provide the first concrete evidence for the induction of novel neural connections in the adult monkey cerebral cortex, which accompanies a process of demanding behaviour in these animals.

Treatment of typical Charles Bonnet syndrome with donepezil.

Charles Bonnet syndrome (CBS) is characterized by the presence of complex visual hallucinations in psychologically normal people. Although visual hallucinations in the elderly are often associated with dementia with Lewy body (DLB), Alzheimer's disease and delirium, they are excluded from the diagnosis of typical CBS, as are cognitive or psychiatric disturbances, sleep disorders and focal neurological lesions. Here, we describe a patient with typical CBS, who responded to donepezil, a cholinesterase inhibitor, and has not shown any symptoms suggestive of Alzheimer's disease or DLB for approximately the past 40 months. However, follow-up examination of her clinical symptoms is necessary for a definite exclusion of Alzheimer's disease and DLB. The effectiveness of donepezil indicates that the patient's visual hallucinations might be related to dysfunction of cholinergic neurones, although she did not exhibit any cognitive decline, or morphological and physiological brain pathology. Because donepezil has fewer adverse effects than anticonvulsants and neuroleptic drugs, it may be a safer option for the treatment of CBS in the elderly.

A case of small cell gastric carcinoma with an adenocarcinoma component operated curatively.

We present a case of a primary advanced gastric tumor that was composed of 2 different pathological components: small cell carcinoma and moderately-differentiated adenocarcinoma. The patient was still alive four years after the surgery was performed, without recurrence. A large part of the tumor consisted of a diffuse sheet of small cell carcinoma, which transitioned into another small portion consisting of moderately-differentiated tubular adenocarcinoma components. Therefore, this case raised the possibility that small cell gastric carcinoma may originate from totipotential stem cells of the stomach. Although small cell carcinoma progresses aggressively, and patients with it have an extremely poor prognosis, this patient recovered uneventfully after the surgical resection, and has remained in good health, without any recurrences.

Pulmonary Langerhans cell histiocytosis associated with lingual carcinoma.

A 57-year-old man who was a heavy smoker was admitted to our hospital for further evaluation of abnormal shadows on a chest X-ray film. Chest radiography and a computed tomography (CT) scan revealed nodular lesions and multiple thin-walled cysts in both lungs. Histopathological examination of one of these cystic lesions showed that the predominant cellular population was Langerhans cells, with the cytoplasm testing positive for S-100 protein and the cell membrane showing a positive reaction for CD1a. The pathological diagnosis was pulmonary Langerhans cell histiocytosis (LCH). A lingual carcinoma that had been detected simultaneously was treated with neoadjuvant therapy and the patient was advised to stop smoking. However, only limited improvement was seen on follow-up chest CT. In view of this, a radical resection of the lingual carcinoma was performed. There was a subsequent dramatic improvement in the pulmonary LCH. Langerhans cells may play a role in the immune response to tumors. In this patient, we suggest the possibility that both the habitual smoking and the lingual carcinoma may have contributed to the development of pulmonary LCH.

Rapid learning of sequential tool use by macaque monkeys.

Earlier reports have described monkeys in their natural habitat as being capable of purposefully using tools for activities such as obtaining food. However, little is known regarding the extent of macaque monkeys' ability to understand the functional meaning of objects as tools. We have trained Japanese macaques in tool-use behavior to demonstrate their abilities to solve stick problems involving the use of a novel tool and a sequential combination of different tools. Results suggest that macaque monkeys have cognitive abilities, such as (1). flexibility in applying previous experience in accordance with the requirements of the learning situation, (2). the foresight needed to conduct a series of acts in a continuous sequential manner and the ability to internally plan the necessary strategy.

Inferior parietal somatosensory neurons coding face-hand coordination in Japanese macaques.

Neuronal activities of the anterior part of the inferior parietal lobule (area 7b or PF) were investigated in five awake Japanese monkeys. There were neurons which had specific combinations of receptive field (RF) locations, most typically in both the face and hand; we refer to the seas Face-Hand neurons. The most interesting property of the Face-Hand neurons is that some of these neurons responded to specific behavior executed with synergism between the face (especially the mouth) and hand movements; namely, face-hand coordinated behavior (e.g., eating behavior). We call these cells Face-Hand coordination neurons (52% of all the Face-Hand neurons). These neurons discharged more strongly when the animal executed face-hand coordinated behavior, especially eating behavior, than when somatosensory stimuli were given to RFs passively, or when face movements and hand movements were executed separately. We thus propose that the neuronal activities of area 7b are related to the representation of face-hand coordination.

A living case of pulmonary ossification associated with osteoclast formation from alveolar macrophage in the presence of T-cell cytokines.

A 61-year-old woman had been coughing up blood-tinged sputum since May 1998. Chest radiography and computed tomography (CT) scans revealed a solitary mass (3 cm in greatest dimension) in the right lower field, accompanied by a surrounding area of ground glass and reticular appearance. Surgical lung biopsy was performed to the surrounding area. The pathological diagnosis was pulmonary ossification of the dendriform type. Alveolar macrophages obtained from her lung differentiated into tartrate-resistant acid phosphatase (TRAP)-positive multinucleated giant cells (MGCs) in the presence of autologous T cells or of macrophage colony stimulating factor (M-CSF) and interleukin-4 (IL-4). This results suggest the possibility that monocytes/macrophages may have the ability to form osteoclasts in the presence of cytokines that may be involved in the development of pulmonary ossification.

[A case of bronchopulmonary actinomycosis diagnosed by transbronchial biopsy of a bronchial polypoid tumor].

A 32-year-old man with history of dental caries had been coughing up blood-tinged sputum since Jan 2000. Chest radiography and computed tomography (CT) scans revealed a solitary nodule with a peripheral infiltrative shadow in the left S 6. Fiberoptic bronchoscopy was performed, but yielded no significant findings. In April 2000, because the nodule had increased in size, bronchoscopy was performed again, and revealed a white smooth-surfaced polypoid tumor in the left B 6 c. A transbronchial biopsy of the polypoid tumor was performed, and the histological findings show long Grocco-positive hyphae that are visible under sulfur granules. Bronchopulmonary actinomycosis was diagnosed. The solitary nodule with a peripheral infiltrative shadow in the left S 6 was eliminated by antibiotic therapy including ABPC/SBT. ABPC and LVFX. This case is important, because there are few reports concerning diagnosis of bronchopulmonary actinomycosis using transbronchial biopsy of a bronchial polypoid tumor.
