[Quality of life and therapy in the elderly patients on chronic peritoneal dialysis].

UNLABELLED: In this study quality of life adequacy of treatment was assessed in 32 PD patients over 65 years of age and compared with younger patients on PD (age 40 on average). Quality of life was examined with help of QLQ-C30 questionnaire, negative emotions with use of HADS and own Aggression Scale, life satisfaction on the Cantril's Ladder. Total weekly clearances were used for adequacy assessment as well as nutrition parameters and rate of anaemia and EPO use. Frequency of complications was also compared. Elderly patients were starting dialysis with lower GFR, but in the course of observation neither total clearances nor nutrition parameters and degree of anaemia did not differ between the groups. Elderly used EPO significantly less frequently in comparison to the younger patients. Peritonitis rate was similar and exit site infections were less frequent in elderly population. There was no difference between quality of life and level of negative emotions between the groups. IN CONCLUSION: PD appears to be effective and acceptable method of dialysis for the elderly.

[Psychological examinations of the personality of patients with cardiac syndrome X].

76 patients with chest pain including 27 men and 49 women from 25 to 75 (mean 49.96 +/- 11.36) years old, were examined by means of the psychological test. All the patients showed positive exercise test and no changes in coronary arteries. Psychological test was done by the use of personality--MMPI--2 (Minnesota Multiphasic Personality Inventory) test and depression was diagnosed by GDS (Geriatric Depression Scale) by J. Yesavage--15 questions version. The highest mean values in MMPI--2 were obtained in neurotic scales. It indicates that emotional disorders in these patients emerge as hypochondriacal and hysterical neurosis. In 37 patients (48.7%) neurotic scales showed typical "conversion valley" and these who show such a change in MMPI--2 talk mainly about somatic complains (not psychological) during long-term stress situation. In 25 (32.9%) patients single psychological symptoms of depression could be seen and depression episodes were diagnosed in 17 patients (22.4%).

[Carbamazepine in the treatment in psychiatric disorders based on retrospective clinical study].

Medical records of patient treated in I Department of Psychiatry Medical University of Gdansk in 1996-2000 were analysed. Carbamazepine was used in 383 patients, 209 male and 174 female. Mean age for the whole group 41.2 years, for male 40.6, for female 41.9. Carbamazepine was mainly used in affective disorders (F30-33) and organic affective disorders (F06.3)-107 persons. Second the most common indication was organic personality disturbances (F07.0)-121 persons. Carbamazepine was used in 100-1200 mg per day (mean dose 415 mg) during 5-189 days (mean 36 days). Treatment was well tolerated. Only 24 patients (6.3%) have mild side effects. Drug level in blood was checked in 19 patients during two last years of analysed material. Carbamazepine was used mainly with tricyclic antidepressants (30.3%), second generation antidepressive drugs (22.2%) and typical neuroleptics (11.1%). More than half of patients (55.6%) were recovered. Based on clinical material carbamazepine seems to be active agent in treatment of affective disorders and diseases connected with organic brain lesion.

[Relationship between medical students and patients during the course of psychiatry--expectations and fears].

AIM: The aim of this study was to discover methods of preparing medical students for contacts with mentally ill people and to create educational procedures considering the exceptionality of the course of psychiatry. METHODS: The examined group consisted of IVth, Vth, VIth-year students of the Medical University in Gdańsk. We have analysed a group of 121 students. To perform our study we have created a questionnaire with 22 questions (enclosed). CONCLUSIONS: We have founds as follows: 1. Most of the students were at first interested in the subject; 2. Almost all of the students have actively participated in conversation with the patients; 3. Many of the students were afraid of relations with the patients; 4. The most difficult cases for the students were schizophrenic psychoses and affective diseases; 5. In the students' opinion there should be less than 4-6 students in a group; 6. In the students' opinion the time spent on conversation with one patient was too short.

[Coping with stress in various rescue groups].

AIM: The main purpose of this study was checking if the above mentioned groups differentiate from each other according to the coping styles used by them; if yes, what does it depend on? METHOD: The Authors studied three groups of rescue services, representing three different formations. There were: Firemen, Ambulance services members and Policemen (preventive troupes). A few well known psychological methods were used, among them: CISS-Coping Inventory for Stressful Situations (in polish adaptation), State-Trait Anxiety Inventory (in polish adaptation) and Eysenck's-Maudsley Personality Inventory (in polish adaptation). RESULTS: The obtained results allowed to formulate some important conclusions. There were no statistically significant differences among the groups as far as the two coping styles were concerned--task and avoidance oriented. Emotionally oriented coping style is the one in which there are statistically significant differences between the researched groups. The Firemen group has the highest score in the emotional coping style. CONCLUSIONS: Using the coping styles (the most effective too), dependence mostly on the personality traits. It means that personality traits should be an essential condition in the way of accepting new members of rescue groups. The personality traits, beside the professional training, are responsible for effective coping in extremely difficult situations.

[Coping with stress in those who experienced a traumatic situation].

AIM: Does the traumatic experience influence the choice of a particular coping style? If yes, which style is preferred by those who experienced trauma? Answering these two essential questions is the aim of this paper. The authors have accepted the assumption (Holman, Silver, 1998), that the individuals who experienced trauma prefer past temporal orientation and they present a higher level of distress. METHOD: The authors investigated two groups. The experimental group consisted of 46 victims of the Gdańsk Shipyard concert hall fire. The control group comprised the 41 individuals who never experienced any trauma. Two psychological methods were used. CISS--Endler and Parker, which measures coping styles. STAI--Spielberger and al., was the second method used for measuring the level of anxiety as an essential distress indicator. RESULTS AND CONCLUSIONS: The results show, that individuals who experienced trauma, presented a significantly higher level of anxiety and that the victims of trauma prefer the emotional coping style. The difference is statistically significant. There is also an indirect conclusion (based on these results), which confirms the above mentioned assumption.

[Psychological changes in the victims of the Gdańsk Shipyard hall fire in the period of 1995-1999].

AIM: The aim of this paper was to answer two questions. First--has a particular traumatic event (the Gdańsk Shipyard concert-hall fire) caused any psychopathological changes in the victims? Secondly--if yes, how do the dynamics of these changes manifest themselves? METHOD: The authors used two psychological methods. Adjective Check List, which measures the structure of psychological needs, and in some way, the picture of oneself (before and after the fire) and HADS-M, which measures the intensity of negative emotions (anxiety, depressiveness and aggression). The first investigation in 1995 included two groups: 46 victims of the fire and a control group (47 youngsters, who did not participate in this particular concert). The investigation of the victims group was then repeated twice, in 1996 (35 persons) and in 1999 (39 persons). RESULTS: The series of investigations lead to the conclusion that the traumatic event (Gdańsk Shipyard hall fire) caused statistically significant, psychopathological changes in the victims (the first investigation). The changes concerned the worsening of the picture of oneself and an increase in the intensity of negative emotions (anxiety, depressiveness, aggression). All these changes still exist, despite the time which passed since the traumatic event (the repeated investigations).
