Stenting of symptomatic M1 stenosis of middle cerebral artery: an initial experience of 40 patients.

OBJECTIVE: To assess the safety and clinical efficacy of stenting for patients with symptomatic M1 stenosis of middle cerebral artery (MCA), and to assess the significance of classification based on location, morphology, and access of intracranial stenosis (LMA classification) in MCA stenting. METHODS: Forty patients with 42 symptomatic M1 stenoses refractory to medical therapy were enrolled in this study. The lesions were situated at M1 trunk (n=13), M1 origin (n=12), and M1 bifurcation (n=17), respectively, which were classified into type N (nonbifurcation lesions, n=13) and type A (prebifurcation, n=11), B (postbifurcation, n=14), C (lesion across the nonstenotic ostium of its branch, n=1), D (across the stenotic ostium of its branch, n=2), F (combinative lesions of prebifurcation and its small branch ostium, n=1) locations, morphologically into type A (n=15), B (n=23) and C (n=4) lesions, and into type I (mild-to-moderate tortuosity and smooth access, n=17), II (severe tortuosity and/or irregular arterial wall, n=18), and III (excessively severe tortuosity, n=7) accesses. RESULTS: The technical successful rate was 97.6% for total lesions and 100%, 100%, and 85.7% for types I, II, and III accesses, respectively. The total complication rate was 10%. The mortality was 2.5% (1/40 patients), and 0%, 0%, and 25% for types A, B, and C lesions, respectively. During the median 10 months follow-up, there was no recurrence of transient ischemic attack or stroke in 38 available patients. Among 8 stenting vessels of seven patients with six-month follow-up angiography, 7 showed good patency and one showed restenosis. CONCLUSIONS: Stenting appears to be an effective and feasible therapy for symptomatic M1 stenoses, but also appears to have the higher periprocedural complications, which need strict procedural and periprocedural management to reduce the mortality and morbidity. The LMA classification seems to be helpful to work out the individual therapy and predict the results of stenting. A further study is needed to confirm the benefits of stenting of MCA stenosis.

[Human antisense-vascular endothelial growth factor gene therapy for laryngeal tumor by cationic liposome-mediated transfection].

OBJECTIVE: To observe the effect of cationic liposome mediated antisense-vascular endothelial growth factor (VEGF) gene transfection on the growth of laryngeal cancer Hep-2 cells in the nude mice. METHODS: The VEGF-cDNA gene was cloned by reverse transcriptase polymerase chain reaction (RT-PCR) from human laryngeal cancer, and its eukaryotic expression vector pcDNA3-VEGF (-) with antisense-VEGF gene was constructed and identified by PCR and double-enzyme digestion. The pcDNA3-VEGF (-) was transfected into laryngeal cancer Hep-2 cell line by using cationic liposome (LP 2000). Then, the transfected Hep-2 cells were injected into nude mice and the size of tumor from different groups was observed while establishing laryngeal cancer xenografts in nude mice, and then treating the tumor-bearing mice with liposome-plasmid complex, observing the size of tumor from different groups. The expression of VEGF mRNA in different groups was observed by RT-PCR. The transfected cell ultranstructure was observed by transmission electron microscopy. RESULTS: The human VEGF-cDNA was successfully cloned and its eukaryotic expression vector with antisense-VEGF pcDNA3-VEGF (-) was constructed. The antisense-VEGF gene was transfected into Hep-2 cell line by using cationic liposome (LP2000). The size of tumor transfected with pcDNA3-VEGF (-) was significantly smaller than that of control groups. While the size of tumor treated with liposome-pcDNA3-VEGF (-) complex was significantly smaller than that of control groups. Many apoptic tumor cells were observed by transmission electron microscopy and the structure of microvessel was also changed. The expression of VEGF mRNA was evidently weaker than that of the control groups. CONCLUSION: The growth of Hep-2 cells could be inhibited significantly by antisense-VEGF gene transfection.

[Biodiv Ysio stent for symptomatic stenosis of middle cerebral artery].

OBJECTIVE: To assess the safety and feasibility of Biodiv Ysio stent for patients with symptomatic M1 stenosis of middle cerebral artery (MCA). METHODS: Forty-three patients with forty-five M1 stenoses, ranged from 50% to 99% in diameter reduction and refractory to medical therapy, were enrolled in this study between March 2002 and November 2003. The lesions were situated at M1 trunk (n = 19), M1 origin (n = 12) and M1 bifurcation (n = 14), respectively. RESULTS: The technical successful rate was 97.8% (44/45) for all the lesions. The rate of complicating subarachnoid hemorrhage was 7.0% (3/43 patients) and the rate of death was 2.3%. During a follow-up of 7.0 months (median), there was no recurrence of transient ischemic attack or stroke in 41 available patients. Six-month angiographic follow-up was obtained in 6 patients (7 vessels), demonstrating good patency in 6 stenting vessels and restenosis in one vessel. CONCLUSIONS: Angioplasty associated with Biodiv Ysio stent appears to be a safe and feasible for the patients with symptomatic M1 stenosis of MCA, under strict control of periperformeral project. Further study is, however needed.

[Detection of RNA of SARS coronavirus in hospital sewage].

OBJECTIVE: In order to explore the existence of SARS coronavirus (Co-V) and/or its RNA in sewage of hospitals administered SARS patients. METHODS: A novel electropositive filter was used to concentrate the SARS-CoV from the sewage of two hospitals administered SARS patients in Beijing, including twelve 2,500 ml sewage samples from the hospitals before disinfection, and ten 25,000 ml samples after disinfection; as well as cell culture, RT-PCR and sequencing of gene to detect and identify the viruses from sewage. RESULTS: There was no live SARS-CoV detected in the sewage in this study. The nucleic acid of SARS-CoV had been found in the 12 sewage samples before disinfection from both hospitals by semi-nested PCR. After disinfection, SARS-CoV RNA could only be detected from the samples from the 309th Hospital, and the others were negative. CONCLUSION: It provides evidence that there is no live SARS-Cov in the sewage from hospitals with SARS patients though SARS-CoV RNA can be detected.

[Influence of oral sodium selenite and vitamin E on thyroid hormones in patients with cardiovascular disease at altitude].

OBJECTIVE: To investigate the effect of oral sodium selenite and vitamin E on thyroid hormones in patients with cardiovascular disease at altitude. METHODS: Ninety patients with cardiovascular disease were divided into A group (n = 42, sodium selenite + VE), B group (n = 28, sodium selenite only) and control group (n = 20). Serum selenium (Se), plasma glutathione peroxidase (GSH-Px), plasma malondialdehyde (MDA) and serum T(3) and T(4) were determined before and after 6 month treatment. RESULTS: Serum Se in A and B group after 6 month treatment were higher than before [(0.71 +/- 0.22) micromol/L vs (0.31 +/- 0.17) micromol/L, (0.68 +/- 0.18) micromol/L vs (0.33 +/- 0.14) micromol/L respectively, P < 0.01], and so were plasma GSH-Px levels [(87.12 +/- 13.61) U/L vs (58.43 +/- 18.93) U/L, (84.79 +/- 12.13) U/L vs (57.12 +/- 17.36) U/L respectively] while plasma MDA were lower than before [(4.86 +/- 1.18) nmol/ml vs (8.66 +/- 0.96) nmol/ml, (4.18 +/- 1.23) nmol/ml vs (8.71 +/- 0.87) nmol/ml respectively, P < 0.01]. Serum T(3) and T(4) levels in A and B group were also obviously decreased (P < 0.01). The levels of plasma GSH-Px were positively correlated with those of serum Se (r = 0.781, P < 0.01). The levels of plasma MDA and serum T(3) and T(4) were negatively correlated with those of serum Se (r = -0.385, -0.687, -0.412 respectively, P < 0.05). 31 cases (73.81%) in A group and 20 cases (71.42%) in B group completely recovered to normal; 4 cases (9.52%) in A group and 2 cases (7.43%) in B group partly recovered. The recovered rates were significantly different from that of control (P < 0.05). CONCLUSION: Supplementation of adequate selenium may correct the abnormal function of secretion in thyroid hormones of patients because of lack of selenium at altitude areas.

[Symptomatic intracranial artery stenosis: angiographic classifications and stent-assisted angioplasty].

OBJECTIVE: To assess the safety and efficacy of stent-assisted angioplasty (SAA) for symptomatic intracranial artery stenosis, and to evaluate preliminarily the significance of classification of location, morphology and access (LMA classification) of intracranial artery stenosis in SAA. METHODS: Forty-two patients with symptomatic intracranial artery stenosis (diameter reduction: 50% - 74%, n = 15; >or= 75%, n = 27), located in middle cerebral artery (n = 27), intracranial internal carotid artery (n = 4), intracranial vertebral artery (n = 7) and basilar artery (n = 4) respectively, refractory to medical therapy were enrolled in this study. RESULTS: LMA classification: 23 of the forty-two lesions (54.8%) located at the site of bifurcation, which were classified according to the location into type A (n = 8), B (n = 11), C (n = 2), D (n = 1) and F (n = 1) respectively. Type A, B and C lesions were 19, 19 and 4 respectively in the light of morphologic classification. Type I, II and III accesses were 15, 23 and 4 respectively in the light of access classification. TECHNIQUE: The technical successful rate of SAA was 95.2% (40/42) for the group overall, and 100% (15/15), 94.7% (22/23), and 75% (3/4) for type I, II, III accesses, respectively. The rate of periprocedural complication and death was 9.5% (4/42), including acute occlusion (n = 1) and high perfusion syndrome (n = 3). After emergency measures, 3 patients were cured completely, and the remaining one with severe MCA trunk stenosis of type C lesion died of subarachnoid hemorrhage (2.4%). During a clinical follow-up period ranging from 1 to 18 months (median 8 months), 39 patients receiving SAA have been still free from ischemic events. There was no restenosisfound angiographically 6 months (n = 7) and 12 months (n = 4) after SAA. CONCLUSIONS: Our results suggest that under rigorous control of procedural and periprocedural measures, SAA appears to be a safe and effective therapy for symptomatic intracranial stenoses of type A and B lesions, but it is not risk-free for type C lesions. The LMA classification is helpful for predicting the results of SAA and to design the procedure. However, further study is needed.

Detection of enteroviruses and hepatitis A virus in water by consensus primer multiplex RT-PCR.

AIM: To develop a rapid detection method of enteroviruses and Hepatitis A virus (HAV). METHODS: A one-step, single-tube consensus primers multiplex RT-PCR was developed to simultaneously detect Poliovirus, Coxsackie virus, Echovirus and HAV. A general upstream primer and a HAV primer and four different sets of primers (5 primers) specific for Poliovirus, Coxsacki evirus, Echovirus and HAV cDNA were mixed in the PCR mixture to reverse transcript and amplify the target DNA. Four distinct amplified DNA segments representing Poliovirus, Coxsackie virus, Echovirus and HAV were identified by gel electrophoresis as 589-,671-, 1084-, and 1128bp sequences, respectively. Semi-nested PCR was used to confirm the amplified products for each enterovirus and HAV. RESULTS: All four kinds of viral genome RNA were detected, and producing four bands which could be differentiated by the band size on the gel. To confirm the specificity of the multiplex PCR products, semi-nested PCR was performed. For all the four strains tested gave positive results. The detection sensitivity of multiplex PCR was similar to that of monoplex RT-PCR which was 24 PFU for Poliovrus,21 PFU for Coxsackie virus,60 PFU for Echovirus and 105 TCID(50) for HAV. The minimum amount of enteric viral RNA detected by semi-nested PCR was equivalent to 2.4 PFU for Poliovrus, 2.1 PFU for Coxsackie virus, 6.0 PFU for Echovirus and 10.5 TCID(50) for HAV. CONCLUSION: The consensus primers multiplex RT-PCR has more advantages over monoplex RT-PCR for enteric viruses detection, namely, the rapid turnaround time and cost effectiveness.

[Study of effects of leptin on cultured human luteinized granulosa cell function].

OBJECTIVE: To assess the effects of leptin on follicle-stimulating hormone (FSH) and insulin-like growth factor-I (IGF-I) induced production of estradiol (E(2))-17beta and progesterone (P) in cultured human luteinized granulosa cell (GC). METHODS: Human luteinized GCs were obtained from pre-ovulatory follicles in an in-vitro fertilization program, and were cultured for 24 hours in the presence of leptin (3.0 ng/ml) or FSH (1.0 ng/ml) or IGF-I (30.0 ng/ml) alone, leptin with FSH or IGF-I or both, and FSH with IGF-I. The conditioned media were aspirated for measuring E(2) and P content. Luteinized GC were also counted for cell number and detected the expression of the leptin receptor by reverse transcription polymerase chain reaction analysis. RESULTS: In cultured luteinized GC system, leptin or FSH or IGF-I alone did not affect the growth of cultured human luteinized GC. Leptin alone or in the presence of FSH had no effect on E(2) production. E(2) levels in culture media without leptin or FSH were (0.103 +/- 0.036) pmol/1 000 cells, (0.323 +/- 0.042) pmol/1 000 cells respectively. When cultured with leptin alone or with leptin and FSH, E(2) levels were (0.120 +/- 0.008) pmol/1 000 cells, (0.343 +/- 0.034) pmol/1 000 cells. Leptin significantly inhibited FSH + IGF-I or IGF-I induced E(2) production. E(2) levels decreased from (0.318 +/- 0.037) pmol/1 000 cells to (0.193 +/- 0.025) pmol/1 000 cells (IGF-I) (P < 0.05) and from (0.493 +/- 0.036) pmol/1 000 cells to (0.251 +/- 0.033) pmol/1 000 cells (FSH + IGF-I) (P < 0.01). However, the production of P did not change. The leptin receptor expression was demonstrated in luteinized GC. CONCLUSIONS: Leptin may directly attenuate the IGF-I or IGF-I and FSH-stimulated E(2) synthesis in cultured human luteinized GC by the leptin receptor mechanism.

[Trabeculetomy combined with implantation of silicon rubber slice for intractable glaucoma].

PURPOSE: To investigate the lowering-pressure outcome and postoperative complications of trabeculectomy with implantation of silicon rubber slice for intractable glaucoma. METHODS: Sixty eyes with intractable glaucoma, including 12 eyes with neovascular glaucoma, 14 eyes with aphakic or pseudophakic glaucoma, 4 eyes with congenital glaucoma, 18 eyes with failed filtering surgery glaucoma and 12 eyes with glaucoma secondary to trauma, were performed trabeculectomy with implantation of silicon rubber slice. The postoperative lowering-pressure outcome and complications also were observed. RESULTS: All patients were follow-up for 5 to approximately 34 months ( mean, 18.3 months) . The postoperative intraocular pressure of patients was 17.30 to approximately 37.19 mmHg [mean, (23.36 +/- 4.06) mmHg] at the last visit. Of 60 eyes, the postoperative intraocular pressure was below 21 mmHg in 38 eyes (63.3%), between 21 to 30 mmHg in 18 eyes (30%) and higher than 30 mmHg only in 4 eyes (6.7%). No serious complications and rejection related to implantation of silicon rubber slice occurred in this group during the follow-up period. CONCLUSION: Trabeculectomy combined with implantation of silicon rubber slice seems to be an ideal procedure for treatment of intractable glaucoma in developing countries, especially in vast country hospital in China, this surgery also can replace the expensive import drainage device implant (Molteno tube implant) therapy for intractable glaucoma.
