MRI of alveolar soft-part sarcoma.

Alveolar soft-part sarcoma (ASPS) is rare. We present a case of a 30-year-old woman with a 6-month history of a palpable mass in her left thigh. MRI showed an 8-cm mass and a satellite nodule at the left gluteus muscle. The main tumor exhibited an isointense signal on T1-weighted images and high-signal-intensity areas with low-signal-intensity scanty solid components on T2-weighted images. MR angiography showed dilated and tortuous veins around the tumor. Histologic findings were compatible with ASPS.

Evidence for a protective role for adiponectin in osteoarthritis.

Obesity has been associated with an increased risk of osteoarthritis (OA). However, the mechanism by which obesity contributes to OA remains uncertain. Adiponectin, an adipocyte-derived hormone, has shown anti-diabetic and anti-atherogenic properties. In the present study, we aimed to investigate the potential role of adiponectin in OA disease. We demonstrated that adiponectin was present in OA synovial fluid (SF) and its expression level was almost 100-fold decrease compared with that in OA plasma. FPLC and ELISA studies revealed the distribution and abundance of the adiponectin complexes in plasma and SF from patients with OA. The percentage of high molecular weight (HMW) per total adiponectin in OA SF was lower than in OA plasma, while that of the hexamer form was similar and the trimer form was higher. The expression levels of adiponectin receptors AdipoR1 and AdipoR2 were examined in human OA tissues by RT-PCR. AdipoR1 was abundantly expressed in cartilage, bone and synovial tissues, whereas AdipoR2 was rarely detected. Finally, the effects of adiponectin on primary chondrocyte functions were studied by using antibody-based protein array and RT-PCR. The patterns of mRNA expression and protein production strongly indicate that adiponectin is involved in the modulation of cartilage destruction in chondrocytes by up-regulating TIMP-2 and down-regulating IL-1beta-induced MMP-13. Together these findings clearly indicate that the adiponectin may act as a protective role in the progression of OA, and this also provide new thinking on the relationship between obesity and OA.

The classification and treatment algorithm for post-burn cervical contractures reconstructed with free flaps.

Neck contractures after burn produce restrictions in motion and unacceptable aesthetic outcomes. Proper planning and tissue selection is essential to minimize donor site morbidity while optimizing outcomes. A classification system and treatment algorithm aids in achieving this goal. Between December 1999 and January 2003, 40 burn patients underwent release and reconstruction with free perforator flaps. Neck extensibility and zone of injury were evaluated. Choice of reconstruction was based on available tissue, restriction degree and zones involved. Cervical territories were classified according to movement restrictions and amount of improvement. Reconstructive territories were classified as central above (CA), central below (CB), central above and below (CAB) and lateral (L). Single, split, double and preexpanded free flaps were used for the reconstructions. Maximal gain in motion was noted at 4 weeks and maintained for the average 11 months follow-up. Types of reconstructive territories showed significant effects on range of motion while etiology and time between injury and reconstruction showed no impact on the functional outcome. Classification of neck territories aids in improving outcomes while minimizing donor morbidity. The central above territory, when reconstructed with free flaps, yielded the most rewarding improvement. A classification and treatment algorithm aids in achieving significant improvements in range of neck motion while taking into consideration the donor sites.

Three-dimensional hip morphology analysis using CT transverse sections to automate diagnoses and surgery managements.

This paper describes an image analysis method that evaluates bone morphology of hip structures including the femur stem, trochanter, neck and head, acetabulum, and pelvis to automate hip diagnoses and surgical managements. On every CT transverse section, radial B-spline curves are used to approximate the ellipse-like acetabulum and femur head and stem. The femur neck is approximated as trapezoid-like and the pelvis horizontally symmetrical structure. The centers of the ellipse-like structures from transverse sections are used to determine 3D axes of the femur stem, head, and acetabulum. The centerlines of the neck or the pelvis on the sections are used to determine the neck axis or pelvis centerplane. Boundary changes of these structures are recognized as concave, convex and hole features that are then identified as fractures, tumors, and spurs. Based on the geometric evaluations of these structures and features, hip surgeries including tumor dissect and bone graft, open reduction using plates, screws and nails, and arthroplasty are automatically managed to achieve the normal hip function including dissection of tumors and reduction of dislocations and angular deviations between the hip structures. This prototype system can be used as a qualitative and quantitative tool for the diagnosis of various hip diseases and for the planning of accurate surgical procedures. A series of examples and four case studies illustrate this automated method can be used to accurately diagnose hip diseases and manage hip surgeries, and train operators.

Volume manipulations for simulating bone and joint surgery.

Bone and joint surgery is widely used in orthopedic, oral, and maxillofacial, and dental and plasty departments to correct bone and joint pathology such as bone and joint tumors and fractures, and skeletal morphological deformities. This article presents a voxel structure to represent topologically and geometrically correct surfaces and algorithms to accurately compute intersections of tool swept surfaces with bones based on this voxel structure. This article then presents various volume manipulation algorithms to operate on virtual bones, bone grafts, and prostheses for bone and joint surgery simulations. A complicated knee arthroplasty illustrates the practicality and versatility of the proposed method.

Production of the chemokine eotaxin-1 in osteoarthritis and its role in cartilage degradation.

The expression of the chemokine, eotaxin-1, and its receptors in normal and osteoarthritic human chondrocytes was examined, and its role in cartilage degradation was elucidated in this study. Results indicated that plasma concentrations of eotaxin-1 as well as the chemokines, RANTES, and MCP-1alpha, were higher in patients with osteoarthritis (OA) than those in normal humans. Stimulation of chondrocytes with IL-1beta or TNF-alpha significantly induced eotaxin-1 expression. The production of eotaxin-1 induced expression of its own receptor of CCR3 and CCR5 on the cell surface of chondrosarcomas, suggesting that an autocrine/paracrine pathway is involved in eotaxin-1's action. In addition, eotaxin-1 markedly increased the expressions of MMP-3 and MMP-13 mRNA, but had no effect on TIMP-1 expression in chondrocytes. However, pretreatment of anti-eotaxin-1 antibody significantly decreased the MMP-3 expression induced by IL-1beta. These results first demonstrate that human chondrocytes express the chemokine, eotaxin-1, and that its expression is induced by treatment with IL-1beta and TNF-alpha. The cytokine-triggered induction of eotaxin-1 further results in enhanced expressions of its own receptor of CCR3, CCR5, and MMPs, suggesting that eotaxin-1 plays an important role in cartilage degradation in OA.

Automatic spinal disease diagnoses assisted by 3D unaligned transverse CT slices.

This paper describes a three-dimensional (3D) reconstruction method and an automatic diagnosis method for spinal diseases using unaligned transverse slices that have arbitrary angles and intervals but do not intersect to each other at regions of interest. The 3D reconstruction method extends the marching cube algorithm to generate triangulated isosurfaces for these unaligned slices. The automatic spinal disease diagnostic method analyses boundaries of discs and vertebral bodies on unaligned transverse slices to estimate the presence and extent of disc herniation and canal compression, and deformities in the spinal curve. The prototype system can be used as a qualitative and quantitative tool for the diagnosis of various spinal diseases using unaligned transverse slices.

Stimulation of inducible nitric oxide synthase by monosodium urate crystals in macrophages and expression of iNOS in gouty arthritis.

From the studies on the involvement of iNOS in arthritis, it is clear that attention has focused primarily on rheumatoid arthritis (RA) and osteoarthritis (OA). To date, little is known about the role of iNOS in the pathophysiology of gouty arthritis (GA). Here, we investigated the significance of iNOS expression in cell culture system as well as in GA patients. Gouty crystals monosodium urate (MSU) appeared to up-regulate inducible nitric oxide synthase (iNOS) mRNA and protein expression in a concentration- and time-dependent manner in RAW264.7 macrophages. This increase of iNOS expression is attributable to the activation of multiple signaling pathways. Evidence for this was initially established by inhibitor treatment of cells in the presence of MSU. While the JAK inhibitor AG490, the PI3K inhibitor LY294002, and the NFkappaB inhibitor PDTC abrogated almost completely the expression of iNOS induced by MSU, the ERK1/2 inhibitor PD98059 was only partially effective. Furthermore, the effect of MSU on the activation of PI3K/Akt, JAK/STAT, ERK1/2, and NFkappaB signaling molecules was carefully examined. Moreover, it was shown that GAS and NFkappaB motifs are required for iNOS expression mediated by MSU. In addition, synovial tissues obtained from GA patients displayed enhanced expression of iNOS when compared with normal synovium. Taken together, these findings provide the first evidence for the potential importance of iNOS in the pathogenesis of GA as well as RA and OA, and in turn raise the possibility that iNOS may be an ideal target for preventive therapy in human arthritis.

Expression of matrix metalloproteinase-9 (gelatinase B) in gouty arthritis and stimulation of MMP-9 by urate crystals in macrophages.

To investigate the relevance of gelatinase-B (matrix metalloproteinase 9, MMP-9) in gouty arthritis (GA), we tested the occurrence of MMP-9 in GA patients and cell culture system. Gelatinolytic activity in the synovial fluid (SF) of patients with different kinds of arthritis was assessed by gelatin zymography. A predominant 92-kDa MMP-9 gelatinolytic activity was evident in rheumatoid arthritis (RA) and GA samples, but no activity was observed in osteoarthritis (OA) samples. Among the 53 SF samples (9 RA, 24 GA, and 20 OA) analyzed for MMP-9 and tissue inhibitor of metalloproteinase (TIMP-1) antigen levels by ELISA, MMP-9 antigen levels were elevated tenfold in GA SF compared with OA SF. In addition, GA synovial tissue extracts revealed elevated levels of MMP-9 expression as compared to OA tissue extracts by Western blot and RT-PCR analysis. Immunohistochemical studies demonstrated that MMP-9 immunoreactivity was more intense in GA than in OA synovial tissues. Furthermore, macrophages activation by gouty crystals in vitro was examined. Crystals stimulated MMP-9 gene expression in macrophage cell line and such stimulation was suppressed by PD98059. These findings suggest that the abnormal production of MMP-9 by macrophages is a reflection of the pathological conditions in joints of patients with GA, and that the activation of MMP-9 in the joint is known to play an important role in joint disease.

Arsenite stimulates cyclooxygenase-2 expression through activating IkappaB kinase and nuclear factor kappaB in primary and ECV304 endothelial cells.

Epidemiological studies have shown that chronic exposure to arsenic can result in liver injury, peripheral neuropathy, arteriosclerosis, and an increased incidence of cancer of the lung, skin, bladder, and liver. The overexpression of inducible cyclooxygenase-2 (Cox-2) has been associated with vascular inflammation and cellular proliferation. However, the effect of arsenite on Cox-2 gene expression in endothelial cells was left to be investigated. Western Blot analysis of HUVECs revealed a two-fold induction of Cox-2 protein by arsenite. This induction was associated with a two-fold increase of prostaglandin E2 in the media. Furthermore, the level of Cox-2 mRNA was correspondingly elevated as demonstrated by both Northern blot and reverse transcriptase-polymerase chain reaction (RT-PCR) analyses. Transfection of an immortalized human endothelium cell line (ECV304) with Cox-2 reporter gene constructs demonstrated that the transcription of Cox-2 gene was enhanced by arsenite. This induction was attenuated by pyrrolidine dithiocarbamate (PDTC), an inhibitor of NFkappaB. In addition, electrophoretic mobility shift assays indicated that NFkappaB activity was induced by arsenite. The kinase activity assay also indicated that IkappaB kinase (IKK) activity was induced by arsenite. These findings indicated that the induction of Cox-2 gene transcription by arsenite was through the stimulation of NFkappaB activity. Arsenite could induce IKK activity, which leads to the phosphorylation and degradation of IkappaB in ECV304 cells. Therefore, it appears that IKK signaling pathway is involved in arsenite-mediated Cox-2 expression.
