Consanguineous marriages in Iranian folktales.

BACKGROUND: Using folktales we can find some aspects of social customs. Here, I show the situation of consanguineous marriages in folktales from different Iranian populations and try to compare it with the present situation of consanguineous marriages. METHODS: The study includes tales from 11 different Iranian (ethnic and/or geographical) populations. Overall, the records of 585 marriages in 296 folktales from the study populations were found. RESULTS: The estimated mean inbreeding coefficient (alpha) for different populations using folktale records is lower than that of the present values. However, there is a very high level of statistically significant correlation between them (r = 0.753, d.f. = 9, p = 0.007). CONCLUSION: The historical background is very important to attitude and practice regarding marriages between relatives.

Correlation between incidences of self-inflicted burns and means of inbreeding coefficients, an ecologic study.

PURPOSE: The aim of the study is to obtain more insight into the possible association between consanguinity and the incidence of deliberate self-burning. METHODS: Data were obtained by analysis of medical records of patients hospitalized in two referral burn centers: Chormy Burn Center (Bushehr Province, south of Iran) from March 21, 1998, through March 20, 2004, and Shahid Sadoqi Center of Burns and Injuries (Yazd Province, center of Iran) from March 21, 2000, through March 20, 2004. RESULTS: The incidence of suicidal burns was 6.51 and 2.32/100,000 person-years for Bushehr and Yazd Provinces, respectively. The observed sex ratio of patients in both centers indicated there was a female predominance in patients with self-inflicted burns. Using patients' home addresses, patients were sorted into 16 cities. The incidence of suicide by self-burning ranged from 0.80 (for Tabas, located in Yazd Province) to 12.60/100,000 person-years (for Dilam, located in Bushehr Province). The coefficient of inbreeding defines the probability that an individual received both alleles of a pair from an identical ancestral source. There was a significant correlation between incidences of suicidal burns and mean coefficient of inbreeding (r = 0.782, df = 14, p < 0.001). CONCLUSION: In addition to other factors, consanguineous marriage may be a risk factor that influences the incidence of suicidal burns in a population.

Genetic polymorphisms of glutathione S-transferase T1 (GSTT1) and susceptibility to gastric cancer: a meta-analysis.

The association between glutathione S-transferase T1 (GSTT1) polymorphism and gastric cancer risk has been both confirmed and refuted in a number of published studies. Most of these studies were based on small sample sizes. We carried out a meta-analysis of the research published up to August 2005 to obtain more precise estimates of gastric cancer risk associated with GSTT1 polymorphism. In the present study, 16 case-control studies (with a total of 6717 subjects) were eligible for meta-analysis. There was no evidence of heterogeneity between the studies. The GSTT1 null genotype conferred a 1.06-fold increased risk of gastric cancer, which was not significant (95% confidence interval [CI]: 0.94-1.19). However, in the analysis of ethnic groups, we observed distinct differences associated with GSTT1 status. Restricting analyses to ethnic groups, the pooled odd ratios for the GSTT1 genotype were 1.27 in Caucasians (95% CI: 1.03-1.57) and 0.98 in Asians (95% CI: 0.86-1.13). Glutathione S-transferase M1 (GSTM1) and GSTT1 are involved in detoxification of a variety of compounds, some that overlap between enzymes and some that are highly specific. To investigate whether the profile of glutathione S-transferase genotypes was associated with risk of gastric cancer, further analyses combining the GSTT1 and GSTM1 genotypes were also carried out. There was a significant trend in risk associated with zero, one and two putative high-risk genotypes (chi2 = 9.326, d.f. = 1, P = 0.0023). Those who had null genotypes of GSTM1 and GSTT1 had an increased gastric cancer risk compared with those who had both active genes (odds ratio = 2.08, 95% CI: 1.42-3.10).

Environmental exposure to natural sour gas containing sulfur compounds results in elevated depression and hopelessness scores.

Some parts of Masjid-i-Sulaiman (MIS) (Khozestan provinces, southwest of Iran) are contaminated with subsurface leakage of natural sour gas containing H(2)S. It is reported that the incidence of suicide by self-burning in MIS is very high. High endogenous H2S levels have been found in the brain and it is involved in the brain functions. Because there is no report about the effect(s) of natural sour gas containing sulfur compounds on the function(s) of human brain, the present study was done. The study was performed on 128 individuals exposed or unexposed to natural sour gas. The exposed group consisted of 64 persons (39 males, 25 females). Unexposed subjects were matched by sex, age, and educational levels. Depression and hopelessness were determined using Beck's depression inventory (BDI) and Beck's hopelessness (BHS) questionnaires. Using multiple linear regression models, the averages of BDI (t=2.637, P=0.009) and BHS (t=3.344, P=0.001) were significantly higher among the exposed subjects than among the control group. In Lali 57.8% and 14.1% of subjects had no depressed mood and moderate to severe depressed mood, respectively, while in MIS 31.3% and 35.9% of subjects had no depressed mood and moderate to severe depressed mood, respectively; the difference was significant (chi2=12.88, df=3, P=0.005). In Lali 35.9% and 12.5% of subjects had no hopelessness at all and moderate to severe hopelessness, respectively, while in MIS 17.2% and 32.8% of subjects had no hopelessness at all and moderate to severe hopelessness, respectively, the difference was significant (chi2=11.49, df=3, P=0.009). Replication is necessary and health implications are discussed.

Alterations of liver function test indices of filling station workers with respect of genetic polymorphisms of GSTM1 and GSTT1.

To determine the health effects of gasoline exposure on liver function test indices of filling station workers the present study was done. This case-control study was conducted in Shiraz on 56 male gasoline workers and 56 age- and sex-matched control subjects with no occupational exposure to gasoline. To elucidate the role of hepatic detoxifying enzymes, the genotypes of glutathione-S-transferases (GST) M1 and T1 were determined. Data analysis was done by multiple linear regression analysis and non-parametric Kruskal-Wallis test. The present study showed that all measurements were in normal range, although sub-clinical changes were detected in some indices. In liver function tests, exposure was associated with lower serum albumin (t=-3.88, P<0.001) and total proteins (t=-3.016, P=0.003) but higher alanine aminotransferase (t=2.856, P=0.005) and aspartate aminotransferase (t=2.11, P=0.038) levels in workers comparing to controls. Other investigators reported that GSTs involved in detoxification of several toxins including some of the compounds present in gasoline. Therefore, the possible influence of GSTT1 and GSTM1 genetic polymorphisms on alteration of liver function tests indices was investigated. The present findings showed that the genotype combinations of GSTM1 and GSTT1 did not alter the effects of exposure to gasoline in workers except for serum albumin. Serum albumin significantly decreased in workers with both active GST enzymes who had more than 5 years of employments (P=0.01). It is suggested that GSTM1 and GSTT1 are not involved in detoxification of toxicants present in gasoline which are hazardous to liver. Overall, due to detection of sub-clinical changes in hepatic test in gasoline station workers, exposure limitation and administrating safety device are recommended.

Epidemiology and mortality of hospitalized burn patients in Kohkiluye va Boyerahmad province (Iran): 2002-2004.

The Kohkiluye va Boyerahmad province is located in the south of Iran. The population of the province is about 615,000, living in 14,261 km(2). The total number of patients in the study was 235. There were admitted to the burn unit of Shahid Beheshti hospital (Yasuj) between 18 July 2002 and 17 July 2004. Of these patients, 149 (63.4%) were accidental and 86 (36.6%) were self-inflicted burns. The hospitalization rates for accidental and self-inflicted burns were 12.1 and 7.0 per 100,000 person-years, respectively. There is a significant difference in the sex ratio (males:females) between accidental (1.40) and suicidal (0.13) patient groups (P((2))<0.00001). The fatality rates for accidental and self-inflicted burns were 2.1 and 59.5%, respectively. The mortality rates for accidental and self-inflicted burns were 0.24 and 3.81 per 100,000 person-years, respectively. The overall mean and median ages for accidental patients were 19.4 and 13 years, and those for self-inflicted burns were 24.9 and 23 years, respectively. In the accidental patients, the BSA of the burns ranged from 5 to 95%, with a median of 15%. In the self-inflicted patients, the median of BSA was 65%. The majority of accidental burns were caused by scalds (43.2%) and of self-inflicted burns by kerosene (87.5%). The median hospital stay was 8 days with a range of 1-43 days for accidental patients. The median hospital stay was 4 days with a range from <1 to 36 days for fatal self-burning patients. In non-fatal self-burning cases, the median hospital stay was 20 days with a range of 4-53 days. There is significant difference between fatal and non-fatal self-burning cases for hospital stay (P((2))<0.0001). No definite correlation was found between the incidence of either accidental or self-inflicted burns and seasons (P((2))>0.05). Most of the burns occurred at home.

Influence of polymorphism of glutathione S-transferase M1 on systolic blood pressure of normotensive individuals.

Studies have indicated that systolic (SBP) and diastolic (DBP) blood pressure are multi-factorial traits and significantly heritable. The aims of the present study are to assess whether the glutathione S-transferase M1 (GSTM1) and T1 (GSTT1) genotypes are associated with SBP and DBP of normotensive subjects and to ascertain whether the level of SBP and DBP given exposure to cigarette smoking is modified by the specific genetic polymorphisms of GSTM1 and GSTT1. This cross-sectional study was conducted on 140 subjects (49 females and 91 males) (mean age+/-SD: 38.7+/-14.7). The genotypes were determined using a polymerase chain reaction based method. Individuals were stratified according to the mean values of DBP and SBP, lower than or maximally same as the mean value defines as group I and higher than the mean value defines as group II. The logistic regression analyses were used. The best models fitted by logistic regression analysis for variables were associated with SBP and DBP. For analysis the combination of genotypes, sex, and smoking behavior was used as qualitative variables, and age, body mass index (BMI), and heart rate were used as covariates. Combination of "present-GSTT1, null-GSTM1" genotype (OR=0.001, 95% CI=0.00-0.439, P=0.025), heart rate (OR=1.065, 95% CI=1.018-1.114, P=0.006), and interaction between BMI and combination of "present-GSTT1, null-GSTM1" (OR=1.319, 95% CI=1.058-1.644, P=0.014) was associated with SBP. There was no association between either combination genotypes of GSTs or interaction of genotypes and smoking behavior on DBP. The present results suggest that the GSTM1 gene is one of the candidate genes that alter the baseline of SBP in normotensive individuals.

Genetic polymorphisms of glutathione S-transferases M1 and T1 modulate hematological changes of individuals chronically exposed to natural sour gas.

In order to find the effect of genetic polymorphisms of glutathione S-transferase M1 (GSTM1) and GSTT1 on hematological changes of individuals chronically exposed to natural sour gas, the present study was done. Study subjects (59 males, 55 females) were residents of contaminated areas of Masjid-i-Sulaiman (southwest of Iran). The GSTM1 and GSTT1 genotypes were determined using a polymerase chain reaction-based method. The multiple linear regression method was applied. There is significant association between GSTs genotypes and either hemoglobin (t=2.185, P=0.031) or hematocrit (t=2.454, P=0.016). Also there is weak association between GSTs genotypes and WBC counts (t=1.802, P=0.074). The hemoglobin and hematocrit levels and WBC counts increased in individuals who had null genotypes of GSTM1 and GSTT1 compared to subjects with one or two active genes. Also the levels of hemoglobin and hematocrit and WBC counts increased in persons with one active genotype compared to subjects who had two active genes. There is no significant association between neither platelet nor WBC differential parameters and GSTs genotypes.

Null genotype of glutathione S-transferase M1 is associated with senile cataract susceptibility in non-smoker females.

In the present study, we investigated whether the polymorphisms of glutathione S-transferase M1 (GSTM1) and T1 (GSTT1) genes are risk factors of cataract among Iranian population in a molecular epidemiological way. Blood samples from 150 subjects with cataract (72 male; 78 female) and 150 age- and sex-matched healthy persons were collected. Both patient and control groups were unrelated Iranian Muslims. Using PCR-based method, the genotypes were determined. The null GSTM1 genotype was associated with a 2.38-fold increase in the risk of developing cataract (OR=2.38; 95% CI=1.46-3.89; P = 0.0003). After stratification by sex of subjects, the association was apparent only among women (OR=3.20; 95% CI=1.58-6.52; P = 0.0007). The GSTT1 null genotype was associated with a 1.10-fold increased risk of developing cataract, but this association was not statistically significant. After stratification by sex of subjects, same results were obtained. Female patients with null genotype for GSTM1 and no history of smoking had a 3.45-fold increased cataract risk (P < 0.05), whereas females who were null for GSTM1 and having history of smoking were not at increased risk of cataract.

Polymorphisms of glutathione S-transferase M1 and T1 modulate blood pressure of individuals chronically exposed to natural sour gas containing sulfur compounds.

In order to find the effect of genetic polymorphisms of GSTM1 and GSTT1 on blood pressure of individuals chronically exposed to sulfur compounds, the present study was done. Study subjects (38 males, 38 females) were residents of contaminated areas of Masjid-i-Sulaiman (southwest of Iran). The GSTM1 and GSTT1 genotypes were determined using a polymerase chain reaction (PCR)-based method. The non-parametric Sign test was applied in order to detect differences between the GSTs genotypes of study subjects and the normal mean values according to the sex and age of subjects. From four combination of genotypes, systolic blood pressure significantly decreased in combination of null-GSTM1 and present-GSTT1 (Z=-2.41; P=0.016), and diastolic blood pressure significantly increased in combination of present-GSTM1 and null-GSTT1 (Z=+2.14; P=0.032). It is speculated about polymorphisms of GSTs in individuals chronically exposed to natural sour gas, which contains H2S, fulfilling a physiological role(s) in regulating blood pressure.
