Cytogenetic damage in workers from a coal-fired power plant.

The aim of this study was to investigate the genotoxic risk to workers occupationally exposed to coal combustion products in Afsin-Elbistan A power plant, located in south-eastern Turkey. We analysed chromosomal aberrations (CAs), polyploidy, sister-chromatid exchanges (SCEs), and micronuclei (MN) in 48 male workers without a history of smoking, tobacco chewing, or alcohol consumption. The results were compared with a control group of 30 healthy male individuals without exposure to any known genotoxic agents. The mean frequencies of CA, polyploidy, SCEs (P<0.01), and MN (P<0.05) were significantly higher in workers than in the control group, by the Mann-Whitney U-test. Spearman's rho correlation analysis revealed a significant increase in the frequency of CA and MN with increasing years of exposure (P<0.05). However, there was no significant effect of age on the cytogenetic markers analysed in both groups (P>0.05). The data obtained from this study clearly showed chromosomal hazard in the peripheral lymphocytes of workers exposed to coal combustion products in Afsin-Elbistan A power plant for several years. This cytogenetic damage might be attributed to the cumulative effects of several substances due to chemical complexity of the coal ash and gaseous emissions rather than a specific substance.

The knowledge, attitude and practice of the primary and secondary care nurse-midwife practitioners on developmental dysplasia of hip.

A detailed clinical examination of the newborn performed by nurse-midwife practitioners has been recommended as a standard practice throughout the most western countries. The purpose of this study was to evaluate and compare the knowledge, attitude and practice of the primary and secondary care nurse-midwife practitioners on developmental dysplasia of hip (DDH) before and after a structured lesson. The study was designed as a cross-sectional, pre- and post-test. The number of participants from primary care and secondary care nurse-midwife practitioners were 155 and 208, respectively. A structured questionnaire consisting of 28 statements of medical and practical knowledge, and traditional attitudes with regard to DDH was administered. The results determined that the pre-test scores were much lower than the post-test scores of both primary and secondary care practitioners. Although there was a lack of knowledge about DDH in both groups, the pre-test and post-test scores were higher in the primary care group compared to the secondary care group. In conclusion, the knowledge, attitude and practice of the primary and secondary care nurse-midwife practitioners on developmental dysplasia of hip are in need of improvement. This improvement may be provided by continuing education programmes.

Clastogenicity of the fungicide afugan in cultured human lymphocytes.

The genotoxic effects of the fungicide afugan were analysed by measuring chromosomal aberrations (CAs), sister chromatid exchange (SCE) and micronuclei (MN) in cultured human peripheral lymphocytes. Concentrations of 2.5, 5, 10 and 20 microg/ml of afugan were used during 24 and 48 h. Afugan significantly increased the frequency of CAs at 5, 10 and 20 microg/ml concentrations during a 48 h treatment period. A significant increase was observed for induction of SCE and MN at all treatments compared with the negative control. A significant dose-response correlation was found in all tests. Afugan did not affect the replicative index (RI), however it significantly decreased the mitotic index (MI) at all treatment concentrations except 2.5 microg/ml, and at both treatment times. The present results indicate that afugan is clastogenic and cytotoxic to cultured human lymphocytes.

Congenital teratoma of the tongue: a case report and review of the literature.

Teratomas of the head and neck are rare. They develop in neonates and are associated with feeding difficulty, airway obstruction, and difficulty in swallowing (which may cause polyhydramnios). Although some tumors consist of immature elements, teratomas of the head and neck are mature in nature, and excision is the most effective treatment. We present a case report of a neonate with a polypoid mass protruding from his mouth. He exhibited respiratory arrest immediately after birth and underwent intubation. The mass, which was completely excised, was histologically identified as a mature teratoma.

Comparison of effects of smoking and smokeless tobacco "Maras powder" use on humoral immune system parameters.

BACKGROUND: The aim of this study is to assess the impacts of "Maras powder" and cigarette smoking on the parameters of the humoral immune system. MATERIAL AND METHODS: One hundred seventy seven subjects were included in the study. The IgA, IgG, IgM, C3 and C4 levels were detected via nephelometric method. RESULTS: In 1.4% of the control group IgM levels were below normal where it was 10.8% and 18.6% in Maras powder group and in cigarette smoking group respectively. The IgM levels of both groups were significantly lower compared to the control group (P < .05). Nonetheless, the IgE levels of Maras powder group and smoking group were found to be remarkably higher compared to the control group (P < .01). CONCLUSION: Effects of Maras powder on humoral immune response were found to be similar to that of smoking.

The effect of posterior bridging of C-1 on craniovertebral junction surgery.

OBJECT: The groove for the vertebral artery (VA) may be bridged on C-1, forming a canal through which the artery passes. Because this variant may significantly affect the commonly performed C-1 posterior laminectomy, the authors studied the incidence and radiological appearance of this anatomical feature in surgical specimens. METHODS: Anatomical measurements were performed using a Vernier caliper (accurate to 0.1 mm) on 166 dry C-1 vertebrae. In addition, a convenience sample of 172 cervical x-ray films was evaluated. Partial osseous bridging was detected in eight (4.8%), and a complete osseous bridge, forming the canalis arteriae vertebralis (canal for the VA), was seen unilaterally (eight on the left and two on the right) in 10 (6%) and bilaterally in eight (4.8%) of the dry vertebrae. On lateral radiography, a complete bone bridge was observed on nine (5.2%) and a partial bridge on 10 (5.8%) of the 172 studies. CONCLUSIONS: The discovery of a common variant may necessitate an immediate change in surgical management, and this anomaly could even increase the rate of surgery-related complications. The VA canal, present in the C-1 vertebra in approximately 5% of the specimens evaluated, was a variant that may be protective against VA injury. Knowledge of this variant may help the surgeon undertaking procedures in the C-1 region.

[Knowledge and attitude of nursing and midwifery college students on developmental dysplasia of the hip].

OBJECTIVES: It is of special importance to educate families and health care providers, in particular midwives and nurses who are in close and frequent contact with families, for the prevention and early diagnosis of developmental dysplasia of the hip (DDH). A knowledge and attitude study was conducted concerning DDH among students of Nursing and Midwifery College of Kahramanmaraş Sütçü Imam University. METHODS: A structured form was prepared consisting of 28 statements about medical and practical knowledge and traditional attitudes with regard to DDH and was administered to 232 voluntary students before and after an educational session of 60 minutes by a specialist in orthopedics. Prior to the study, the students of grade I and II had not received any theoretical or practical lectures about DDH, which were included in the curriculum of grades III and IV. RESULTS: The knowledge of the students about risk factors and prevention strategies for DDH was of a moderate level. They were not sufficiently furnished with information about traditional attitudes and applications that predispose infants to DDH. The mean test scores before and after training were 51.52+/-13.90 and 87.86+/-5.90, respectively (p=0.000). Compared to grade III and IV students, grade I students performed significantly less in the former test (p=0.00); however, the scores of the latter test did not differ significantly between the grades (p>0.05). CONCLUSION: The curriculum during the license education of midwives and nurses should include essential information on traditional attitudes. With sufficient knowledge and application skills, midwives and nurses may play an important role in the primary and secondary prevention of DDH in our country.

In vitro effect of karathane LC (dinocap) on human lymphocytes.

Karathane LC (active ingredient dinocap), a contact fungicide and a non-systemic acaricide was investigated for its ability to induce chromosome aberrations (CAs) and sister chromatid exchanges (SCEs) in cultured human lymphocytes of peripheral blood. In addition to the cytogenetic analysis, the effect of karathane LC on the cell proliferation kinetics (CPK) by the replication index (RI) was studied. The mitotic index (MI) was also determined to detect the cytotoxic effect. Lymphocytes were treated with four different concentrations (5, 10, 15 and 20 microg/ml) of karathane LC for 24 and 48 h. Significant differences between exposed and non-exposed groups found in CAs, SCEs and MI demonstrate the mutagenic, clastogenic and also the cytotoxic effect of karathane LC.

Wound approximation with tissue glue in circumcision.

BACKGROUND: To assess the feasibility of using tissue glue in the approximation of circumcision wounds in children. PATIENTS AND METHODS: A prospective randomized trial was carried out on 30 boys (age, 1-10 years) admitted to the Kahramanamaras Sutcuimam University Medical Faculty, Turkey. The results of wound approximation in circumcision with cyanoacrylate tissue glue and suturing with interrupted 5/0 plain catgut were compared. The operations were carried out by the same surgeon using an identical technique. The wounds were assessed 1 day, 3 days, 1 week and 1 month postoperatively. RESULTS: There was no difference between the two groups in the rates of wound inflammation, infection, bleeding and dehiscence, although cosmetic appearance was better in the tissue glue group. The operation time was also shorter in this group. CONCLUSION: Using tissue glue for wound approximation in circumcision is a feasible alternative.

[Psychopathology and personality patterns in the first-degree relatives of bipolar patients].

OBJECTIVE: This is a cross-sectional study designed to assess psychopathology and personality patterns in a group of high-risk subjects for bipolar disorder compared to a control group. As high-risk subjects first-degree relatives of bipolar patients were selected. METHOD: Ninety-five first-degree relatives of 54 bipolar patients and 93 first-degree relatives of 54 subjects without any psychiatric disorder were recruited in the study. Control subjects were matched to bipolar patients according to age, gender and educational status. SADS-L (Schedule for Affective Disorders and Schizophrenia-Lifetime Version) was used both to ascertain the psychiatric status of the patient and control subjects, and to evaluate the psychopathology in probands' and controls' relatives. MMPI-2 (Minnesota Multiphasic Personality Inventory-2) profiles of relatives of patient and control groups were compared as well. RESULTS: In the relatives of bipolar patients the SADS-L diagnoses of hypomania, minor depression and schizotypal personality were statistically more prevalent than in the relatives of the control group. MMPI-2 profiles of both relatives of bipolar patients and controls were within "normal" range, whereas relatives of patients were more defensive in disclosing psychopathology. Any specific profile characteristic for relatives of bipolar patients could not be described. CONCLUSION: Minor mental disorders were more prevalent in the relatives of bipolar patients group. A personality pattern specific to high risk group for bipolar disorder couldn't be detected.

Correction of buttock ptosis with anchoring deepithelialized skin flaps.

A new technique for correction of a buttock ptosis resulting from a degloving injury is presented. With this technique, the deepithelialized crescent-like zone in lateral thigh redundancy anchored to gluteal fascia through three separate incisions as used for creation of a new gluteal crease. The patient's satisfaction was good with long-term follow-up evaluation. Infragluteal fold reconstruction is achieved by reconstruction of the buttock's sustaining apparel through deepithelialized skin flaps anchored to deep gluteal fascia.

Genotoxicity study in lymphocytes of offset printing workers.

The potential cytogenetic damage in offset printing workers was evaluated using sister chromatid exchanges (SCEs), chromosome aberrations (CAs) and micronuclei (MN) as biomarkers in peripheral lymphocytes of 26 volunteers (14 workers, 12 controls). The CA, SCE and MN frequency of offset printing workers was significantly higher than in their controls. The replication index (RI) was not affected while the mitotic index (MI) was affected most in the workers. It can be concluded from this study that chronic occupational exposure to printing dyes and thinner may lead to a slightly increased risk of genetic damage among offset printing workers.
