A comparative study of linear and non-linear regression analysis for ammonium exchange by clinoptilolite zeolite.

Ammonium ion exchange from aqueous solution using clinoptilolite zeolite was investigated at laboratory scale. Batch experimental studies were conducted to evaluate the effect of various parameters such as pH, zeolite dosage, contact time, initial ammonium concentration and temperature. Freundlich and Langmuir isotherm models and pseudo-second-order model were fitted to experimental data. Linear and non-linear regression methods were compared to determine the best fitting of isotherm and kinetic model to experimental data. The rate limiting mechanism of ammonium uptake by zeolite was determined as chemical exchange. Non-linear regression has better performance for analyzing experimental data and Freundlich model was better than Langmuir to represent equilibrium data.

Effect of atorvastatin on the recurrence rates of atrial fibrillation after electrical cardioversion.

To study the effect of atorvastatin on recurrence of atrial fibrillation (AF) after electrical cardioversion (EC), 48 patients with AF lasting 48 hours who were scheduled for EC were randomized to the atorvastatin (group I) and control (group II) groups. Six patients in group I (25%) and 2 patients in group II (8.3%) had spontaneous conversion before EC (p >0.05). The end point was the recurrence of AF during 3 months of follow-up. Eighteen patients in group I (12.5%) and 11 patients in group II (45.8%) had recurrence (p = 0.01, log-rank test). With the Cox proportional model, the predictors of recurrence included a body mass index of 25 to 30 kg/m2 (relative risk [RR] 0.07, 95% confidence interval [CI] 0.008 to 0.59), body mass index > or = 30 kg/m2 (RR 0.24, 95% CI 0.08 to 0.72), AF duration of > or = 3 months (RR 0.28, 95% CI 0.09 to 0.83), diabetes mellitus (RR 0.34, 95% CI 0.12 to 0.98), and left atrial diameter of > or = 45 mm (RR 0.23, 95% CI 0.07 to 0.74). Atorvastatin was associated with a significantly reduced risk of developing AF (unadjusted RR 0.23, 95% CI 0.064 to 0.82, p = 0.024). This association remained significant after adjustment for these predictors (adjusted RR 0.19, 95% CI 0.052 to 0.72, p = 0.01). High-sensitivity C-reactive protein levels at baseline were not different between the 2 groups (p = 0.92). Although the high-sensitivity C-reactive protein levels decreased significantly 48 hours after EC compared with the baseline levels in group I (2.82 +/- 1.46 vs 2.56 +/- 1.3 mg/dl, p = 0.02), no significant change occurred in group II (2.87 +/- 0.8 vs 2.84 +/- 0.8 mg/dl, p = 0.09). In conclusion, atorvastatin decreased the recurrence rate of AF after EC.

Reverting cholesterol auxotrophy of NS0 cells by altering epigenetic gene silencing.

NS0 is a cholesterol-requiring mouse myeloma cell line widely used in the production of recombinant antibodies. We have previously reported that the deficiency of 17beta-hydroxysteroid dehydrogenase type7 (Hsd17b7) is responsible for the cholesterol auxotrophy of NS0 cells. Here we demonstrate DNA methylation to be the mechanism underlying transcriptional suppression of Hsd17b7 in cholesterol dependent NS0 cells. Analysis of the DNA methylation pattern revealed methylation of the CpG-rich region upstream of the Hsd17b7 transcription start site in NS0 cells. This is in contrast to the unmethylated status of this sequence in a naturally isolated cholesterol independent revertant cell population (NS0_r). This transcriptional repression was relieved after treating cells with the demethylating drug, 5-azacytidine. Drug treatment also gave rise to high frequency cholesterol-independent variants. Characterization of revertants revealed substantially elevated transcript level of 17beta-hydroxysteroid dehydrogenase type7 (Hsd17b7) gene along with hypomethylation of the CpG-rich region. These results affirm that deficiency of Hsd17b7 causes cholesterol dependence of NS0 cells. Furthermore, induction of cholesterol independence by altering DNA methylation pattern alludes to the role of epigenetics in the metabolic adaptation of NS0 cells. With the widespread use of NS0 cells, this finding will have a significant impact on the optimization of recombinant antibody production processes.

Bladder and bowel control in children with cerebral palsy: case-control study.

AIM: To determine the age of development of bladder and bowel control and the frequency of enuresis, encopresis, and urinary infections in children with cerebral palsy. METHODS: The study included 45 children with cerebral palsy who regularly attended a rehabilitation center in Isparta, Turkey, and two groups of age- and sex-matched children, 37 siblings of the children with cerebral palsy and 37 healthy children. Demographic data and information on the age of development of total bladder and bowel control and presence of possible urinary symptoms in children were collected from their caregivers by use of a questionnaire. Frequency of enuresis and encopresis was estimated among the children aged > or =5 years. A mid-way urinary sample was obtained from 40, 22, and 21 children in the cerebral palsy, siblings, and healthy children, respectively. RESULTS: The mean age of nighttime bladder and bowel control development was 47 months (95% confidence interval [CI], 35-58) and 45 (36-55) months, respectively, for the children with cerebral palsy, 35 months (95% CI, 24-46) and 26 months (95% CI, 24-28), respectively, for their siblings, and 27 months (95% CI, 22-33) and 25 months (95% CI, 23-27) months, respectively, for the healthy children. Among the children aged > or =5 years, enuresis was present in 11 of 34 children with cerebral palsy, 7 of 30 siblings, and 4 of 30 healthy children (P = 0.200), whereas encopresis was present in 5 children with cerebral palsy, one sibling, and one healthy child. Constipation was significantly more present in chidlren with cerebral palsy than in other two groups (P<0.001). Urine culture was positive in 13 children with cerebral palsy, 1 sibling, and 2 healthy chidlren (P = 0.024). There were no significant differences in other urinary symptoms and laboratory findings among the three groups. CONCLUSION: The children with cerebral palsy gained bladder and bowel control at older age in comparison with their siblings and healthy children. They also had more frequent enuresis and urinary infections.

Seroprevalence of fasciolosis and the difference of fasciolosis between rural area and city center in Isparta, Turkey.

OBJECTIVE: To investigate the seroprevalence of fasciolosis and the possible causes of differences between rural and city center. METHODS: We undertook a multi-stage sampling analysis of data from Isparta, Turkey, between March and June 2004. Four hundred and fifteen individuals participants from Isparta center and 171 from Asagi Gokdere village were included in the study. Fasciola hepatica (F. hepatica) specific antibodies were analyzed using excretory-secretory (ES)-enzyme-linked immunosorbent assay (ELISA) method. RESULTS: Fasciola hepatica antibodies were detected as positive in 10 (2.4%) of 415 people whose sera were collected from the city center and 16 (9.3%) of 171 people from Asagi Gokdere village. The positivity rates between village and city center were found statistically significant. A statistical difference was noted for fasciolosis positivity between individuals who have ingested water cress and who have not. Fasciolosis was not detected in the individuals who used to wash vegetables with water containing vinegar. CONCLUSION: Most of the patients in this region reported consumption of uncooked or unwashed water cress. Watering channel is one of the major risk factors of fasciolosis. Therefore, it is essential to determine the watering systems in this region. Moreover, ES-ELISA would be useful in investigating the laboratory diagnosis of fasciolosis.

Comparison of the four formulas of adjusting QT interval for the heart rate in the middle-aged healthy Turkish men.

OBJECTIVE: The aim of this study was to evaluate the QT intervals at different rest heart rates in healthy middle-aged Turkish men and to compare the known four QT adjusting methods for heart rate. METHODS AND RESULTS: The QT intervals were measured in electrocardiograms of 210 healthy men (mean age = 35-60 years). A curve relating QT intervals and heart rates from 45 to 135 beats/min was constructed for study population. Based on the formula of Bazett, Fridericia, and Framingham, adjusted QT intervals in these range of heart rates were separately estimated. An adjusting nomogram for different heart rates was created using a reference value, which was the measured QT interval at heart rate of 60 beats/min (QT(No) = QT + correcting number). These four QT correction methods were compared with each other. The reference value of QT interval at heart rate of 60 beats/min was 382 ms. The relationship between QT and RR interval was linear (r = 0.66, P < 0.001). Nomogram method corrected QT interval most accurately for all the heart rates compared with other three adjusting methods. At heart rates of 60-100 beats/min, the equation of linear regression was QT = 237 + 0.158 x RR (P < 0.001). Bazett's formula gave the poorest results at all the heart rates. The formulas of Fridericia and Framingham were superior to Bazett's formula; however, they overestimated QT interval at heart rate of 60-110 beats/min (P < 0.01). At lower rates (<60 beats/min), all methods except nomogram method, underestimated QT interval (P = 0.03). CONCLUSION: Among four QT correction formulas, the nomogram method provides the most accurately adjusted values of QT interval for all the heart rates in healthy men. Bazett's formula fails to adjust the QT interval for all the heart rates.

Global fibrinolytic capacity increased exponentially in metastatic colorectal cancer.

Colorectal cancers are one of the most common malignancies associated with coagulation abnormalities ranging from asymptomatic laboratory changes to massive thromboembolism or hemorrhage. It was previously shown that global fibrinolytic was increased in non-metastatic colorectal cancer. In this study global fibrinolytic capacity was measured in patients with colorectal cancer and metastatic liver disease, which always more commonly displays various coagulation disorders. Nineteen patients with biopsy-proven colorectal cancer, 30 patients with metastatic colorectal cancer, and 20 healthy control subjects were involved into the study. Using standart silicated fibrin pellets and tissue plasminogen activator, fibrinolytic capacity of the plasmas was detected with the amount of d-dimer produced before the reaction was stopped by adding aprotinin to the medium. Mean global fibrinolytic capacity (GFC) was increased to higher levels in patients with metastatic disease compared to levels in non-metastatic disease (p<0.05). Fibrinogen/GFC ratio correlated to the increase of d-dimer levels. Global fibrinolytic capacity was much higher in metastatic disease, reflecting a progression to overt disseminated intravascular coagulation.

Fluorosis and its hematological effects.

Although it has been reported that fluoride ingestion has no influence on various indices of hematopoiesis, some research has been published that excessive fluoride developed anemia and eosinophilia of leukocytes. Isparta is situated on the lake region of Turkey where fluorosis is endemic. Our aim was to explore the hematological effects in rats induced by fluoride. In this study, Wistar-Albino rats were used, divided into two groups as control and fluorized. While the control group was administered commercial water (including 0.07 ppm fluoride), the fluorized group was administered 100 ppm fluoride in commercial drinking water for four months. At the end of four months, hematological indices (Hb, Hct, MCV, MCH, RDW, RBC, WBC, and platelet counts) were measured. In addition, bone marrow samples were investigated. Mean leukocyte counts (WBC) in the control group and fluorized group were 7.07 (2.62-12.25) and 2.76 (3.13-5.24)x 10(3)/mm3, respectively. We observed displastic changes on granulocytes in the bone marrow samples of the fluorized group. Although there were significant statistical changes in WBC, we did not determine red blood cell and platelet changes in the fluorized group.

P-wave dispersion for predicting maintenance of sinus rhythm after cardioversion of atrial fibrillation.

P-wave measurements and left atrial function were investigated to predict the maintenance of sinus rhythm after cardioversion of atrial fibrillation. Left atrial dimension <45 mm (p = 0.02) and P-wave dispersion <46 ms (p <0.001) were independent predictors of sinus rhythm maintenance, with a sensitivity of 89% and 96%, respectively. Duration of atrial fibrillation, maximum P-wave duration, and no spontaneous echocardiographic contrast were also univariate predictors.

Prevalence of internet usage and access to health information among dental school outpatients.

The aim of this study was to investigate the prevalence of Internet access and use among dental school outpatients to evaluate the type of information they seek and their views regarding health-related information. A total of 400 consecutive outpatients were surveyed. A questionnaire was used to collect data on demographic characteristics and Internet use. Users were asked about the frequency and location of their Internet access. Access to health-related information and medical and dental topics of interest was recorded. Participants expressed their opinions on the usefulness of the information and the improvements that might be needed. A total of 33.0% of the participants were Internet users. Those in the 15- to 24-year age group were male, unmarried, and at school and were much more likely to use the Internet than their counterparts. Twelve percent of the users were seeking online health information. Dental information was sought by 16.7% (n = 8) of online health seekers. Those in the 25- to 34-year age group were married, employed, and who have a university degree and were much more likely to seek health information on the Internet than their counterparts. Currently, persons seeking online health information in this population in Turkey are a small minority. It is important to respond to the specific health needs of the Internet users to post accurate information.

P wave dispersion and left atrial appendage function for predicting recurrence after conversion of atrial fibrillation and relation of p wave dispersion to appendage function.

BACKGROUND: We investigated P wave dispersion and left atrial appendage (LAA) function for predicting atrial fibrillation (AF) relapse, and the relationship between P wave dispersion and LAA function. METHODS: Sixty-four consecutive patients with AF lasting </=3 months were evaluated to predict the recurrence after successful cardioversion. P wave duration and dispersion were measured in a 12-lead electrocardiograph (ECG). The size and function of the left atrium (LA) and LAA were assessed by transthoracic and transesophageal echocardiography. RESULTS: After 6 months, 28 patients experienced recurrent AF and 36 remained in sinus rhythm. There was no difference between patients with and without recurrence in gender, age, underlying heart disease, AF patterns, left ventricular function, and maximum LAA area. AF duration >/=5 days, LA size >/=45 mm, maximum P wave duration >/=112 ms, P wave dispersion >/=47 ms, spontaneous echo contrast, minimum LAA area >/=166 mm(2), and LAA emptying velocity <36 cm/sec were univariate predictors of recurrence (each P < 0.05). By multivariate analysis, LA size (P = 0.02), P wave dispersion (P < 0.001), and LAA emptying flow (P = 0.01) identified patients with recurrent AF. Their positive predictive values were 91, 97, and 72%, respectively. CONCLUSION: The increased P wave dispersion in addition to the dilated LA and the depressed LAA emptying flow can identify patients at risk of recurrent AF after cardioversion.

Improving control of high blood pressure among middle-aged Turkish women of low socio-economic status through public health training.

AIM: To assess the effects of health training course aimed at educating middle-aged women of low socio-economic status in Turkey how to better control high blood pressure. METHODS: The intervention study included 400 randomly selected women aged (mean+/-standard deviation) 34.1+/-8.6 years. After their body size and blood pressure were measured, the women were randomly divided into two groups. The intervention group underwent public health training on high blood pressure control and obesity reduction, whereas control group had no training at all. After six months, blood pressure and body size of women in both groups were measured again. RESULTS: Overall, the initial high blood pressure and obesity ratio in the intervention and control group were 20.0% and 31.7%, respectively. After 6 months of the intervention program, the number of women within the optimal weight range significantly increased in the intervention group compared with control group (p=0.009). After the intervention program, the number of women with normal weight and normal blood pressure in the intervention group significantly increased (weight: 130 women before vs 150 after, p=0.001; blood pressure: 160 women before vs 173 after, p<0.001). After the intervention, the percentage of meals consisting of fried food in the intervention group was lower by 31.9%, and by 5.0% in the control group. After the intervention the percentage of boiled food increased by 15.7% in the intervention group, compared with no change in the control group. The consumption of food high in salt decreased by 5.5% in the control and by 72.2% in the intervention group. Similarly, the percentage of women who exercised almost doubled in the intervention group, whereas there was no change in the control group. CONCLUSION: The changes in the lifestyle significantly reduced hypertension and obesity ratio among women. The public health center can provide initial health training support as a part of the broader public health management program, aimed at helping people tackle health problems.

Socio-demographical examinations on disability prevalence and rehabilitation status in southwest of Turkey.

PURPOSE: Disability is one of the most important social issues in the community. In contrast to the developed countries, Turkey has, unfortunately, no goods records on the current issues of disability such as the number of disabled and handicapped, and their socio-demographic properties due to the lack of sufficient studies in the specified area. The present study was, therefore, aimed to determine the status of disabled in the entire region of Isparta and to establish relationships between the distinctive disability properties in order for a better understanding of its important aspects. MATERIALS AND METHODS: Data were collected in 2002 from 3500 people residing throughout the region by the gradual sampling method according to the evaluation criteria of International Classification of Impairments, Disabilities and Handicaps (ICIDH) by WHO. RESULTS: The disability prevalence in the studied group was found to be 5.3%, and 4.8% of the disabled were previously rehabilitated. Forty one percent of the disabled were found to have two types of disability. In the study, it was shown that the disability prevalence significantly increased by the age. The disabled were shown to be under-educated and not benefit from the rehabilitation services in one hand, and there were insufficient activities of rehabilitation established in the region in other hand. CONCLUSIONS: We concluded that the medical and social rehabilitation services must be initiated and freely provided to these local people, particularly to those young disables whose life will greatly change to a better living standard.

The role of inducible nitric oxide synthase inhibitor, meropenem, and taurine in experimental acute necrotizing pancreatitis.

INTRODUCTION: Translocation of bacteria from the gut is one of the most important factors in the development of septic complications and mortality in acute pancreatitis. AIMS: To investigate whether S-methylisothiourea (SMT), taurine (TAU), and meropenem (MER) could effect bacterial translocation and the course of acute necrotizing pancreatitis. METHODOLOGY: Seventy male Sprague-Dawley rats were studied. Rats were randomly allocated into seven groups. Acute pancreatitis was induced in group II (MER), group III (TAU), group IV (TAU + MER), group V (TAU + SMT), group VI (TAU + MER + SMT), and group VII (positive control) by retrograde injection of taurocholate into the common biliopancreatic duct. Group I rats (sham) received normal saline infusion into the common biliopancreatic duct as negative control. Rats were treated with drug combinations intraperitoneally for 48 hours after induction of pancreatitis. At the 48th hour of induction, all animals were killed, and specimens were collected. RESULTS: Bacterial translocation to peritoneum and pancreas in groups treated with MER were lower than in the other groups. Pancreatic tissue GSHpx and SOD levels were higher in all groups in comparison with levels in group VII. Pancreatic tissue MDA levels were also lower in all treatment groups except group II. The most favorable results were obtained in group VI (TAU + MER + SMT). Also, the lowest pathologic score between the groups in which acute pancreatitis developed was obtained in group VI. CONCLUSIONS: Addition of TAU and SMT to the treatment protocol for acute pancreatitis seems to improve the pathologic score and oxidative stress parameters. Also, antibiotherapy with MER decreases the risk of bacterial translocation.

Global fibrinolytic capacity in colorectal cancer: a new clue to occult fibrinolysis.

Hemostatic disorders in cancer patients range from asymptomatic laboratory changes to massive thromboembolism or hemorrhage. Routine laboratory tests often fail to identify patients at high risk for hemostatic complications. The postoperative risk of thromboembolic events in colorectal cancer was reported as approximately 2% to 5%. A new diagnostic test was used to assess global fibrinolytic activity, which may detect occult fibrinolysis or disseminated intravascular coagulation in patients with colorectal cancer. Twenty patients with colorectal cancer and 20 healthy control subjects were involved. Using standard silicated fibrin pellets and tissue plasminogen activator, the fibrinolytic capacity of the plasma samples was detected with the amount of D-dimer produced before the reaction was stopped by adding aprotinin to the medium. Mean global fibrinolytic capacity was increased in patients before and after surgery when compared to controls (p = 0.002, p = 0.001, respectively). In conclusion, a hemostatic imbalance was detected toward primary fibrinolysis in the preoperative period and low-grade disseminated intravascular coagulation postoperatively.

Does fibromyalgia affect the outcomes of local steroid treatment in patients with carpal tunnel syndrome?

Carpal tunnel syndrome (CTS) and fibromyalgia (FM) are common diseases in adult women. The aim of this study was to investigate the effect of FM on the outcome of local steroid treatment in patients with CTS. Forty-eight female patients with CTS and 26 female patients with CTS who also met the 1990 American College of Rheumatology (ACR) criteria for FM were enrolled the study. All patients underwent single steroid injections into the carpal tunnel. Response to injection was measured using Boston Questionnaire (BQ) and electrophysiological tests. The BQ scores and electrophysiological findings showed significant improvement 3 months after the treatment in both groups ( P<0.001). However, less improvement in BQ symptom severity scores was observed in the FM group than the other group ( P<0.05). Bilateral CTS was also more common in the FM group ( P<0.05). The present data suggest that FM must be kept in mind in patients with CTS giving poor response to local steroid treatment.
