Positive effects of phenytoin on experimental colonic anastomoses.

BACKGROUND AND AIMS: Anastomotic dehiscence following colorectal surgery is a significant cause of morbidity and mortality. Phenytoin has wound-healing promoting and collagenase inhibitory effects. This study assessed these effects on healing of experimental colonic anastomoses in a rat model. MATERIALS AND METHODS: Ninety Wistar rats weighing 240-290 g were divided into six groups: 3rd-day control group (n=15), 3rd-day oral administration of phenytoin (n=15), 3rd-day rectal administration of phenytoin (n=15), 7th-day control group (n=15), 7th-day oral administration of phenytoin (n=15), and 7th-day rectal administration of phenytoin (n=15). In oral phenytoin groups the agent was given at 10 mg/kg daily per orogastric route by 4-F fine feeding catheter; in rectal phenytoin RAP groups the agent was administered at 10 mg/0.5 cc daily to the anastomoses transrectally via a fine anal catheter. RESULTS: There were significantly higher anastomosis bursting pressure values and hydroxyproline contents in phenytoin groups than in controls. In histopathological examination it was seen that phenytoin treatment caused greater collagen deposition, fibroblast, and blood vessel ingrowth than in controls. Immunohistochemical analysis showed the stimulatory effect of phenytoin in expression of vascular endothelial growth factor and basic fibroblast growth factor. Anastomosis bursting pressure, histopathological analysis, hydroxyproline content, and immunohistochemical results were better in the groups with rectal administration than in those with oral administration. CONCLUSION: These results had showed us that phenytoin administration resulted in enhanced stability of colonic anastomoses during the first postoperative week and rectal administration showed better results than oral administration.

Influence of media characteristics on energy dissipation in filter backwashing.

Effective cleaning of granular filters during backwashing processes needs maximum turbulence and maximum shear in the fluid particle field. The energy dissipation in a backwashed filter as a particulate fluidized bed arises due to the suspending and random motions of particles and turbulent fluctuations in the bed. Size, density, and sphericity of the filter materials greatly influence the fluidization behavior of the media. In this study, a new model is proposed for predicting the energy dissipation parameters namely the hydrodynamic shear stress (tau(a)), the velocity gradient (G(a)), the turbulence dissipation coefficient (C(a)), and the turbulence parameter (C(a)0.5/Re) in backwashing of filters for different types of filter materials (sand, anthracite, and glass ball). The hydrodynamic shear stress is the dominant mechanism of filter cleaning and appears to increase with increasing the density and size of the filter media particles. Using the basic set of data, a step by step procedure is developed to compute the velocity gradient G(a), the turbulence dissipation coefficient C(a), the hydrodynamic shear stress tau(a), and the turbulent parameter (C(a)0.5/ Re).

Effect of nifedipine on the healing of left colonic anastomoses in rats.

PURPOSE: Ca(2+) channel blockers inhibit collagenase production and have a vasodilatatory effect. They also restrict the formation of ischemia-reperfusion induced free oxygen radicals. The aim of this study was to assess the effect of nifedipine on the healing of colonic anastomoses in a rat model. METHODS: Sixty Wistar rats weighing 240-290 g were divided into four groups of 15 rats each: a 3rd day control group (group A), a 3rd day treatment group (group B), a 7th day control group (group C), and a 7th day treatment group (group D). The treatment groups were given Nifedipine 3 mg/kg per day orally as three divided doses. RESULTS: The bursting pressure values of the anastomoses in the treatment groups were significantly higher than those in the control groups ( P < 0.05). The hydroxyproline content was also significantly higher in the treatment groups than in the control groups ( P < 0.05). Histologic examination confirmed that nifedipine treatment significantly increased collagen deposition and fibroblast ingrowth compared with controls ( P < 0.05). CONCLUSIONS: These results clearly showed that nifedipine enhanced the stability of colonic anastomoses during the first postoperative week.

Thyroid hormone supplementation in sepsis: an experimental study.

PURPOSE: The levels of thyroid hormones decrease in septic, critically ill, and major trauma patients who show a bad prognosis. The aim of this study was to investigate the effects of thyroid hormone supplementation on the sepsis criteria and mortality in an experimental sepsis model. METHODS: Forty-eight Wistar-Albino rats were divided into four experimental groups. A cecum ligation and puncture was used as a sepsis model. Group I: sham; group II: a cecum ligation and puncture (control); group III: a cecum ligation and puncture +T3 (0.4 microg/100 g body weight/day) intraperitoneally; group IV: a cecum ligation and puncture +T4 (1 500 ng/100 g body weight/day) intramuscularly. The PCO(2), leukocyte count, peritoneal bacterial content, neutropenia, and mortality rates were assessed in all groups. RESULTS: The PCO(2) levels decreased only in the control group ( P < 0.05). The leukocyte counts were lower in the therapy groups ( P < 0.05). The rate of neutropenia was seen much more in the control group ( P < 0.05). The peritoneal bacteria content decreased in the therapy groups ( P < 0.05). The mortality rate significantly decreased in the thyroid hormone-treated animals ( P < 0.05). CONCLUSIONS: Thyroid hormone supplementation in septic rats had a beneficial effect on the sepsis criteria and also resulted in a lower rate of mortality.

Sigmoid colon perforation as an unusual complication of Behçet's syndrome: report of a case.

A 47-year-old man with long-standing Behçet's syndrome presented with an acute abdomen, and was found to have perforation of the sigmoid colon. Laparotomy revealed gangrenous changes in the sigmoid colon and perforation in the center of the affected segment. This is a very rare complication of Behçet's disease, and we report this case to stress the importance of performing careful abdominal examination while evaluating patients with Behçet's disease.

Clay mineral batch process for color removal of textile wastewaters.

Textile industry effluents exhibit large amounts of dye chemicals, which create severe water pollution. It is therefore important to reduce the dye concentration in the wastewater before discharging into the environment. In this study, the ability of sepiolite to uptake different reactive dyes is investigated. The dyes used in the experiments are Everzol Black B, Everzol Yellow 3RS H/C, and Everzol Red 3BS. A number of parameters on adsorption has been investigated in order to establish the adsorption pattern of sepiolite. The effect of conditioning time, solid/liquid ratio, pH, temperature, and dye concentrations on adsorption are studied. Adsorption isotherms for different reactive dyes have been obtained using both natural and modified sepiolites. The maximum capacity of natural sepiolite for the reactive dyes occurs at around 0.1-1 mg/g. Therefore, the natural sepiolite appears to have a little capacity for reactive dyes. The zeta potential profiles of natural and modified sepiolites against pH are also examined. The modified sepiolite exhibited ten times higher performance than the natural sepiolite.

Comparison of different thyroidectomy techniques for benign thyroid disease.

Extent of thyroidectomy in the management of benign thyroid disease remains controversial. In this clinical study, three different thyroidectomy techniques were compared by means of the complication, short period recurrence and L-thyroxin requirement rates. Two hundred consecutive patients who had bilateral subtotal thyroidectomy (BST) (n = 71), unilateral total lobectomy + contralateral subtotal lobectomy (Dunhill Procedure (DP)) (n = 71), or total thyroidectomy (TT) (n = 58) for benign thyroid disorders were included in this study. One patient was re-operated due to bleeding in BST group. Wound infection was observed in 1 patient both in BST and DP group and 2 patients in TT group. Temporary hypocalcaemia was seen in 14 (19.7%) of BST group, in 19 (26.7%) of DP group, and in 14 (24.1%) patients of TT group (p>0.05). Transient recurrent laryngeal nerve palsy developed in 1 patient both in DP and TT group. One patient of DP group had secondary thyroidectomy due to postoperative diagnosis of papillary carcinoma. There was no significant difference in the mean durations of hospitalization between the groups. Mean postoperative follow-up periods were 27.7 months (6-56), 34.8 months (8-55), 26.5 months (6-54) in BST, DP and TT groups, respectively. While all patients were administered L-thyroxin in TT group, 26 (36.6%) patients in DP group and 34 (47.8%) patients in BST group needed no L-thyroxin supplementation and L-thyroxin requirement rates were not different in either group. We think that total thyroidectomy should be adopted for benign thyroidal diseases, because its complications are no different than those for BST and DP. If individual factors and patient's preference are not in favor of lifelong L-thyroxin supplementation, however, DP may be carried out for benign thyroidal diseases instead of BST.

Traumatic pelvic hydatid cyst rupture: report of a case.

Secondary peritoneal echinococcosis is one of the important complications of cyst hydatid disease. Although many ruptured hydatid cysts of a primary organ have been reported, there is only one case of peritoneal hydatid cyst rupture in the literature. In this case report we present a 25 year-old male patient, who had been operated for hydatid cyst of the liver four years ago and had pelvic hydatid cyst rupture due to blunt abdominal trauma.

A simplified operative technique for single-staged resection of left-sided colon obstructions: report of a 9-year experience.

PURPOSE: The operative strategy for left-sided large bowel obstruction remains controversial. Because a safe and definitive single-staged operation that avoided a colostomy would clearly be in the patients' best interest, we conducted a prospective study to compare the efficacy of single-staged surgery (SSS) supplemented by the milking and swabbing technique (MST), with the conventional Hartmann's procedure (HP). METHODS: In group 1 ( n = 37) we performed traditional HP by resecting the site of obstruction and constructing an end colostomy. In group 2 ( n = 33), after resecting the site of obstruction, instead of on-table lavage, we cleaned out the fecal content from the proximal and distal parts of the obstruction by milking the colon with the fingers, then swabbing the proximal and distal 10 cm of lumen of the colonic anastomosis with povidone iodinized stick sponges. Finally, we performed a primary one-layer anastomosis without fecal diversion. RESULTS: The mortality, morbidity, and postoperative hospital stay after emergency left-side colonic resection and primary anastomosis by MST was comparable with those after HP ( P > 0.05). CONCLUSIONS: We believe that SSS with MST is a viable choice in the surgical management of selected patients with obstructing lesions of the left colon. The advantages of SSS with MST lie in its good long-term results, and short-term reduced surgical intervention and hospital stay.

Antimicrobial susceptibility of Bacillus anthracis in an endemic area.

We aimed to test the antimicrobial susceptibility of 28 Bacillus anthracis strains isolated from cutaneous anthrax cases to various antimicrobial agents using the Sceptor automatic system in an anthrax endemic area. All strains tested were susceptible to penicillin (MIC < or = 0.03 microg/ml). Piperacillin-tazobactam and carbapenems showed good activity towards all strains. Trimethoprim-sulfamethoxazole and cefepime had no activity. Strains were also tested with other antimicrobials.

Cumulative prior dose of cisplatin as a cause of the nephrotoxicity of high-dose chemotherapy followed by autologous stem-cell transplantation.

BACKGROUND: Nephrotoxicity is one of the most frequent dose-limiting factors of high-dose chemotherapy to create tolerance of autologous peripheral blood stem-cell transplantation (PBSCT) for the treatment of malignant diseases. The relative importance of factors that may be responsible for the development of nephrotoxicity varied in different trials. METHODS: The factors affecting nephrotoxicity in the early period of high-dose ifosfamide, carboplatin and etoposide treatment (ICE) followed by autologous PBSCT was investigated in 47 patients. ICE was given as a conditioning regimen for 6 days. Nephrotoxicity was defined as an increase in the serum creatinine concentration of 0.5 mg/dl or more over individual baseline levels. RESULTS: Eleven patients developed nephrotoxicity (23.4%). There was no significant difference in baseline renal function between patients with nephrotoxicity and those without. No differences were found between the two groups in terms of average total doses of ICE, infections and antibiotic use. The age of patients was higher in those with nephrotoxicity (37+/-3.7 vs 26+/-1.7 years, P=0.019). The cumulative cisplatin dose administered prior to this regimen was higher in the group that developed nephrotoxicity (470 vs 227 mg/m(2), P=0.02). The overall mortality rate was 17%, but the transplant-related deaths were higher in the presence of nephrotoxicity (54.5 vs 5.5%, P=0.001). CONCLUSIONS: The cumulative dose of cisplatin is a strong risk factor for the development of nephrotoxicity in patients who receive high doses of ICE followed by PBSCT. Nephrotoxicity may occur with much lower doses than the currently recommended maximum doses.

Pancreatic pseudotumor due to peripancreatic tuberculous lymphadenitis.

Peripancreatic tuberculous lymphadenitis is a very rare and difficult diagnosis. We report herein a patient with a clinically solitary abdominal tuberculoma. A 68-year-old woman was admitted to our hospital with moderate-level obstructive jaundice due to a mass located between pylorus and head of the pancreas. There were no clinical signs or symptoms of tuberculosis in lungs or abdomen. After the diagnosis of a neoplasm of the pancreas was made, exploratory laparotomy was performed which revealed a conglomerated mass penetrating into the pancreas. Since an exact diagnosis could not be reached on the basis of frozen sections prepared during the operation, a standard Whipple procedure was performed. After the histopathological examination of the resected specimen revealed tuberculous lympadenitis, the patient was given antituberculous medication. The patient recovered well. An abdominal tuberculoma is often mistaken for a malignant neoplasm, and nonsurgical diagnosis and treatment of this entity continues to be a challenge.

The effect of leptin on engraftment in patients undergoing peripheral blood stem cell transplantation.

AIM AND BACKGROUND: To evaluate the alterations of serum leptin levels during stem cell transplantation and its possible role in engraftment. Thirty-two patients (19 male, 13 female) with various hematological and solid tumors and 28 healthy subjects (15 male, 13 female) as a control group were enrolled in the study. METHODS: Serum leptin levels were measured on the day before administering G-CSF, at the time of leukapheresis harvest, on day +1st and +7th after transplantation and on the day of leukocyte engraftment. RESULTS: There was no significant difference in serum leptin levels between patients (mean +/- SEM, 11.62 +/- 2.75 ng/ml) before transplantation and control groups (9.79 +/- 1.73 ng/ml). Pre-G-CSF (baseline) level of serum leptin (11.62 +/- 2.75 ng/ml) was significantly decreased to 7.73 +/- 2.02 ng/ml at the time of apheresis harvest (P = 0.0029). Later, serum leptin levels increased to 16.75 +/- 3.26 ng/ml on day +1 after transplantation (P < 0.0001). Subsequently serum leptin levels both on day +7th posttransplant (12.11 +/- 2.17 ng/ml) and leukocyte engraftment day (9.26 +/- 1.50 ng/ml) were gradually decreased. There was no correlation between the serum leptin levels and the leukocyte or platelet engraftment. CONCLUSION: The present study concludes that serum leptin level does not change remarkably during peripheral blood stem cell transplantation and no association exists between circulating leptin levels and the onset of engraftment suggesting that circulating serum leptin does not have a significant direct influence on engraftment.

Total parenteral nutrition delays platelet engraftment in patients who undergo autologous hematopoietic stem cell transplantation.

OBJECTIVES: One of the major challenges in the post-transplant period is nutrition. In this prospective, non-randomized study, total parenteral nutrition (TPN) was given to 31 patients and partial parenteral nutrition (PPN) was given to 30 patients undergoing autologous hematopoietic stem cell transplantation for solid tumors or hematologic malignancies to compare the effects of these parenteral nutrition modalities on post-transplant hematological engraftment, blood chemistry, and supportive therapy requirements. METHODS: All patients in the TPN group and 17 patients in the PPN group received growth factor in the post-transplant period. Both groups did not differ with respect to sex, age, and reinfused CD34(+) cell numbers. RESULTS: After transplantation body mass index and body weight decreased significantly in both groups (P < 0.001). Whereas serum albumin concentrations did not decrease significantly in the TPN group, it fell markedly in the PPN group at the end of parenteral nutrition (P = 0.019). After parenteral nutrition, blood chemistry was also remarkable for serum urea and glucose levels, which were elevated significantly in the TPN group (P < 0.001 and P = 0.03, respectively). Patients receiving TPN had a higher incidence of positive microbial cultures and clinical infection than did patients receiving PPN (64.5% versus 40%, P = 0.05). The most striking result was a delay in platelet engraftment for the TPN group compared with the PPN group (15.54 and 12.93 d, respectively; P = 0.014). This difference was also noted in patients using growth factor in the PPN group (P = 0.017). Parallel to these results, platelet transfusion requirement increased in the TPN group compared with the PPN group (1.93 versus 1.16 U, P = 0.004). Both groups were unremarkable for leukocyte recovery and red blood cell transfusion requirement. CONCLUSIONS: Consequently, TPN has some pitfalls of hyperglycemia, infection tendency, delayed platelet engraftment, and increased platelet transfusion requirement. Therefore, it should not be used as a standard nutrition support for patients undergoing autotransplantation.

Surgical management strategies in patients with primary hyperparathyroidism: which technique in which patients?

OBJECTIVE: This study was planned to investigate the efficacy of either 99mTc-sestamibi scan or ultrasonography in predicting the operative treatment in patients with primary hyperparathyroidism (PHPT). SUBJECTS AND METHOD: Thirty patients (25 female, 5 male; mean age: 53 years) being operated for symptomatic primary PHPT were included in this study. Ultrasonography was used in 29 patients while 99mTc-sestamibi scintigraphy was done in 28 patients to localize the hyperfunctioning gland(s). Standard bilateral neck exploration was done in 6 patients. Although unilateral intervention had been planned for 24 patients, bilateral intervention was performed in 9 of them. RESULTS: Sensitivity of 99mTc-sestamibi was 81%, while that of ultrasonography was 42%. 99mTc-sestamibi localization method led to misleading results in 10/28 (35.7%) patients. False-positive localization and accompanying thyroid pathologies played an important role in determining transition from unilateral to bilateral intervention. CONCLUSION: Our findings indicate that bilateral intervention remains a successful management option without prior localization in patients with PHPT especially in an endemic goiter region.
