[Relationships between arterial hypertension, metabolic disturbances and uric acid nephropathy].

AIM: To study effects of uric acid disbolism in combination with other metabolic disturbances on prevalence and clinical symptoms of arterial hypertension; contribution of uric acid nephropathy to its development. MATERIAL AND METHODS: A cohort of 243 representatives from a general population (107 males, 136 females, age 19-59 years) was studied in 1986 and 15 years later. At the beginning and end of the trial the participants were examined for arterial hypertension, microalbuminuria, uric acid nephropathy, blood biochemical tests were made. RESULTS: For 15-year follow-up prevalence of arterial hypertension doubled, incidence of hyperuricemia and hyperuricosuria, hypertriglyceridemia increased considerably. The number of hypertensive subjects was maximal in the group with the greatest number of signs of uric acid nephropathy. CONCLUSION: The population studied exhibited increasing frequency of some risk factors, especially of uric acid metabolism, contributing to formation of arterial hypertension.

[The features of uric acid metabolism in women in climacterium].

The purpose of the study was to establish the prevalence of uric acid (UA) metabolism disturbances as a possible risk factor of urate nephropathy in women of various age groups. The authors carried out a retrospective analysis of 757 case histories and performed uricemia and uricosuria screening in women aged 18 to 69 years. The study revealed significant age-dependent elevation of uricemia, with hyperuricemia (HUE) at the age of 60 to 69 years old. The study also found age-dependent increase of HUE rate with its significant elevation in postmenopausal women (older than 50 years), compared with HUE rate in reproductive age women (18 to 49 years old) -37.9% vs. 11.4%. For the first time the researchers established UA excretion level and found a significant tendency of uricosuria to decrease with age (p < 0.016 between the groups of 40 to 49 and 50 to 59 years old). The highest rate of hyperuricosuria was revealed in women of 40 to 49 years old (22.2%), while in women of 18 to 29 and 60 to 69 years old it was 10.1% to 12.1%. For the first time the authors established the rate of hypouricosuria in women, which culminated in women older than 50 (22% of cases). The increase of HUE rate, which is most prominent in postmenopausal women, is associated with the decrease in UA excretion, accompanied by the decrease of hyperuricosuria rate and increase of hypouricosuria rate. Taking into account high prevalence of UA metabolism disturbances in women older than 50, menopause may be considered a risk factor of HUE and urate nephropathy.

[Hormone replacement therapy in women with arterial hypertension in peri- and postmenopause: hemodynamic effects].

AIM: To study hemodynamic effects of replacement hormone therapy (RHT) with trisecvens, beta-blocker betaksolol and their combination in perimenopausal and menopausal women with hypertension. MATERIAL AND METHODS: The study was made of 60 menopausal women aged 45-60 with mild and moderate arterial hypertension (AH). The women had no contraindications to either RHT or beta-blockers. They were randomized into three groups: group 1 received trisekvens, group 2--trisekvens plus betaksolol in a dose 10-20 mg/day, group 3--betaksolol. Arterial pressure (AP) and heart rate (HR) were measured before the treatment, in one month and each three months for a year. ECG and echo-CG were registered before treatment and each 3 months of the therapy. AP monitoring covered 50% of the patients of each group before the treatment and after 1 and 3 months of it. The data were analysed according to SAS system. RESULTS: Group 1 patients showed no significant changes in AP and HR. Group 2 and 3 patients' AP lowered, in group 2 the fall of systolic AP being more pronounced. 12-month therapy brought about a 10.3% decrease in left ventricular myocardial mass index in group 2. CONCLUSION: RHT with trisekvens in combination with beta-blocker in long-term use potentiates the effect on systolic AP and reduced hypertrophy of the left ventricle. RHT does not produce a significant effect on AP in women with AH recorded before the menopause.

[Betaxolol treatment of hypertension in postmenopausal women].

AIM: The study of hypotensive efficacy of high-cardioselective beta-blocker betaxolol, its effects on lipid and carbohydrate metabolism, menopausal syndrome in females with mild and moderate arterial hypertension. MATERIALS AND METHODS: 20 postmenopausal 45-59-year-old women entered the trial of betaxolol. They had diastolic blood pressure 90-114 mm Hg. Beta-blockers were not contraindicated. Arterial pressure, heart rate, body mass, blood lipid spectrum, glucose and insulin levels were evaluated before the treatment, 1, 3 and 6 months after it. RESULTS: Betaxolol safely and effectively lowered blood pressure. Blood lipid spectrum remained unchanged. A transitory rise in fasting insulin levels did not worsen glucose tolerance despite a small gain in body mass. Menopausal syndrome relieved due to good effect of betaxolol on vasomotor disorders. CONCLUSION: Betaxolol has a good hypotensive effect in the absence of adverse effects on lipid and carbohydrate metabolism. A reduction of menopausal syndrome was also achieved.

[The Chediak-Higashi syndrome].

The Chediak-Higashi syndrome is described. The morphology of the blood and bone marrow in a girl aged 4.5 months [correction of years] is described in detail. Hematological, immunological, and karyological characteristics of her parents are presented.

The effect of low-dose irradiation on proliferation of mammalian cells in vitro.

The effect of low doses of gamma radiation on proliferation of Raji lymphoma cells and Chinese hamster fibroblasts in vitro has been investigated. It is shown that irradiation in the dose range between 2 and 20 cGy (maximum at 10 cGy) stimulates growth of cultured cells due to the shortening of the lag phase of cell growth, with the duration of the doubling time being unchanged. Preirradiation of growth medium in this dose range also stimulates cell proliferation, though to a lesser extent than irradiation of cells and the medium does. The stimulatory effect manifests itself in an increase in the cell number as well as in the size and number of colonies.

[Results of 2-year nondrug treatment of patients with a mild form of arterial hypertension at the health centers of an industrial enterprise].

Results of two years of observation over groups of intervention among men and women indicate that the efficacy of non-medicamentous treatment of mild arterial hypertension in women was 1.5 times higher than in men (90 and 65%, respectively). During the same observation period, the mean level of the DBP in women dropped to a larger degree than in men while in women, the treatment usually made the DBP return to normal whereas in men, only the borderline DBP level could be attained. Thus, the systematic control of the BP in patients with mild arterial hypertension, the patients' counselling as regards the healthy way of life, aiding the patients in the observance of health recommendations, early transfer of patients to drug therapy, if the disease takes an unfavourable course, make it possible to attain satisfactory therapeutic effects, the hypotensive effect included, without resorting to permanent use of drugs.

[Interrelation between arterial hypertension and the data of their obstetrical and gynecological anamnesis in women aged 30 to 54--workers in a Moscow industrial enterprise].

The population studies carried out among women engaged in automotive industry have demonstrated the relationship between arterial hypertension (AH) and an aggravated obstetric and gynecological anamnesis. In climacteric women, AH has been found to occur twice as frequently as in those of the same age with preserved menstrual function.

[Diagnostic value of electrocardiographic criteria of left ventricular hypertrophy in hypertension in a population study].

The correlation between the diagnosis by the criteria of the Minnesota code and the known indices of left ventricular hypertrophy (LVH) (RAVL greater than or equal to 7 mm, Rv5,6 + Sv1,2 greater than greater than 35 mm, Rv6 greater than Rv4 Sv2 greater than 24 mm) in 12 standard leads and the roentgenocardiometric data has been analyzed. 156 males (35-54 years of age) and 175 females (30-52 years of age) suffering from first and second stage of essential hypertension as well as 60 male and 30 female controls from the staff population of the Likhachev Plant have been studied. The study has shown that sensitivity of the above ECG criteria for LVH, diagnosed by roentgenocardiometry, is higher in males than in females, their specificity being higher in females. The correlation ratio of ECG criteria with established LVH is 0.18 for males, and 0.26 for females. In the first stage of essential hypertension no correlation between ECG criteria for LVH and roentgenocardiometric data were observed. The second stage of essential hypertension was characterized by negative correlation between ECG criteria for LVH and roentgenocardiometric data.
