Adenoma, Sweat Gland↗

Hyponatraemia and hypothyroidism in a previously irradiated case of carcinoma of the tongue.

A case of squamous cell carcinoma of the tongue was complicated by post operative hyponatraemia. The criteria for the syndrome of inappropriate secretion of anti diuretic hormone, [ADH] (SIADH) were met but the patient remained hyponatraemic despite adequate treatment. The patient had previously received radical external radiotherapy to the neck and was found to be profoundly hypothyroid. Correction of the hypothyroid state led to clinical and biochemical recovery. The frequency of post-irradiation hypothyroidism and the possible mechanisms of hypothyroid-induced hyponatraemia are discussed.

The versatility of pectoralis major and latissimus dorsi myocutaneous flaps in the reconstruction of cancrum oris defects of children and adolescents.

The application of the recently described myocutaneous flaps in 7 cases of cancrum oris defects is shown and the consistency of the blood supply in children is stressed. The surgical anatomy is mentioned and the use of the flaps in the reconstruction of defects of the cheek, angle of mouth, lips and chin is illustrated. The excellence of the muscle island flap in preventing reankylosis if interposed into new joints in the body of the mandible is emphasised, together with the advantages over the forehead and delto-pectoral flaps when used for the same purpose.

The use of the Mini-Flap wound suction drain in maxillofacial surgery.

Preliminary experience using the Mini-Flap closed suction drain after maxillofacial surgery is described. The drain fulfils many of the criteria required for drainage of small wounds about the face and neck, and is associated with a low incidence of postoperative haematoma, oozing and infection.
