[Local signs of the reperfusion syndrome in obliterating atherosclerosis of the lower extremity arteries and their treatment].

There were examined 32 patients with obliterating atherosclerosis of the lower extremities arteries, in whom the reperfusion syndrome have occurred after performance of the restoration operation. In 8 patients aorto-femoral shunting was performed and in 24--femoro-popliteal one. There was established that local signs of the reperfusion syndrome, as a rule, have occurred in patients with critical ischemia of the lower extremities preoperatively. Characteristic signs of the reperfusion syndrome were noted, the methods of its prophylaxis and treatment were adduced.

[Application of octreotide in patients with acute pancreatitis].

In 1986-2002 yrs period 515 patients were treated in the clinic for an acute pancreatitis. The severity of the disease was assessed according to clinical and laboratory data, basing on Ranson scale. Severe acute pancreatitis was diagnosed in 45 (8.7%) of patients. In 16 patients together with conventional treatment octreotide was applied in 100-300 mkg/day dosage during 5 days. Fast improvement of the patients state and elimination of acute pancreatitis symptoms was noted. All the patients are alive.

[Changes of regional hemodynamics and microcirculation in patients with obliterating atherosclerosis in stage of chronic critical ischemia of the lower extremities before and after of restorational operation performance].

The state of regional hemodynamics and microcirculation in 47 patients with obliterating atherosclerosis of the lower extremities vessels in stage of chronic critical ischemia before and after performance of restorational operation was studied up. Ileo-femoral shunting using synthetic prosthesis was done in 9 patients and femoro-popliteal shunting--in 38. In 46 patients, in whom before the operation, basing on the Doppler flowmetry data, the preservation of the microcirculation bed functional reserves was noted, after the operation the increase of the regional blood flow and microcirculation indexes was observed beginning from the first up to the fifth day. In one patient the insufficient functional reserve of the microcirculation bed was revealed, the realization of restorational operation turned out to be ineffective, the lower extremity amputation was performed. The dynamics of the microcirculation changes on the first, third, fifth and seventh postoperative days was shown.

[Revascularization of the lower limbs for ischemic diabetic foot with pyonecrotic tissue lesions].

This paper analyzes the results of the treatment of 213 patients with ischemic diabetic foot marked by pyonecrotic tissue lesions. On examination of the patients the authors were governed by the clinical findings as well as by the data supplied by the research methods such as ultrasound Doppler, percutaneous oxymetry, contrast angiography and laser flowmetry. Altogether 89 patients underwent revascularisation of the lower limbs by means of restoring arterial circulation; 72 patients were provided lumbar sympathectomy. The indications were elaborated for use of one or another revascularization technique. It has been established that of the 161 patients provided limb revascularization, the satisfactory results with limb salvage were obtained in 132 (81.9%) patients; high amputations were performed in 22 (13.6%) cases; 7 (4.3%) patients did not improve; after operation no lethal outcomes were recorded. Of the 52 patients who did not undergo lower limb revascularization, the beneficial results were obtained in 37 (71.1%), high amputations were performed in 15 (28.9%) patients. The conclusion is drawn that limb revascularization allows to minimize the number of high amputations more than two fold.

[Restorative operations on main vessels for diabetic angiopathy of lower extremities with purulent-necrotic complications].

There were analyzed the results of treatment of 75 patients with diabetic angiopathy of the lower extremities and purulent-necrotic affection of the foot tissues, to whom restoration operation on the main vessels was accomplished. In 11 patients aorto-femoral bilateral shunting together with lumbar sympathectomy was performed, in 7--femoro-femoral shunting, in 38--femoro-popliteal, in 12--iliofemoral shunting together with lumbar sympathectomy, in 7--thrombendarterectomy from a. femoris communis in conjunction with the vessel dilatation using the patch. In 89.2% of patients satisfactory result with the extremity preservation was noted. In 10.8% of patients high amputation of extremity was done, all the patients are alive.

[Surgical strategies and choice of treatment methods of the sequelae of the main vessels damage in extremities].

The data on diagnosis, surgical tactics and method of treatment of 75 patients with consequences of the extremities main vessels damage were presented. In 18 patients pulsating hematoma have occurred, in 16--the arterial one, in 12--arteriovenous aneurysm, in 29--the disease of ligated vessel. The data on clinical investigations, ultrasonic dopplerography and roentgencontrast angiography were used for the diagnosis. There were operated on 66 patients. Restorational operation on vessels was done in 47 (71.2%), of them ligation of the vessels--in 16 (24.2%), the lumbar sympathectomy--in 3 (4.5%). The immediate positive result of the treatment was noted in 63 (95.5%) of patients. All the patients are alive. Original method of traumatic aneurysm treatment was adduced.

[Surgical policy in diabetic angiopathies of the lower limbs with suppurative-necrotic lesions].

Results of treatment of 158 patients aged from 17 to 26 years with pyonecrotic complications of diabetic angiopathies are presented. Classification is proposed taking into account characteristics of purulent lesions (local, general, progressive), state of circulation in the damaged limb (major or collateral) and severity of ischemia for choice of treatment policy. Thanks to a sound policy based on these data recovery with limb safety was achieved in 130 (82.3%) patients, including 55 patients who had undergone reconstructive vascular operations or lumbar sympathectomy. High amputations were performed in 28 (17.6%) patients, in 12 of them--on the crus, and in 16--on the thigh. There were no lethal outcomes.

[Application of lumbar sympathectomy for the lower extremities diabetic angiopathy in presence of purulent necrotic changes on the foot].

Immediate results of treatment of 63 patients with diabetes mellitus and occlusion of main arteries of lower extremity on different level in chronic ischemia and presence of limited purulent-necrotic changes of toes were adduced. Indications for the operation performance were substantiated and immediate results were analyzed. Favourable result was achieved in 48 (76%) patients, fair--in 8 (13%), in 7 (11%) patients high amputation of extremity was done.

[Changes in immune state in patients with duodenal ulcer disease].

There were examined 73 patients with giant ulcer (GU) of the duodenum and 62--with the smaller size ulcer of the duodenum. In majority of patients essential disorders of immune state were revealed: reduction of the T-lymphocytes quantity, disbalance of their subpopulation composition, the rise of the circulating immune complexes level, activization of autoimmune reactions. In patients with duodenal GU immune disorders were more pronounced, than in the presence of the smaller size duodenal ulcer. Maximum changes of the immune status were observed in patients with complications of duodenal GU--perforation, penetration, haemorrhage.

[Purulent pancreatitis].

For last 12 years 455 patients with an acute pancreatitis were treated in the clinic. In 29 (6.3%) of them purulent pancreatitis, originating from pancreonecrosis, was diagnosed. Operative intervention consisted of laparotomy conduction, bursa omentalis draining through lig, gastrocolicum, excision of purulent-necrotic foci in pancreas and retroperitoneal fat, draining purulent cavity using two silicon tubes, introduced via left and right epigastral regions, durable lavage of purulent cavity with antiseptic solutions. Five patients died.

[Indications for and prognostication of results of lumbar sympathectomy in patients with obliterating atherosclerosis of the lower limbs with chronic critical ischemia].

The instrumental methods of investigation data, permitting to estimate the regional hemodynamics state in affected extremity, was used for the lumbar sympathectomy (LS) result prognosis in patients with obliterating atherosclerosis of the lower extremities vessels while chronic critical ischemia. The LS conduction is indicated in patients with critical ischemia in the stage of the regional blood flow compensation if it is impossible to conduct the restorative operation.

[Surgical interventions in acute pancreatitis].

Results of treatment of 240 patients with an acute pancreatitis are adduced. Of them 42 (17,5%) were operated on. In 6 patients an acute edema of pancreas was revealed (one patient have died), in 36--destructive forms of an acute pancreatitis, 8 (22%) died. Results of treatment of patients, operated on in one week after disease beginning, were the best.
