Phenotype of peripheral blood lymphocytes and serum immunoglobulin concentration in patients with early rheumatoid arthritis.

We studied the phenotype of peripheral blood lymphocytes and serum immunoglobulin concentration in patients with early rheumatoid arthritis and rheumatoid arthritis of more than 12 months duration. Subpopulations of CDIgM+, CD25+, and HLA-DR+ lymphocytes and IgA concentration differed in these groups of patients with rheumatoid arthritis. The count of lymphocytes carrying CDIgM+ and HLA-DR+ receptors correlated with activity of rheumatoid arthritis.

[C677T mutation in methylentetrahydrofolatereductase gene in patients with venous thromboses from the central region of Russia correlates with a high risk of pulmonary artery thromboembolism].

AIM: To investigate genetic factors of risk (RF) to develop venous thrombosis and pulmonary artery thromboembolism (PATE) in population of central Russia. MATERIAL AND METHODS: We studied polymorphism of the genes of coagulation factor II (G20210A), factor V (G1691A) and methylentetrahydrofolatereductase (MTHFR) with polymerase chain reaction and restriction analysis of DNA amplified sites. We estimated prevalence of the mutations in healthy population and in patients with flebothrombosis as well as effects of the mutations on a PATE rate in patients with thrombosis. We examined 97 patients with documented flebothrombosis. PATE was detected in 54 of them. The control group consisted of 56 healthy volunteers matched by age and gender. RESULTS: G1691A mutation in the gene of coagulation factor V (Leiden mutation of factor V--LMFV) in healthy population occurred in 3.6%, in patients with flebothromboses--in 19.6% (OR = 6.58; 95% CI from 1.47 to 29.42; p = 0.006). Heterozygous mutation G20210A in prothrombine gene was detected in 8 (8.2%) patients (p = 0.027), while this mutation was registered in none controls. Polymorphism of MTHFR gene (C677T) was seen both in the control and patients (60.7 and 52.6%, respectively). LMFV occurrence in patients with flebothrombosis and PATE is less than in patients with flebothrombosis without TEPA (16.7 and 23%, respectively). The PATE risk is significantly higher in carriers of mutant allele 677CT and 677TT of MTHFR compared to patients free of this mutation (OR = 3.11; CI 95% from 1.35 to 7.15; p = 0.006). Homozygous inheritance of this mutation in males combined with PATE in 100% cases. Of 8 carriers of heterozygous mutation G20210A in prothrombin gene PATE was detected in 5 carriers. CONCLUSION: LMFV and mutation G20210A in prothrombin gene are genetic risk factors of venous thrombosis. LMFV is not a PATE risk factor. Mutation C677T in MTHFR gene has no influence on the risk of venous thromboses but makes PATE much more probable. This suggests that it may be a genetic risk factor of PATE in this disease.

[Antiphospholipid syndrome in the structure of hematogenic thrombophilia in young and middle-aged patients with venous thrombosis].

AIM: To specify detectability and clinical presentation of antiphospholipid syndrome (APS) in young and middle aged patients with phlebothromboses (PT). MATERIAL AND METHODS: Enzyme immunoassays for lupus anticoagulant, PCR determination of G1691A mutation in the gene of coagulation factor V, mutation G20210A in prothrombin gene, mutation C677T in methylenetetrahydrofolate reductase (MTHFR) gene were made in 97 patients (57 males and 40 females) with venous thrombosis as well as estimation of external and internal coagulation, antithrombin III activity, protein C activity, plasma fibrinogen, stimulated platelet aggregation, blood and plasma viscosity. RESULTS: APS was detected in 20.6% young and middle-aged patients with venous thrombosis, in 18.5% of them--primary APS. In APS patients acquired risk factors of thrombosis occurred significantly less frequently than in patients with venous thrombosis free of APS (30 and 70%, respectively). Recurrent pulmonary artery thromboembolism (TEPA) prevailed in APS patients. In patients with combined hemostatic disturbances (APS+mutation) TEPA was diagnosed more frequently than in APS patients and in the absence of markers of genetic thrombophilia. Plasma viscosity is most important diagnostically among rheological indices.

[Crioglobulinemia in cerebral and coronal atherothrombosis].

Forty-seven patients: group A--17 patients with acute ischemic atherothrombotic stroke and group B--30 patients with acute Q-formation myocardial infarction. The levels of dynamics and comparative clinical relevance of crioglobulinemia in patients of the both groups have been evaluated. Crioglobulines (CG) in the peripheral blood serum were determined at 1, 2, 3, 7th and 14th days after the disease development. On the 1st day, the elevated CG level similar in the both groups as well as similarity of crioglobulinemia dynamics in whole was observed. In the group A, but not in the group B, the CG level was directly correlated with disease severity (p<0,05). In the group B, there was a trend, which depended on the blood CG level on the 1st day of the disease and on the presence of previous stenocardia (p<0,1).

[Changes in indices of inflammatory activity in patients with rheumatoid arthritis during early stages of basic therapy with leflunomide].

AIM: To assess leflunomide efficacy and tolerance in patients with rheumatoid arthritis (RA) during the first four months of the treatment. MATERIAL AND METHODS: The study included 200 RA patients treated in four Moscow clinical centers. Leflunomide was given in a dose of 100 mg/day for 3 days, then 20 mg/day for 16 weeks. The activity of the disease according to the criterion DAS 28 was assessed before the treatment and 4, 8, 12 and 16 weeks after the treatment start. RESULTS: RA activity diminished considerably after one month of leflunomide treatment. Later, the articular syndrome continued to improve. A significant improvement by DAS 28 was observed after 16 weeks of the treatment in 65% (129 of 200) patients, high RA activity persisted only in 17 of 90 patients. CONCLUSION: Leflunomide reduces articular inflammation and raises RA patients' quality of life at early stages of the treatment. This reduction continued for 4 months of the study. Therefore, adequate assessment of leflunomide efficacy should be made only after 4-6 months of therapy.

[Effect of moxonidine on parameters of lipid metabolism in patients with metabolic syndrome.].

AIM: To compare effects of moxonidine and lisinopril on parameters of lipid metabolism in patients with metabolic syndrome. METHODS: Moxonidine (0.2-04 mg/day) and lisinopril (10-20 mg/day) were given to 25 and 24 patients, respectively. Blood pressure, heart rate, parameters of lipid profile and fasting blood glucose were measured before and after 12 weeks of treatment. RESULTS: Antihypertensive efficacy of 2 drugs was similar. No changes of lipid profile occurred in lisinopril treated patients while significant elevation of high density lipoprotein cholesterol and tendency to lowering of triglyceride level were observed in moxonidine group. CONCLUSION: Therapy with moxonidine was associated with favorable changes of lipid profile.

[Circadian blood pressure profile in patients with metabolic syndrome and osteoarthrosis during therapy with nonsteroidal anti-inflammatory drugs].

Twenty-three patients with metabolic syndrome (MS) concurrent with ostearthrosis (OA) who had been long taking various nonsteroidal anti-inflammatory drugs (NSAID) (a study group) and 23 patients with MS without locomotor diseases (a control group) were examined. The groups were matched by gender, age, body mass index, and office blood pressure values. All the patients underwent 24-hour blood pressure (BP) monitoring. The mean 24-hour, mean diurnal and nocturnal variables of BP and the proportion of non-dippers were estimated in the study and control groups. In the study group, there were higher levels of the mean 24-hour systolic BP (SBP) (p = 0.044), pulse BP (p = 0.023), mean diurnal and mean nocturnal SBP (p = 0.040 and 0.036, respectively). There was also a predominance of non-dippers in the study group as compared with the control one (p = 0.037). Thus, the use of NSAID in patients with MS and concomitant OA causes negative changes in the 24-hour profile of BP.

[Gastrointestinal hemorrhages as complications of gastropathies associated with intake of nonsteroidal anti-inflammatory drugs].

AIM: To examine gastroduodenal mucosa in patients treated with nonsteroidal anti-inflammatory drugs (NSAID), to evaluate the rate and severity of gastrointestinal hemorrhage (GIH) as NSAID-gastropathy manifestation by the results of clinical and device tests. MATERIAL AND METHODS: A retrospective analysis of 2042 case histories of patients admitted to the surgical department of the Moscow city hospital N 1 in 1997-2001 with diagnosis GIH confirmed at esophagogastroduodenoscopy (EGDS). 989 patients (566 males and 423 females) had acute GIH from the upper gastrointestinal tract. RESULTS: Relationship between intake of NSAID and GIH was documented in 342 patients (16.7% of overall number of the examinees and 34.6% of patients with acute GIH). According to EGDS, GIH was provoked by gastric lesions (68%), duodenal lesions (20%), gastroduodenal lesions (9%), esophageal erosions (3%). GIH was caused primarily by low-dose aspirin, indometacin, diclofenak and ibuprofen. Severe GIH occurred in 28.9% cases, moderate and mild ones in 20.8 and 50.3%, respectively. CONCLUSION: To reduce the incidence of GIH, it is necessary to take preventive measures against NSAId-gastropathies.

[Clinical and instrumental characteristics of gastrointestinal bleeding caused by non-steroidal anti-inflammatory drugs].

UNLABELLED: There was a study of the condition of mucous membranes of upper parts of the gastrointestinal tract in patients with gastrointestinal bleedings (GIB), who were taking non-steroidal anti-inflammatory drugs (NSAID); the blood loss severity was assessed. 2,042 health records of patients, who came to surgical departments with the gastrointestinal bleeding diagnosis, were studied pro- and retrospectively. A group of 342 patients with GIB induced by NSAID was selected. The age of the group of patients under examination was from 17 to 86. RESULTS: Chronological relation between the application of NSAID and GIB was discovered in 16.7% of all cases. When assessing the bleeding source area, stomach affections were discovered in 68% of cases; duodenal affections were discovered in 20% of cases; associated stomach and duodenal affections were recorded in 9% of cases; esophagus erosion was found in 3% of cases. Bleedings were most often induced by the application of Aspirin, Indometacin, Diclofenac and Ibuprofen. Severe bleedings were revealed in 28.9% of cases, medium bleedings were recorded in 20.8% of cases, and 50.3% of cases were attributed to light bleedings.

[Pain in the lower part of the spine in spinal osteochondrosis: experience in using chondroprotective drugs].

AIM: To assess the response to chondroitin sulfate (structum) in low back pain (LBP) due to spinal osteochondrosis. MATERIAL AND METHODS: 30 patients (mean age 51.4 years) with a definite primary LBP took structum in a dose 1 g/day for 24 weeks. The diagnosis was made according to WHO recommendations (2000). The response was assessed with uniform international questionnaires and visual analogue scale. RESULTS: The treatment reduced pain syndrome and improved spinal function in 73.3% patients with LBP studied. CONCLUSION: It would be valid to include long-acting chondroprotective drugs in the program of LBP in spinal osteochondrosis.
