[Measurement of alcohol in saliva with a test strip].

As the concentration of alcohol in saliva follows the concentration of alcohol in the blood meticulously, a test strip to measure alcohol in saliva would provide an easy and rapid method of semiquantitative estimation of the blood alcohol concentration. The authors have submitted Alco-Screen test strips to trial by comparing five different batches by measurement on aqueous solutions of alcohol, saliva to which alcohol had been added and also in practice. The batch variations were very great and the results obtained on aqueous solutions and saliva with added alcohol were too high whereas the results in practical use were, in general, too low. In addition, it was found that the colour of the reagent field was difficult to compare with the reference fields. Instructions for use were incomplete and easily misunderstood.

Partial obstruction of the sigmoid colon by surgical implantation.

Invasive transitional cell carcinoma of the bladder is known to extend directly to other organs in the pelvis. When metastases occur they almost always adhere to a rather consistent pattern of dissemination. We present an unusual case of partial obstruction of the sigmoid colon due to a metastatic transitiocellular bladder tumour four months after transurethral resection of the tumour complicated with a perforation into the peritoneal cavity. Direct implantation of the tumour with the resectoscope is the most possible explanation, but seeding on an intact serosa cannot be excluded either.

The development of a Ki-1-positive large cell non-Hodgkin's lymphoma in pagetoid reticulosis.

A case of the disseminated variant of pagetoid reticulosis is described which progressed after many years of disease to a large cell anaplastic (Ki-1) lymphoma of T-cell type. The lymphoma cells showed abnormal cellular DNA and a high proliferative rate, as revealed by immunophenotypic examination and single-cell DNA measurements. The cells were positive for activation associated antigens and expressed a T-helper/inducer phenotype. A similar phenotype was expressed by the neoplastic cells in the co-existing lesions of pagetoid reticulosis. These findings support the view that pagetoid reticulosis is a variant of the cutaneous T-cell lymphomas which originates from activated lymphoid cells and which may on occasion progress to a potentionally more aggressive lymphoid malignancy.

Immunohistochemical steroid receptor detection in frozen breast cancer tissue. A multicenter investigation.

Now that monoclonal antibodies against the estrogen receptor (ER) have become available, it is possible to detect the presence of ER immunohistochemically. In order to evaluate the validity of the immunohistochemical (ERICA) method in a routine detection of ER, we have tested the method in a multicenter investigation. As reference is used a conventional biochemical assay, dextran-coated-charcoal assay (DCC). The investigation analyzes the validity of the ERICA-assay, including a semiquantitative estimation of the ER-content, when the method is used in several independent departments of pathology, and the interobserver variation of the analysis. The results show that the qualitative detection of ER, obtained by the ERICA-assay and the DCC-assay has a concordance of about 90% for all the participating departments. The concordance is around 60-70%, when a semiquantitative estimation of the ER-content is used. The interobserver variation is also very low, with full agreement in 58 of 59 cases. The ERICA-assay may, therefore, be a relevant alternative to the DCC-assay. However, its clinical importance still has to be fully elucidated.

Adenosarcoma uteri.

Adenosarcoma of the uterus is a rare tumor of low-grade malignancy. It belongs to the family of mixed mesodermal (Müllerian) tumors. It is a biphasic tumor with benign epithelium and malignant stromal components. A case in a 76-year-old woman is presented.

Successful treatment of cold angio-oedema by H2-antihistamine therapy.

Two patients with severe cold angio-oedema were practically relieved from their symptoms during H2-blocker therapy (cimetidine 1 000 mg daily). Classic H1-blocker therapy had no effect on their symptoms and combined H1- and H2-blocker treatment was just as effective in ameliorating the symptoms of cold angio-oedema as H2-blocker treatment alone. A low dose of the H2-blocker (400 mg cimetidine daily) was almost able to control the angio-oedema formation after cold exposure, but provoked typically urticarial lesions. This observation strongly indicates that H2-receptors may play an important role in the pathogenesis of cold angio-oedema and that a massive, subcutaneous, cold angio-oedema may obscure cold urticarial lesions.

Pachymeningitis cervicalis hypertrophica syphilitica.

A rare form of neurosyphilis with hypertrophic cervical pachymeningitis is presented. It is concluded that serological tests for syphilis are still indicated in patients with unexplained neurological or psychiatric symptoms.

Multiple kerato-acanthoma: a case report.

A case of multiple keratoacanthomas in a 36-year-old healthy seaman is presented. Four months prior to the onset of the disease the patient had been exposed to strong sunlight. Histopathological examination gave the diagnosis of keratoacanthoma.
