Anaerobic and aerobic microflora of pouch of Douglas aspirate v/s high vaginal swab in cases of pelvic inflammatory disease.

The anaerobic and aerobic bacterial flora in pouch of Douglas (POD) aspirate and high vaginal swabs (HVS) was studied in 43 cases of pelvic inflammatory disease (PID) using standard techniques. High vaginal swabs from 20 healthy women were included as controls. Anaerobic and aerobic bacteria were isolated from 37 (86%) HVS and 31 (72%) POD aspirates from these 43 women. A total of 100 aerobic and 10 anaerobic bacterial strains were recovered from both the sites. Coagulase negative staphylococci (28), Escherichia coli (23) and Streptococcus faecalis (14) were predominant aerobic bacteria. Of the anaerobes, peptostreptococci species and Bacteroides species were more common, polymicrobial flora (more than one type of organism) was present in a total of 27 specimens. However mixture of anaerobic and aerobic bacteria were seen in only 5 specimens. Of the 20 control specimens, ten were positive for organisms. Ten aerobic and 3 anaerobic bacterial strains were recovered.

Pleiotropic changes in the activities of extracellular toxins of Pseudomonas aeruginosa protease-deficient mutants.

Protease-deficient mutants of Pseudomonas aeruginosa obtained by ethyl methane sulphonate treatment were found to show pleiotropic effect on various extracellular toxin activities. Of eight mutants five were completely deficient in lecithinase and four in collagenase activity. The decreased hemolytic activity was obtained in five mutants and enterotoxin activity in seven mutants as compared to parent strain.

Effect of the mi allele on mast cells, basophils, natural killer cells, and osteoclasts in C57Bl/6J mice.

The osteopetrotic, microphthalmic (mi/mi) mouse lacks functional osteoclasts and has also been reported to be deficient in mast cells and natural-killer (NK) cells. The later deficiencies could be secondary to the osteopetrotic marrow, or a direct result of the mi allele. Therefore, heterozygotes were examined for these cell types, since these mice do not exhibit osteopetrosis. Adult +/mi animals have approximately 50%, and mi/mi animals examined by histologic techniques or tissue histamine levels have 0-10%, of the peritoneal, dermal, and intestinal mast cells compared with that of +/+ animals. Leukocyte histamine, indicative of the number of basophils, demonstrates the same pattern. Histamine content per mast cell in +/+ and +/mi animals is identical. The number of large granular lymphocytes (LGL) in splenic leukocyte preparations from +/mi animals is 50% that of +/+ animals, and these cells are undetectable in preparations from mi/mi mice. NK activity against YAC-1 cells paralleled the number of LGL present. The resorptive response of neonatal calvaria to parathyroid hormone was delayed in the case of cultured +/mi bone compared with that of +/+ bone, but the final rate of calcium release was identical. These data indicate that 1) the presence of one mi allele can affect the development of four distinct cell types, and 2) osteopetrosis alone does not account for the lack of mast cells, basophils, and NK cells in mi/mi mice.

Transneuronal retrograde degeneration in the cat retina following neonatal ablation of visual cortex.

Bilateral removal of the cortical visual area in newborn cats produces degeneration of retinal ganglion cells. Measurements of over 4,000 cells and calculation of neuron densities from sample areas of retina in the adult show that the medium sized cell population in peripheral retina is reduced by 68%, whereas the populations of small and large cells are not affected. The degeneration is greater in peripheral retina than in area centralis.

Probable Kawasaki disease in a 52-year-old man presenting with acute myocardial infarction treated with percutaneous revascularization.

This is a case report of a 52-year-old man who presented with an acute myocardial infarction. In the process of performing an angiogram to delineate the anatomy for possible percutaneous transluminal coronary angioplasty, large, diffuse coronary aneurysms were observed. When the vessel was opened, several aneurysms were seen to contain thrombus. The size, location, and diffuseness of the aneurysms are suggestive of Kawasaki disease.

Is hysterosalpingography an important tool in modern gynecological practice?

OBJECTIVE: To assess the value of hysterosalpingography (HSG) in diagnosing tubal patency and peritubal adhesions. PATIENTS AND METHODS: HSG was followed whenever possible by diagnostic laparoscopy with chromopertubation to give the final prognosis in cases of infertility. RESULTS: Normal HSG findings were obtained in 669 women, and abnormal ones in 306. No significant correlation was found between age of patient, duration, type of infertility, and HSG findings. Tubal blockage was seen in 89.2% and uterine factor in 9.4% of cases. Peritubal adhesions were suggestive in 12.75% of cases. On laparoscopy, done in about half of these women, findings were in agreement with HSG in 59.87% of cases. Follow-up was carried out whether the woman conceived, had a laparotomy, or required further investigations. CONCLUSION: Although somewhat insensitive, the high specificity of hysterosalpingography makes it very helpful for ruling out tubal disease, even where endoscopic evaluation is available.

Congenital atresia of left main coronary artery.

We report a rare case of congenital atresia of left main coronary artery in an elderly male presenting with angina and positive stress thallium test. Coronary angiogram showed absence of left main coronary artery in the left aortic sinus. Collaterals from right coronary artery supplied left anterior descending artery and left circumflex artery territories. Despite collaterals, the left anterior descending and left circumflex arteries were rudimentary. Poor size of left-sided arteries precluded any surgical revascularization which is the treatment of choice in this rare entity.

Acute myocardial infarction secondary to myocardial bridge treated with drug-eluting stent.

We report a case of myocardial bridge in left anterior descending coronary artery associated with acute anterior wall myocardial infarction. The patient had many unusual features like coronary ectasia and atherosclerosis within the myocardial bridge segment. The patient was treated with drug-eluting stent under intravascular ultrasound guidance with good result over 6 months follow-up.
