Medical interns knowledge of TB in Pakistan.

Of 460 interns from five Pakistani teaching hospitals surveyed, only 22% correctly identified the estimated number of new TB cases in Pakistan. The majority (96%) knew that droplet infection was the usual mode of transmission. Only 38% considered sputum smears for acid-fast bacilli as the best test for diagnosis of pulmonary TB and 43.5% for follow-up during TB treatment. The recommended four-drug anti-TB regimen was prescribed by 56.5% in the initiation phase and the recommended two-drug combination in the continuation phase by 52%. Most interns (82%) were unable to identify a single component of directly observed treatment short course (DOTS) strategy. Our study reflects poor awareness of and low compliance to the World Health Organization/National Tuberculosis Programme guidelines among interns. For effective control of TB, immediate action to improve undergraduate and continuing medical education is essential, with special emphasis on national guidelines.

Cultural barriers to asthma management.

This article reviews published evidence which addresses the relevance of cultural factors in the delivery of health services for asthma patients. In addition, it suggests a framework within which further research could be carried out to advance our knowledge on this topic.

[Principles and organization of tuberculosis control].

Tuberculosis control must be organized by establishing programmes that are then implemented rigorously. The aim is to eradicate tuberculosis by applying a set of treatment procedures. These include, in particular, the detection of infectious cases in symptomatic patients who present to health services and their rapid treatment with chemotherapy, which makes them non-infectious and ensures their full recovery. An essential part of the programme is the information system, which enables treatment results to be evaluated and the efficacy of the treatment to be monitored. The model of the International Union against Tuberculosis and Lung Disease (IUATLD) for low income countries has proved to be effective, and has been recognized as one of the most efficient health interventions. The eradication of tuberculosis is possible: tuberculosis has a slow endemic cycle and tuberculosis can be reduced faster than drug resistance can emerge. In addition, all the tools required for tuberculosis control are already available and HIV infection is not yet widespread in several of the countries where tuberculosis is prevalent. Nevertheless, there is an urgent need for the rigorous and standardized application of the methods we have developed.

Prevalence of asthma in a region of Algeria.

In 1984-85 an epidemiological survey on asthma was carried out on a representative population sample in the "Daira de Chéraga" region in Algeria. It had two objectives: to estimate the prevalence of asthma and to describe its epidemiological pattern. The study was divided into two parts: the first was a household survey based on a questionnaire and the second, another interview at hospital, with clinical examination and pulmonary function tests with pharmacodynamic tests. The cumulative prevalence of respiratory symptoms was 2.38%. The cumulative prevalence of asthma was 1.34% and the annual prevalence 1.21%. Asthma was more prevalent in boys than girls aged under 20 years (sex ratio = 2.66) with an equal proportion after age 20 years. Asthma most often occurred in childhood in males and at an adult age in females. In 40% of cases there were family antecedents of asthma. It is impossible to extrapolate these results to the entire country and the existence of regional disparities pleads in favour of further research in this field.
