[Assessment of the degree of severity of mitral regurgitation by the systolic inversion of the pulmonary venous flow. Transesophageal echocardiography study].

It is controversial the value of an echocardiographic mitral regurgitation evaluation based on planimetric patterns color codification area. We made this study using as alternative methods of quantification, the parameter of pulmonary venous flow, concretly maximum velocity and area of pulmonary systolic inversion venous flow. Considered the values of this related parameters to 3 crescent degrees on mitral regurgitation severity and obtained the following results: Lightness mitral insufficiency with maximum inversion area, 10 +/- 21 mm2 and maximum inversion velocity, 3.2 +/- 6 cm/seg. Moderate mitral insufficiency with maximum inversion area, 171 +/- 140 mm2 and maximum moderate velocity, 21 +/- 13 cm/seg. Several mitral insufficiency with maximum inversion area, 648 +/- 615 and maximum inversion velocity, 50 +/- 20 cm/seg. We conclude that systolic parameters of pulmonary venous flow evaluated by transesophagic echocardiography constitute considerable sensibility rules in the evaluation of severity degree in echocardiographic mitral regurgitation.

Splenic hydatidosis.

This report is concerned with ten patients treated surgically for a splenic hydatidosis. Splenic echinococcosis represents 3.5% of abdominal hydatid cysts treated in our Service. When splenic hydatidosis was diagnosed, the spleen was the first and only localization of hydatid disease in three cases. One of these patients underwent surgery for a pulmonary cyst four years later. Splenic cysts were asymptomatic in six patients. The most frequent clinical sign of splenic hydatidosis was a painful mass in the left upper abdominal quadrant. Ultrasonography and axial computed tomography (axial C.T.) were the most useful diagnostic tools. The complications arising from splenic hydatidic cysts were infection, rupture in the abdominal cavity and fistulization to the colon. A splenectomy was performed in every case. There was no mortality and morbidity was principally related to the hepatic-associated hydatidosis. It is concluded that splenic involvement is a rare manifestation of hydatid disease but should be looked for in a systematic way in patients with this diagnosis.

Cell responses to acoustic stimuli in the pterothoracic ganglion of two noctuoid moths.

The spike activity of various types of cell responses in the pterothoracic ganglion of Ascalapha odorata (Noctuidae) and Empyreuma pugione (Arctiidae) was studied. Pure tones (16 kHz for A. odorata and 20 kHz for E. pugione, 45 ms pulses) were presented at a 1 Hz rate over 9 s and at intensities ranging from 25 to 95 dB SPL. The values of the latency period and the interspike intervals allowed us to describe the intensity-latency and intensity-response functions as well as the spike distribution during the responses, the latter being given by the 'instantaneous frequency', i.e., as the inverse value of the mean of the nine measurements of each interspike interval during the response time. 'Repeater' (RA1 and RA2) is a type of cell response that shows a phasic-tonic spike distribution similar to that of the receptor cells (A1 and A2), but that differs from the latter in a longer (ca. 1.0 ms) latency period, a lower number of spikes per pulse, and a lower instantaneous frequency during the response time. Another repeater type of cell response (RA) differs from the receptors and the other two repeaters in the form of its intensity-latency function, having the widest dynamic range (from 40 to 50 dB), and exhibiting the highest maximal number of spikes per pulse of all the response types recorded. We recorded also strictly phasic responses (1 or 2 spikes per pulse), which are considered as pulse markers. Of these, one (PM1) has a shorter latency period (ca. 10 ms) and higher sensitivity than the other (PM2). Two other types of cell responses showed significant differences in their latency period and the number of spikes per pulse under binaural and monoaural stimulation and are assumed to be the consequence of binaural summation, one by inhibition (BSI) and the other by excitation (BSE); they also differ in the spike distribution during the response. For the other types of cell responses recorded we used names that reflect the form of their spike distribution: chopper, build, On-S, tonic, and suppression. The spike distributions during the response time recorded in the pterothoracic ganglion of these two noctuoid moths are compared with the temporal patterns of discharge described in the auditory neurons of the first relay stations of birds and mammals.(ABSTRACT TRUNCATED AT 400 WORDS)

Ovarian and feto-placental factors and the regulation of prolactin release during pregnancy in the rat.

Administration of progesterone (5 or 10 mg) to pregnant rats increased serum prolactin significantly on the afternoon of Days 4, 6, 7 and 8 of pregnancy, but had no effect on later days. On Day 10 progesterone administration increased serum prolactin only in rats treated with oestrogen the day before. A similar treatment with oestrogen and progesterone was unable to stimulate secretion on the afternoon of Day 13 of pregnancy. In rats from which the corpora lutea had been unilaterally removed and hence endogenous progesterone levels were 50% of the normal values, or in those that carried 4 conceptuses, progesterone treatment after oestradiol priming was partly effective in inducing prolactin release on Day 13. However, in rats ovariectomized, with bilateral excision of the corpora lutea, or with 2 conceptuses left, treatment with ovarian steroids markedly increased serum prolactin values. By Day 13 all the rats from the ovariectomized group or with bilateral excision of the corpora lutea had aborted. On Day 13, therefore, the high serum concentrations of feto-placental factors and of progesterone are responsible for the blockade of the spontaneous and ovarian steroid-induced prolactin release. On the other hand, on Day 16 of pregnancy the decrease of circulating progesterone by excision of the corpora lutea or by ovariectomy followed by oestradiol treatment significantly increased serum prolactin on Day 17. Removal of all the conceptuses did not modify the effects of these treatments. The present results demonstrate different roles of progesterone upon the control of prolactin secretion. After a stimulatory action during the first days of pregnancy, there is a change to an inhibitory control at the end of pregnancy.(ABSTRACT TRUNCATED AT 250 WORDS)

The distal pedicle fascia flap of the leg.

An anatomical study was undertaken in order to obtain data on the vascularity of the posterior fascia of the leg. Injection studies revealed there to be three "distal" pedicles; one between the peroneal and soleus muscles, emerging 10-15 cm above the lateral malleolus and two other, smaller ones, 5 cm above the medial and lateral malleolus. Clinical experience included seven cases: five with trauma and two with varicose ulcers. We had good results in five, a partial loss in one and a total loss in one. The distal pedicle fascia flap should be considered an alternative method for repairing defects in the lower third of the leg.

Effect of synthetic thyrotrophin releasing factor on prolactin and luteinizing hormone secretion in male and female rats during various reproductive states.

The effect of synthetic thyrotrophin releasing factor (TRF) on serum prolactin and LH concentrations was determined by radioimmunoassay in male, cyclic and pseudopregnant female rats. A solution of TRF (0-1, 0-25, 0-5 and 1 mug/rat) was injected i.v. at 17.00 h into rats pretreated with sodium pentobarbitone at 13.00 h. A group of male rats was also treated with TRF at 11.00 h after pretreatment with sodium pentobarbitone at 07.00 h. Fifteen minutes after TRF administration, blood samples were obtained by heart puncture. Doses of 0-25, 0-5 and 1 mug TRF significantly increased the serum prolactin concentration in pro-oestrous rats. The mean serum prolactin level after the injection of 0-5 and 1 mug into oestrous rats and 0-5 mug TRF into dioestrous day 2 rats, was significantly greater than the control values. Injection of TRF on day 1 of dioestrus had no effect. Serum LH concentration was not significantly modified by the various doses of TRF administered. On day 3 of pseudopregnancy a significant increase of serum prolactin values was obtained with 0-5 and 1 mug TRF. On day 7 of pseudopregnancy a dose of 0-5 mug produced the same effect, but on day 10 of pseudopregnancy only 1 mug TRF significantly increased serum prolactin levels when compared with the control rats. In male rats serum prolactin concentration was significantly greater than the control values after TRF treatment either in the morning or the afternoon. The response was similar to that obtained in pro-oestrous rats. The results suggest that the ability of synthetic TRF to stimulate prolactin release exists in both female and male rats and that TRF does not affect LH secretion.

[Treatment of losses of bone substance of the frontal region with outer table autogenic graft].

Ten patients with losses of frontal bone were operated with the use of bone grafts taken from the parietal area without craniotomy. In all patients the contour was restored without complications at the donor area. Results were evaluated clinically and radiologically one year later and no absorption was observed. We conclude that bone grafts from outer layer of parietal bone offer a satisfactory esthetic results. The morbidity at the donor area was not significant.

[Treatment of complex nose deformities by external approach].

The authors present thirty patients with complex nasal deformities operated on by external approach. The incision was on the nasal columella. They used bone and cartilage grafts for the correction of the deformities. The external transcolumellar incision provided a good visualization of nasal structures and the columella scar was not a significant problem.

[Treatment of cutaneous losses of the heel with lateral calcaneus artery flap].

The lateral calcaneal artery flap is an axial flap which comprises also the saphenous vein and the sural nerve. Its use has been reported for coverage of skin loss of the malleolus and calcaneal area. The donor area should be closed by a split thickness skin graft. In the present report, eight patients who had skin loss on the foot were submitted to reconstruction with the lateral calcaneal flap. In seven there were island flaps and in one a bridge of skin similar to that described by Grabb and Argenta. The etiology was trauma in five patients and decubitus ulcer in three. The results were good. The flap was adequate for the cover on the posterior foot area even after submitted to the strain of normal walking. The advantages were simpler technique and consistency of the anatomical landmarks.

[Autologous fat transplantation in rats].

The authors perform an experimental study in rats to demonstrate the integration of fat cells transplanted from the inguinal region to the dorsal region of the same animal. Histological studies were performed with the material removed and with the material injected. Final results show the transplanted fat cell integration after 360 days.

[Lower limb repair. Bipedicled fasciocutaneous flap. A new technical approach].

The authors present an anatomical study of a fasciocutaneous flap of the lower limb and its clinical application. Sixteen patients with loss of cutaneous substance and exposure of the leg bone were operated on. The bipedicled fasciocutaneous flap presented by the authors allows the coverage of large areas of the lower limb in the initial stage of treatment. Preservation of the superficial venous system allows for a better vascularization of the flap and, thus, for the use of larger flaps.
